Ga-G7 SCINTIGRAPHIC AND RADIOGRAPHIC EVALUATION OF THE ACTIVITY OF EXPERIMENTALLY INDUCED MANDIBULAR INFLAMMATORY LESION (Ga-67 골주사와 X선사진을 이용한 실험적 하악골 염증성 병변의 활성도 평가)

  • The purpose of this study was to evaluate the usefulness of Ga-67 scintigram in predicting the disappearance of inflammatory activity in mandibular lesions of dogs. Inflammatory bony lesions were produced artificially by the injection of sclerosing solution and bacterial suspension through the distal root canals of the lower 4th premolars in 10 dogs. The serial Ga-67 scintigrams and periapical radiograms were obtained. After 3 to 6 weeks duration of antibiotic therapy, the experimental animals were killed, and the mandibles were dissected. Bacteriologic culture and antibiotic sensitivity test were done. 1. The following results were obtained. The negative Ga-67 scan has 100% predictive value, and is a strong evidence of the resolution of inflammatory activity. In 2 animals of positive Ga-67 scan, I animal had negative culture. In 14 cases of positive Ga-67 scan, radiographically there was increase in the size of lesion in 9 cases, no change in 5 cases. In 8 cases of negative Ga-67 scan, radiographically there was increase in the size of lesion in I case, no change in 6 cases and decrease in 1 case.

  • The Inflammatory myofibroblastic tumor (inflammatory pseudotumor) is a rarely occurring soft tissue lesion of unknown etiology. It can be of any location, but commonly it is found in lungs. It has been considered as a nonneoplastic reactive inflammatory lesion, but nowadays, confusion and dispute about its character is increasing due to its high recurrence rate and metastasis. We present a patient who had been diagnosed with an inflammatory pseudotumor in the right maxilla area, 1 year before visiting our hospital. After that, her pain and swelling did not resolved and she visit our hospital. On radiographic examination, aggressively infiltrative growth of the lesion with destruction of adjacent bony structure was noted. We found unusual aggressiveness of the inflammatory myofibroblastic tumor of the head and neck region. Because the typical behavior of the inflammatory myofibroblastic tumor is not defined yet, we recommend the surgical excision of the lesion and close follow-up.

The Protective Effects of Sopung-tang on Brain Damage in Photothrombotic Ischemia Mouse Model (뇌경색 마우스의 뇌손상에 대한 소풍탕(疎風湯)의 보호효과)

  • Objectives : The water extract of Sopung-tang (SPT) has been traditionally used in the treatment of acute stroke in Oriental Medicine. Pro-inflammatory cytokines play a critical role in the onset of post-ischemic inflammatory cascades. The present study was designed to investigate the effects of SPT on pro-inflammatory cytokine production in a photothrombotic ischemia mouse model. Methods : After SPT oral administration to the mice for five days, with using Rose Bengal and cold light, photothrombotic ischemia lesion was induced in stereotactically held male BALB/c mice. Also, results including, gross finding lesion size, histopathological finding changes, and inflammatory cytokine expression changes from the photothrombotic ischemia mouse model were observed. Results : The photothrombotic ischemia lesion was decreased by the oral injection of SPT. Also, SPT inhibited the expression of TNF-$\alpha$, IL-$1{\beta}$, IL-6, the active form of caspase-3 protease, and transglutaminase-2 in the photothrombotic ischemia lesion. Conclusions : These results suggest that SPT protects the ischemic death of brain cells through suppression of the production of anti-inflammatory cytokines and catalytic activation of caspase-3 protease in the photothrombotic ischemia mouse model.

Platysma Infiltration on CT or MRI in Parotid Pathology (이하선 병변에서 활경근 침윤의 빈도와 조건)

  • Purpose To evaluate the incidence and condition of platysma infiltration in benign tumors, malignant tumors, and inflammatory disease in the parotid gland using CT or MRI. Materials and Methods Patients with benign tumors (n = 314), malignant tumors (n = 52), and inflammatory disease (n = 22) in the parotid gland were included. The incidence of platysma infiltration and the relationship between platysma infiltration and the location, capsular involvement, and focality of the mass were retrospectively evaluated using CT or MRI. Results The incidence of platysma infiltration was 0% in benign tumors,19.2% in malignant tumors (10/52), and 50.0% in inflammation (11/22). Platysma infiltration was positive in 10 of 13 patients with inflammatory lesion with capsular involvement. Platysmal infiltrations in inflammatory lesion showed diffuse lesion in 10 patients and focal lesion in one patient. Malignant tumor with platysmal infiltration showed all capsular involvement, and diffuse lesion in seven and focal lesion in three. Conclusion Platysma infiltration was more common in patients with inflammatory disease than those with malignant tumors or benign tumors. In inflammatory disease, platysma infiltration was more common in patients with capsular involvement or diffuse lesion.

  • Inflammatory pseudotumor is an uncommon lesion with unknown etiology characterized by sclerosing inflammation which clinically and radiographically mimics a neoplastic lesion. A 47-year-old man presented with sudden headache and dysarthria. Brain CT scan revealed a $2.6{\times}2.2\;cm$ sized, round, and hyperdense mass in the anterolateral wall of the left lateral ventricular trigone. On MR imaging studies, the mass showed low signal intensity in the wall of the trigone on T2-weighted image, central mixed (iso- and high-) signal intensity with peripheral low-signal intensity on T1-weighted image. Subtle staining of left choroid plexus with irregular shaped distal branch of anterior choroidal artery was found on the cerebral angiography. These findings suggested a small tumorous lesion originated from the left choroid plexus. During the hospital days, the mass manifested as repeated hemorrhages. The mass was successfully removed via left occipital transcortical approach. The histopathological report of the specimen was hemorrhage and fibrosis, with dense lymphoplasma cell infiltration, suggestive of an inflammatory pseudotumor.

  • The authors describe a rare case of inflammatory pseudotumor involving the clivus, where a soft tissue mass lesion, with extension into the prevertebral retropharyngeal space and the cavernous sinuses, was detected by CT and MRI. The mass resembled a malignant tumor or aggressive infectious lesion, and the final diagnosis of inflammatory pseudotumor was a diagnosis of exclusion, decided after histopathological examination.

  • The periapical response to injury is a complex interaction of inflammatory, immune, neural, vascular and synthetic activity. TGF-${\beta}$ is a potent modulator of proliferation and differentiation in various tissue, seems to lead to an increase in extracellular matrix. MMP are a family of proteolytic enzyme that mediate the degradation of extracellular matric macromolecules, but little is known about theirs possible role in periapical tissue. The purpose of this study is to investigate the differential expression of TGF-${\beta}$ and MMP-1 in tooth follicle, periapical abscess, granuloma and cyst. The expression of TGF-${\beta}$ and MMP-1 in Periapical tissue was evaluated by immunohistochemical staining and Western blot analysis. Correlationship among the periapical lesions were stastically analyzed. The degree of MMP-1 expression in periapical abscess was higher than in any other periapical lesion, and stastically significant. TGF-${\beta}$ expression is the prominent in granuloma than other periapical lesion, which was stastically significant. The increased expression of MMP and TGF-${\beta}$ was not co-related with inflammatory cell infiltration degree of the periapical cyst. The expression degree of MMP and TGF-${\beta}$ was not co-related with periapical abscess and cyst, but expression of MMP and TGF-${\beta}$ showed strong positive co-relationship with periapical granuloma, which was stastically significant. TGF-${\beta}$ expression by Western blot analysis was prominent in granuloma and cyst, and similar to the results by imunohistochemistry. MMP-1 expression is less than TGF-${\beta}$, but there is not extreme difference between periapical lesion. These results suggest that TGF-${\beta}$ and MMP may be involved in tissue remodeling and has an important role in progress or mediation of periapical lesions.

Application of artificial neural network to differential diagnosis of lung lesion: Preliminary results

  • It is difficult to differentially diagnose between lung cancer and benign inflammatory lung lesion due to high false positive rate on F-18 FDG-PET. We investigated whether application of artificial neural network to this diagnosis may be helpful. We reviewed the medical records and F-18 FDG PET images of 12 patients, selecting clinical and PET variables such as SUV. For selected variables and confirm, multilayer neural perceptron was applied in crossvalidation method and compared to visual interpretation. Neural network correctly classified the lung lesions in 83%, and reduced greately the false positive rate. However, false negative rate was not influenced. Application of neural network to the differential diagnosis between lung cancer and benigh inflammatory lesion may be helpful. Further studies with more patients are warranted.

RADIOLOGIC EVALUATION OF THE MAXILLARY ANTRAL PATHOLOGIC CONDITIONS IN CHILDREN (소아 상악동 병변의 방사선학적 평가)

  • The authors evaluated the distribution of the antral pathologic conditions and their radiographic features on the Waters' radiolgraphs of 151 children patients who had been radiographed at the department of Oral Radiology, Seoul National University Hospitals. The obtained results were as follows: 1. The most common pathologic condition was inflammatory change(58%). Percentage of cases showing cystic lesion and fibro-osseous lesion were 25% and 11% respectively. 2. In cases of inflammatory change, odontogenic origins were 12 cases(13%) and bilateral occurrences were 37 cases(39%). 3. The most common radiographic feature of the inflammatory conditions was various types of mucosal thickenkng(78%).Percentage of cases showing totally increased radiopacity was 18%. 4. Intrinsic cystic lesions were 26 cases(65%) and dentigerous cyst was the most common extrinsic cyst. 5. Most of the fibro-osseous lesions(15 from 17 cases) were fibrous dysplasia. 6. Maxillary sinus hypoplasia(5 cases) and Burkitt's lyphoma(4 cases) were also observed.

Spinal Nerve Root Compression by Acute Inflammatory Granuloma after Spine Surgery -A case report- (척추수술 후 급성 염증성 육아조직 형성으로 인한 신경근 압박 -증례 보고-)

  • This report describes a case of spinal nerve root compression due to an acute inflammatory granuloma after lumbar surgery. A 39 year-old man with a history of increasing back pain with a 3-week duration was diagnosed with a herniated intervertebral disc (HIVD). The diagnosis of a HIVD was confirmed by magnetic resonance imaging (MRI) with indications for surgery. A discectomy and a partial laminectomy was performed and the symptoms were alleviated immediately after surgery for a five-day period. However, a slowly progressing pain was subsequently noted along a different dermatome. There was no pain relief despite the patient being given pharmacological treatments, combined with an epidural steroid injection. The follow up MRI images showed severe compression of the nerve roots by a epidural lesion. Another procedure was performed 17 days after the initial operation. The lesion responsible for the compression of the nerve roots was found to be an acute inflammatory granuloma. The pain was relieved after the second procedure and there were no other symptoms or neurological problems. This case is remarkable in that a granuloma formed relatively quickly and grew to such a size that it was able to severely compress the surrounding nerve roots.
