  • The purpose of this study was to investigate the interrelationship of the maxillary interincisal diastema, spacing and crowing to the type of superior labial frenum and the type of intermaxillary suture by analyzing histories, intraoral radiographs, orthopantomographs, intraoral slide films, and dental casts. The data for this study were compiled from 500 outpatients of the Department of Orthodontics, Infirmary of Dental College, Seoul National University. The following conculusions were obtained: 1. The occurrence of maxillary interincisal diastema in the permanent dentition decreased rapidly compared with that in the mixed dentition. And there was no sex difference in the occurrence of diastema in the both dentitions. 2. Frenum attachment remained relatively constant between the two dentitions but assumed a higher level in the permanent dentition than in the mixed dentition. 3. Except suture type D which was not yet fused completely, there was no significant difference in the distribution of suture type between the mixed dentition and the permanent dentition. 4. Frenum and suture type were highly related; frenum types 5 and 6 were associated with suture types III and V. 5. In the permanent dentition, frenum type 5 and 6 and suture types III and IV occurred more frequently in the spacing group than in the normal or crowding group. 6. The relationship between the diastema and frenum types 5 and 6 and sture types III and IV showed a strong correlation.

  • The purpose of this study was designed to evaluate the existence of the junctional epithelia on the tooth surface in periodontal pocket, and what were the morphologic differences between the junctional epithelia on the healthy and advanced periodontitis tooth. Fifteen premolar teeth from patients of Yon Sei University, Dental Infirmary were selected for this study. After extration, the teeth were prepared and examined in Scanning electro microscope. The results were as follows. 1. The junctional epithelia from healthy tooth surface were irreguraly round, oval, polygonal and slightly elongated while those from periodontal pocket were so elongated that difficult to distinguished the individual cell boundary. 2. There were a lot of round space so called 'HOLE or WHORL' which seemed tunnel in periodontal pocket with advanced periodontitis. 3. Microvilli were going to destructed and disappeared on surfaces of junctional epithelia in periodontal pocket with advanced periodontitis. 4. There were a lot of Filopodia on Junctional epithelia from healthy surfaces. %. Junctional epithelia from periodontal pocket with advanced periodontitis contained more inflammatory cells than healthy junctional epithelia did.

치근단병소에 관한 방사선학적 연구 (A RADIOGRAPHIC STUDY ON PERIAPICAL LESIONS)

  • The author studied the age distribution, etiology, affected site and several radiographic features of periapical granulomas, cysts, and abscesses. The material consisted of 928 films obtained from the patients who were diagnosed and treated under the diagnosis of periapical granulomas, cysts, and abscesses during the past 8 years (1979-1986) at the Infirmary of Dental School, Chosun University. The obtained results were as follows: 1. The order of incidence was as follows: periapical abscess (67.2%), granuloma, and cyst. 2. The age distribution revealed the highest incidence around the age of 30 and relatively higher incidence over the age of 60 in the case of periapical abscess and granuloma. 3. In the frequency of location: Periapical abscesses occured most frequently in the mandibular molars. Granulomas showed relatively higher incidence in maxilla than in mandible. Cysts were most common in the maxillary anterior teeth. 4. The mean diameter of dental granuloma was 5.9㎜, however, all dental granulomas were less than 9.3㎜ in diameter. The mean diameter of periapical cyst was l3.8㎜. 5. Periapical cyst revealed well circumscribed radiolucent lesions and 77.8% of the lesion showed white line. 86.0% of dental granuloma showed well circumscribed border, 54.5% sclerosis on surrounding bone and 38.5% partial white line. Periapical abscess revealed diffuse radiolucent lesion, 89.6% of the lesions had sclerosis on surrounding bone, and 38.0% sinus tract.

Osteogenic Sarcoma에 관한 연구 (A STUDY ON OSTEOGENIC SARCOMA)

  • The author observed a case of osteogenic sarcoma in a 11-year-old female with complaint of painful swelling on face in right side. The observed results were as follows: 1. Large hematoma was obsered, and patient complanited painfull swelling on c/c site. 2. Predisposing factor of osteogenic sarcoma was not clear, but patient had history of extraction before patient visiting infirmary of our dental collage. 3. Serologic findings were not specific, and serum alkaline level was normal. 4. Radiographic findings were as follows: ① Diffuse faint radiopacity in the lesion ② Bony destruction and increased radiopaciy in right antrum ③ Displacement of multiple teeth on involved area(i. e ;#12, 15. 55, 16, 17, 18), ④, Increased periodontal space in singel tooth(#13) ⑤Destruction of bony crypt on involved teeth(#13, 14, 15, 17, 18) ⑥ Loss of lamina dura of three teeth in involved area(#11, 12, 16) 5. Computed tomographic findings were as follows: ① Large calcific and hetergenous component mass in the Rt. maxillary sinus, and this mass extending to Rt. maxilla, alveolar bone, ethmoid sinus. ②, Soft tissue bulging into Rt. side nasal cavity and oral cavity. ③ Bone destruction of maxillary sinus wall and Rt. alveolar bone.

부정교합 치료기간의 예측에 관한 연구 (A STUDY HO THE PREDICTION OF DURATION OF ORTHODONTIC TREATMENT IN MALOCCLUSION)

  • To predict duration of orthodontic treatment of malocclusion, 304 male and female patients whose orthodontic treatment was finished in department of orthodontics, Infirmary of dental college, Chosun University were studied. The duration of treatment was studied according to types of malocclusion, beginning ages of treatment, Hellman's dental ages, the determination of tooth extraction, types of tooth extraction and types of orthodontic appliance. The following results were obtained. 1. The duration of treatment was 18 months in Class I malocclusion, 20 months in Class II malocclusion and 24 months in Class III malocclusion. 2. The more early treatment was done, the more duration of treatment was needed according to beginning age of treatment and Hellman's dental ages. 3. Treatment of tooth extraction case was needed for 78 days more than that of non-extraction case. 4. The duration of treatment with unilateral extraction of premolars was the shortest (598 days) and that with single arch extraction of premolars was the longest (685 days) according to types of tooth extraction. 5. The duration of treatment by removable appliance was the shortest (237 days) and the combination of removable appliance, headgear and full banded appliance was the longest (1425 days) according to types of orthodontic appliance. 6. The duration of treatment was 18 months in typical orthodontic treatment with 4 extraction of premolar and full banded appliance.

교정치료를 위한 발치빈도에 관한 연구 (A STUDY ON THE FREQUENCY OF TOOTH EXTRACT10N FOR ORTHODONTIC TREATMENT)

  • To study diagnosis and treatment plan of malocclusion, comparison of race and nation and consideration of treatment technique, 885 orthodontic patients of the department of orthodontics, Infirmary of dental college, Chosun University were evaluated. The frequency and pattern of tooth extraction were studied according to malocclusion types and developmental stages of occlusion and sex. Thefollowingresultswereobtained. 1. Average frequency of tooth extraction for orthodontic treatment was $38.3\%$. 2. The frequency of tooth extraction was $43.7\%$ in class I malocclusion, $34.2\%$ in class II malocclusion and $32.1\%$ in class III malocclusion. 3. The drequency of tooth extraction was the highest in permanent dentition $(49.2\%)$. 4. The frequency of tooth extraction was not different between male and female. 5. The frequency of 3-4 premolars extraction was the highest $(57.5\%)$ and that of lower incisor extraction was the lowest $(2.9\%)$ in patterns of tooth extraction. 6. Both frequency of unilateral and single arch extraction were the highest in class III malocclusion.

한국인 소아의 유치치열에서 치간열궤의 깊이에 관한 임상학적 연구 (CLINICAL STUDY OF THE DEPTH OF THE GINGIVAL SULCUS IN THE PRIMARY DENTITION ON KOREAN CHILDREN)

  • Thirty-four children who were patients at the Pedodontic Department of Infirmary of Dental College, S.N.U., aged from 3 years 1 month to 6 years 10 months. One quadrant jaw was measured for each child, Each of the 5 teeth was measured in 6 specific areas, therefore, a total of 1,020 measurements were observed. The results were as follows: 1. Facial surface was the shallowest of all. It was 0.14 mm. shallower than lingual, 0.62mm. shallower than mesial, 0.98mm. shallower than distal. 2. The sulcus generally increased in depth from anterior to posterior. 3. The anterior segment in the maxilla was the shallowest of all. It was 0.68mm. shallower than posterior of maxilla, 0.08 mm. shallower than anterior of mandible, 0.63 mm. shallower than posterior of mandible. 4. The most frequent measurement observed in the both jws was 1.4mm. 5. The mean for the complete anterior segment in the maxilla was 1.3±0.50 mm. In the posterior segment of the maxilla, the mean was 2.0v0.74mm. In the mandible, the mean for the anterior segment was 1.4±0.46 mm. In the posterior segment of the mandible, the mean was 2.0±0.56 mm. 6. The mean gingival sulcus depth for the entire Primary Dentition was 1.7±0.59 mm.

강명길(康命吉)의 생애(生涯)와 의학사상(醫學思想)에 대한 고찰 (A Study on the Life of Kang Myeong-gil and His Medical Theories)

  • Gang Myeong-gil(康命吉) wrote Jejung sinpyeon(濟衆新編), one of three major medical books produced in the latter half of the Joseon Period. Jejung sinpyeon, edited at the behest of King Jeongjo, was intended to overcome the shortcomings of the then best medical book, Donguibogam(東醫寶鑑), and to update medical knowledge in a bid to spread that knowledge widely to the general public. Jejung sinpyeon contains the life and medical theories of the physician Gang Myeong-gil. In order to examine Gang Myeong-gil's life and medical theories, which influenced the edition and characteristics of Jejung sinpyeon, this paper studies his family lineage, medical records and the editorial characteristics of Jejun sinpyeon. Gang Myeong-gil and his ancestors served in government medical offices for some 200 years, and Gang Myeong-gil himself served as a physician at Naeuiwon for thirty years. His descendants also served as physicians upon passing the state-administered examination. Gang Myeong-gil preferred to prescribe drugs, and this inclination led him to include Yakseongga in Jejung sinpyeon, and he took an open and practical attitude towards the editing of the book. This approach is supported by the chapter on experience-based prescriptions in Jejung sinpyeon and the prescriptions of the Royal Infirmary, thus revealing his humanitarian and open thinking on medicine.

근관치료 환자에 대한 임상적 고찰 (A CLINICAL STUDY ON BOOT CANAL TREATED PATIENTS)

  • In order to observe the frequency of visits and the distribution of endodontically treated patients according to age, sex, diagnosis, type of teeth, the author had surveyed the 17,250 outpatients of the Infirmary of the College Dentistry, Yonsei University from January 1976 to December 1978. 773 cases, 660 patients among 728 endodontically treated patients were studied. The obtained results were as follow; 1. The average frequency of visits were 5.95 in uppers and 5.88 in lowers. 2. The average frequency of visits were 5.79 in males and 6.02 in females. 3. At the age of under 14, the average frequency of visits were 7.42 in uppers and 6.76 in lowers. 4. According to diagnoses, the average frequency of visits in periapical granuloma or cyst were 2.80 in uppers and 3.16 in lowers. 5. The number of endodontically treated teeth had revealed as follow; upper incisors 27.94%, lower molars 25.87%, and upper molars 15.13% 6. The number of endodontically treated teeth by diagnoses were as follow; periapical abscess 45.27%, pulpit is 37.51%, tooth fracture 8.40%. 7. The number of endodontically treated teeth were 51.99% in the age of from 20 to 34.

The Net Promoter Score with Friends and Family Test applied to arthroscopic shoulder surgery

  • Background: The Friends and Family Test (FFT) developed by the UK National Health Service evaluates whether patients are satisfied with a service provided, where improvements are needed, and how likely patients are to recommend the intervention. Calculated from the FFT, the Net Promoter Score (NPS) creates a recommendation metric for treatment. The primary aim of this prospective study is to evaluate NPS for arthroscopic subacromial decompression (ASD) and rotator cuff repair (RCR). Secondary aims are to postoperatively evaluate 1-year changes in patients' Oxford Shoulder Scores (OSSs) in terms of the proportion of patients satisfied with their surgery and correlation with FFT. Methods: During a 2-year period, all patients undergoing ASD or RCR completed questionnaires prospectively. Collected preoperatively and postoperatively at 1 year. Results: NPSs were 31 for ASD (n=32) and 52 for RCR (n=39). OSSs increased by 4.3 and 6.9 for ASD and RCR, respectively (P<0.001). Overall, 75% of ASD and 77% of RCR patients were either "satisfied" or "very satisfied," respectively, with procedure outcomes. Scores from FFT had a positive correlation with improvement in OSS and satisfaction scores among patients undergoing arthroscopic shoulder surgeries (P<0.001). Conclusions: The current study shows positive NPS outcomes in patients with ASD and RCR. Scores from FFT correlate well with both satisfaction and OSS among patients. NPS can be an adjunct to traditional patient-reported outcome measures to provide global evaluation of patient experiences to aid in determining the clinical value of common procedures in shoulder orthopaedics.
