THE ERUPTION GUIDANCE OF AN IMPACTED DILACERATED MAXILLARY CENTRAL INCISOR (변위 매복된 상악 중절치의 맹출유도)

  • Tooth impaction is defined as a cessation of the eruption of a tooth at the level of the oral mucosa or alveolar bone by any causes. Any tooth in the dental arch can be impacted, but the teeth frequently involved in a descending order are the mandibular and maxillary third molars, the maxillary canines, the mandibular and maxillary second premolars, and the maxillary central incisors. In these teeth, impaction of maxillary incisor occurs in about 0.1-0.5% and major causes are trauma, supernumerary teeth and periapical inflammation of primary maxillary incisor. Delayed eruption of a maxillary central incisor results in midline shift, the space's being occupied by an adjacent tooth and different levels of alveolar height. Treatment options are observation, surgical intervention, surgical exposure and orthodontic traction, transplantation and extraction. These cases were about the patients with delayed eruption of maxillary central incisor. We surgically exposed impacted tooth and guided it into normal position by the orthodontic traction. At the completion of traction, the maxillary central incisor was positoned fairly within the arch and complications such as root resorption were not observed.

  • The term 'impaction' is used to designate a tooth which remains unerupted in the jaw beyond the time at which it should normally be erupted. The main causal factors are local (lack of space, ectopic positions of teeth, supernumerary teeth, cyst, the occurrence of infectious process in the eruption path, traumatic facial injury etc.). Systemic and genetic disorders, however, may have primary failure of eruption and retarded eruption as additional symptoms (cleidocranial dysplasia, osteopetrosis etc.). Most cases of impacted teeth reported in the literature are of permanent teeth. The absence of primary teeth occur rarely whereas impaction of second primary molars is more numerous than all other impactions. Impaction due to primary failure of eruption must be distinguished from the secondary infraocclusion. The etiology of impaction of primary teeth is probably related to early ankylosis of primary teeth, but it is not clear. Failure of eruption of primary teeth may cause a number of complications, such as interference with development and eruption of succedaneous teeth, formation of cyst, and damage to adjacent teeth. This study is to report cases of primary failure of eruption in the primary dentition.

Gardner syndrome associated with multiple osteomas, intestinal polyposis, and epidermoid cysts

  • Gardner syndrome is known as a variant of familial adenomatous polyposis. This syndrome is characterized by multiple intestinal polyposes, osteomas, and epidermoid cysts. In addition, dental abnormalities include an increased frequency of multiple odontomas, as well as supernumerary and impacted teeth. The authors report the case of a 7-year-old male patient with Gardner syndrome. Radiographic findings revealed multiple osteomas in both sides of the maxilla, multiple diffuse enostoses in both jaws, and a complex odontoma in the left mandibular body. Two years later, multiple epidermoid cysts on the scalp were found. Since this patient was suspected to have Gardner syndrome, the authors recommended gastrointestinal endoscopy to check for intestinal polyposis. Gastrointestinal endoscopic examination revealed multiple polyposes in the upper gastrointestinal tract and fundus of the stomach. As a result, the final diagnosis was Gardner syndrome.

Association of maxillary dental developmental abnormality with precocious puberty: a case-control study

  • Background: Dental studies of precocious puberty have focused on examination of jaw and dentition growth. The aim of the study was to analyze the relationship between precocious puberty and maxillary dental developmental abnormalities (DDAs). Methods: This retrospective study was conducted on the Korean patients in whom dental panoramic and hand-wrist radiographs had been taken before they were 15 years of age. The maxillary DDAs were assessed as mesiodens, congenital missing teeth, peg-shape lateral incisors, or impacted teeth. The chronological ages of the control group members were within the normal range of the hand-wrist bone age. Others with a peak luteinizing hormone of ≥ 5 and < 5 IU/L were allocated to central precocious puberty (CPP) and peripheral precocious puberty (PPP), respectively. Results: Of the enrolled 270 patients, 195, 52, and 23 were allocated to the control, CPP, and PPP groups, respectively. The maxillary DDAs were significantly more prevalent in the CPP group than in the other groups. Among those with maxillary DDA, the mesiodens predominated. Age- and sex-adjusted multivariate analysis revealed maxillary DDA (odds ratio, 3.36; 95% CI, 1.60-7.05) and especially mesiodens (odds ratio, 5.52; CI, 2.29-13.28) to be significantly associated with CPP. Conclusions: Maxillary DDAs were significantly more prevalent in the CPP group than in the PPP or control groups. Among the many types of maxillary DDAs, mesiodens was significantly associated with CPP and may be considered a predictor of the development of CPP.

Relationship with Passage Time of Human Dental Pulp Stem Cells from Supernumerary Tooth by Classification (과잉치 분류에 따른 치수유래줄기세포 계대 배양 시간의 연관성)

  • For this research 20 supernumerary teeth impacted in the maxillary anterior have been extracted and pulp cells have been collected from them. From the collected pulp cells, total of 17 (10 males, 7 females) have been selected as subjects. From this research, the run-time of successive culture of the cell from tooth number pulp tissue was $2.91{\pm}0.29$ days. From the gathering of cells from the initial pulp tissue until gaining 80% confluency took $4.53{\pm}0.94$ which was the longest. The following successive cultures took $2.73{\pm}0.32$ days. Average runtime for female was $2.81{\pm}0.27$ days whereas male had average runtime of $2.98{\pm}0.29$ days. Average run-time for inversion was $2.94{\pm}0.30$ days and for normal location, $2.80{\pm}0.20$ days. Average runtime was $2.92{\pm}0.31$ days and other forms took $2.88{\pm}0.22$ days. In the future, follow up research would be needed to evaluate the efficiency of the cells collected from the initial passage and the latter passage as stem-cells and taking into consideration the less than 3 days'time for the subculture, it could be concluded that the research efficiency and fast cultivation would be sufficiently effective.

AUTOTRANSPLANTATION OF IMPACTED MAXILLARY CANINES (자가이식을 이용한 상악 매복 견치의 치료)

  • Maxillary canine impaction is a frequently encountered clinical problem, and it may cause the resorption of adjacent tooth or cystic change. Treatment plan for maxillary canine impaction should be decided among extraction, orthodontic traction and autotransplantation according to several factors such as direction and position of unerupted tooth, degree of developing root apex, eruption space, exsitance of supernumerary tooth, odontoma, or cyst. Autotransplantation is a valuable alternative to extraction of impacted teeth, where surgical exposure and subsequent orthodontic realignment are difficult or impossible due to unfavorable impaction position. And its prognosis is dependent on a number of factors such as preservation of periodontal ligament, degree of root development, surgical technique, patient's age, endodontic treatment, time and type of splinting and storage medium, etc. The patients in these cases visited our dental clinic in the late permanent dentition with the chief complaint of unerupted maxillary canines. And it was thought that the spontaneous eruption guidance or orthodontic traction and alignment were difficult because of its unfavorable impacted position. Therefore, autotransplantaion and endodontic treatment were done and have been checked periodically until now.

Management of Maxillary Impacted Canines (매복 상악 견치의 처치)

  • The canine tooth is important both functionally and aesthetically, being positioned between the anterior and posterior teeth. The upper canine has the longest eruption path, forming in the deepest part of the maxillary bone and often experiencing eruption disorders, leading to significant aesthetic and functional issues. Early detection and management of canine impaction are crucial in pediatric dentistry, which focuses on tooth growth and eruption. The prevalence of maxillary canine impaction ranges from 1.1% to 3.0%. In Western populations, palatal impaction is twice as common as labial impaction, while some Korean studies report more labial impaction. Maxillary canine impaction occurs more frequently in women and is associated with various factors such as structural obstacles, pathological conditions in surrounding tissues, developmental abnormalities, and genetic factors. Labial displaced canines are linked to narrow maxillary intercanine width, total dental crowding, and skeletal Class III malocclusion. Maxillary palatal displaced canine impaction is more prevalent in patients with familial agenesis of lateral incisors or conical supernumerary lateral incisors. Understanding these factors aids in early diagnosis and appropriate intervention for canine tooth impaction, ensuring optimal oral health and aesthetics.

THE EVALUATION OF THE PATIENTS TAKING CBCT IN DEPARTMENT OF PEDIATRIC DENTISTRY (소아치과에서 Cone beam형 전산화단층영상을 이용한 환자의 평가)

  • Cone beam computed tomography (CBCT) has become widely available in recent years and is recognized as an important diagnostic tool for varies disease and condition of the orofacial structure. Clinician is easy to determine adequate treatment plan for pediatric patients by using CBCT. CBCT is used in Chonbuk National University Dental Hospital since 2005. This research presents clinical application of CBCT on patients visiting department of pediatric dentistry in Chonbuk National University Dental Hospital from Jan, 2005 to July, 2011. 1. Total number of patients taken CBCT is 252, and total number of area taken CBCT is 279. 2. An age group form 9 years to 12 years showing 53% was highest and percentage of 6~8 years showed 24%. 3. Chief complaints for CBCT taking are position and shape of impacted teeth (49.1%), mesiodens (19.4%), supernumerary teeth (7.9%), position and root canal shape of erupting teeth (7.2%), cyst (5.4%), inflammatory lesion (3.9%), odontoma (3.9%), tumor (2.2%), and et al. 4. Treatments are extraction (29.7%), orthodontic traction and leveling (24.0%), follow up (16.5%), refer to other professional part (11.5%), endodontic treatment (3.9%), surgical removal (2.9%), malsupialization (3.9%), enucleation (1.1%), and fail to follow up (5.0%), and et al.

Distribution of the Peg-Laterals and Associated Dental Anomalies in Korean Children: A Radiological Study (상악 왜소측절치와 치아발육장애에 관한 연관성 조사 : 방사선학적 연구)

  • The purpose of this study was to find out prevalence of peg-laterals and to evaluate the frequency of dental anomalies in Korean children with peg-laterals. For this study, panoramic radiographs of 3,278 patients (aged 7 to 15 years) from the Department of Pediatric Dentistry admitted from January, 2008 to January, 2013 were selected for an investigation on peg-laterals distribution. The prevalence of peg-laterals was 2.62% (86 subjects). Among the peg-laterals children, the distribution of associated dental anomalies were as follows: congenitally missing teeth (29.1%), dens invaginatus (19.8%), impacted teeth (12.8%), supernumerary teeth (9.3%), and transposition (4.7%). Due to this study showing frequent occurrences of peg-laterals with other dental anomalies, one suggestion is to consider such relationships before deciding on a diagnosis and treatment plan.

ERUPTION DISTURBANCE OF THE LOWER LEFT FIRST PERMANENT MOLAR CAUSED BY AMELOBLASTIC FIBROMA (법랑모세포섬유종에 의한 하악 제1대구치의 맹출 장애)

  • Impaction is the cessation of eruption process caused by physical obstacles on the eruption pathway, abnormal tooth position, or lack or space. It often occurs in association with supernumerary teeth, odontogenic tumor, or cystic lesions, and ameloblastic fibroma is one of the odontogenic tumors that can cause impaction of teeth. In many cases, ameloblastic fibroma occurs in association with one or more unerupted teeth. The proper management of ameloblastic fibroma is determined between conservative resection or more aggressive block resection, based size and morphologic features of the lesion and age of the patient. This is a case of a 8 year and 6 month old boy whose lower left permanent molar showed eruption disturbance. The impacted tooth was successfully repositioned favorably through surgical exposure and orthodontic traction using a modified halterman appliance. Long term follow-up, longer than 10 years, is planned considering relatively high recurrence rate and possibility of malignant transformation of ameloblastic fibroma, which cause impaction of the lower left permanent molar in this case.
