요척주관 협착증 혹은 추간판 탈출증 환자에서 실시한 경추간공 경막외 스테로이드 주입의 결과에 관한 분석 (An Analysis of the Outcome of Transforaminal Epidural Steroid Injections in Patients with Spinal Stenosis or Herniated Intervertebral Discs)

  • Background: Spinal stenosis and herniated intervertebral discs are the principal causes of lumbosacral radiculopathy. This study was conducted to compare the therapeutic value and duration of pain relief of fluoroscopic guided transforaminal epidural steroid injections (TFESIs) in patients with refractory radicular leg pain. Methods: Between August 2006 and March 2007, 87 patients (H group: patients with herniated intervertebral disc, S group: patients with spinal stenosis) who met the inclusion criteria were treated with fluoroscopic guided TFESIs. Prior to treatment, the VAS and ODI scores were determine to evaluate the degree of pain and level of disability. The degree of pain relief was then assessed 1 month after treatment with the TFESIs and graded as excellent (no residual pain), good (improvement of pain symptoms by more than 50%), fair (improvement of pain symptoms by less than 50%) and Poor (no improvement of pain). In addition, the duration of pain relief was evaluated by regular outpatient visits for 6 months, and by telephone interviews after 6 months. Results: The H and S group both had excellet results at 1 month after treatment with TFESIs showing improvements of 44.1% and 20.8% respectively. However this difference was not significant between groups. In addition, a duration of pain relief greater than 6 months was achieved in 32.4% of the patients in the H group and 37.7% of those in the S group. Conclusions: TFESIs had a similar degree of therapeutic effectiveness and duration of pain relief in patients with spinal stenosis and herniated intervertebral discs.

Magnetic resonance imaging analysis of rotator cuff tear after shoulder dislocation in a patient older than 40 years

  • Background: This study was designed to evaluate characters of the rotator cuff tear (RCT) recognized after primary shoulder dislocation in patients older than 40. Methods: From 2008 to 2019, patients who visited two hospitals after dislocation were retrospectively reviewed. Inclusion criteria were patients over 40 who had dislocation, with magnetic resonance imaging (MRI) undergone. Exclusion criteria were patients who lost to follow-up, combined with any proximal humerus fracture, brachial plexus injury, and previous operation or dislocation history in the ipsilateral shoulder. Also patients who had only bankart or bony bakart lesion in MRI were excluded. We evaluated RCTs that were recognized by MRI after the primary shoulder dislocation with regard to tear size, degree, involved tendons, fatty degeneration, the age when the first dislocation occurred, and the duration until the MRI was evaluated after the dislocation. Results: Fifty-five RCTs were included. According to age groups, the tear size was increased in coronal and sagittal direction, the number of involved tendons was increased, and the degree of fatty degeneration was advanced in infraspinatus muscle. Thirty-two cases (58.2%) conducted MRI after 3 weeks from the first shoulder dislocation event. This group showed that the retraction size of the coronal plane was increased significantly and the fatty accumulation of the supraspinatus muscle had progressed significantly. Conclusions: Age is also a strong factor to affect the feature of RCT after the shoulder dislocation in patients over 40. And the delay of the MRI may deteriorate the degree of tear size and fatty degeneration.

Effects of Induction Heat Bending Process on Microstructure and Corrosion Properties of ASME SA312 Gr.TP304 Stainless Steel Pipes

  • The usage of bending products recently have increased since many industries such as automobile, aerospace, shipbuilding, and chemical plants need the application of pipings. Bending process is one of the inevitable steps to fabricate the facilities. Induction heat bending is composed of compressive bending process by local heating and cooling. This work focused on the effect of induction heat bending process on the properties of ASME SA312 Gr. TP304 stainless steel pipes. Tests were performed for base metal and bended area including extrados, intrados, crown up, and down parts. Microstructure was analyzed using an optical microscope and SEM. In order to determine intergranular corrosion resistance, Double Loop Electrochemical Potentiokinetic Reactivation (DL-EPR) test and ASTM A262 practice A and C tests were done. Every specimen revealed non-metallic inclusion free under the criteria of 1.5i of the standard and the induction heat bending process did not affect the non-metallic inclusion in the alloys. Also, all the bended specimens had finer grain size than ASTM grain size number 5 corresponding to the grain sizes of the base metal and thus the grain size of the pipe bended by induction heat bending process is acceptable. Hardness of transition start, bend, and transition end areas of ASME SA312 TP304 stainless steel was a little higher than that of base metal. Intergranular corrosion behavior was determined by ASTM A262 practice A and C and DL-EPR test, and respectively step structure, corrosion rate under 0.3 mm/y, and Degree of Sensitization (DOS) of 0.001~0.075% were obtained. That is, the induction heat bending process didn't affect the intergranular corrosion behavior of ASME SA312 TP304 stainless steel.

  • 경각형 결정화에 의하여 벤젠-사이클로헥산 혼합물로부터 벤젠의 결정화에서 결정에 내포된 불순물(사이클로헥산)의 분포가 조사되었다. 결정의 순도에 미치는 결정성장속도의 영향을 파악하였으며 모든 실험결과는 Wintermantel 모델에 의해 도시될 수 있었다. 결정의 순도는 과냉각 정도가 클수록, 주입조성이 낮을 수록, 결정성장속도가 클 수록 낮았으며 결정성장속도는 불순물의 내포를 지배하는 가장 중요한 변수이다. 결정화 초기에 형성된 결정은 불순물을 많이 내포하고 있으며 결정의 두께가 증가함에 따라 불순물은 잔여용융액쪽으로 이동되어 배제됨을 알 수 있었다. 경막결정화에서 결정에 내포된 불순물은 일정두께의 결정층에 온도구배를 이용하여 결정을 부분용해시키면 불순물의 확산에 의하여 제거될 수 있음을 알 수 있었다.

전염성 훼브리셔스낭병 (Infectious Bursal Disease-A Review)

  • IBDV는 양계업이 전업화된 모든지역에서 발생하고 있다. 이 질병의 임상증상은 매우 특징적이기도 하지만 IBDV 감염시의 닭의 연령 또는 바이러스의 병원성에 따라 임상증상 업이 내과 하기도 한다. IBDV에 의한 BF의 종장이나 출혈성 병변은 감염 후 짧은 기간에만 관할되며 질병이 경과함에 따라 BF는 위축된다. IBDV는 감염력이 극히 높으며, 오염된 계분, 사료, 물 등에 의하여 전파된다. 이 바이러스는 외계 환경에 대하여 극히 높은 저항성을 갖고 있으며 Ribovirus군의 Diplorna 바이러스로서 크기는 55-60nm이다. IBDV는 계태아나 계태아 조직 또는 감수성이 있는 닭의 BF에서 용이하게 증식시킬 수 있다. agar gel을 사용한 면역 확산법은 IBDV 감염을 진단할 수 있는 가장 간편한 방법이다. IBDV에 대한 항체는 감염 후 1주일 후부터 증명되며 장기간 지속된다. 감수성 계군일 경우 높은 역가의 항체형성이나 지속은 계군의 연령에 비례한다. IBDV에 대한 모체이행 항체는 critical age때의 병아리를 야외 감염으로부터 보호하여 준다. IBDV 단독 감염에 의한 IBD 피해는 경미한 것이지만 면역부전증에 의하여 유발되는 복합성 질병인 Inclusion body hepatitis, 전염성 빈혈, 괴저성 피부염 등은 닭의 생산성을 크게 저하시킨다.

Molecular characterization of Malaysian fowl adenovirus (FAdV) serotype 8b species E and pathogenicity of the virus in specific-pathogen-free chicken

  • Background: Inclusion body hepatitis (IBH) is an economically important viral disease primarily affecting broiler and breeder chickens. All 12 serotypes of fowl adenovirus (FAdV) can cause IBH. Objectives: To characterize FAdV isolates based on phylogenetic analysis, and to study the pathogenicity of FAdV-8b in specific-pathogen-free (SPF) chickens following virus inoculation via oral and intramuscular (IM) routes. Methods: Suspected organ samples were subjected to virus isolation and polymerase chain reaction (PCR) for FAdV detection. Hexon gene sequencing and phylogenetic analysis were performed on FAdV-positive samples for serotype identification. One FAdV-8b isolate, UPM/FAdV/420/2017, was selected for fiber gene characterization and pathogenicity study and was inoculated in SPF chickens via oral and IM routes. Results: The hexon gene phylogenetic analysis revealed that all isolates belonged to FAdV-8b. The fiber gene-based phylogenetic analysis of isolate UPM/FAdV/420/2017 supported the grouping of that isolate into FAdV species E. Pathogenicity study revealed that, chickens infected with UPM/FAdV/420/2017 via the IM route had higher clinical score values, higher percent mortality, higher degree of the liver lesions, higher antibody response (p < 0.05), and higher virus shedding amounts (p < 0.05) than those infected via the oral route. The highest virus copy numbers were detected in liver and gizzard. Conclusions: FAdV-8b is the dominant FAdV serotype in Malaysia, and pathogenicity study of the FAdV-8b isolate UPM/FAdV/420/2017 indicated its ability to induce IBH in young SPF chickens when infected via oral or IM routes.

Effects of Dietary Buffering Characteristics and Protected or Unprotected Acids on Piglet Growth, Digestibility and Characteristics of Gut Content

  • We condicted two experiments to evaluate the interaction among fumaric acid (FA), protected acids (PA), or no additional acid (NO) and two different levels of acid buffering capacity (BC) in diets for 14-d-old weaned pigs. BC was varied substituting mono-calcium phosphate and calcium sulfate for dicalcium phosphate and calcium carbonate. In the high BC diet plus PA, FA was also added. In Exp. 1, 48 gilts were raised for 31 days on the six diets, evaluating growth performance and fecal digestibility. In Exp. 2, 42 gilts were raised. With each diet three subjects were sacrificed after 19 days and four after 38 days. In addition, six subjects were sacrificed at weaning. Growth and carcass performance, ileal digestibility, bacterial populations and pH in the gut were assessed. The piglet performance and stomach, ileal and cecal pH, and empty body composition were not affected by the diets. Empty body composition other than ash content was affected by piglet age (p<0.01). The BC did not influence digestibility. The dietary inclusion of PA improved fecal digestibility of protein (p<0.05) compared to the addition of FA and NO. Ileal digestibility slightly increased with both acid additions (p<0.10), the groups receiving PA showing the higher values. Piglets fed diets with low BC had lower Lactobacillus and E. coli counts in the ileum (p=0.07) and higher Lactobacillus in the colon (p=0.08). Acidified diets tended to reduce E. coli counts in the ileum (p=0.10) and increased Lactobacillus in the colon (p=0.09). The addition in the diet of PA increased Lactobacillus in the ileum compared to the sole addition of free fumaric acid (p=0.07). The addition of protected acids, combined with free fumaric acid in the case of high BC diets, increased protein digestibility and Lactobacillus counts and reduced E.coli counts. Only some changes in the concentration of bacterial population can be expected with a diet of low BC.

삼성 금-은광상의 생성환경: 광석광물, 유체포유물 및 안정동위원소 연구 (Genetic Environment of the Samsung Gold-Silver Deposit, Republic of Korea: Ore Minerals, Fluid Inclusion and Stable Isotope Studies)

  • 삼성 금-은광상은 백악기 셰일과 사암 내에 발달된 단층대를 충진한 석영맥광상이다. 이 광상의 광화작용은 단층-각력대에 수반되며 2시기로 구분된다. 광화I시기는 주된 광화시기이고 광화II시기는 광화작용이 관찰되지 않는다. 광화 I시기는 모암변질광물(견운모, 황철석, 녹니석, 석영), 금홍석, 천금속 황화광물(자류철석, 황철석, 섬아연석, 황동석, 방연석)과 에렉트럼 등이 관찰된다. 광화II시기는 석영, 방해석 및 황철석만 관찰된다. 유체포유물 자료에 의하면, 광화II시기의 균일화온도와 염농도는 각각 $145\sim309^{\circ}C$, 0.4~12.4 wt.% NaCl 로서 광화유체가 천수의 혼입에 의한 냉각과 희석이 있었음을 지시한다. 천금속 황회광물과 에렉트럼은 온도 $200\sim300^{\circ}C$에서 냉각과 희석작용에 의해 침전되었다. 황(9.3~10.8‰) 기원은 화성기원과 모암내의 황에서 유래된 것으로 해석된다. 산소 [-2.3~0.9‰(석영 0.3‰과 0.9‰, 방해석: -2.3‰)] 및 수소[-86~-76‰(석영: -86‰과 -82‰, 방해석: -76‰)]동위원소값의 자료로 볼 때, 이 광상의 광화유체는 천수 기원의 유체가 주종을 이룬 것으로 보이며 광화작용이 진행됨에 따라 기원이 다른 천수의 혼입이 작용한 것으로 해석할 수 있다.

부영 금-은광상의 광석광물, 유체포유물 및 안정동위원소 연구 (Ore Minerals, Fluid Inclusion and Stable Isotope Studies of the Buyeong Gold-silver Deposit, Republic of Korea)

  • 부영 금-은광상은 백악기 고성층 내에 발달된 NS단층대의 열극을 충진한 석영맥광상이다. 이들 석영맥의 광화작용은 hypogene 시기와 supergene 시기로 구분된다. Hypogene 시기의 광물은 견운모, 황철석, 녹니석 및 녹염석으로 구성된 열수변질광물과 황철석, 자류철석, 섬아연석, 백철석, 황동석, 방연석 및 갈레노비스무타이트로 구성된 황화광물이 관찰된다. Supergene 시기에는 공작석, 침철석, 휘동석 및 섬아연석 산화물 등이 생성되었다. 유체포유물 자료에 의하면, 광화시기의 균일화온도와 염농도는 각각 $112{\sim}340^{\circ}C$, 0.2~7.9 wt.% NaCl 로서 광화유체가 천수의 혼입에 의한 냉각과 희석이 있었음을 지시한다. 황(3.2~3.9‰) 기원은 주로 화성기원과 일부 모암내의 황에서 유래된 것으로 해석된다. 산소(4.3~6.0‰)와 수소(-60~-64‰) 동위원소값의 자료로 볼 때, 이 광상의 광화유체는 천수 기원의 유체가 주종을 이룬 것으로 보이며 광화작용이 진행됨에 따라 기원이 다른 천수의 혼입이 작용한 것으로 해석할 수 있다.

봉상 금-은광상의 광석광물, 유체포유물 및 안정동위원소 연구 (Ore Minerals, Fluid Inclusion and Stable Isotope Studies of the Bongsang Gold-silver Deposit, Republic of Korea)

  • 봉상 금-은광상은 백악기 안산암질 래필리응회암 내에 발달된 단층대를 충진한 석영맥광상이다. 이 광상의 광화작용은 단층-각력대에 수반되며 2시기로 구분된다. 1시기는 다시 조기와 말기로 구분되며 주된 광화시기이다. II시기는 광화작용이 관찰되지 않는다. I시기 조기는 모암변질과 천금속 황화광물이 관찰된다. I시기 말기는 금-은광물 정출시기로 자연은, 함은사면동석 및 휘안동은석과 함께 황철석, 섬아연석, 황동석 및 방연석 등의 황화광물이 관찰된다. 유체포유물 자료에 의하면, 광화I시기의 균일화온도와 염농도는 각각 $137{\sim}336^{\circ}C,\;0.0{\sim}10.6wt.%$ NaCl 로서 광화유체가 천수의 혼입에 의한 냉각과 희석이 있었음을 지시한다. 또한, 광화I시기 말기에 관찰되는 광물공생군으로부터 구한 생성온도와 황분압은 $<210^{\circ}C$$<10^{-15.4}$ atm를 갖는다. 황(3.4%o 기원은 화성기원과 모암내의 황에서 유래된 것으로 해석된다. 산소{2.9%o과 10.3%o(석영: 7.9%o과 8.9%o, 방해석: 2.9%o과 10.3%o)}, 수소(-75%o) 및 탄소(-7.0%o과 -5.9%o)동위원소값의 자료로 볼 때, 이 광상의 광화유체는 천수 기원의 유체가 주종을 이룬 것으로 보이며 광화작용이 진행됨에 따라 기원이 다른 천수의 혼입이 작용한 것으로 해석할 수 있다.
