부분 무치악의 고정성 임플랜트 보철의 저위교합에 관한 3차원 유한요소법적 연구 (3-DIMENSIONAL FINITE ELEMENT ANALYSIS ON THE INFRAOCCLUSION OF FIXED IMPLANT PROSTHESIS FOR PARTIAL EDENTULISM)

  • The purpose of this study was to examine, by the method of 3-dimentional finite element analysis. how infraocclusion affected the stress distribution in surrounding bone and osseointegrated prosthesis. The 3-dimentional finite element mandibular models were made, in which the first and second molars were removed and the two osseointegrated implants were placed in the first and second molar sites and implant supported fixed prostheses were constructed. Analysis of equivalent stress and displacement induced by strong occlusion or infraocclusion was performed under vertical or inclined distributed loads. The results were as follows; 1. Under vertical load of 50N or 500N, the model in which infraocclusion had not been allowed showed greater stress on implants and the supporting bone than on natural teeth. 2. In the model in which infraocclusion of $30{\mu}m$ had been allowed, implant-prosthesis on the molars had no contact with opposing teeth under vertical load of 50N, However with the same allowed infraocclusion and the model under vertical load of 500N, implant prosthesis on the second molar had contact with opposing teeth, and stress distribution occured properly on natural teeth and implants. 3. Under $45^{\circ}$ inclined load, the model in which infraocclusion had not been allowed showed greater stress on implants and the supporting bone than on natural teeth. There was greater stress in the case of $45^{\circ}$ inclined load than in the case of vertical load. 4. Under $45^{\circ}$ inclined load of 50N or 500N, the model in which infraocclusion of $30{\mu}m$, had been allowed showed no occlusal contact on the implants and occlusal contact on the natural teeth. 5. In partially edentulous cases with implant supported prosthesis, we can prevent excessive load on implants by allowing infraocclusion.

골유착성 임프란트에 의해 지지되는 새로운 설계의 Hybrid Prosthesis (Hybrid Prosthesis Supported by Osseointegrated Implants in Maxilla)

  • A fixed bridge is preferred as a prosthetic option supported by oral implants. However, it is very difficult to re tore edentulous maxilla with fixed prosthesis in cases with improper position and angulation of fixtures, abnormal jaw relation, and need for proper lip support. Six Br${\aa}$nemark implants were installed in edentulous maxilla opposing mandible with natural dentition. A removable hybrid prosthesis attached to a bar milled by 6 degrees was therefore designed to overcome such disadvantages of fixed prosthesis. Lateral stabilization of removable prosthesis was obtained by framework closely fitting the milled bar. Support for the prosthesis was ensured by three elevated areas on the bar. Two precision attachments(CEKA REVAX) provided appropriate direct and indirect retention without influencing support. A clinical and laboratory procedure was presented.

레이저 적층 제조 기술을 이용한 상악 무치악 환자의 임플란트 고정성 보철 수복 증례 (Implant-supported prosthetic rehabilitation for the edentulous maxilla using the additive manufacturing technology: A case report)

  • 직접식 금속 레이저 소결 방식 시스템은 기공 오차 및 주조 결함 없이 제작 비용 및 제조 시간 단축을 장점으로 전악 임플란트 지지 고정성 보철물 제작에 유용하다. 본 증례에서는 직접식 금속 레이저 소결 방식과 치과용 캐드캠으로 제작된 단일구조 지르코니아 크라운을 사용하여 무치악 상악에서 임플란트 지지 고정성 치과 보철물의 성공적인 결과를 보고하고 임상적 의의를 평가하였다. 51세의 건강한 중년 여성이 상악 전 치아의 동요도를 주소로 서울대학교 치과병원에 내원하였다. 모든 상악 치아 발치 후 직접식 금속 레이저 소결 기술 및 캐드캠 단일구조 지르코니아 크라운을 사용하여 코발트-크롬 프레임을 포함하는 임플란트 지지 고정성 치과 보철물을 제작하였다. 최종 수복물 장착 6개월 후, 기계적 및 생물학적 합병증은 발견되지 않았고 보철물은 기능 및 심미적으로 만족스러운 결과를 보였다. 직접식 금속 레이저 소결 시스템을 이용하여 추가적인 기공 작업 없이 임플란트 지지 고정성 보철물을 제작하여 제조 시간을 단축하고 신뢰할 수 있는 정확도와 적합성을 얻었다.

골유착성 임프란트와 자연치를 이용한 고정성 국소의치에서 응력분산 및 충격흡수에 관한 유한요소법적 응력분석 (A FINITE ELEMENT STRESS ANALYSIS OF THE STRESS DISTRIBUTION AND THE SHOCK ABSORPTION IN AN OSSEOINTEGRATED IMPLANT-NATURAL TOOTH SUPPORTED FIXED PARTIAL DENTURE)

  • The long-term success of any dental implant is dependent upon the optimization of stresses which occur during oral function and parafunction. Especially, it has been suggested that there is an unique set of problems associated with joining an osseointegrated implant and a natural tooth with a fixed partial denture. For this particular case, although many literatures suggest different ways to avoid high stress concentrations on the bone surrounding the implant under static and dynamic loading conditions, but few studies on the biomechanical efficacy of each assertion have been reported. The purpose of this investigation was to evaluate the efficacies of clinically suggested methods on stress distribution under static load and shock absorption under dynamic load, using two dimensional finite element method. In FEM models of osseointegrated implant-natural tooth supported fixed partial dentures, calculations were made on the stresses in surrounding bone and on the deflections of abutments and superstructure, first, to compare the difference in stress distribution effects under static load by the flexure of fastening screw or prosthesis, or intramobile connector, and second, to compare the difference in the shock absorption effects under dynamic load by intramobile connector or occlusal veneering with composite resin. The results of this analysis suggest that : 1. Under static load condition, using an implant design with fastenign screw connecting implant abutment and prosthesis or increasing the flexibility of fastening screw, or increasing the flexibility of prosthesis led to the .increase in height of peak stresses in cortical bone surrounding the implant, and has little effect on stress change in bone around the natural tooth. 2. Under static load condition, intramobile connector caused the substantial decrease in stress concentration in cortical bone surrounding the implant and the slight increase in stress in bone around the natural tooth. 3. Under dynamic load condition, both intramobile connector and composite resin veneering showed shock absorption effect on bone surrounding the implant and composite resin veneering had a greater shock absorption effect than intramobile connector.

Effect of repetitive firing on passive fit of metal substructure produced by the laser sintering in implant-supported fixed prosthesis

  • PURPOSE. The aim of the present study was to investigate the passive fit of metal substructure after repetitive firing processes in implant-supposed prosthesis. MATERIALS AND METHODS. Five implants (4 mm diameter and 10 mm length) were placed into the resin-based mandibular model and 1-piece of screw-retained metal substructure was produced with the direct metal laser sintering (DMSL) method using Co-Cr compound (n = 10). The distance between the marked points on the multiunit supports and the marginal end of the substructure was measured using a scanning electron microscope (SEM) at each stage (metal, opaque, dentin, and glaze). 15 measurements were taken from each prosthesis, and 150 measurements from 10 samples were obtained. In total, 600 measurements were carried out at 4 stages. One-way ANOVA test was used for statistical evaluation of the data. RESULTS. When the obtained marginal range values were examined, differences between groups were found to be statistically significant (P<.001). The lowest values were found in the metal stage (172.4 ± 76.5 ㎛) and the highest values (238.03 ± 118.92 ㎛) were determined after glaze application. When the interval values for groups are compared with pairs, the differences between metal with dentin, metal with glaze, opaque with dentin, opaque with glaze, and dentin with glaze were found to be significant (P<.05), whereas the difference between opaque with metal was found to be insignificant (P=.992). CONCLUSION. Passive fit of 1-piece designed implant-retained fixed prosthesis that is supported by multiple implants is negatively affected by repetitive firing processes.

임플란트 즉시 식립에 의한 연속된 상악 전치부의 심미적 수복 증례 (Esthetic restoration in continuous maxillary anterior area using immediate implant placement: A case report)

  • 상악 전치부를 발치 해야 하는 경우, 즉시 식립을 통한 임플란트 고정성 보철물의 수복을 고려할 수 있다. 이 경우, 수술 횟수 및 치료기간의 감소, 흡수가 일어나지 않은 잔존 골상에 식립이 가능하다는 장점이 있으나, 연속된 상악 전치부인 경우 임플란트와 임플란트간 거리에 대한 고려 및 예지성 있는 치료를 위한 정확한 진단 및 치료계획의 수립이 추가적으로 고려되어야 한다. 본 증례에서는 63세 남환의 잔존치근을 발거 후 연장된 상악 전치부에 골이식을 동반한 2개의 임플란트 즉시 식립 및 임시 보철물을 장착 후, 임플란트 고정성 보철물로 수복하여 심미적, 기능적으로 만족할만한 임상 결과를 보였기에 이를 보고하는 바이다.

Three-dimensional finite element analysis of the splinted implant prosthesis in a reconstructed mandible

  • PURPOSE. The purpose of this study was to analyze the effects of the splinted implant prosthesis in a reconstructed mandible using three-dimensional finite element analysis. MATERIALS AND METHODS. Three-dimensional finite element models were generated from a patient's computed tomography data. The patient had undergone partial resection of the mandible that covered the area from the left canine to the right condyle. The mandible was reconstructed using a fibula bone graft and dental implants. The left mandibular premolars and molars remained intact. Three types of models were created. The implant-supported prosthesis was splinted and segmented into two or three pieces. Each of these models was further subcategorized into two situations to compare the stress distribution around normal teeth and implants. Oblique loading of 300 N was applied on both sides of the mandible unilaterally. The maximum von Mises stress and displacement of the models were analyzed. RESULTS. The stress distribution of the natural mandible was more uniform than that of the reconstructed fibula. When the loading was applied to the implant prosthesis of reconstructed fibula, stress was concentrated at the cortical bone around the neck of the implants. The three-piece prosthesis model showed less uniform stress distribution compared to the others. Displacement of the components was positively correlated with the distance from areas of muscle attachment. The three-piece prosthesis model showed the greatest displacement. CONCLUSION. The splinted implant prosthesis showed a more favorable stress distribution and less displacement than the separated models in the reconstructed mandible.

상악 완전 무치악 환자의 CAD/CAM 을 이용한 임플란트 식립(NobelGuide$^{TM}$) 및 즉시하중 후 고정성 보철수복 증례 (Implant Supported Fixed Restoration for Maxillary Edentulism using CAD/CAM Guided Implant Surgery (NobelGuide$^{TM}$) and Immediate Loading)

  • NobelGuide$^{TM}$ 임플란트 시스템은 골량과 골질을 수술 전에 판단하여 점막을 젖히지 않고 미리 계획된 스텐트와 보철물을 이용하여 즉시하중을 부여하는 CAD/CAM 기반의 임플란트 보철 치료방법이다. 환자의 불편감을 최소화하여 미리 예측 가능한 위치에 임플란트를 식립함으로써 술 후 합병증 및 보철과정을 간소화 할 수 있는 장점이 있다. 본 증례의 환자는 56세 남성으로 상,하악 부분 무치악 상태였으며, 상악전치부 보철물 탈락과 하악 전치가 흔들린다는 주소로 내원하였다. 이에 상악 잔존치 모두와 하악 4전치 발거 및 치주치료를 실시한 뒤 임시 의치를 6개월간 사용하여 발치와 치유 및 의치 적응여부를 평가하였다. 골량은 충분하다고 판단되었고 환자의 협조도와 경제적인 여건, 전신건강 등 여러 사항이 양호하여 CAD/CAM 기반의 수술템플렛을 이용한 무판막 수술을 통해, 미리 제작된 임시 고정성 보철물로 즉시하중을 부여하는 상악 NobelGuide$^{TM}$ 임플란트 시스템을 계획하였다. 사용중인 임시의치를 이용하여 제작한 방사선 스텐트로 CT를 촬영하고, computer based planning (Procera$^{(R)}$ software)을 통해 3차원적인 골형상과 각 부위별 단면상을 참고하여 상악에 8개의 임플란트 식립체를 생역학적 관점 및 골조건을 고려하여 분산 배치하였다. 미리 제작된 임시 고정성 보철물을 임플란트 수술 후 즉시 장착 및 하중을 부여하여 기능하도록 하였으며 6개월 후 골유착 정도를 평가한 뒤 금속구조물을 이용한 최종 보철물을 제작하였다. 하악의 경우 전치부 도재전장주조금속관과 우측 구치부 임플란트 보철물 제작을 통해 상, 하악 치료를 마무리하였다.

An evaluation of the stress effect of different occlusion concepts on hybrid abutment and implant supported monolithic zirconia fixed prosthesis: A finite element analysis

  • PURPOSE. The aim of this study is to evaluate the effects of canine guidance occlusion and group function occlusion on the degree of stress to the bone, implants, abutments, and crowns using finite element analysis (FEA). MATERIALS AND METHODS. This study included the implant-prosthesis system of a three-unit bridge made of monolithic zirconia and hybrid abutments. Three-dimensional (3D) models of a bone-level implant system and a titanium base abutment were created using the original implant components. Two titanium implants, measuring 4 × 11 mm each, were selected. The loads were applied in two oblique directions of 15° and 30° under two occlusal movement conditions. In the canine guidance condition, loads (100 N) were applied to the canine crown only. In the group function condition, loads were applied to all three teeth. In this loading, a force of 100 N was applied to the canine, and 200-N forces were applied to each premolar. The stress distribution among all the components of the implant-bridge system was assessed using ANSYS SpaceClaim 2020 R2 software and finite element analysis. RESULTS. Maximum stress was found in the group function occlusion. The maximum stress increased with an increase in the angle of occlusal force. CONCLUSION. The canine guidance occlusion with monolithic zirconia crown materials is promising for implant-supported prostheses in the canine and premolar areas.

A retrospective comparison of clinical outcomes of implant restorations for posterior edentulous area: 3-unit bridge supported by 2 implants vs 3 splinted implant-supported crowns

  • PURPOSE. To compare the clinical outcomes of two types of implant restoration for posterior edentulous area, 3-unit bridge supported by 2 implants and 3 implant-supported splinted crowns. MATERIALS AND METHODS. The data included 127 implant-supported fixed restorations in 85 patients: 37 restorations of 3-unit bridge supported by 2 implants (2-IB), 37 restorations of 3 implant-supported splinted crowns (3-IC), and 53 single restorations (S) as controls. Peri-implantitis and mechanical complications that occurred for 14 years were analyzed by multivariable Cox regression model. Kaplan-Meier curves and the multivariable Cox regression model were used to analyze the success and survival of implants. RESULTS. Peri-implantitis occurred in 28.4% of 2-IB group, 37.8% of 3-IC group, and 28.3% of S control group with no significant difference. According to the implant position, middle implants (P2) of the 3-IC group had the highest risk of peri-implantitis. The 3-IC group showed a lower mechanical complication rate (7.2%) than the 2-IB (16.2%) and S control group (20.8%). The cumulative success rate was 52.8% in S (control) group, 62.2% in 2-IB group, and 60.4% in 3-IC group. The cumulative survival rate was 98.1% in S (control) group, 98.6% in 2-IB group, and 95.5% in 3-IC group. There was no significant difference in the success and survival rate according to the restoration type. CONCLUSION. The restoration type was not associated with the success and survival of implants. The risk of mechanical complications was reduced in 3 implant-supported splinted crowns. However, the middle implants of the 3 implant-supported splinted crowns had a higher risk of peri-implantitis.
