중족부에 발생한 Mycobacterium abscessus 골수염 (Mycobacterium abscessus Osteomyelitis in the Mid Foot)

  • Non-tuberculous mycobacterium has a wide-spread occurrence in nature, and skin, soft tissue, bone, lung and disseminated infection can be involved. Non-tuberculous mycobacterium infection occurs both in immunocompetent patients without underlying diseases and in immunocompromised hosts. Non-tuberculous mycobactrial osteomyelitis is a rare cause of granulomatous osteomyelitis, and has been previously reported in the sternum, spine, humerus, femur, tibia or metatarsal. Mycobacterium abscessus osteomyelitis is a very rare infection in the foot and only 1 case has been reported. Authors report a case of Mycobacterium abscessus osteomyelitis involving the tarsal and metatarsal bones in a non-immunocompromized middle aged women.

  • 뮤코르진균증은 비교적 드문 질환으로 면역이 저하된 환자에게 있어서 치명적인 기회감염을 일으킨다. 감염은 주로 혈액암, 만성 신부전, 당뇨 환자나 장기이식을 받은 환자에게 일어난다. 저자들은 조절되지 않은 당뇨를 동반한 신장이식을 받은 환자에서 발생한 폐뮤코르진균증을 암포테리신과 함께 수술적 절제를 통해 성공적으로 치료하였다.

Pseudomonas aeruginosa Biofilm, a Programmed Bacterial Life for Fitness

  • A biofilm is a community of microbes that typically inhabit on surfaces and are encased in an extracellular matrix. Biofilms display very dissimilar characteristics to their planktonic counterparts. Biofilms are ubiquitous in the environment and influence our lives tremendously in both positive and negative ways. Pseudomonas aeruginosa is a bacterium known to produce robust biofilms. P. aeruginosa biofilms cause severe problems in immunocompromised patients, including those with cystic fibrosis or wound infection. Moreover, the unique biofilm properties further complicate the eradication of the biofilm infection, leading to the development of chronic infections. In this review, we discuss the history of biofilm research and general characteristics of bacterial biofilms. Then, distinct features pertaining to each stage of P. aeruginosa biofilm development are highlighted. Furthermore, infections caused by biofilms on their own or in association with other bacterial species (i.e., multispecies biofilms) are discussed in detail.

Primary Central Nervous System Lymphoma in Organ Recipient

  • We report a case of primary central nervous system(CNS) lymphoma in an organ recipient. A 33-years-old man who underwent a renal transplantation 3years previously presented with headache and vomiting. In Brain computed tomography scans and magnetic resonance images showed multiple periventricular cystic rim enhancing masses. Pathologic diagnosis by stereotactic biopsy revealed malignant non-Hodgkins B-cell lymphoma. After pathologic confirmation, methotrexate chemotherapy and whole brain radiation therapy were done. Having experienced such a case, the authors strongly recommend to add primary CNS lymphoma as one of the differential diagnoses to brain abscess, metastatic brain tumor and glioblastoma multiforme in cases of multiple ring enhancing periventricular lesions of immunocompromised patient or organ recipient.

  • Deep neck infections were flirty common and a source of considerable morbidity and mortality. Although the advent of antibiotics has reduced the overall number of deep neck infections, they still occur in the general population. There are several new groups of patients at risk for deep neck infections, such as immunocompromised individuals, those with underlying diseases. Prevention of the severe sequale that may be associated with deep neck infections- mediastinitis, airway obstruction, carotid artery hemorrhage, aspiration pneumonia, septicemia - requires a knowledge of various portals of entry for infection, the presenting sign and symptoms, the possible microbiologic features, appropriate laboratory and radiologic workups, therapeutic techniques, and the ongoing medical management. A prompt diagnosis and institution of therapy will shorten the course of required treatment and reduce morbility and mortility. The authors have experienced one case of acute mediastinitis in deep neck infection patient with diabetes mellitus.

하악 구치부 치주조직에 발생한 침윤성 국균증환자의 치험례 (INVASIVE ASPERGILLOSIS ON LOWER MOLAR PERIODONTAL TISSUES : CASE REPORT)

  • This paper describes that invasive aspergillosis was found in oral cavity, especially in lower molar periodontium in a immunocompromised 34-year-old woman who had been admitted in hemato-oncology with the diagnosis of acute myeloid leukemia. Antifungal therapy and surgical excision of involved teeth, bone and gingiva were the treatment of choice. After treatment infected area was healed very well. We would like to report our case because we could not find any paper reporting on invasive aspergillosis occurring in lower molar periodontium.

Septicemia Caused by Leuconostoc lactis with Intrinsic Resistance to Vancomycin in a Patient with Biliary Stent

  • Leuconostoc spp. is intrinsically resistant against vancomycin and rarely causes the infection in immunocompromised patients. In this report, we describe a fatal case of Leuconostoc lactis bacteremia in a patient with biliary tract stent insertion to resolve the biliary tract obstruction by multiple pseudocysts in the pancreatic head region. Leuconostic lactis isolated from the blood of the patients was confirmed by 16S rRNA sequencing and this isolate was susceptible against most antibiotics, including levofloxacin, penicillin, erythromycin and cefotaxime except vancomycin. The septic shock and multi-organ failure was abruptly progressed due to delayed use of adequate antibiotic. Using vancomycin as the empirical antibiotics in a bacteremic patient by Gram positive cocci, the treatment failures by the isolates with intrinsic resistance against vancomycin have to be considered. In addition, the prompt and accurate identification of Leuconostoc spp. are very important to select the adequate antibiotics.

Candida tropicalis arthritis of the elbow in a patient with Ewing's sarcoma that successfully responded to itraconazole

  • Fungal infections are rarely responsible for arthritis. Few cases of fungal arthritis have been reported, even in immunocompromised hosts susceptible to low-virulence organisms. Herein, the authors report the first case of Candida tropicalis arthritis in a child with a solid tumor. A 13-year-old boy with Ewing's sarcoma developed arthritis in his elbow during the neutropenic period after chemotherapy. Despite treatment with broad-spectrum antibiotics, his condition did not improve and serial blood cultures failed to reveal any causative organisms. After surgical drainage, culture of the joint fluid revealed the presence of C. tropicalis. Itraconazole treatment was started and after 3 months of therapy, the patient completely recovered full elbow function.

Ileal Perforation with Norovirus Gastroenteritis in a 3-Month-Old Infant

  • Noroviruses have been recognized as the leading cause of epidemic and sporadic gastroenteritis since the advent of molecular diagnostic technique. They have been documented in 5-31% of pediatric patients hospitalized with gastroenteritis. Although norovirus gastroenteritis is typically mild and self-limited, it causes severe, but sometimes fatal, conditions in the vulnerable population such as immunocompromised patients, young children, and the elderly. Bowel perforation due to norovirus infection is rare. We report a case of small bowel perforation with norovirus gastroenteritis in the infant with Down syndrome during the hospitalization with pneumonia. Severe dehydration may cause bowel ischemia and could have triggered bowel perforation in this case. Physicians should be alert to the potential surgical complications followed by severe acute diarrhea, especially in high risk groups.

  • Listeria monocytogenes is a facultative bacterium that cause severe clinical disease including meningoencephalitis, septicaemia, and abortion in pregnant women, newborn infants, the debilitated elderly or immunocompromised people. In preliminary experiments on murine listeriosis we noticed suppurative gastritis in mice infected with L. monocytogenes by the intragastric route. The aims of the present study were ⅰ) to describe the histopathology of the experimentally listeria-induced gastroenteritis ⅱ) to investigate the influence of bacterial strain and laboratory mouse strain on infectivity and on the severity of the infection; [3] to examine possible effects of preliminary intragastric administration of sodium bicarbonate.
