This paper on an experiment, using System Dynamics, on the affect of increase in number of beds and medical instruments used for the care of premature infants, which constitute the physical requirements in quality of medical services, on changes in the survival rate of premature in ants that leads to demographic changes of Newborn infants. The model has four sectors: take-in capacity, survival rate of premature infants, demographics without newborn infants and demographics with newborn infants. The model simulates the changes in demographics of the newborn infants from 2002 to 2022. The study results show that the survival rate of premature infants can be increased by improving the physical aspects in the quality of medical services. An average of 1,900 premature infants can survive as a result of the physical quality improvements in medical services, adding up to an increase of 37,300 newborn infants by the year 2022.
The Purpose of this study was to observe the body temperature changes of newborn infants in general crib and electric heat crib after birth for the period required to reach the optimum body temperature. Forty-seven newborn infants who were delivered at Seoul National University Hospital during the period from June 12 to September 13, 1973 were chosen as Subjects for this study. The criteria for the choice of subjects were the babies with normal spontaneous delivery; body weight 2.5kg and over at birth; Apgar so ore seven and over and gestation period over thirty-six weeks. Of these subjects, by random sampling thirty-one newborn infants were placed in the general crib and sixteen in the electric-heat crib. The rectal body temperature of these newborn infants were taken and recorded at fifteen-minute interval for the first one hour period after birth, at thirty minute interval for the next two hours and at one hour interval for the remaining period up to eight hours. The results of the study were as follows: 1. The mean body temperature of the newborn infants on admission to nursery ranged from 98.7℉. to 99℉. irrespective of the body weight and the room temperature. 2. There was a significant difference in the body temperature changes of the newborn infants as a total between the general crib and the electric-heat crib from three to eight hours after birth. It was found that the body temperature of the newborn infants in the electric-heat crib was significantly higher than that of the newborn infants in the general crib. 3. In comparison with the body temperature changes of the newborn infants in the general crib, the newborn infants in the electric- heat crib exhibited significantly higher body temperatures in all three body weight groups; from four to eight hours after birth in the 2.5-2.9kg body weight group; from three to seven hours after birth in the 3.0-3.4kg body weight group; from two and half to six hours after birth in the group with body weight over 3.5kg. 4. Time required to reach 98℉. of body temperature was four hours in the 3.5-2.9kg body weight group, three hours in the 3.0-3.4kg. body weight group and two and half hours in the group with body weight over 3.5kg in the electric- heat crib. In the general crib, it took over eight hours in the 2.5-2.9kg body weight group and five hors in both the 3.0-3.4kg and over 3.5kg body weight group to reach 98℉ of body temperature. 5. The lowest mean body temperature of newborn infants in both general and electric- heat crib appeared in forty-five minute after birth and the temperature ranged from 96.4℉ to 96.5℉. 6. The mean body temperature of the newborn infants in the general crib was increased as the room temperature. 7. The body weight, the room temperature and the time elapsed after birth fore proved to be significant factors influencing the body temperature changes of newborn infants. From tile above results, the three hypotheses were positively accepted.
We frequently encounter newborn infants with thrombocytosis in the neonatal intensive care unit. However, neonatal thrombocytosis is not yet fully understood. Thrombocytosis is more frequently identified in newborns and young infants, notably more often in those younger than 2 years than in older children or adults. The production of megakaryocytes (megakaryopoiesis) and platelets (thrombopoiesis) is mainly regulated by thrombopoietin (TPO). Increased TPO levels during infection or inflammation can stimulate megakaryopoiesis, resulting in thrombopoiesis. TPO concentrations are higher in newborn infants than in adults. Levels increase after birth, peak on the second day after birth, and start decreasing at 1 month of age. Initial platelet counts at birth increase with gestational age. Thus, preterm infants have lower initial platelet counts at birth than late-preterm or term infants. Postnatal thrombocytosis is more frequently observed in preterm infants than in term infants. A high TPO concentration and low TPO receptor expression on platelets leading to elevated plasma-free TPO, increased sensitivity of megakaryocyte precursor cells to TPO, a decreased red blood cell count, and immaturity of platelet regulation are speculated to induce thrombocytosis in preterm infants. Thrombocytosis in newborn infants is considered a reactive process (secondary thrombocytosis) following infection, acute/chronic inflammation, or anemia. Thrombocytosis in newborn infants is benign, resolves spontaneously, and, unlike in adults, is rarely associated with hemorrhagic and thromboembolic complications.
The present analysis was undertaken to find out the relationships between birth weight & Apgar Score of newborn infant & maternal factors. The medical records of 1436 newborn infants who had been at the Korea University III Seoul from January. 1.1984. to December. 31. 1985, were examined. Measurements include weight and Apgar Score. As the possible factors influencing the birth weight & Apgar Score of newborn infant, 9 variables such as : mother's age, frequency ·of pregnancy, frequency of fullterm delivery, frequency. of premature, frequency of abortion, mother's hemoglobin level, complications during pregnancy gestational period and infant sex at birth were selected among the items recorded in the medical records of newborn infants and their mothers. The weight & Apgar Score of newborn infants were compared separately by sex with group percent of those variables. The results were summarized as follows: 1. All of those factors chosen are supposed to be influencing upon the birth weight and Apgar Score examined at birth indirectly through inducing early termination of pregnancy. 2. The most influencing variable of birth weight of newborn infants was gestation period. The most influencing variable of Apgar Score of infant newborn was gestation period. 3. The relationships of those influencing factors are more clear on the birth weights of newborn than on the Apgar Score. 4. More then half of low birth weight infants are turned out to be physiologically normal through the evaluation by Apgar Scoring. Conclusively, All of those factors chosen are supposed to be influencing upon the birth weight and apgar score examined at birth indirectly through inducing early termination of pregnancy.
The purposes of this study are to assess iron status in mothers and their newborn infants at birth and to analyze the influence of maternal iron status on their newborn babies. Venous bloods samples were drawn from 144 pregnant women just before delivery and cord bloods of their newborn babies were collected immediately after birth for measurement of hemoglobin, hematocrit, serum iron, ferritin, total binding capacity and transferrin saturation. The values of hemoglobin and hematocrit were significantly lower in the mothers(10.9$\pm$1.43g/dl and 33.7$\pm$3.67%) than in their newborn infants(14.7$\pm$1.43g/dl and 45.3$\pm$4.76%)(p<0.0001). At delivery, serum iron levels in cord blood were about twice as high as those in the maternal blood, and serum ferritin levels in the cord blood were about four times higher than those in the maternal blood. The serum ferritin levels of multigravidas were higher than those of primigravidas,. but there was no difference between the serum ferritin levels of their infants. The serum ferritin levels of the mothers and their infants were higher in maternal group with iron supplement regularly than in other maternal group without iron supplement during pregnancy. Among the mothers, 26.4% had a serum ferritin levels below 12ng/ml(i.e. depleted iron stores)and 78.9% had a hemoglobin below 12g/dl(i.e.iron deficient anemia). When the maternal group was classified according to their serum ferritin levels by 9ng/ml, 12ng/dl or 20ng/ml, there was no significant difference in the iron status of their newborn infants among the three groups. The hemoglobin and serum ferritin levels of the mothers were well correlated with those of their babies. The maternal hemoglobin values negatively correlated with infant birth weight. It is possible that the demands of iron of the mother might be increased in the case of a newborn infant of greater size. The results of this study provide useful information regarding establishment of RDA for iron in pregnant women and guidance about the need for iron supplement during pregnancy.
Intestinal microflora in 17 breast-fed(BF) and 14 formula-fed (FF) newborn infants at about 1 week after birth were investigated to determine the floral differences of the two lactation groups. The fecal conditions were significantly different between the two groups in that the number of subjects who showed watery feces, and the times of defecation per day, were higher in the BF group. In addition, the mean fecal PH of the BF group was lower than that of the FF group. The dominant species in the BF groups which showed the highest count among the species of microflora was bifidobacteria , while the dominant species in the FF group was streptococci. The frequency of staphylococci and the count of bifidobacteria were significantly higher in the BF group. However, the frquencies of streptococci and Peptococcaceae were higher in the FF group. Through the differences were not significant, the frequency of bvifidobacteria and the count of staphylococci were higher in BF group, whereas the frequencies of bacteriodes, clostridia, enterobacter and the count of clostridia were higher in FF group.
To evaluate the effects of pregnancy-induced hypertension (PIH) to the iron status of fetuses, umbilical cord blood of 35 newborn infants borne by PIH mothers and of 37 normal term infants delivered at Yeungnam University Hospital from September 1, 1993 to September 30, 1994, were studied. The serum hemoglobin concentration of women with PIH was significantly higher than normal full-term pregnant women. There was no significant difference in serum hemoglobin concetration between women with PIH and normal full-term pregnant women and their newborn infants. There was no significant difference in serum hemoglobin concentration beween infants of women with PIH and normal full-term infants. The serum iron concentration of newborn infants of women with PIH was higher and the serum ferritin concentration of newborn infants of women with PIH was lower than normal full-term infants, but there were no significant difference between the two groups. The serum total iron-binding capactity and unsaturated iron-binding capacity of infants of women with PIH were significantly higher than normal full-term infants. The newborn infants of PIH women seemed that they might have occult depletion of iron store and need meticulous follow up during early neonatal period.
Hearing loss in newborns is the most frequently occurring birth defect. If hearing impaired children are not identified and managed early, it is difficult for many of them to acquire the fundamental language, social and cognitive skills that provide the foundation for later schooling and success in society. All newborns, both high and low risk, should be screened for hearing loss in the birth hospital prior discharge (Universal Newborn Heaing Screening, UNHS). Objective physiologic measures must be used to detect newborns and very young infants with hearing loss. Recent technological developments have produced screening methods and both evoked otoacoustic emission (EOAE) and auditory brainstem response (ABR) have been successfully implemented for UNHS. Audiologic evaluation should be carried out before 3 months of age and infants with confirmed hearing loss should receive intervention before 6 months of age. All infants who pass newborn hearing screening but who have risk indicators for other auditory disorders and/or speech and language delay receive ongoing audiologic surveillance and monitoring for communication development. Infants with sensorineural hearing loss are managed with hearing aids and receive auditory and speech-language rehabilitation therapies. Cochlear implants can be an outstanding option for certain children aged 12 months and older with severe to profound hearing loss who show limited benefit from conventional amplifications.
Radiation dose estimation on the newborn and infants during radiation examinations, unlike for the adults, is not actively being progressed. Therefore, as an index to present exposure dose during radiation examinations on newborn and infants, entrance skin dose was measured, and the result was compared with results of monte carlo simulation to raise reproducibility of entrance skin dose measurement, and it was proved that various geometry implementation was possible. The resulting values through monte carlo simulation was estimated using normalization factors for entrance skin dose to calibrate radiation dose and then normalized to a unit X ray radiation field size. Average entrance skin dose per one time exposure was $78.41{\mu}Gy$ and the percentage error between measurement by dosimeter and by monte carlo simulation was found to be -4.77%. Entrance skin dose assessment by monte carlo simulation provides possible alternative method in difficult entrance skin dose estimation for the newborn and infants who visit hospital for actual diagnosis.
Purpose: Multiple studies have documented that high resting levels of cardiac vagal tone suggest higher levels of self-regulation. The aim of this study was to evaluate cardiac vagal tone as an indicator of autonomic nervous function in healthy newborn and premature infants. Methods: This study was conducted using a descriptive comparison design and a convenience sampling strategy. The participants were 72 healthy and 62 premature infants delivered in a university hospital. Continuous heart rate data recordings from the infant's ECG were analyzed and Mxedit software was used to calculate mean heart period and an index of cardiac vagal tone. Results: The healthy infants had significantly higher cardiac vagal tone than the premature infants, when the influence of gestational age was removed using analysis of covariance. However, there were no significant differences in heart rate and heart period between the two groups when the influence of gestational age was removed using analysis of covariance. Conclusion: The results of this study show that cardiac vagal tone may be used as an index for determining infant's autonomic nervous function. Nursing staff in pediatric departments can use cardiac vagal tone with ease, as this index can be calculated in a noninvasive method from the ECG.
