Purpose: To investigate the frequency, presentation, management, and outcome of cytomegalovirus (CMV) infection in pediatric patients who underwent renal transplantation. Methods: We performed a retrospective chart review of 70 patients under the age of 18, who underwent renal transplantation between January 1990 and November 2014. A diagnosis of CMV infection was based on serology, molecular assays, antigenemia assays, and culture. CMV infection was defined as detection of virus and CMV disease was diagnosed when clinical signs and symptoms were present. Results: The number of patients with CMV infection was 18 (25.7% of renal transplant recipients). Twelve were male (66.7%), and the $mean{\pm}standard$ deviation (SD) age at infection was $13.3{\pm}3.9$ years. Median time of infection after renal transplantation was 4 months (range 1.0-31.0 months). Pretransplantation CMV status in the infected group was as follows: donor (D)+/recipient (R)+, 11 (61.1%); D+/R-, 7 (38.9%); D-/R+, 0; and D-/R- 0. Nine patients had CMV disease with fever, leukopenia, thrombocytopenia, or organ involvement such as enteritis, hepatitis, and pneumonitis. The age of disease occurrence was $13.1{\pm}3.9$ years and the median time to disease onset after renal transplantation was 8 months (range 1.0-31.0). Immunosuppressive agents were reduced or discontinued in 14 patients (77.8%), antiviral agents were used in 11 patients (61.1%), and all patients with CMV infection were controlled. Conclusions: A quarter of the patients had CMV infection about 4 months after renal transplantation. CMV infection was successfully treated with reduction of immunosuppressants or with antiviral agents.
We report a pediatric patient admitted with abdominal pain, diffuse lower extremity edema and watery diarrhea for two months. Laboratory findings including complete blood count, serum albumin, lipid and immunoglobulin levels were compatible with protein losing enteropathy. Colonoscopic examination revealed diffuse ulcers with smooth raised edge (like "punched out holes") in the colon and terminal ileum. Histopathological examination showed active colitis, ulcerations and inclusion bodies. Immunostaining for cytomegalovirus was positive. Despite supportive management, antiviral therapy, the clinical condition of the patient worsened and developed disseminated cytomegalovirus infection and the patient died. Protein losing enteropathy and disseminated cytomegalovirus infection a presenting of feature in steroid-naive patient with inflammatory bowel disease is very rare. Hypogammaglobulinemia associated with protein losing enteropathy in Crohn's disease may predispose the cytomegalovirus infection in previously healthy children.
This study was carried out to investigate the prevalence of pocine circovirus type 2(PCV2) infection and post-weaning multi systemic wasting syndrome(PMWS) of swine in Gyeongbuk province from June to December in 2003. Gyeongbuk Veterinary Service Laboratory had tested PCV1 and PCV2 by the PCR method against 210 infected pigs of 105 farms, which were requested diagnosis of disease and investigate compositive infections with bacterial disease. The following results were obtained; 1. Among 210 pigs, The pigs infected with PCV1 were 134(63.8%) heads and with PCV2 were 143(68.1%) heads. 2. Using PCR, the rate of PCV1 infection of 105 farms which were requested of diagnosis of disease is positive in 75(71.4%) farms and 78(74.3%) farms were positive in PCV2, and compositive infections of PCV1 and PCV2 were 75(71.4%) farms. 3. According to age of requested pig, the frequency of PCV infection rate was following, the pig under 4 weeks of age were 28.6% in PCV1 and PCV2, respectively, and the pig of 4-7 weeks of age were 58.6% in PCV1 and PCV2, respectively. The pig of 8-11 weeks of age were 73.1% in PCVl and 74.6% in PCV2, the pig of 12-18 weeks of age were 80.9% in PCV1 and 88.1% in PCV2. 4. In the distribution of compositive infection with bacterial disease related to PCV2 among requested samples, PCV2 infection was 39.7% in 78 cases, two kinds of compositive infection was 38.5%, three kinds of compositive infection was 18.1% and four kinds of complex infection was 3.9%.

Ebola virus disease is a lethal viral hemorrhagic fever that has been boiling in sub-Saharan Africa since 1970s. Last year, The Ebola virus epidemic that has spread not only mainly in West Africa, but also in locals such as USA, Europe and the Antipodes via infected travelers, was brought up. Human-to-human transmission of Ebola virus disease is known only through direct contact with the blood, secretions, tissues or other bodily fluids, including saliva. Although there has not been reported infection cases in the dental healthcare settings, the fact that the infection of the Ebola virus may be made from human secretions such as saliva suggests that there is a high risk of infection for the Ebola virus of dental healthcare workers. Therefore, it is important dental healthcare workers to identify infection-suspected patients through the oral findings for infection prevention. This article will review the oral signs and symptoms of Ebola virus disease and discuss the pathogenesis, treatment and prevention. Furthermore, Infection control guidelines for oral healthcare workers are also proposed.
Purpose: Clostridium difficile colonization and infection are commonly associated with poor outcomes in patients with pediatric inflammatory bowel disease (PIBD). We aimed to investigate the prevalence of C. difficile colonization and infection at the time of diagnosis and to evaluate risk factors associated with the development of C. difficile infection during the course of PIBD treatment. Methods: We retrospectively enrolled a total of 59 children who were newly diagnosed with PIBD at the tertiary medical center. All patients underwent C. difficile toxin assays and cultures initially and at every follow-up during the disease course. Kaplan-Meier survival analysis and Cox regression test were used for statistical analysis. Results: Initial cultures for C. difficile were positive in 13 (22.0%) of 59 PIBD patients, whereas initial toxin assays were positive in 3 patients (5.1%). During treatment, C. difficile cultures converted to positive in 28 (47.5%) in addition to 13 patients who were initially culture-positive, and C. difficile toxins converted to positive in 13 (22.0%) in addition to 3 originally toxin-positive patients. Antibiotic usage alone was significantly associated with the development of C. difficile colonization (p=0.011), and the length of hospitalization was associated with the development of C. difficile infection (p=0.032). Conclusion: C. difficile colonization and infection occur frequently during the disease course of PIBD. Antibiotic usage and longer hospital stay were significant risks factors for the conversion of C. difficile status in PIBD patients undergoing treatment.
Porcine pleuropneumonia is an infectious disease caused by Actinobacillus pleuropneumoniae. The identification of A. pleuropneumoniae genes, specially expressed in vivo, is a useful tool to reveal the mechanism of infection. IVIAT was used in this work to identify antigens expressed in vivo during A. pleuropneumoniae infection, using sera from individuals with chronic porcine pleuropneumonia. Sequencing of DNA inserts from positive clones showed 11 open reading frames with high homology to A. pleuropneumoniae genes. Based on sequence analysis, proteins encoded by these genes were involved in metabolism, replication, transcription regulation, and signal transduction. Moreover, three function-unknown proteins were also indentified in this work. Expression analysis using quantitative real-time PCR showed that most of the genes tested were up-regulated in vivo relative to their expression levels in vitro. IVI (in vivo-induced) genes that were amplified by PCR in different A. pleuropneumoniae strains showed that these genes could be detected in almost all of the strains. It is demonstrated that the identified IVI antigen may have important roles in the infection of A. pleuropneumoniae.
Most of physical therapists has thought be exposed themselves to risk of hospital infection but it is reported that have been low concern about infection management. The control situation of infection waste articles, only 53% responded that the controller has managed very intensive so we can feel to need more intensive It's reported that air culture investigation of physical therapy room has never initiated. To wash the hand, before, after treatment of the patients of physical therapist, is very low frequency. And 73% have responded that the time to wash the hand stays 15-45 second. It is examined that 70% physical therapy room is equipped with washing system, a response of 58% disinfects physical therapy room 1-2times per one month. 36% responded disinfection of treatment modality have done everyday, 25% responded have never done. The location physical therapy room is above one floor - 65%. A response of 57% is ventilation system sufficiency, it is considered that physical therapists needs more efforts on management of hospital infection.

Purpose: Emergency room(ER) is the first place to enter a hospital where patients who might have been infected with contagious disease. Therefore, ER should be designed with infection control in mind. Researchers examined hospital ER layouts to identify layout design that support infection control. Methods: This study analyzed the hospital ER layout of Korean and other hospitals abroad. Researchers focused on route of incoming patients who potentially have infectious disease. Crossing of this route with other routes such as for imaging and testing should be avoided for infection control. Results: There were certain hospital ERs with better control of infection related incidents. ER floor plan layout is analyzed about allocation of key functions with movement routes for each role such as patients and medical staff in mind. To identify layout strategies for ER functions researchers simplified the routes in ER into diagrams. Layout options show that bypassing infection suspected routes over other routes is possible. Implications: Hospitals can control infection easier when they adopt strategic ER layout identified in this study.

Leishmania tropica is one of the causative agents of leishmaniasis in humans. Routes of infection have been reported to be an important variable for some species of Leishmania parasites. The role of this variable is not clear for L. tropica infection. The aim of this study was to explore the effects of route of L. tropica infection on the disease outcome and immunologic parameters in BALB/c mice. Two routes were used; subcutaneous in the footpad and intradermal in the ear. Mice were challenged by Leishmani major, after establishment of the L. tropica infection, to evaluate the level of protective immunity. Immune responses were assayed at week 1 and week 4 after challenge. The subcutaneous route in the footpad in comparison to the intradermal route in the ear induced significantly more protective immunity against L. major challenge, including higher delayed-type hypersensitivity responses, more rapid lesion resolution, lower parasite loads, and lower levels of IL-10. Our data showed that the route of infection in BALB/c model of L. tropica infection is an important variable and should be considered in developing an appropriate experimental model for L. tropica infections.
Objective : Infectious spinal disease is regarded as an infection by a specific organism that affects the vertebral body, intervertebral disc and adjacent perivertebral soft tissue. Its incidence seems to be increasing as a result of larger proportion of the older patients with chronic debilitating disease, the rise of intravenous drug abuser, and the increase in spinal procedure and surgery. In Korea, studies assessing infectious spinal disease are rare and have not been addressed in recent times. The objectives of this study are to describe the epidemiology of all kind of spinal infectious disease and their clinical and microbiological characteristics as well as to assess the diagnostic methodology and the parameters related to the outcomes. Methods : A retrospective study was performed in all infectious spinal disease cases presenting from January 2005 to April 2010 to three tertiary teaching hospitals within a city of 1.5 million in Korea. Patient demographics, risk factors, clinical features, and outcomes were assessed. Risk factors entailed the presence of diabetes, chronic renal failure, liver cirrhosis, immunosuppressants, remote infection, underlying malignancy and previous spinal surgery or procedure. We comparatively analyzed the results between the groups of pyogenic and tuberculous spinal infection. SPSS version 14 statistical software was used to perform the analyses of the data. The threshold for statistical significance was established at p<0.05. Results : Ninety-two cases fulfilled the inclusion criteria and were reviewed. Overall, patients of tuberculous spinal infection (TSI) and pyogenic spinal infection (PSI) entailed 20 (21.7%) and 72 (78.3%) cases, respectively. A previous spinal surgery or procedure was the most commonly noted risk factor (39.1%), followed by diabetes (15.2%). The occurrence of both pyogenic and tuberculous spondylitis was predominant in the lumbar spine. Discs are more easily invaded in PSI. At initial presentation, white cell blood count and C-reactive protein levels were higher in PSI compared to TSI (p<0.05). Etiological agents were identified in 53.3%, and the most effective method for identification of etiological agents was tissue culture (50.0%). Staphyococcus aureus was the most commonly isolated infective agent associated with pyogenic spondylitis, followed by E. coli. Surgical treatment was performed in 31.5% of pyogenic spondylitis and in 35.0% of tuberculous spondylitis cases. Conclusion : Many previous studies in Korea usually reported that tuberculous spondylitis is the predominant infection. However, in our study, the number of pyogenic infection was 3 times greater than that of tuberculous spinal disease. Etiological agents were identified in a half of all infectious spinal disease. For better outcomes, we should try to identify the causative microorganism before antibiotic therapy and make every effort to improve the result of culture and biopsy.
