Usefulness of Inferior Turbinate Bone-Periosteal-Mucosal Composite Free Graft for Cerebrospinal Fluid Leakage (하비갑개 골-골막-점막 복합이식을 이용한 뇌척수액 유출 복원술)

  • Background and Objectives: Endoscopic repair of cerebrospinal fluid (CSF) leak can avoid morbidity of open approaches and has shown a favorable success rate. Free mucosal graft is a good method, and multi-layered repair is more favorable. The inferior turbinate has been commonly utilized for the free mucosal graft, but we newly designed it as a bone-periosteal-mucosal composite graft for multilayered reconstruction. Subjects and Method: Four subjects with a skull base defect were treated with this method. The inferior turbinate was partially resected including the conchal bone and was trimmed according to defect size. Both bony parts and periosteum were preserved on the basolateral side of the mucosa as a composite graft. The graft was applied to the defect site using an overlay technique. Results: All cases were successfully repaired without any complications. Three of them had a defect size greater than 10-12 mm, and the graft stably repaired the CSF leakage. Conclusion: Endoscopic repair of CSF leakage using inferior turbinate composite graft is a simple and easy method and would be favorable for defect sizes greater than 10 mm.

VASCULAR EFFECTS OF THE INFERIOR ALVEOLAR NERVE INJECTION OF 2% LIDOCAINE HCI WITH 1:100,000 EPINEPHRINE (백서에서 하치조신경 전달마취후 설배면 혈관반응에 관한 실험연구)

  • This article discusses the vascular change of dorsal surface of tongue following inferior alveolar nerve block of 2% lidocaine HCl with 1:100,000 epinephrine. Twenty-two rats were randomly allocated to three groups: control group; without mandibular nerve block, experimental group 1; 2% lidocaine HCl, experimental group 2; 2% lidocaine HCl plus 1:100,000 epinephrine. The assessments were performed at 1-, 5-, 30-, 60-, 120-minute after inferior alveolar nerve block. Vessel diameters using the micrometer were measured and compared. Results showed that the increase in vascular constriction in group 3 was greater than those in groups 1 and 2. In conclusion, local anesthesia combined with epinephrine assures safer anesthesia for hemostasis.

  • The widespread use of diagnostic radiography, especially using magnetic resonance imaging, has helped to increase the diagnosis of paralabral cysts in patients with chronic shoulder pain. These paralabral cysts are frequent in the anterior, the superior, and the posterior compartment of the shoulder joint but are rare in the inferior compartment. Paralabral cysts in the shoulder appear particularly in men in their third and fourth decades but rarely in elderly patients. We report a case of an inferior paralabral cyst in an elderly patient whom we treated through arthroscopic decompression.

  • Damage to the inferior alveolar nerve(IAN) is a relatively infrequent complication in endodontic treatment. However, endodontic overfilling involving the mandibular canal may cause an injury of the inferior alveolar nerve resulting in sensory disturbances such as pain, dysesthesia, paresthesia or anesthesia. Two mechanism(chemical neurotoxicity and mechanical compression) are responsible for the IAN injury. When absorbent materials overfilled, it can be treated as a non-surgical procedure. But early surgical intervention required when mechanical, chemical nerve damage expected. We report surgical removal of overfilled gutta-percha and IAN decompression through sagittal split osteotomy in case of dysesthesia after overfilling of endodontic material into the mandibular canal. Dysesthesia recovered 3 months after surgical treatment.

A Case of Budd-Chiari Syndrome Associated with Alveolar Echinococcosis

  • Although alveolar echinococcosis (AE) can cause a serious disease with high mortality and morbidity similar to malign neoplasms. A 62-year-old woman admitted to a hospital located in Sivas, Turkey, with the complaints of fatigue and right upper abdominal pain. On contrast abdominal CT, a $54{\times}70{\times}45$ mm sized cystic lesion was detected in the left lobe of the liver that was seen to extend to the posterior mediastinum and invade the diaphragm, esophagus, and pericardium. The cystic lesion was seen to be occluding the inferior vena cava and left hepatic vein at the level where the hepatic veins poured into the inferior vena cava. Bilateral pleural effusion was also detected. We discussed this secondary Budd-Chiari Syndrome (BCS) case, resulting from the AE occlusion of the left hepatic vein and inferior vena cava, in light of the information in literature.

  • The look up table method conventionally applied to control the inner temperature and humidity of a laver drying machine has repeatedly occurred not only laver's damage but also inferior goods since the reaching time at the optimum state takes a long time. In this paper, a fuzzy control theory instead of the look up table was proposed to reduce the reaching time at the optimum state. The proposed method used six input variables and four output variables for the fuzzy control, and a triangle rule for a fuzzifier, The Mandani's min-max method was applied to a fuzzy inference. Also, the mean method of maximum was applied to a defuzzifier. The method applied to the fuzzy controller contributed to reduce the reaching time at the optimum state, and to minimize not only laver's damage but also inferior goods.

Surgical Correction of Intrahepatic Inferior Vena Cava Obstruction (간내 하공정맥 폐색증의 수술요법)

  • Seven patients [six women and one man] with obstruction of hepatic portion of inferior vena cava was operated on, from May, 1969 to January, 1985. Of seven patients, six were undergone corrective operation for IVC obstruction and another one was not operated because of far advanced liver cirrhosis. The occlusions were found at or close to the level of diaphragm and they were membranous or diffuse with or without thrombi. Most of their symptoms were referable to either inferior vena caval or hepatic venous obstruction and onset of the symptoms was usually gradual, beginning between the age of their thirties and forties. Most of the patients showed marked elevation of peripheral venous pressure of lower extremity [29-40 cm H2O] preoperatively, which decreased significantly after corrective operation [17-30 cm H2O]. Venous catheterization for pressure study and venography were essential for confirming the diagnosis. Of six cases, in which corrective operations were done, Transatrial membranotomy with or without IVC dilatation were performed in five cases [case 1, 2, 3, 5, 6], using cardiopulmonary bypass and in another one case, bypass operation between IVC, distal to obstruction, and RA was done using Dacron tube graft under the thoracoabdominal incision. All survived and their conditions were improved.

Isolated Dissecting Posterior Inferior Cerebellar Artery Aneurysm

  • Dissecting aneurysms frequently involve the vertebral arteries and their branches, but those involving the posterior inferior cerebellar artery [PICA] and not vertebral artery at all are extremely rare. We present a case of an isolated dissecting aneurysm of the PICA without involvement of vertebral artery. A 54-year-old man presented with dizziness and headache. MR imaging of the brain showed a cerebellar infarction of the left PICA territory. MR angiographic and cerebral angiographic studies revealed a dissecting fusiform aneurysm involving the left proximal PICA. Subsequently, the patient underwent GDC embolization. A postembolization angiogram demonstrated complete obliteration of the aneurysm. In this report, the treatment modalities for this rare condition is described with review of the literature.

  • A 34-year-old female was seen with Budd-Chiari syndrome caused by an obstruction of the inferior vena cava above the hepatic veins. Transatrial dilation or membranotomy was not available due to severe hourglass constriction of the inferior vena cava. Retrohepatic cavoatrial bypass with a low porosity woven Dacron graft[24mm in the diameter] was performed. For increasing the patency of venous graft, we used anticoagulation with warfarin. Postoperative course was uneventful except pleural effusion of the right luring.

In Situ Intersegmental Anastomosis within a Single Artery for Treatment of an Aneurysm at the Posterior Inferior Cerebellar Artery : Closing Omega Bypass

  • A 74-year-old patient was diagnosed with a subarachnoid hemorrhage suspected from a dissecting aneurysm located at the lateral medullary segment of the posterior inferior cerebellar artery (PICA). Because perforators to the medulla arose both proximal and distal to the dissecting segment, revascularization for distal flow was essential. However, several previously reported methods for anastomosis, such as an occipital artery-PICA bypass or resection with PICA end-to-end anastomosis could not be used. Ultimately, we performed an in situ side-to-side anastomosis of the proximal loop of the PICA with distal caudal loops within a single artery, as a "closing omega," followed by trapping of the dissected segment. The aneurysm was obliterated successfully, with intact patency of the revascularized PICA.
