Paraffin을 이용한 Thyroid Phantom제작에 따른 유용성에 관한 연구 (A study on the Effectivness of Hand-made Paraffin Thyroid Phantom)

  • 방사성의약품을 이용한 핵의학적 영상기록에 있어 장기가 함유하고 있는 정보량을 최대한 묘출시켜 양질의 진단정보를 제공해야 한다는 것은 매우 중요하다. 그것을 위한 기술적인 문제를 비롯하여 장비성능의 유지관리에 많은 노력을 기울이고 있으며, 다양한 종류로 상업화되어 이용되고 있는 우수한 phantom들이 사용되고 있다. 본 연구에서 인체의 연부조직과 매우 흡사한 물리적 조건을 가지고 있으며 구입하기 쉬운 Paraffin을 이용하여 기존에 사용되고 있는 acryl thyroid phantom과 동일하게 paraffin phantom을 제작하였으며, 판매되고 있는 acryl thyroid phantom에서 측정할 수 있는 것 보다 더 높은 분해능을 측정할 수 있도록 작은 3mm와 6mm 직경의 cold area를 삽입한 paraffin thyroid phantom을 수작업으로 제작하였다. 방사성의약품 $^{99m}TcO_4$를 이용하고 pinhole collimator를 이용하여 상업화되어 사용되고 있는 acryl thyroid phantom과 본 연구를 위하여 제작한 paraffin thyroid phantom의 특성을 비교 분석한 결과 paraffin을 이용한 phantom도 상업화되어 사용 중인 acryl thyroid phantom과 동일한 물리적 특성을 유지하고, 오히려 분해능이 높은 부분을 측정할 수 있고 연부조직의 특성을 연구할 수 있는 등가물질로서 구입하기 쉽고 제작이 수월한 장점을 비롯하여 경제성이 있음을 증명하는 계기가 되었다.

자장의 세기에 따른 유방 확장기 주위의 인공물 평가 (Evaluation of artifacts around the breast expander according to magnetic field strength)

  • 유방조직확장기의 Magnetic Valve는 MRI 검사 시 영상 인공물이 발생하여 MRI 검사가 제한적이다. MRI 검사가 필요한 유방조직확장기 삽입 환자에 대해 영상 인공물이 진단영역에 미치는 영향을 평가한다. 자체 제작한 팬텀과 실제 임상 조건을 이용하여 영상 인공물 측정을 실시하였다. 영상 인공물은 1.5 Tesla와 3.0 Tesla 환경에 따라 상이하게 측정되었으며, C-spine, L-spine 검사 시 영상 인공물의 영향이 T-spine에 비해 상대적으로 적었다. 유방암 전이로 인한 MRI 검사가 꼭 필요한 경우 주로 1.5 Tesla에서 Head & Neck 검사와 L-spine 아래 부위를 검사 할 수 있으나 일부 시퀀스에서는 영상 인공물로 인한 왜곡이 발생할 수 있다. 유방조직확장기 삽입 환자의 MRI 검사는 안전성 측면에서는 3.0T는 피하고 1.5T에서 조건부로 가능하다.

소동물 영상을 위한 Siemens Inveon PET 스캐너의 성능평가 (Performance Measurement of Siemens Inveon PET Scanner for Small Animal Imaging)

  • Inveon PET은 최근에 출시된 소동물 전용 PET 시스템이다. 이 연구에서는 Inveon PET 스캐너의 성능을 평가하기 위하여 공간 분해능, 민감도, 산란분획, 잡음등가계수(Noise equivalent count rate: NECR)를 측정하였다. 공간 분해능 측정은 에너지창 350~625 keV, 민감도, 산란분획, NECR 측정은 350~750 keV에서 수행하였고 동시계수창은 3.432 ns였다. 크기 $1\;mm^3$의 F-18 점 선원을 만들어 중심에서부터 5 cm 벗어난 위치까지 공간 분해능을 측정하였다. 민감도를 측정하기 위하여 스캐너의 축방향 길이와 동일한 길이 12.7 cm의 F-18 선 선원을 만들고 두께 2 mm의 알루미늄 관을 1개에서 5개까지 차례로 씌우며 절대 민감도를 계산하였다. 산란분획과 NECR 측정하기 위하여 두 가지 NEMA 산란 팬텀(랫(rat): 지름 50 mm, 길이 150 mm/마우스(mouse): 지름 25 mm, 길이 70 mm)을 이용하였고, F-18 선 선원(랫: 353 MBq, 마우스: 201 MBq)를 만들어 14반감기(25.6시간) 동안 데이터를 획득하였다. F-18의 중심에서 공간 분해능은 반경, 접선, 축 방향에서 각각 1.53, 1.50, 2.33 mm이고, 체적 공간 분해능은 $5.43\;mm^3$이었다. 절대민감도는 6.61%이었다. F-18 최대 NECR은 486 kcps @121 MBq (랫 팬텀), 1056 kcps @128 MBq (마우스 팬텀)이었다. 랫과 마우스의 산란분획은 각각 20.59%, 7.93%이었다. 이 연구에서 최신 소동물용 PET인 Inveon PET의 표준성능을 평가하였고 소동물 PET영상 획득에 유용함 을 보여주었다.

골판톰을 이용한 골밀도측정 참조체의 결정 (The determination of reference material for bone density by using bone phantom)

  • Purpose: To determine the proper reference step wedge for digital Cu-Equivalent Image analyzing systems for measurement of bone density. Meterials and Methods : Radiograms of lumbar vertebrae phantom (1g/㎠) with 3 test copper step wedges of 0.03, 0.05 and, 0.1 mm thickness unit were taken and analyzed using NIH image software on a Macintosh personal computer. Measured densities of the lumbar areas in the Cu-Equivalent images made by utilizing 3 different copper stepwedges were compared with a known bone density. Results: The values of r2 for all copper equivalent images were over 0.99. The mean Cu-Eq value of lumbar in copper equivalent image made by a 0.1 mm copper stepwedge was 0.22 ± 0.06 mm and converted to hydroxyapatite density of 1.03 g/㎠. The stepwedges of 0.03 and 0.05 mm produced results having higher values than the actual known bone density. They did not show the blue and green color level that appeared in lumbar on color enhanced image. Conclusion : A copper stepwedge of adequate thickness and range of steps which can express the range of density of bone being measured should be used.

Study on Changes in Shape of Denatured Area in Skull-mimicking Materials Using Focused Ultrasound Sonication

  • Recently, ultrasound therapy has become a new and effective treatment for many brain diseases. Therefore, skull-mimicking phantoms have been developed to simulate the skull and brain tissue of a human and allow further research into ultrasound therapy. In this study, the suitability of various skull-mimicking materials(HDPE, POM C, Acrylic) for studies of brain-tumor treatments was evaluated using focused ultrasound. The acoustic properties of three synthetic resins were measured. The skull-mimicking materials were then combined with an egg white phantom to observe the differences in the ultrasound beam distortion according to the type of material. High-intensity polyethylene was found to be suitable as a skull-mimicking phantom because it had acoustic properties and a denatured-area shape that was close to those of the skull,. In this study, a skull-mimicking phantom with a multi-layer structure was produced after evaluating several skull-mimicking materials. This made it possible to predict the denaturation in a skull in relation to focused ultrasound. The development of a therapeutic protocol for a range of brain diseases will be useful in the future.

Gradient Optimized Gradient-Echo Gradient Moment Nulling Sequences for Flow Compensation of Brain Images

  • Gradient moment nulling techniques require the introduction of an additional gradient on each axis for each order of motion correction to be applied. The additional gradients introduce new constraints on the sequence design and increase the demands on the gradient system. The purpose of this paper is to demonstrate techniques for optimization of gradient echo gradient moment nulling sequences within the constraints of the gradient hardware. Flow compensated pulse sequences were designed and implemented on a clinical magnetic resonance imaging system. The design of the gradient moment nulling sequences requires the solution of a linear system of equations. A Mathematica package was developed that interactively solves the gradient moment nulling problem. The package allows the physicist to specify the desired order of motion compensation and the duration of the gradients in the sequence with different gradient envelopes. The gradient echo sequences with first, second, and third order motion compensation were implemented with minimum echo time. The sequences were optimized to take full advantage of the capabilities of the gradient hardware. The sequences were used to generate images of phantoms and human brains. The optimized sequences were found to have better motion compensation than comparable standard sequences.

Performance Evaluation of a Rapid Three Dimensional Diffusion MRI

  • MRI, particularly diffusion weighted imaging (DWI), plays vital roles in detection of the acute brain infarction$\^$1-4/ and others metabolic changes of biological tissues. In general, every molecule in biological tissues may diffuse and move randomly in three-dimensional space. However, in clinical diagnosis, only 2D-DWI is used. The authors have developed a new method for rapid three-dimensional DWI (3D-DWI). In this method, by refocusing of the magnetized spin with the applied gradient field, direction of which is opposite to phase encoding field. Magnetized spin of $^1$H is kept under the SSFP (steady state free precession)$\^$5-6/. Under SSFP, in addition of FID, spin echo and stimulated echo are also generated, so the acquired signal is increased. The signal intensity is increased depending on flip angle (FA) of magnetized spin. This phenomenon is confirmed by human brain and phantom studies. The performance of this method is quantitatively analyzed by using both of conventional spin echo DWI and 3D-DWI. From experimental results, three dimensional diffusion weighted images are obtained correctly for liquid phantoms (water, acetone and oil), diffusion coefficient is enhanced in each image. Therefore, this method will provide useful information for clinical diagnosis.

Influence of Iodinated Magnetic Resonance Contrast Media and Isotope 99mTc on Changes of Computed Tomography Number

  • The purpose of the study was to identify how isotope and magnetic resonance imaging (MRI) contrast media impact on noise to computed tomography (CT) examination. For the study, divide the phantoms to two groups: 1) saline, saline + different kinds of contrast agent without $^{99m}Tc$ administration; 2) $^{99m}Tc$ administration: saline, saline + different kinds of contrast agent with $^{99m}Tc$ administration. CT contrast agent was used for Iopamidol$^{(R)}$ and Dotarem. And MRI contrast agent was used for Primovist$^{(R)}$ and Gadovist$^{(R)}$. To obtain an image, we used CT scanner. With an obtained image, we set the $1cm^2$ region of interest in the middle of bottle to measure the noise and CT number. As a result, there was no difference in CT number before and after inserting $^{99m}Tc$ into all contrast media including Normal Saline. However, when it comes to Noise, there was a difference before and after inserting $^{99m}Tc$ into every contrast media except MRI contrast media such as Primovist$^{(R)}$ and Gadovist$^{(R)}$.

자기공명영상검사 시 Array coil에서 element를 선택하여 신호를 수집하는 기법의 유용성 (Usefulness of the Technique of Collecting Signals by Selecting Elements from RF Receive Phase Array Coil in Magnetic Resonance Imaging)

  • 본 연구는 자기공명영상에서 array coil을 이용한 부분영역의 영상화 시 SNR을 증가시킬 수 있는 방안으로 element를 선택하여 신호를 수집하는 기법의 유용성에 대해 알아보고자 하였다. 연구방법은 array coil에 두 개의 원통형 fluid phantom을 나란히 위치시킨 다음, 부분영역인 한 개의 fluid phantom 영상화 시 element 전체를 사용하여 신호를 수집하는 경우와 선택한 경우로 나누어 SNR을 비교하였다. 연구결과, T1, T2 강조영상 모두 element를 선택하여 신호를 수집하는 경우가 전체를 사용한 경우보다 T1강조영상은 5.49%, T2강조영상은 14.64% 유의하게 증가하였다. 결론적으로 array coil을 이용한 부분영역의 영상화 시 element를 선택하여 신호를 수집하면 손쉽게 SNR을 증가시킬 수 있으리라 판단된다.

Metal artifact production and reduction in CBCT with different numbers of basis images

  • Purpose: To evaluate the effect of different numbers of basis images and the use of metal artifact reduction (MAR) on the production and reduction of artifacts in cone-beam computed tomography images. Materials and Methods: An acrylic resin phantom with a metal alloy sample was scanned, with 450 or 720 basis images and with or without MAR. Standard deviation values for the test areas (around the metal object) were obtained as a way of measuring artifact production. Two-way analysis of variance was used with a 5% significance level. Results: There was no significant difference in artifact production among the images obtained with different numbers of basis images without MAR (P=.985). MAR significantly reduced artifact production in the test areas only in the protocol using 720 basis images (P=.017). The protocol using 450 basis images with MAR showed no significant difference in artifact production when compared to the protocol using 720 basis images with MAR (P=.579). Conclusion: Protocols with a smaller number of basis images and with MAR activated are preferable for minimizing artifact production in tomographic images without exposing the patient to a greater radiation dose.
