  • This study was carried out to investigate how chlorella dry powder added to the feed of laying hens influences on the egg quality and the composition of egg yolk's fatty acid. Moisture content, ash content, crude protein content, and crude fat content were 12.8%, 10.8%, 18.0% and 2.5%, respectively. The moisture content of the chlorella powder added to the feed was about 1.54%, the ash content was 6.53%, the crude protein content was 54.56%, and the crude fat content was 2.45 %. After feeding chlorella, compared to the control, the color of egg shell significantly became darker. The hardness of egg shell was increased for 10 days after chlorella feeding and was significantly strengthened. Thickness of egg shell was significantly thicker. The height of egg whites was increased. After 10 days of providing chlorella, the quality of egg white was 92.0 HU (Haugh Unit), which was significantly higher than 84.8 HU, the quality of the control. The color of egg yolk significantly revealed more yellow than that of the control. Egg weight was increased by 7.5% after 15 days of feeding chlorella. Protein content was increased by 11.9% and 10.7% after 10 and 15 days of feeding, respectively. The major compositions of fatty acid content of egg yolk fed with chlorella diet were oleic acid, trans-linoleic acid, palmitic acid, ${\alpha}$-linolenic acid, stearic acid, DHA, EPA, palmitoleic acid, and heptadecanoic acid, respectively. Palmitoleic acid was decreased in the eggs fed with chlorella diet compared to the control. The Saturated Fatty Acid (SFA) content of the control was higher than that of the eggs fed with chlorella. The content of Unsaturated Fatty Acid (UFA) was higher in egg yolks fed with chlorella than in the control. The ratio of UFA to Saturated Fatty Acid (SFA) was higher in egg yolks fed with chlorella than in the control. These results suggest that the addition of chlorella to the feed of the laying hens brings positive effects on the improvement of egg quality and lowering of the Unsaturated Fatty Acid of egg yolk.

  • The On-X valve was recently introduced. It was the aim of this study to assess the safety and feasibility from the data derived from 28 patients who underwent aortic and/or mitral valve replacement with this prosthesis in National Medical Center. Material and Method: From May 1999 and May 2003, a series of 28 consecutive patients who had been implanted with 32 On-X prosthesis were reviewed, The operative procedure comprised of 12 MVR, 10 AVR and 6 DVR. The study followed the guidelines of AATS/STS. Mean follow-up was 27 months (total 04 patient-years). Result: Early ($\leq$30 days) mortality was 7.44% (2/28) and no late mortality occurred in the study. Total actuarial freedom from mortality at 2 years was 92.86$\pm$4.87% for all cases, 100% for MVR, 90$\pm$9.49% for AVR, and 83.3$\pm$1.52% for DVR. Thromboembolic event occurred in 2 MVR patients and that was the only complication; therefore, the linearized incidence of valve related complications was 3.17%/ patient-years for all cases and 6.5%/patient-years for MVR and the actuarial freedom from valve related complications at 2 years was 84.85$\pm$10.75%. Preoperatively, 24 (85.71%) patients were in NYHA functional class III or IV but postoperatively, 25 (89,29%) patients were in NYHA functional class I or II. The levels of hemoglobin, hematocrit, serum LDH, reticulocyte rate and indirect bilirubin were all within normal range at postoperative 3 month. In mitral position, the peak gradient was 6.1$\pm$1.8 mmHg and the mean gradient was 3.0$\pm$0.6 mmHg and EOA were 2.54$\pm$0.56 $m^2$, 2.39$\pm$0.73 $m^2$, 2.34$\pm$0.55 $m^2$, 2.40$\pm$0.63 $m^2$ at 27 mm, 29 mm, 31 mm, 33 mm respectively. In aortic postion, the peak gradient was 21.1 $\pm$14.12 mmHg and the mean gradient was 12.3$\pm$6.52 mmHg. Conclusion: Since there was no significant difference in the postoperative mortality, valve related complications and echocardiographic hemodynamic data compared to standard bileaflet design and since there was an improvement in the NYHA functional class and normal values of hemolytic indicators, it can be assumed that On-X valve is safe and feasible. However, accumulation of cases and long-term follow-up of this patient group is needed to establish this result.

  • From August 1986 until June 1995, single aortic valve replacement was performed in 65 patients at the Chonnam National University Hospital. worthy-eight were male and 17 were female patients, ranging from 19 to 68 years of age(median : 43 years). The causes of the valve lesions were rheumatic in 29 patients (44.6%), bicuspid aortic valve in 6 patients (6.2%), endocarditis in 6 patients(6.2%), unknown in others. Concomitant surgical procedures were performed in 10 patients : repair of congenital defect in 5, pericardiectomy in 1, coronary artery bypass grafting in 1, noncoronary sinus plication in 1, Valsalva sinus aneurysmectomy in 1, subaortic membrane resection in 1 Used valves were 51. Jude-Medical valve in 42, Duromedics valve in 22, Bjork-Shiley valve in 2, Carpentier-Edward valve in 1. There were 3 hospital deaths (4.6%), and 2 late deaths (3.2%). Follow-up was 95.2% complete. The 10-year acturlal survival rate was 85.3%. Postoperative complications were low cardiac utput in 8, arrythmia in 5, valve related hemolysis in 1, cerebral infarction in 1, and gastrointestinal bleeding in 2. Reoperation was performed in 4 for surgical bleeding, in 3 for paravalvular leak. The mean improvement in New York Heart Association functional class is from 2.79 $\pm$ 0.66 preoperatively to 1.25 $\pm$ 0.49 postoperatively(p < 0.001) The change of cardiothoracic ratio from preoperative to postoperative is 0.57 $\pm$ 0.06 to 0.54 $\pm$ 0.05 (p < 0.05). The left ventricular ejection fraction change is not significant perioperatively. There are no mechanical failures. This early and intermediate-term follow-up suggests that in adults in whom valve repair is not possible, the mechanical valve is a reliable and durable prosthesis with good hemodynamic function and a low rate of thromboembolic event.

  • Between May 1984 and January 1996, 130 patients were replaced cardiac valve using 150 St. Jude Medical prosthetic valves(42 aortic, 68 mitral, 20 aortic and mitral valve replycements). Follow-up was 97.6% complete. The early mortality rate was 5.4%, and late mortality rate was 4.9%. The valve-related late mortality rate was 3.3%. Of late complications, there were 6 anticoagulant related hemorrhages, 4 thromboembolisms and 1 paravalvular leakage. Linearized rates of late complication and valve-related late mortality were as follows: total late complications, .1.68o per patient-year: anticoagulant related hemorrhages, 0.92% per patient-year: thromboembolism, 0.61% per patient-year: paravalvular leakage, 0.15% per patient-year: reoperation, 0.15% per patient-year: and valve-related late mortalities, 0.61% per patient-year. Actuar al event free rate at 10 years was 87.4 $\pm$ 3.2%. The overall actuarial survival rate was 90.4$\pm$2.7% at 5 years, 87.5$\pm$3.3% at 10 years. Ninety eight percent of the survivors were in the New York Heart Association functional class I or II at the end of follow-up. There was significant improvement of cardiothoracic ratio. In conclusion, this study suggests the excellent durability of the St. Jude Medical Heart valve and remarkable functional benefit for the majority of the patients. However, prosthesisrelated complications are still common. Outcome is strongly related to the patient's preoperative cardiac condition and to the adequacy of anticoagulation control.

  • Purpose: The aim of this study is to report the efficacy of infliximab, a monoclonal antibody directed against tumor necrosis factor alpha which is used for both treatment of refractory pediatric Crohn disease (CD) and induction of remission. Methods: Among pediatric patients who were diagnosed with CD at Samsung Medical Center between March 2001 and August 2007, a total of 16 patients were given infliximab to treat conventional therapyresistant refractory CD and severe active CD for induction of remission. Patients needing maintenance therapy were treated with an infliximab infusion every 8 weeks, and fistulizing CD patients occasionally received the infusion upon the condition that a fistula developed. The efficacy of treatment was assessed by comparing the Pediatric Crohn Disease Activity Index (PCDAI), Hct, ESR, CRP, and serum albumin levels using paired t-test. Results: The male/female ratio was 13:3, and the median age was 13 years (range, 21 months~15 years). The patients included 7 cases of therapy-resistant refractory CD, 7 cases of severe active CD, and 2 cases of fistulizing CD. Mean PCDAI before infliximab therapy was 34.19${\pm}$14.96, and mean follow-up PCDAI within 2 to 4 weeks after the last infusion was significantly lower, at 6.88${\pm}$10.31 (p=0.000). Hematological markers such as ESR (p=0.000), serum albumin (p=0.016), and CRP (p=0.009) also improved significantly after infusion. Remission was achieved in 2 of 4 patients refractory to conventional therapy. Among 3 steroid-dependent patients, 2 were able to discontinue steroid therapy, and dose reduction was possible in 1 patient. Remission after top-down therapy without prior use of other immunomodulators was achieved in 6 weeks in all 7 of the patients who had severe CD. Nine of ten refractory fistulizing CD patients also showed improvement after infliximab therapy. Conclusion: Infliximab was effective in pediatric refractory CD for induction of remission and maintenance therapy, as well as in severe CD for top-down induction therapy. Furthermore, infliximab has contributed to steroid cessation and dose reduction. Long-term follow-up evaluation is needed to determine safety and efficacy of infliximab in the future.

  • Purpose : Urachal anomalies are rare but are known to develop several complications, especially infection. Moreover, uniform guidelines for management have not been presented because of the variable clinical characteristics of these anomalies. The purpose of this report is to review our experience with urachal anomalies and attempt to determine the optimal management. Methods : We retrospectively reviewed the records of fourteen children with a variety of urachal anomalies who had been treated from January 1996 to June 2005 at Dong Kang General Hospital. Results : The age distribution of the patients(mean age; 3.8 years) was six neonates, one infant, five preschool-age and two school-age children. The male to female ratio was 1:1. Six cases of urachal cyst, four cases of patent urachus, two cases of urachal sinus and two cases of urachal diverticulum were found. Three patients with patent urachus and one with urachal cyst had hydronephrosis. Other associated anomalies included an inguinal hernia in one patient with urachal sinus and a vesicoureteral reflux in one patient with urachal diver ticulum. As a first-line diagnostic tool, high-resolution ultrasound examination was performed in thirteen cases and computed tomography in one case. Surgical excision was performed in nine patients with urachal anomaly. Five cases out of six neonatal cases experienced spontaneous improvement during a three-month follow up period. Due to frequent infection of the umbilicus, surgical excision was performed on one neonate with urachal sinus. Conclusion : All patients with urachal anomalies should undergo investigation for associated anomalies. The neonate with urachal anomalies, especially patent urachus, do not require surgical excision unless the patient has multiple episodes of recurrent infection. • Background: Homografts and bioprostheses are most commonly used for Rastelli operation in congenital heart disease, but the limited durability is responsible for multiple reoperations associated with increased morbidity This study evaluated long-term results after Rastelli operation with a mechanical valved conduit. Material and Method: A total of 20 patients underwent Rastelli operation with mechanical valved conduit from January 1990 to July 1992. Operative mortality was 1 of 20 patients, and a retrospective review of 19 patients(10 males, 9 females) was done. Initial diagnosis was congenitally corrected transposition of great arteries(cc-TGA, n=4), complete TGA (n=2), ventricular septal defect with pulmonary atresia(VSD with PA, n=9), truncus arteriosus(n=2), double outlet right ventricle with pulmonary stenosis(DORV with PS, n=2). The mean age at Rastelli operation was $4.6{\pm}3.4$ years, and mean follow-up period was $12.8{\pm}2.7$ years. Patients underwent Rastelli opearation using 16 CarboMedics mechanical valve, and 3 Bjork-Shiley mechanical valve($17{\pm}2$ mm). Result: There were 15 reoperations for failed mechanical valved conduit. The freedom from reoperation at 5 and 10 years was 53% and 37%. Most patients were received oral anticoagulation with warfarin, and maintained the international normalized ratio(INR) of 1.5 to 2.0. There was no anticoagulation or thromboembolism related complication. There was a significant difference in the causes of a conduit failure between early(within 3 years) and late(after 3 years) failure groups. The six patients reported early prosthetic valve failure, mainly due to valvular dysfunction by thrombosis or pannus formation. The other nine patients reported late prosthetic valve failure, mainly due to dacron conduit stenosis at anastomosis sites, whereas their valvar motion was normal except 1 patient. Conclusion: To avoid early prosthetic valve failure, strict anticoagulation therapy would be helpful. About the late development of obstructive intimal fibrocalcific peels within the Dacron conduit, an improvement of conduit material is necessary to reduce late prosthetic valve failure. In selected patients, the long term results were satisfactory.

  • Methods of establishment of AOEL (Acceptable Operator Exposure Level), application of oral absorption by country, and calculation of exposure dose for operator risk assessment in USA, EU and Korea were investigated. Oral absorption of 141 active substances for pesticides was also investigated, then operator risk assessment was conducted with AOEL including oral absorption and Korean AOEL. Internal dose converted to external dose with oral or dermal absorption in USA and EU, but external dose to which oral absorption was not applied was used for establishment of AOEL in Korea. Oral absorption of 50 active substances among 141 were below 80%. In case of application of oral absorption as a correction factor in below 80%, AOELs of about 36% active substances were considered to be lower than the current Korean AOELs. Operator risk assessment of 28 active substances among 50 active substances with oral absorption below 80% was conducted with EU AOELs. TER (Toxicity Exposure Ratio) of 12 plant protection products including chlorothalonil WG (Water-dispersible Granule) was less than 1 and the risk was high. Operator risk assessment of 24 active substances among 50 active substances with oral absorption below 80% was conducted with Korean AOELs. TER of 6 plant protection products including chlorothalonil WG were less than 1 and the risk was high. Operator risk assessment of 4 plant protection products not having Korean AOEL was conducted with converted EU AOEL into AOEL not including oral absorption. The results indicated TER of 4 products including daminozide WP (Wettable Powder) was over 1 and risk was low. 22 products except 6 products such as oxadiagyl SC (Suspension Concentration) were shown the same results of risk assessment between EU AOELs and Korean AOELs. As a result, it was considered that AOELs including oral absorption was possible to be used for operator risk assessment. It was considered operator risk assessment with AOEL including oral absorption was more like real assessment method, and improvement of assessment was needed for application to evaluate pesticides in registration.

  • Quantitative and qualitative surveys were conducted during scuba diving activity between May 2017 and June 2017 in order to analyze spring marine algal community structure at Gijang-gun, Busan. A total of 121 species including 13 (10.7 %) Chlorophyta, 18 (14.9 %) Phaeophyta, and 90 (74.4 %) Rhodophyta were identified. The species count in the subtidal zone (110 species) was twice as high as in the intertidal zone (56 species). In terms of regions, 88,76, and 75 species were found in Mundong, Gongsu, and Daebyeon, respectively. Mean biomass for the whole study area was 1,501.5 g·m-2, higher for the subtidal zone (1,869.4 g·m-2) than for the intertidal zone (1,133.5 g·m-2). Mean biomass by region values were 2,234.0 g·m-2, 1,228.1 g·m-2, and 1,044.4 g·m-2 for Mundong, Gongsu, and Daebyeon, respectively. Ulva australis, Sargassum thunbergii, and Corallina pilulifera had high biomass in the intertidal zone, while Sargassum macrocarpum, Undaria pinnatifida, and Phycodrys fimbriata had high biomass in the subtidal zone. The composition ratio of marine algal functional forms suggests that the entire study area had 'Low' status, and all sites fell within 'Low' to 'Moderate' status. Results indicate that opportunistic species are widely distributed throughout the Gijang-gun coast, hence potential effects of disturbance and environmental pollution should be considered. Seaweed farming in particular - a major activity along the Gijang-gun coast -, together with effects of other anthropogenic activities such as national fishing port development and the coastal improvement project, could have long-term effects on marine algal communities. Therefore, long-term monitoring and management plans for marine algal communities will be required.
