The Relationship Between Bone mineral Density and The Environmental Factors in Korean Pubescent Girls (한국 사춘기 소녀들의 골밀도와 환경요인들과의 관계(II))

  • Bone mass accretion during puberty appears to be critical in the development of peak bone mass. Although bone density of females in Korea has been studied, only a few studies have related bone mass with anthropometric patterns or puberty in the pubescent girls. This study was conducted as part of a study of major determinants of bone development during puberty. Subjects were aged 14∼16 yr(mean 14.97), and had no history of disorders or dedication use likely to influence bone or calcium metabolism. Bone mineral density and content were measured by dual energy X-ray absorptiometry using a Lunar DPX+Scanner (Lunar Madison, WI). Also, total body fat, and total lean body mass were assessed using a Lunar DPX dual-energy X-ray absorptiometer, Pubertal status was assessed according to the Marshall and Tanner guidelines. Serum levels of osteocalcin was measured by RIA using a commercial kit assay. Skinfold measurements were taken with a skinfold caliper(Lange Caliper, USA). Data were analyzed using the regression and GLM procedure of the statistical package SAS. The results indicated that the observed means for lumbar spine BMD and femoral BMD correspond to approximately 91% and 96% of the means for young adult females, respec tively. All subjects were menarchal, with the majority being in the middle to end stages of pubertal development. Total body BMD was positively related to fat mass(P<0.001), lean body mass and time since menarche, and negatively related to urine pyridinoline, serum alkaline phosphatase and osteocalcin. The data indicate that girls who reported lower age for menarche had significantly higher bone densities than girls who reported higher age for menarche. Attaining peak skeletal bone mass during puberty may reduce the incidence of osteoporosis in later life. this finding suggests that early menarche may augment peak bone mass, influencing the extent of bone loss later in adulthood. The results suggest that good nutrition in childhood appears to be needed not for growth and development, but possibly also to assure an optimal peak of bone mass and thus greater latitude for the maintenance or skeletal integrity in the face of bone losses. Troeps skinfold thickness was a better predictor of total BMD and total BMC than was any other skinfold thickness. The study did not find a relationship between total BMD and body fat %, but total fat was significantly positively related to total BMD(r=0.49) and total BMC(r=0.60). It supports earlier report that there was a significant correlation between TBMD and body weight. Conclusively, total fat, lean body mass and pubertal development could influence BMD in pubescent girls. Clearly, longitudinal studies are required to assess the effect of puberty on peak bone mass, and to define further the potential determinants of peak bone mass.

Effect of Water Hyacinth (Eichhornia Crassipes) Silage on Intake and Nutrient Digestibility in Cattle Fed Rice Straw and Cottonseed Cake

  • Four crossbred Sindhi heifers with an average body weight (BW) of 135 kg and a mean age of 17 months were used to investigate the effect of feeding different combinations of rice straw and ensiled water hyacinth (EWH) supplemented with a source of protein in the form of cottonseed cake (CSC) on intake and digestibility. Four treatments consisting of graded levels of EWH were arranged in a $4{\times}4$ Latin square. The levels of EWH were set at: 0 (EWH0), 15 (EWH15), 30 (EWH30), and 45% (EWH45) of an expected total dietary dry matter (DM) intake of 30 g total DM per kg BW per day. Rice straw was offered ad libitum, while CSC was given at a fixed level of 5 g DM/kg body weight (BW). Voluntary intake and digestibility were measured consecutively in the 4 experimental periods which each lasted 28 days. The crude protein (CP) content of EWH, rice straw and CSC were 174, 53 and 370 g/kg DM, respectively. Rice straw had the highest neutral detergent fibre (NDFom) content (666 g/kg DM), followed by EWH (503 g/kg DM) and the lowest content was 418 g/kg DM in the CSC. The actual EWH contents in the consumed diets were 0, 17, 32 and 52% for EWH0, EWH15, EWH30 and EWH45, respectively. Rice straw intake decreased with level of EWH offered from 3049 for EWH0 to 1014 g/day for EWH45. Crude protein intake was 16, 25 and 33% higher (p<0.001) in EWH15, EWH30 and EWH45 treatments, respectively, as compared to EWH0. Digestibility of organic matter (OM), CP, NDFom and acid detergent fibre (ADFom) increased with increasing level of EWH offered. The highest OM digestibility (72.2%) was found for treatment EWH45 and the lowest (47.4%) for treatment EWH0. In spite of similar dietary CP contents, CP digestibility increased by 21 (EWH15), 31 (EWH30) and 40% (EWH45) with increasing level of EWH in comparison with treatment EWH0. It is concluded that increasing level of EWH in cattle diets considerably improved CP intake and digestibility of nutrients.

Impact of Treatment Time on Chemoradiotherapy in Locally Advanced Cervical Carcinoma

  • Background: Adverse effects of treatment prolongation beyond 8 weeks with radiotherapy for cervical cancer have been established. Clinical data also show that cisplatin increases the biologically effective dose of radiotherapy. However, there are no data on the effect of overall treatment time in patients with locally advanced cervical cancer treated with concomitant chemo-radiotherapy (CCRT) in an Indian population. The present study concerned the feasibility of concurrent chemotherapy and interspacing brachytherapy during the course of external radiotherapy to reduce the overall treatment time and compare the normal tissue toxicity and loco-regional control with a conventional schedule. Materials and Methods: Between January 2009 and March 2012 fifty patients registered in the Gynaecologic Oncology Clinic of Institute Rotary Cancer Hospital with locally advanced cervical cancer (FIGO stage IIB-IIIB) were enrolled. The patients were randomly allocated to treatment arms based on a computer generated random number. Arm I (n=25) treatment consisted of irradiation of the whole pelvis to a dose of 50 Gy in 27 fractions, and weekly cisplatin $40mg/m^2$. High dose rate intra-cavitary brachytherapy (HDR-ICBT) was performed after one week of completion of external beam radiotherapy (EBRT). The prescribed dose for each session was 7Gy to point A for three insertions at one week intervals. Arm II (n=25) treatment consisted of irradiation of the whole pelvis to a dose of 50 Gy in 27 fractions. Mention HDR-ICBT ICRT was performed after 40Gy and 7Gy was delivered to point A for three insertions (days 23, 30, 37) at one week intervals. Cisplatin $20mg/m^2/day$ was administered from D1-5 and D24-28. Overall treatment time was taken from first day of EBRT to last day of HDR brachytherapy. The overall loco-regional response rate (ORR) was determined at 3 and 6 months. Results: A total of 46 patients completed the planned treatment. The overall treatment times in arm I and arm II were $65{\pm}12$ and $48{\pm}4$ days, respectively (p=0.001). At three and six months of follow-up the ORR for arm I was 96% while that for arm II was 88%. No statistically significant difference was apparent between the two arms. The overall rate of grade ${\geq}3$ toxicity was numerically higher in arm I (n=7) than in arm II (n=4) though statistical significance was not reached. None of the predefined prognostic factors like age, performance status, baseline haemoglobin level, tumour size, lymph node involvement, stage or histopathological subtype showed any impact on outcome. Conclusions: In the setting of concurrent chemoradiotherapy a shorter treatment schedule of 48 days may be feasible by interspacing brachytherapy during external irradiation. The response rates and toxicities were comparable.

A study on the Quality Characteristics of Rice Bread Containing Trehalose (트레할로스를 첨가한 쌀식빵의 품질특성)

  • This study was conducted to evaluate the quality characteristics. Trehalose was used to manufacture rice bread in two ways: one was to substitute trehalose for sugar by (0, 25, 50, and 75%) trehalose in each sample and the other was to analyze the temporal changes in rice bread on Day 0, on Day 2, and on Day 4. The results concerning the characteristics of specific volumes, water content, water activity, variation in weight and height, physical property test, and sensory test and measurements of the temporal changes are as follows: First, as for variation in specific volumes, rice bread containing trehalose had the content and temporal changes decrease significantly (p<0.001). Second, as for texture, a higher trehalose content led to a slightly lower level of hardness. As for the temporal changes, addition of trehalose is generally effective in decreasing hardness of rice bread. the sample containing trehalose showed an almost similar value. The control group not containing trehalose showed the significantly lowest level of cohesiveness, springiness, Chewiness. Finally, the sensory evaluations revealed that the taste, Flavor, and Sweetness was at a significantly higher level in the group with trehalose substituted for sugar than in the control group. An increase in the trehalose content led to an increase in overall eating quality.

The Effect of Self-help Group Program on Adaptation and Quality of Life of Mastectomy Patients (자조집단 프로그램이 유방절제술 환자의 적응과 삶의 질에 미치는 효과)

  • This quasi-experimental study was intended to test the effect of self-help group program, which is one of the way to enhance adaptation and quality of life to mastectomy patients. Data was collected from July 14, 1998 to Oct. 31, 1998 at two Medical Center in Seoul. The subjects for this study were the patients who had undergone mastectomy and were follow-up ; 14 in experimental group and 14 in control group matched with age and treatment. The instruments for this study were adaptation in Lee(1994)'s physical symptom questionnaire, Zung's Self-rating Depression Scale(SDS, 1965), and Self-rating Anxiety Scale(SAS, 1970), quality of life in Spranger(1996)'s and No(1988)'s Quality of Life Questionnaire. The self-help group program for mastectomy patients was developed based on literature review and pilot study by the investigator. The subjects of experimental group were participated in 6 weeks self-help group program and were received arm and shoulder exercise, informational support, and interpersonal support by group members. The control group were received no intervention, Both group answered questionnaires prior to intervention and 6 weeks later. The data analyzed by frequency, $X^2$-test, Mann-Whitney U test. Wilcoxon Signed Rank test, Pearson's Correlation Coefficient and Stepwise Multiple Regression using SPSS WIN. The results are as follows ; Hypothesis 1. "The experimental group with the self-help group program will have a higher score on adaptation state than control group." was not supported. But the post test score of anxiety and depression in experimental group were declined and the depression score was reduced relatively. Hypothesis 2. "The experimental group with the self-help group program will have a higher score on quality of life than control group." was not supported. But the posttest score of quality of life in experimental group was reduced relatively. Hypothesis 3. "The higher adaptation state of mastectomy patients, the higher quality, of life." was supported(r=,80, p<.001). Additionally, the lower physical symptom, depression and anxiety, the higher quality of life And depression, which was the main predictor of quality of life, accounted for 59.5%, depression and anxiety accounted for 65.5% of the variance in quality of life. In conclusion, when the self-help group program was intervened to mastectomy patients, it was tended to increase quality of life and to reduce depression and anxiety. So self-help group program can be considered useful nursing inter vention effect on adaptation and quality of life of mastectomy patients. With discussion, I suggest repeated further re search on self-help group with appropriate sample size and longitudinal study. Also during adjuvant therapy, it is needed to develop convenient method to be supported from peer group and family, such as computer mediated support group.

MACRO-SHEAR BOND STRENGTH AND MICRO-SHEAR BOND STRENGTH OF CEROMER BONDED TO METAL ALLOY AND FIBER REINFORCED COMPOSITE

  • Statement of problem. According to the fracture pattern in several reports, fractures most frequently occur in the interface between the ceromer and the substructure. Purpose. The aim of this in vitro study was to compare the macro shear bond strength and microshear bond strength of a ceromer bonded to a fiber reinforced composite (FRC) as well as metal alloys. Material and methods. Ten of the following substructures, type II gold alloy, Co-Cr alloy, Ni-Cr alloy, and FRC (Vectris) substructures with a 12 mm in diameter, were imbedded in acrylic resin and ground with 400, and 1, 000-grit sandpaper. The metal primer and wetting agent were applied to the sandblasted bonding area of the metal specimens and the FRC specimens, respectively. The ceromer was placed onto a 6 mm diameter and 3 mm height mold in the macro-shear test and 1 mm diameter and 2 mm height mold in the micro-shear test, and then polymerized. The macro- and micro-shear bond strength were measured using a universal testing machine and a micro-shear tester, respectively. The macro- and micro-shear strength were analyzed with ANOVA and a post-hoc Scheffe adjustment ($\alpha$ = .05). The fracture surfaces of the crowns were then examined by scanning electron microscopy to determine the mode of failure. Chi-square test was used to identify the differences in the failure mode. Results. The macro-shear strength and the micro-shear strength differed significantly with the types of substructure (P<.001). Although the ceromer/FRC group showed the highest macroand micro-shear strength, the micro-shear strength was not significantly different from that of the base metal alloy groups. The base metal alloy substructure groups showed the lowest mean macro-shear strength. However, the gold alloy substructure group exhibited the least micro-shear strength. The micro-shear strength was higher than the macro-shear strength excluding the gold alloy substructure group. Adhesive failure was most frequent type of fracture in the ceromer specimens bonded to the gold alloys. Cohesive failure at the ceromer layer was more common in the base metals and FRC substructures. Conclusion. The Vectris substructure had higher shear strength than the other substructures. Although the shear strength of the ceromer bonded to the base metals was lower than that of the gold alloy, the micro-shear strength of the base metals were superior to that of the gold alloy.

Suppression of Experimental Liver Tumors by Estradiol-3-Benzoate Treatment or Castration in Male Rats

  • Epidemiologically the incidence of liver cancer is markedly sex-differentiated, with a much higher frequency in men than in women. In experimental animals, it is also higher in male than in female irrespective of carcinogen-induced or spontaneous tumors. Therefore, we tried to investigate the modulating effects of sex hormones in experimental hepatocarcinogenesis. For induction of liver tumors, mini-osmotic pump containing diethylnitrosamine at a dose level of 47.5mg was implanted into the peritoneal cavity of the rat at 6 weeks old. To remove the effects of male sex hormones, the animals of group 2 were castrated one week prior to DEN treatment. To see the effects of estrogen, pellet containing 1g or 10g of estradiol-3-benzoate was infused subcutaneously to the animals of group 3 and 4 one week prior to DEN treatment. The pellets were exchanged every 4 weeks until sacrifice. All animals were sacrificed at 26 weeks after DEN treatment. The tumor incidences in group 1 (DEN alone), group 2 (DEN +castration), group 3(DEN +EB 1g) and group 4 (DEN +EB 10g) were 100% (15/15), 93.3% (14/15), 85.7% (12/14) and 66.7% (10/15), respectively, showing that the value of group 4 is significantly different from that of group 1. Tumor multiplicity data of group 1, 2, 3 and 4 were 5.470.73, 2.800.51, 2.070.41 and 1.670.46, respectively, showing castration or EB treatment reduced number of liver tumors significantly (P<0.001). With immunohistochemistry and Western blotting of ER the expressions were detected in normal adjacent liver cells but decreased or lost in tumor cells. From these results we conclude that female sex hormone, especially estrogen, may act as a liver tumor suppressor, and it seemed that the down regulation of ER may be associated with liver tumor development.

Ipsilateral Cerebral and Contralateral Cerebellar Hyperperfusion in Patients with Unilateral Cerebral Infarction; SPM Analysis (일측 뇌경색 환자에서 반대측 뇌의 보상성 뇌관류 증가에 대한 SPM 분석)

  • Purpose: Cortical reorganization has an important role in the recovery of stroke. We analyzed the compensatory cerebral and cerebellar perfusion change in patients with unilateral cerebral infarction using statistical parametric mapping (SPM). Materials and Methods: Fifty seven $^{99m}TC-Ethylene$ Cystein Diethylester (ECD) cerebral perfusion SPECT images of 57 patients (male/female=38/19, mean age=$56{\pm}17\;years$) with unilateral cerebral infarction were evaluated retrospectively. Patients were divided into subgroups according to the location (left, right) and the onset (acute, chronic) of infarction. Each subgroup was compared with normal controls (male/female=11/1, mean age=$36{\pm}10\;years$) in a voxel-by-voxel manner (two sample t-test, p<0.001) using SPM. Results: All 4 subgroups showed hyperperfusion in the ipsilateral cerebral cortex, but not in the contralateral cerebral cortex. Chronic left and right infarction groups revealed hyperperfusion in the ipsilateral primary sensorimotor cortex, meanwhile, acute subgroups did not. Contralateral cerebellar hyperperfusion was also demonstrated in the chronic left infarction group. Conclusion: Using $^{99m}Tc-ECD$ SPECT, we observed ipsilateral cerebral and contralateral cerebeller hyperperfusion in patients with cerebral infarction. However, whether these findings are related to the recovery of cerebral functions should be further evaluated.

The Usefulness of Vaginal Cytology during the Estrous Cycle in Miniature Schnauzer Dogs (Miniatur Schnauzer견에서 발정 주기 동안 질세포 검사의 유용성)

  • Vaginal cytology with behavioral observation was performed in 34 estrous cycles in 16 Miniature Schnauzer dogs to evaluate the its usefulness. The mean duration of proestrus and estrus in Miniature Schnauzer based on behavioral observation and vaginal cytology was $7.58{\pm}1.50$ (Mean ${\pm}$S.D., range: $5{\sim}10$) and $8.46{\pm}2.82\;(6{\sim}17)$ days for proestrus, and $6.97{\pm}1.66\;(4{\sim}10)$ and $10.29{\pm}2.61\;(6{\sim}19)$ days for estrus, respectively. The duration of each phase of the estrous cycle was not significantly different based on between behavioral observation and vaginal cytology. The gestational length from the first day of male acceptance was $66.55{\pm}2.91\;(64{\sim}76)$ days, $57.70{\pm}1.92\;(54{\sim}62)$ days from the first male refusal, and $56.90{\pm}1.62\;(54{\sim}60)$ days from the onset of cytologic diestrus, respectively. Vaginal cytology during the estrous cycle were significantly characteristic of large intermediate cells in proestrus, anuclear cells in estrus, small intermediate cells in diestrus, and parabasal cells and small intermediate cells in anestrus (p<0.001), respectively. Cornification index (CI) by vaginal cytology was higher in proestrus ($69.90{\pm}1.44%$) and estrus ($91.35{\pm}1.09%$), then it was decreased in diestrus ($13.53{\pm}1.28%$) and anestrus ($1.16{\pm}1.26%$).

A study on influential factors for dental hygiene students' image of dental hygienist (치위생과 학생들의 치과위생사 이미지에 영향을 미치는 요인 연구)

  • Objectives : The purpose of this study was to examine dental hygiene students' image of dental hygienist and factors affecting the image of dental hygienist in an effort to provide some information useful for dental hygiene education. It's basically meant to recreate the image of dental hygienist as a professional. Methods : The subjects in this study were 310 sophomores and seniors who ere selected by convenience sampling from three different three-year-course colleges located in Gyeonggi Province, Chungcheong Province and Gwangju. After a survey was conducted, the answer sheets from 277 respondents were analyzed except for 33 incomplete ones. The collected data were analyzed with a SPSSWIN 17.0 program, and statistical data on frequency and percentage were obtained to find out the general characteristics of the subjects, and another statistical data on mean and standard deviation for each item were acquired to check their image of dental hygienist. Besides, t-test, ANOVA and Kruscal-Wallis test, one of nonparametric test, were utilized to look for connections between their general characteristics and image of dental hygienist. Results : 1. They gave a mean of 3.53 to the image of dental hygienist, which showed that the dental hygiene students took a favorable view of dental hygienists in general. 2. Their response to four items showed that they looked at the image of dental hygienist in a positive manner. The four items respectively described dental hygienists as professionals, as ones who played a crucial role in the improvement of national oral health, as ones with professional knowledge and sophisticated knowhow, and as neat and decent people. And their response to the following five items indicated that they looked at the image of dental hygienists in a negative way. One was that dental hygienists were considered to be dental nurses, and another was that the occupation of dental hygienist was a demanding and stressful job. The third was that there was no single, fixed title to call dental hygienists, and the fourth was that dental hygiene wasn't recognized as an independent area. The fifth was that the social standing of dental hygienists was sort of high. 3. In regard to the impact of the general characteristics of the dental hygiene students on their image of dental hygienist, they had a more favorable image of dental hygienist when they perceived dental hygiene in a more positive manner(p<0.001). Conclusions : In order to boost the image of dental hygienist, the working conditions of dental hygienists should be improved, and educational programs should be prepared to provided them extended continuing educational opportunities. And the kind of educational environments that could nurture dental hygienists who are convinced and have a vision should be created.
