  • Objective: To investigate whether the Minimal Clinically Important Difference (MCID) clinically defines improvement of Berg Balance Scale (BBS) scores in people with acute stroke in response to rehabilitation. Design: Retrospective study. Methods: Seventy-three participants with acute stroke participated in the study. Balance evaluation was performed using the BBS. All patients received rehabilitation with physical therapy for 4 weeks, 5 times a week, for 2 hours and 20 minutes a day. An anchor-based approach using the clinical global impression was used to determine the MCID of the BBS. The MCID was used to define the minimum change in the BBS total score (postintervention-preintervention) that was needed to perceive at least a 3-point improvement on the global rating of change. Receiver operating characteristic (ROC) curves was used to define the cut-off values of the optimal MCID of the BBS in order to discriminate between improvement and no improvement groups. Results: The optimal MCID cut-off point for the BBS change scores was 12.5 points for males with a sensitivity (Sn) of 0.62 and a specificity (Sp) of 0.89, and 12.5 points for females with a Sn of 0.69 and Sp of 0.85. The area under the curve of the ROC curve for all participants were 0.84 (95% confidence interval [CI], 0.72; 0.95, p<0.001), and 0.89 (95% CI, 0.77; 1.00, p<0.001), respectively. Conclusions: The MCID for improvement in balance as measured by the BBS was 13.5 points, indicating that the MCID does clinically detect changes in balance abilities in persons with stroke.

  • Purpose: It is estimated that the odontogenic keratocyst (OKC) makes up 10% to 12% of all developmen-tal odontogenic cysts. The lesion has been of particular interest because of its specific histopathologic fea-ture, high recurrence rate, and aggressive behavior. Materials and Methods: We investigated 266 OKCs of Korean patients for the sex of patient, the age of the patient, the location of OKC, the recurrence rate related to radiographic impression. Results: The male-to-female ratio was 1.47:1, showing a slight male predilection. Odontogenic keratocysts had a peak of occurrence in the third decade of life. The mandibular angle and ascending ramus area (49.6%) is the most frequent site of OKCs in the jaws. Fourteen cases of unilocular (12%) and 5 cases of multilocular (20%) OKCs recurred. Thirteen cases of smooth (12.9%) and 6 cases of lobulated (14.6%) OKCs recurred. Seventeen cases of OKCs without perforation of cortical bone (12.5%) and 2 cases of OKCs with perforation of cortical bone (33.3%) recurred. Fifteen people of patients with single lesion (12.2%) and 4 people of patients with multiple lesions (66.7%) recurred. Conclusion: In this resul, we consider multiple odontogenic keratocysts can recur more easily. So we have to treat them more carefully and need long-time follow-ups.

  • Objectives:This study was designed to investigate the characteristics of personality changes and emotional distress using the Personality Assessment Inventory (PAI) in patients with traumatic brain injury(TBI), divided into mild (MTBI) and severe (STBI) groups according to the severity of injury. Methods:The subjects were consisted of 25 patients with MTBI, 25 patients with STBI, and 25 normal controls. They were interviewed with the PAI. The data were analyzed by ${\chi}^2$ test, analysis of variance and Tukey test. Results:The results were the followings. First, Negative Impression in validity scales was elevated above cutoff point(T score 70) in both MTBI and STBI groups. Second, the clinical scales of which scores elevated above the cutoff point were Somatic Complaints, Anxiety, Anxiety-Related Disorders, Depression, and Schizophrenia in the MTBI, and Somatic Complaints and Depression in the STBI. Third, the clinical subscales above the cutoff point were Conversion, Somatization, Health Concerns, Affective Anxiety, Physiological Anxiety, Traumatic Stress, Cognitive Depression, Affective Depression, Physiological Depression, Thought Disorder, and Affective Instability in the MTBI, and Health Concerns, Cognitive Depression, Affective Depression, and Physiological Depression in the STBI. Fourth, Suicide Ideation in treatment scales was the only scale above the cutoff point in the MTBI and the others of the treatment and interpersonal scales in the MTBI and all of these scales of the STBI were not elevated above the cutoff point. Fifth, the scales of which scores showed significant difference between the MTBI and the STBI were Somatic Complaints, Anxiety, Depression, and Suicide Ideation, the subscales were Conversion, Somatization, Health Concerns, Affective Anxiety, Physiological Anxiety, Physiological Depression, and Psychotic Experiences. Conclusion:These results suggest that the patients with MTBI had more somatic and anxiety symptoms, depressed mood, and suicidal ideation than the patients with STBI. These characteristics are generally consistent with clinical observation and findings from previous studies of the patients with TBI, and the PAI seems to be a beneficial adjunctive assessment tool for the evaluation of patients with traumatic brain injury.

  • PURPOSE. The trueness and precision of acquired images of intraoral digital scanners could be influenced by restoration type, preparation outline form, scanning technology and the application of power. The aim of this study is to perform the comparative evaluation of the 3-dimensional reproducibility of intraoral scanners (IOSs). MATERIALS AND METHODS. The phantom containing five prepared teeth was scanned by the reference scanner (Dental Wings) and 5 test IOSs (E4D dentist, Fastscan, iTero, Trios and Zfx Intrascan). The acquired images of the scanner groups were compared with the image from the reference scanner (trueness) and within each scanner groups (precision). Statistical analysis was performed using independent two-samples t-test and analysis of variance (${\alpha}=.05$). RESULTS. The average deviations of trueness and precision of Fastscan, iTero and Trios were significantly lower than the other scanners. According to the restoration type, significantly higher trueness was observed in crown and inlay than in bridge. However, no significant difference was observed among four sites of preparation outline form. If compared by the characteristics of IOS, high trueness was observed in the group adopting the active triangulation and using powder. However, there was no significant difference between the still image acquisition and video acquisition groups. CONCLUSION. Except for two intraoral scanners, Fastscan, iTero and Trios displayed comparable levels of trueness and precision values in tested phantom model. Difference in trueness was observed depending on the restoration type, the preparation outline form and characteristics of IOS, which should be taken into consideration when the intraoral scanning data are utilized.

  • Objective: This study was conducted to evaluate payer-driven medication adherence intervention program from the patient's and counselor's perspectives. Methods: Target patients for intervention were selected by retrospective adherence measures based on national health insurance claims data for hypertension, diabetes and hyperlipidemia. As a serial intervention for higher risk groups of medication non-adherence, initial direct mailing, the first direct telephone call and the second direct call or a home visit were followed. Interview approach to qualitative inquiry was used to evaluate intervention results. Results: Participants including 4 patients received telephone calls, and 4 National Health Insurance Service staff and 4 pharmacists participated as counselors were interviewed regarding their impression of the intervention program. Three major themes arose: overall perception; necessities; and suggestions for success, of the intervention. Despite short period of intervention, educational intervention by telephone counseling involving pharmacists shows potential to improve self-management of chronic disease, and pharmacist-involvement. But more sophisticated selection of target patients requiring the intervention and complementation of electronic database system would be necessary. In addition, personal disposition of counselor was revealed to be an important factor for achieving successful outcome of intervention. Conclusion: The findings suggest that the individualized counseling intervention would be an efficient option for improved medication adherence. Further researches should include longer periods of interventions, a quantitative analysis using adherence measures based on claims data and consideration of clinical benefits associated with the intervention.

  • The common errors in preoperative treatment plan for the orthognathic surgery can be occurred during cast impression, cast mounting procedure with face-bow transfer, surgical stent fabrication, and so on. One of the most common errors exists during mounting process of the model on the articulator. Accurate mounting of dental casts to articulator should be achieved by transferring the 3-dimensional spatial relationship of the maxillary arch to an articulator. A face-bow is used for transfer this relationship to articulator, usually by relating the face-bow to a plane of reference of maxillary cast. The purpose of this study is evaluation of the accuracy of face-bow transferring of maxillary model to the articulator. The maxillary casts of thirty patients for orthognathic surgery were mounted on articulator with an face-bow instrument. The relationship of occlusal plane angle to Frankfort horizontal plane relations were compared the cephalogram with the cast-mounted articulator. As a result of this study, the significant difference between the maxillary occlusal planes angle in the cephalogram and articulator were found. The results were followed, 1. The mean occlusal plane angle in cast-mounted articulator was $13.5^{\circ}\;(SD{\pm}5.4)$. 2. The mean occlusal plane angle in cephalogram was $10.4^{\circ}\;(SD{\pm}4.3)$. 3. The mean difference of occlusal plane angle between cast-mounted articulator and cephalogram was $3.3^{\circ}\;(SD{\pm}4.6)$. According to the result, we should suggest that the occlusal plane angle to Frankfort plane in cast-mounted articulator is more steeper than that of cephalogram. And then, maxillofacial surgeon should try to get a more predictable result by suggesting the proper correction method and mounting the cast accurately.

  • PURPOSE. To evaluate the reproducibility of scan-based abutments using a blue light model scanner. MATERIALS AND METHODS. A wax cast abutment die was fabricated, and a silicone impression was prepared using a silicone material. Nine study dies were constructed using the prepared duplicable silicone, and the first was used as a reference. These dies were classified into three groups and scanned using a blue light model scanner. The first three-dimensional (3D) data set was obtained by scanning eight dies separately in the first group. The second 3D data set was acquired when four dies were placed together in the scanner and scanned twice in the second group. Finally, the third 3D data set was obtained when eight dies were placed together in the scanner and scanned once. These data were then used to define the data value using third-dimension software. All the data were then analyzed using the non-parametric Kruskal-Wallis H test (${\alpha}=.05$) and the post-hoc Mann-Whitney U-test with Bonferroni's correction (${\alpha}=.017$). RESULTS. The means and standard deviations of the eight dies together were larger than those of the four dies together and of the individual die. Moreover, significant differences were observed among the three groups (P<.05). CONCLUSION. With larger numbers of abutments scanned together, the scan becomes more inaccurate and loses reproducibility. Therefore, scans of smaller numbers of abutments are recommended to ensure better results.

  • Purpose: This in vitro study was conducted to evaluate the marginal and internal fit of three-unit Co-Cr frameworks fabricated by computer-aided metal milling and direct metal laser sintering(DMLS) systems in comparison to conventional casting method. Methods: Three-unit Co-Cr frameworks were fabricated by conventional wax up with casting(CWC), computer-aided metal milling(MM) and direct metal laser sintering(DMLS)(n=10 each). The marginal and internal fit of specimens were examined using a light-body silicone impression material. The thickness of light-body silicone was measured at eight reference points each, divided in the mesio distal and bucco lingual directions. All measurements were conducted by a stereomicroscope. Digital photos were taken at 150× magnification and then analyzed using a measurement software. The Kruskal-Wallis test and Bonferroni correction were used for analyzing the results. Results: The mean(SD) is ㎛ for fabrication methods, the mean marginal fit were recorded respectively, DMLS 39(27), followed by CWC 63(38), MM 220(128). and the mean internal fit CWC 95(47), DMLS 116(49), MM 210(152). In addition, the largest gap was found in the occlusal surface area among the internal measurement areas of all groups. Conclusion: As a result, the direct metal laser sintering method showed better marginal and internal fit than the metal milling method. The marginal and internal fit were statistically different according to the three fabrication methods(p<0.001). Except the MM group, the marginal fit of the CWC and DMLS groups was below the clinical standard of 120 ㎛. Based on the results of this study, it can be applied to clinical use in the future.
