운영 효율성을 고려한 감염병 전문병원의 일반병동 건축계획에 관한 연구 (A study on the Planning of a general ward in infectious diseases hospital considering the efficiency of hospital operation)

  • Purpose: As the need for a hospital specializing in infectious diseases has increased, construction is being promoted. Hospitals specializing in infectious diseases receive some state subsidies, but in the case of private hospitals, hospital operation efficiency should be considered to prevent cost loss. Therefore, we aim to derive a building plan for a general ward in a hospital specializing in infectious diseases that can be used not only in normal times but also in times of crisis. Methods: In this study, relevant literature review and field interviews were conducted with medical staff working in facilities designated as infectious disease hospitals. Results: The general ward building plan of the hospital specializing in infectious diseases was classified into three categories and presented. 'Spatial composition' for nursing unit and ward zoning, 'Spatial plan' for ward space conversion in normal times and crises, 'Bedroom plan' for effective dimensions and area of the ward. Implications: It can be used as a guideline when designing an infection-facility ward. And it can be a basis for inducing improvements to prevent infection in the ward of existing medical facilities.

중환자실 입원 환자의 설사 발생과 영향요인: 후향적 조사 (Factors Influencing the Occurrence of Diarrhea in Patients Admitted to Intensive Care Units)

  • Purpose: This study was done to examine incidence of diarrhea and explore factors influencing occurrence of diarrhea in patients admitted to intensive care units (ICU). Methods: For this retrospective research, data based on inclusion criteria were collected from the electronic medical records for 142 patients admitted to a university hospital ICU from September 2014 to August 2015. Statistical analysis was conducted using SPSS/WIN 22.0 program. Results: Incidence of diarrhea was 53.5% during the 12-month study period. Diarrhea occurred at 4.54 days and continued for 1.79 days on average. Average total frequency of diarrhea was 5.56 times. Increased ICU stay, enteral nutrition, and infection state were significant predictors of the occurrence of diarrhea. Infection increased risk of diarrhea 3.4 times and enteral nutrition increased risk of diarrhea to 2.2 times greater than patients not receivng enteral nutrition. Conclusion: Diarrhea in ICU patients is associated with multiple factors that should be considered to implement preventive strategies. Infection control should be emphasized, and close monitoring of diarrhea should be provided for those with enteral nutrition. Further studies are warranted to determine standardized clinical definition of diarrhea and diarrhea risk factors in ICU patients with different levels of severity and comorbidity.

  • The neurotropic psudorabies virus(PRV) to replicate within neurons is very useful pathogen for neuronal tracing. I carried out this study to investigate the parametric analysis on the viral infection in the rat circadian control center with two genetically engineered strains out of PRV. The two strains are isogenic with the attenuated Bartha strain of PRV ; in one strain a lacZ reporter gene was inserted into the gC locus (PRV-BaBlu ; $4.75\times10^8pfu/ml$) and the other strain contained a PRV envelope glycoprotein gene(PRV-D ; $2.5\times10^8pfu/ml$) theat is absent in PRV-BaBlu. simultaneous or temporally separated sequential injection of$2{\mu}l$ of each strain into the vetreous body of eye produced a course of transsynaptic infection of retinohypothalamic circuitry. The results were as follows; 1. PRV-BaBlu and PRV-D infected the suprachiasmatic nucleus in hypothalamus and intergeniculate leaflet in lateral geniculate nucleus of thalamus. 2. The rate of PRV infection was dependent upon PRV strain. 3. Pre-infected neurons by PRV-D were interfered with the replication of PRV-BaBlu. 4. Dual injection of PRV-D and PRV-BaBlu showed more virulent than the parental strain.

가잠의 품종별 세포질다각체 바이러스에 대한 저항성 및 이형 바이러스 첨식에 의한 유발에 관한 연구 (Interstrain Difference in the Resistance of the Silkworm, Bombyx mori,. to Peroral Infection and Induction with the Two Strains of Cytoplasmic Polyhedrosis Virus)

  • 세포질다각체 바이러스에 대한 가잠의 품종별 저항성과 이형 세포질다각체 바이러스 첨식에 의한 유발에 관한 시험을 한 결과는 다음과 같다. 1. 품종간의 저항성 1) 일본종 및 중국종 사이에 뚜렷한 차이가 없었다. 2) 장잠기(4령)가 추잠기(2령) 보다 저항성이 강했다. 3) 육각형 세포질다각체 바이러스가 사각형 세포질다각체 바이러스보다 병원성이 높았다. 4) 육각형 세포질다각체 바이러스의 LC$_{50}$의 평균치는 원종에서는 2령기잠 처리에서 6.12$\times$$10^{6}$/$m\ell$, 4령기잠 처리에서 1.57$\times$$10^{7}$ /$m\ell$ 이었고 교잡종에서는 2령기잠 처리에서 1.28$\times$$10^{6}$/$m\ell$, 4령기잠 처리에서 4.99$\times$$10^{6}$/$m\ell$이었다. 5) 사각형 세포질다각체 바이러스의 LC$_{50}$의 평균치는 원종에서는 2령기잠 처리에서 2.06$\times$$10^{7}$ /$m\ell$, 4령기나 처리에서 5.67$\times$$10^{7}$ /$m\ell$ 이었고 교잡종에서는 2령기잠 처리에서 9.84$\times$$10^{6}$/$m\ell$, 사령기잠 처리에서 3.86$\times$$10^{7}$ /$m\ell$이었다. 2. 이형 세포질다각체 바이러스 첨식에 의한 유발사각형 세포질다각체 바이러스 첨식에 의한 육각형 세포질다각체 바이러스의 유발은 1) 영기별로 볼 때 2령기잠 처리에서가 4영기잠 처리에서보다 월등히 높은 유발률을 보였다. 2) 유형별로 볼 때 대부분이 육각형과 사각형의 혼합감염 형태로 나타났다. 3) 농도별로 볼 때 대체로 1.3$\times$$10^{6}$/$m\ell$ 농도를 중심한 중간농도에서 높은 유발률을 나타냈다.타냈다.

  • 1980년 9월 2일부터 1981년 8월 2일까지 금강 하구역인 충남 서천군 서면 솔리산 바지락의 생식소를 조사한 바. Gymnophallidae에 속하는 신종의 cercaria를 발견하여 Cercaria tapes n. sp.로 기재하였다. sporocyst의 크기는 $380-650{\times}130-160{\mu}m$이고, 전단에 분출구와 인두가 있으며, 안쪽에는 30여개의 cercara 및 배세포가 들어 있다. cercaria는 긴 타원형으로 각부의 크기는 충체 $160-187{\times}62-77{\mu}m$, 꼬리 $47-52{\times}17{\mu}m$, 갈라진 꼬리부분 $67-72{\times}12-15{\mu}m$이다. 배설낭은 Y형이고, flame cell식 은 2[(1)+(1)]=4이다. 바지락 2,086개중 42개가 기생되어 연평균 $2.01\%$의 기생률을 나타냈고, 기생률이 가장 높은 시기는 3월의 $5.74\%$, 가장 낮은 시기는 1월의 $0.4\%$이었고, 5,6월에는 기생패가 검출되지 않았다. 각장 2.1-3.0 cm가 $0.38\%$, 3.1-4.0cm가 $1.53\%$, 4.1-5.0cm가 $0.09\%$의 기생률을 나타냈고 2.0cm이하의 치패에는 기생되지 않았다.

$textsc{k}$-Casein, GMP, Sialic Acid가 한우송아지 Rotavirus와 제주도 Bovine Rotavirus(JBR)의 MA-104 세포감염에 미치는 영향 (Effect of $textsc{k}$-Casein, GMP and Sialic Acid on the Infection of MA-104 Cells by Korean Native Cattle Rotavirus and JBR)

  • 로타바이러스는 유아와 어린 포유동물에 있어서 위장염을 일으키는 원인체로서 다양한 혈청형에 의해 발병되기 때문에 효과적인 백신 개발이 되지 않고 있다. 본 연구는 국내 분리 한우송아지 로타바이러스 S97과 JBR(Jeju island bovine rotavirus)에 $textsc{k}$-casein, GMf, sialic acid를 첨가하여 MA-104세포에 감염시켰을 때 그 억제 효과를 규명하기 위해 시행하였다. 한우송아지 로타바이러스 597과 JBR을 무혈청 Ml99배지의 MA-104세포에 감염시켜 37$^{\circ}C$에서 6일간 회전 배양하여 활성화시킨 다음, 로타바이러스 역가를 분석하였고, MA-104 세포에 활성화된 BRV를 감염시키고, $textsc{k}$-casein, GMP sialic acid를 각각 농도에 따라 첨가하여 15시간 배양한 다음 AEC 염색법으로 염색시켜 현미경 상에서 감염된 세포수를 계산하였다. 한우송아지 로타바이러스 S97과 GMR의 역가는 각각 2.5$\times$107과 2.0$\times$106 PFU/ml이었다. $textsc{k}$-casein, GMP의 농도 20001M에서 S97의 세포감염율은 97.4%와 97.44%로 나타났고, $textsc{k}$-casein, GWP의 농도 2000$\mu$M에서 JBR의 세포감염 억제율은 99.52%와 99.78%로 나타났다. Sialic acid의 농도 2000$\mu$M에서의 S97과 JBR의 세포감염 억제율은 3.85%와 3.63%로 나타났다. $textsc{k}$-casein, CU는 로타바이러스 S97과 JBR에 대해 농도 2000UM에서 97%이상의 억제효과를 나타냈으며, sialic acid는 억제효과가 거의 없었다. K-casein, GMP는 송아지뿐만 아니라 유아의 로타바이러스에 의한 설사를 억제할 수 있을 것으로 기대된다.

Interaction of Antibiotic with PAN and Cationic-Dyeable PET Fibers in Development of Infection Resistant Biomedical Materials

  • Interaction of a representative antibiotic, doxycycline(Doxy), with commercial poly(acrylonitrile) (PAN) and cationic-dyeable poly(ethylene terephthalate)(PET) fiber was studied in development of infection resistant biomedical materials. Regular PET was also employed for a comparison purpose. Their interactions were investigated at different treatment temperatures, times, and pHs. Fibers were also hydrolyzed by 1% NaOH for 1 or 2 hours at $85^{\circ}C\;and\;100{\circ}C$ to study effect of hydrolysis on antibiotic sorption. Infection-resistant characteristics of the substrates were evaluated by zone of inhibition (ZOI) test. Results revealed that a significant chemical change occurred in PAN and cationic-dyeable PET due to hydrolysis. Additional functional groups obtained by hydrolysis not only enhanced sorption of the antibiotics but also provided greater ZOI values, indicating substantial improvement in sustained infection resistance properties.

Enterobius vermicularis egg positive rates in primary school children in Gangwon-do (Province), Korea

  • A survey of the infection rate of Enterobius vermicularis among students in 4 primary schools located in Gangwon-do (Province) was done from May to .Tune 2001. Among the 398 examinees,39 (9.8%) were infected with E. vermicularis demonstrated by the adhesive cellotape anal swab method. The infection rates ranged from 8.3% to 11.8% among the four schools. The infection rate of males and females was 10.7% and 7.7% respectively. The first grade students showed the highest infection rate, 28.7%. The confirmed cases were treated with albendazole three times at an interval of 15 days. We were able to confirm that E. vermicularis infection is still prevalent among students in Gangwon-do , Korea.

  • 현장려잠품종에 대하여 FV 및 이나주바이러스의 2령기잠 경구접종에 의한 품종간저항성을 비교검정한 결과는 다음과 같다. 1. FV 에 대한 품종별 저항성은 한생 1 호 $\times$ 한생 2 호가 가장 강하였고 무등$\times$선호가 가장 약했다. 2. 이나주 바이러스에 대한 저항성은 잠 117$\times$잠118, 경추$\times$소일, 무등$\times$선호, 한생 1호$\times$한생 2호 및 한생 3호$\times$4호 등 5개 품종은 완전저항성을 나타냈으며 잠 115$\times$잠116이 가장 약했다.

바베시아 원충감염 예방과 치료에 대한 비타민 E의 응용 (Effects of vitamin E on prevention and treatment of canine babesiosis)

  • This study was carried out to investigate the effects of vitamin E on the prevention and treatment of Babesia gibsoni. Fifteen mongrel dogs, uninfected with Babesia spp, were assigned to three groups according to vitamin E(${\alpha}-tocopherol$) concentrations in the RBC. The concentrations in each of the three groups were, respectively : ${\alpha}-tocopherol$ in RBC less than $30{\mu}g/{\mu}l$(Group I), $30{\mu}g/{\mu}l{\sim}60{\mu}g/{\mu}l$(Group II), more than $60{\mu}g/{\mu}l$(Group III). Artificial infection was accomplished by injecting $2{\times}10^7{\sim}2{\times}10^8$ erythrocyte of Babesia gibsoni-infected dog into the cephalic vein. We investigated the clinical signs, vitamin E concentrations in RBC and serum, Vitamin A concentrations in serum, hematological values, white blood cell(WBC) viability and RBC membrane osmotic fragility after infection of Babesia gibsoni for a period of 20 days at 5 day intervals. The results obtained are summarized as follows : 1. After infection by Babesia gibsoni, clinical examination revealed depression, anorexia, pale mucous membranes, dark brown urine and diarrhea in proportion as time went on. After 10 days of infection, one dog each of Groups I, II and III revealed depression and anorexia. Two dogs in Group I and one dog each of Groups II and III showed dark brown urine after 15 days. Diarrhea was observed in one dog in each of the 3 groups after 20 days of infection. 2. After 5 days of infection, two dogs in each of Groups I, II and III showed Babesia gibsoni in RBC of blood smear stained with Giemsa. At the 15th day after infection with Babesia gibsoni, they were observed in all experimental animals. After both 5 days and 10 days of infection, the rate of Babesia gibsoni parasitized RBC(permillage, ‰) was 1‰, and increased as time went on. 3. After 5 days of infection by Babesia gibsoni, Group I, which had the lowest vitamin E concentration, showed significantly decreased RBC and PCV levels(p < 0.01). Group II and group III also showed significantly decreased RBC and PCV levels after 15 days of infection(p < 0.05). Particularly after 10 days of infection, Group I showed lower values in RBC and PCV levels compared to Groups II and III. WBC, RBC, fibrinogen and total protein levels between the groups did not differ during experimental periods. 4. According to the WBC differential counts, the ratios of neutrophil to lymphocyte showed a tendency to be slightly higher in Group III (more than $60{\mu}g/{\mu}l$) than in Groups I and II. 5. WBC viability did not differ between the groups. 6. RBC membrane osmotic fragility did not differ between the groups.
