Personal and Parental Factors Influencing Influenza Vaccination in Adolescents: Based on the 6th Korea National Health and Nutrition Examination Survey (한국 청소년 인플루엔자 예방접종에 영향을 미치는 개인요인과 부모요인에 관한 융합적 연구 : 제 6기 국민건강영양조사를 바탕으로)

  • Influenza vaccination in adolescents is crucial to prevent the influenza expansion. The aim of this study was to investigate the difference of personal factors and parental factors affected influenza vaccination coverage. Study data on 12-18 years old adolescents and their parents were obtained from the 6th Korea National Health and Nutrition Examination Survey. Rao-Scott Chi-square test and multivariate logistic regression were used for the analysis. The percentage of influenza vaccination coverage in adolescents was low (26.2%). The personal factors influencing influenza vaccination were age, discomfort within two weeks, history of pneumonia, and wearing safety belt in adolescents, and the parental factors were age and influenza vaccination. In order to increase the influenza vaccination coverage for adolescents, media and healthcare professionals should provide education to adolescents and their parents about influenza vaccination.

A Comparative Study on Primary Health Care in Republic of Korea and Republic of Uzbekistan

  • Background: Primary health care (PHC) plays a major role to ensure the basic right and equal distribution of the essential health care services. This study presents comparative analyses of PHC in Korea and Uzbekistan, discusses the existing scenario and the challenges, and provides recommendations. Methods: This study reviewed secondary data from Korea's National Statistical Information Service and the State Committee of the Republic of Uzbekistan on Statistic, regulatory legislation, research reports, and policy papers by research and international institutions. We focus on comparing input and outcome health data, PHC structure, and health expenditure. Results: Overall health status of the population in Korea is better than in Uzbekistan; both countries achieved more than 95% immunization coverage. The reforms implemented in both countries provide initial health care service delivery. However, there are several challenges such as the distribution of the staff between urban and rural areas and interest of the graduates on specialization rather than working in PHC system. Conclusion: PHC plays an important role in the provision of medical services to the population, addressing both health and social problems; it is the best tool for achieving universal coverage for basic health needs of the population. The community health practitioners in Korea and nurses in Uzbekistan plays main role in universal coverage through providing essential health care services. Continuous reform of the PHC system should be directed to strengthen the capacity of the PHC staff in health promotion knowledge and activities as well as to encourage population to improve their own health.

  • Pertussis is a highly contagious respiratory tract disease caused by $Bordetella$ $pertussis$ infection. The clinical manifestation of this infection can be severe enough to cause death. Although pertussis has been supposed to be a vaccine-preventable disease ever since the widespread vaccination of children against pertussis was started, since the 1990s, cases of pertussis and related fatalities are on the rise, especially in countries with high vaccination coverage. In Korea, there have been no deaths due to pertussis since 1990, and the vaccination rate continues to be approximately 94%. However, the number of pertussis cases reported to the Korea Center for Disease Control and Prevention has tended to increase in the 2000s, and in 2009, there was an obvious increase in the number of pertussis cases reported. This review aims to present the latest information about the pathogenesis, diagnosis, treatment, and prevention of pertussis.

A Survey for Changed Control Policies of Hepatitis B in Republic of Korea (우리나라의 B형 간염 조절 정책의 변화에 대한 조사)

  • Purpose : We have tried to search all concerning data on the policies of hepatitis B vaccination or surveillance of medical equipments that can transmit blood mediated diseases, including disposable syringe, in Republic of Korea, to propose references to other medical professionals. Methods : Data from domestic journals, government websites, computerized newspapers, etc. were reviewed. The data were arranged into four categories (governmental policies on hepatitis B control, hepatitis B mass immunization program in school, using process of hepatitis B vaccine, and policies on medical equipments surveillance, including disposable syringes, which is considered as source of blood mediated infection), and the periodic changes on each subjects in policies were reviewed. Results : Due to growing social concerns on hepatitis B in early 1980s, swift introduction and production of hepatitis B vaccine was made. After establishment of "5 year project for eradication of hepatitis B" in late 1983, number of vaccinated population jumped up to 6 million within 2 years (1984-85). However, since the immunization targets were mainly adult, not infant, this project was annulled in late 1985. Following this project, hepatitis B mass immunization program in school was carried out. In year 1995, hepatitis B vaccine was included in national immunization program. The use of disposable syringe was recommended from year 1980 but legislated in year 1985, finally. Conclusion : The mainstay in controlling hepatitis B in Republic of Korea was coordination of prompt introduction of vaccine, mass immunization, high vaccination coverage rate, or use of disposable syringes. However, since there is lack of official data available, it is urgent to arrange and computerize all government data related to infectious diseases.

Factors Influencing on Influenza Vaccination Coverage (인플루엔자 예방접종률에 영향을 미치는 요인)

  • The aim of this study was to investigate the factors influencing on influenza vaccination among socio-demographic characteristics and health behaviors. Data of 18,299 adults of age between 19 and 65 years who answered to the survey on National Health and Nutrition and influenza vaccination from 2007 to 2010 was used and analyzed. Overall influenza vaccination rate was 21.7%. Logistic regression analysis revealed that factors influencing on influenza vaccination were an increasing age, a residence in rural area, a high personal income, a low educational level, jobless women, experience of hospital visits, and morbidity of chronic illnesses. In conclusion, influenza vaccination rate in Korea is low and influenced by multiple socio-demographic factors and health behaviors. It is needed to seek a strategy to develop a vaccination program in consideration of factors associated with influenza vaccination.

Mapping HPV Vaccination and Cervical Cancer Screening Practice in the Pacific Region-Strengthening National and Regional Cervical Cancer Prevention

  • Objective : To provide background information for strengthening cervical cancer prevention in the Pacific by mapping current human papillomavirus (HPV) vaccination and cervical cancer screening practices, as well as intent and barriers to the introduction and maintenance of national HPV vaccination programmes in the region. Materials and Methods: A cross-sectional questionnaire-based survey among ministry of health officials from 21 Pacific Island countries and territories (n=21). Results: Cervical cancer prevention was rated as highly important, but implementation of prevention programs were insufficient, with only two of 21 countries and territories having achieved coverage of cervical cancer screening above 40%. Ten of 21 countries and territories had included HPV vaccination in their immunization schedule, but only two countries reported coverage of HPV vaccination above 60% among the targeted population. Key barriers to the introduction and continuation of HPV vaccination were reported to be: (i) Lack of sustainable financing for HPV vaccine programs; (ii) Lack of visible government endorsement; (iii) Critical public perception of the value and safety of the HPV vaccine; and (iv) Lack of clear guidelines and policies for HPV vaccination. Conclusion: Current practices to prevent cervical cancer in the Pacific Region do not match the high burden of disease from cervical cancer. A regional approach, including reducing vaccine prices by bulk purchase of vaccine, technical support for implementation of prevention programs, operational research and advocacy could strengthen political momentum for cervical cancer prevention and avoid risking the lives of many women in the Pacific.

Post-marketing Surveillance Study of an Inactivated Split-Virion Influenza Vaccine in Korea (불활화 분할 인플루엔자 백신의 국내 시판 후 조사 연구)

  • Purpose : This post-marketing surveillance study (NCT00750360) assessed the safety and reactogenicity of an inactivated, trivalent split-virion influenza vaccine licensed for use in the Korea since 2002. Methods : Eight hundred and eighty three subjects aged 6 months received a single dose of the ${\geq}$ vaccine; an additional dose was administered to those aged <9 years and unprimed with an influenza vaccine. Four hundred and eleven subjects used diary cards to record safety information; this report presents data from these subjects. Incidence of solicited local, general and unsolicited adverse events (4-days and 21-days post-vaccination follow-up periods, respectively) were recorded. Serious adverse events (SAEs) were recorded throughout the study period. Results : Injection site pain (subjects aged <6 years: 12.6% of subjects, ${\geq}$6 years: 34.7%), fever (<6 years: 1.3%) and myalgia (${\geq}$6 years: 13.9%) were the most frequently recorded solicited local and general adverse events. Grade 3 solicited adverse events were reported by ${\leq}$4.0% subjects. No vaccine-related SAEs were recorded (KFDA criteria). Conclusion : Considering the vaccine's well-established immunogenicity and its favourable safety and reactogenicity profile across all age groups and its high coverage rate in Korea, it may be recommended as a candidate to facilitate annual seasonal influenza vaccination for all ages as part of the Korean National Immunization Program.

  • 현재 우리나라는 보건소 중심의 예방접종등록사업을 진행 중에 있다. 2004년부터 시작된 이 사업은 현재는 전국민을 대상으로 실시하고 있다. 예방접종등록사업은 피접종자들은 대체적으로 예방접종에 대한 적절한 시기를 판단할 수 있는 정보가 부족하기 때문에 예방접종과 관련된 정보들을 국가나 사회에서 적시에 피접종자들에게 제공해 줄 수 있다. 따라서 이에 대한 확인이 등록에 앞서 그 정확성을 확인 필요가 있다. 예방접종등록사업의 가장 기본적인 사업으로 진행된 것은 예방접종 수첩의 제공과 그 관리를 통한 예방접종 사업으로 지속적으로 전개되어 왔으며, 일반 국민과 개원의사들에서도 대중적으로 보급되어 있다. 국가 예방접종률의 산출 그리고 취학 어린이 예방접종력 증명서 제출 제도를 위하여 예방접종 수첩을 활용하는 방법이 현실적으로 유용할 수 있다. 따라서 예방접종 수첩의 예방접종력 정확성을 확인할 필요가 있다. 지역조사에서 접종기관으로 확인된 민간 의료기관 365곳 가운데 의료기관 이전 및 폐업으로 반송된 경우가 4건(1.1%)이었으며, 응답한 경우는 129곳으로 응답률은 35.3%였다. 조사된 의료기관 전체를 대상으로 확인된 1,201건에 대한 BCG 접종여부의 정확도는 69.5%였다. B형 간염의 정확도는 1차가 41.3%로 낮았으며, 2차와 3차는 각각 76.6%, 79.7%였다. DTaP의 정확도는 약 80%였으며, 정확도가 제일 높은 것은 DTaP 3차로 82.5%였으나 다른 것과 가장 낮은 정확도와 2% 정도 밖에 차이가 나지 않았다. 폴리오의 정확도는 약 80%였다. MMR의 정확도는 83.2%였다. 일본뇌염의 경우 약 80.0%였으며, 수두의 정확도는 74.9%로 다른 질병과 비교하여 낮은 수치를 보였다. 조사된 의료기관을 전체로 한 기타예방접종별 예방접종수첩의 접종여부의 정확도는 인플루엔자는 정확도는 74.1%였으며, 뇌수막염은 72.7%의 정확도를 보였다. A형 간염 1차의 정확도79.5%였으며, 폐렴구균 1차의 경우 73.2%로 나타났다. 국가필수예방접종별 예방접종수첩의 접종일자에 대한 정확도는 BCG 80.1%로 확인되었고, B형 간염 1차 89.7% 2차는 82.1% 3차는 79%로 B형 간염 중 가장 낮은 정확도를 보였다. DTaP는 1차와 2차는 약 87% 3차는 85.1% 4차는 83.5%로 확인이 되었다. 폴리오는 1차가88.1%로 가장 높은 정확도를 보였고 그 다음으로 2차가 86.2%, 3차가 84.8%로 확인되었다. MMR의 정확도는 84%였으며, 일본뇌염 1차의 정확도는 83.1%로 나타났다. 수두의 접종일자 정확도는 83.7% 이고, 인플루엔자의 정확도는 55.3%로 상대적으로 낮은 수치를 보였다. 기타예방접종별 접종일자 정확도의 조사 결과 뇌수막염 1차과 폐렴구균 1차는 약 90%로 상대적으로 높은 정확도를 보였고, A형 간염 1차는 88.4%의 정확도를 보였다.

Efficient Delivery of Toxoid Antigens using Micro/Nano-carriers (마이크로/나노-운반체를 이용한 톡소이드 항원의 효과적인 전달 방법)

  • Immunization has been performed for centuries and is generally accepted as a sustainable method of controlling bacteria, viruses, and mediated and infectious diseases. Despite many studies having been performed on animal subjects to demonstrate the importance of toxin immunity, the use of toxoid vaccines in humans and animals has been limited for a long time. Recently, the development of the toxoid antigen delivery system has been facilitated using novel nano-medicinal technology. The micro/nano-carrier has been used to improve vaccination coverage as well as reduce vaccine costs. A micro/nano-carrier is a micro/nano-sized material that delivers immune cargo, including recombinant or peptide toxoid antigens. These toxoid antigens are either encapsulated in the interior or displayed on the surface of micro/nano-carriers as a way to protect them from the cellular machinery. In particular, the combination of toxoid antigens and micro/nano-carriers can induce phagocytosis through the specific interactions between GCs and macrophages; thus, the toxoid antigens can be delivered easily into the macrophages. This paper reviews recent achievements of micro/nano-carriers in the field of vaccine delivery systems such as microbial ghost cells (GCs, Bacterial ghost cells and Yeast ghost cells), gene-manipulated outer membrane vesicles (OMVs) and biocompatible, polymer-based nanoparticles (NPs, NP-Carrier and NP-Cage). Finally, this review shows various aspects in terms of the hosts' immune responses.
