Modified Nance appliance를 이용한 매복치의 교정치료 (Impacted tooth treatment with modified Nance appliance)

  • Traction of impacted tooth often requires patient compliance to place elastics to surgically exposed impacted tooth. It can be very difficult and time-consuming. The aim of this article was to describe an impacted tooth tractor, Moon's appliance, which was modified from the Nance holding arch appliance. Moon's appliance eliminates patient compliance and generates a light continuous force. We achieved the desired treatment results for impacted tooth using this appliance. This new fixed appliance can be a reasonable alternative to conventional appliances.

Forced eruption을 이용한 매복 전치의 치험례 (CASE REPORT : FORCED ERUPTION FOR IMPACTED ANTERIOR TOOTH)

  • 영구 전치의 매복은 제 3 대구치나 견치에 비해서 드물게 나타나지만 학령기 아동에서 관찰할 수 있다. 매복치아의 원인은 외상, 공간 부족, 치아의 맹출 경로상에 위치하는 과잉치, 유치의 치근단 감염 등 다양하다. 매복된 치아는 부정교합과 인접치아의 치근 흡수, 낭종 등의 병적 변화를 유발할 수 있으므로 임상적 검사와 방사선학적 검사를 통해 정확한 위치를 확인하고, 적절한 치료계획을 통해 정상 맹출을 유도하여야 한다. 매복된 치아가 병적 변화를 일으킨 경우나 인접치에 손상을 주었을 경우에 발치를 시행하게 된다. 그러나 교정적 견인을 통해 기능과 심미성을 회복시켜 줄 수 있으므로 조기 발견과 발견시의 올바른 진단과 치료 계획을 수립하는 것이 중요하다. 교정적 견인시에는 매복치를 배열할 공간의 확보와 고정원에 대한 고려가 선행되어야 한다. 견인시 적절한 방향의 힘을 통해서 치근의 흡수 등의 부작용이 일어나지 않도록 해야 한다. 본 증례는 매복된 전치들을 forced eruption을 통해서 정상적인 맹출 방향으로 유도한 치험례이다.

  • Tooth eruption disturbance is seen commonly in mixed dentition or early permanent dentition. During this period of time, children experience the growth and development of craniofacial skeleton and dentition, so the impaction and eruption disturbance of permanent teeth make many problems in oral and maxillofacial areas. Eruption guidance of impacted tooth is not simple because many factors related to impaction are considered. Several factors such as etiology, tooth development stage, location of impacted teeth or eruption pathway, patient cooperation, etc. need to be taken into consideration in deciding the management of the impacted tooth. In the present study, it was assessed the etiology and impaction status of impacted tooth through various cases, and try to explain the treatment method to guide eruption by surgical exposure.

  • Purpose: An impacted tooth is defined as a tooth that shows delayed eruption and is expected to erupt incompletely by clinical and radiograph examination despite it reaching its expected time of eruption. The aims of this study were to investigate the clinical and radiological characteristics and treatment duration and method of impacted teeth in Korean patients. Materials and Methods: For this study we used clinical records, study models, panoramic radiographs and intraoral photographs of patients who attended Gachon University Gil Hospital Orthodontic Department between 2005 and 2008. There were 164 patients with a total number of 202 impacted teeth. Results: Male patients shows a little more prevalence than female patients (1.13:1). The under 12 age group had the highest prevalence of tooth impaction, and the over 19 age group showed the least prevalence of tooth impaction. The ratio of tooth impaction between the left to right ratio was 1.73:1 and maxilla and mandible was 1.84:1. The impacted teeth were most commonly positioned buccally (76 cases, 41.5%). Full nap closure technique (108 cases, 81.2%)was most frequently used for attachment of surgical traction hooks. Maxillary canine impaction was most commonly encountered both in male and female patients. The mean treatment period was 12.2 months and the success rate of treatment was 90.3%. The canine tooth shows the longest treatment time and highest failure rate. The ankylosis was the major cause of failure. Conclusion: Impacted teeth most commonly show in left side maxilla in the under 12 age group. And it is most commonly positioned buccally. The mean treatment period was 12.2 months, and the success rate of treatment was 90.3%.

교정적 견인을 위한 매복치의 수술적 노출 방법에 관한 증례보고 (Surgical exposure of impacted tooth for orthodontic forced eruption : case report)

  • 치아의 매복은 병리적인 변화, 기능적 결손, 심미적인 문제 등 다양한 문제점을 만든다. 이러한 매복치를 때론 제거하기도 하지만 매복치를 구강내로 노출시켜주는 수술적인 치료와 교정적인 forced eruption을 통해서 향후 발생할 수 있는 많은 문제점들을 해결할 수 있다. 교정적 forced eruption을 위해서 매복치아를 노출시킬 때 매복치아가 치조골 안에 완전하게 매복되어있지 않으면서 단지 연조직에만 덮혀있는 매복양상과 치조골 내부에 완전하게 매복되어있는 두가지의 양상으로 존재한다. 교정정 forced eruption을 위해서 매보되어있는 치아를 수술적으로 노출시키는 방법에관해 다양한 증례와 문헌과 함께 알아보고자 한다.

  • Impaction of tooth is a situation in which an unerupted tooth is wedged against another tooth or teeth or otherwise located so that it cannot erupt normally. The supernumerary tooth is also called as hyperdontia and defined as the condition of having additional tooth to the regular number of teeth. The most common supernumerary tooth is a mesiodens, which is a mal-formed, peg-like tooth that occurs between the maxillary incisors. The supernumerary tooth is commonly impacted but they are frequently impacted on maxilla. Ectopic impaction of supernumerary tooth on mandibular condyle, coronoid process, ascending ramus, and pterygomandibular space is very rare condition. In this case, we report a case of impacted supernumerary tooth on mandibular sigmoid notch without definite pathologic change.

Jaw lesions associated with impacted tooth: A radiographic diagnostic guide

  • This review article aimed to introduce a category of jaw lesions associated with impacted tooth. General search engines and specialized databases such as Google Scholar, PubMed, PubMed Central, MedLine Plus, Science Direct, Scopus, and well-recognized textbooks were used to find relevant studies using keywords such as "jaw lesion", "jaw disease", "impacted tooth", and "unerupted tooth". More than 250 articles were found, of which approximately 80 were broadly relevant to the topic. We ultimately included 47 articles that were closely related to the topic of interest. When the relevant data were compiled, the following 10 lesions were identified as having a relationship with impacted tooth: dentigerous cysts, calcifying odontogenic cysts, unicystic (mural) ameloblastomas, ameloblastomas, ameloblastic fibromas, adenomatoid odontogenic tumors, keratocystic odontogenic tumors, calcifying epithelial odontogenic tumors, ameloblastic fibro-odontomas, and odontomas. When clinicians encounter a lesion associated with an impacted tooth, they should first consider these entities in the differential diagnosis. This will help dental practitioners make more accurate diagnoses and develop better treatment plans based on patients' radiographs.

Pathologic conditions associated with impacted third molars: A retrospective study of panoramic radiographs in a Southern Brazilian population

  • Purpose: This study investigated the prevalence of developmental and acquired pathologic conditions associated with impacted third molars (3Ms) in a Southern Brazilian population and evaluated whether demographic and tooth characteristics were correlated with the presence of bone or tooth lesions. Materials and Methods: Panoramic radiographs were assessed for developmental (bone-related) or acquired (tooth-related) pathoses associated with impacted upper or lower 3Ms. Data on tooth positioning, tooth development, and patient demographics were collected. A trained, calibrated postgraduate student evaluated all images. Binary and multivariate logistic regression models were used to assess associations between outcomes and the demographic and radiographic variables. The threshold for statistical significance was set at 5% (P<0.05). Results: The sample comprised panoramic radiographs from 2054 patients, predominantly female (59.2%), with a mean age of 27.2±11.5 years. Overall, 4066 impacted 3Ms were evaluated, revealing 471 (11.6%) developmental and 710 (17.5%) acquired pathoses. Among the developmental pathoses, 460 (95.2%) were indicative of dentigerous cysts. Male sex, lower 3M location, vertical or distoangular positioning, and incomplete root formation were associated with an elevated likelihood of developmental pathology. Lower tooth position, complete root formation, and partial eruption were linked to an increased probability of an acquired pathology in the third or second molar. Conclusion: The prevalence of pathologic conditions associated with impacted 3Ms was low. Male sex, lower 3M placement, horizontal or distoangular positioning, and incomplete root formation were associated with developmental pathoses, while lower tooth position, complete root formation, and partial eruption were related to acquired pathoses.

변형된 설측호선 장치를 이용한 매복 대구치의 인위적 맹출

  • Patients who have impacted tooth are found commonly in orthodontic treatment. Although it is difficult to find the cause of impacted teeth, the most common, causes are prolonged retention of the deciduous teeth, trauma, aberrant sequence of eruption, lack of space and deficiency of Vitamin D. Impacted teeth may lead to esthetic and functional problems and root resorption of adjacent teeth, so we should treat it as soon as possible. Commonly used treatment method is the following: After surgically uncovering of the impacted teeth, a bond of orthodontic appliance is established, and orthodontic traction is started with a removable or fixed appliance. We used the modified lingual arch with a soldered auxiliary appliance in lingual arch for traction of lower impacted teeth. The modified lingual arch could control the magnitude and direction of the applied force with one-arch treatment, and also could give continuous force to impacted tooth without patient patient cooperation. We achieved good results with the modified lingual arch.

Which factors are associated with difficult surgical extraction of impacted lower third molars?

  • Objectives: The aim of this retrospective study was to investigate factors associated with increased difficulty in the surgical extraction of impacted lower third molars and to improve identification of difficult cases. Materials and Methods: A total of 680 patients who required 762 surgical extractions of impacted lower third molars from 2009 to 2014 were enrolled in the study. Demographic factors, clinical factors, radiographic factors, surgical extraction difficulty, and presumed causes of difficulty were collected. Data were statistically analyzed using IBM SPSS Statistics version 23. Results: Age, sex, depth of impaction, and blurred radiographic image influenced difficulty in surgical extraction. The position of the impacted tooth influenced surgical difficulty, especially when it was accompanied by other factors. Conclusion: It is challenging to design a reliable and practical instrument to predict difficulty in surgical extraction of impacted lower third molars. To identify very difficult cases, root investigation using computed tomography is advised when impacted tooth position suggests difficult extraction.
