In vivo quantification of mandibular bone remodeling and vascular changes in a Wistar rat model: A novel HR-MRI and micro-CT fusion technique

  • Purpose: This study was performed to introduce an in vivo hybrid multimodality technique involving the coregistration of micro-computed tomography (micro-CT) and high-resolution magnetic resonance imaging (HR-MRI) to concomitantly visualize and quantify mineralization and vascularization at follow-up in a rat model. Materials and Methods: Three adult female rats were randomly assigned as test subjects, with 1 rat serving as a control subject. For 20 weeks, the test rats received a weekly intravenous injection of 30 ㎍/kg zoledronic acid, and the control rat was administered a similar dose of normal saline. Bilateral extraction of the lower first and second molars was performed after 10 weeks. All rats were scanned once every 4 weeks with both micro-CT and HR-MRI. Micro-CT and HR-MRI images were registered and fused in the same 3-dimensional region to quantify blood flow velocity and trabecular bone thickness at T0 (baseline), T4 (4 weeks), T8 (8 weeks), T12 (12 weeks), T16 (16 weeks), and T20 (20 weeks). Histological assessment was the gold standard with which the findings were compared. Results: The histomorphometric images at T20 aligned with the HR-MRI findings, with both test and control rats demonstrating reduced trabecular bone vasculature and blood vessel density. The micro-CT findings were also consistent with the histomorphometric changes, which revealed that the test rats had thicker trabecular bone and smaller marrow spaces than the control rat. Conclusion: The combination of micro-CT and HR-MRI may be considered a powerful non-invasive novel technique for the longitudinal quantification of localized mineralization and vascularization.

Lateral pterygoid muscle volume and migraine in patients with temporomandibular disorders

  • Purpose: Lateral pterygoid muscle (LPM) plays an important role in jaw movement and has been implicated in Temporomandibular disorders (TMDs). Migraine has been described as a common symptom in patients with TMDs and may be related to muscle hyperactivity. This study aimed to compare LPM volume in individuals with and without migraine, using segmentation of the LPM in magnetic resonance (MR) imaging of the TMJ. Materials and Methods: Twenty patients with migraine and 20 volunteers without migraine underwent a clinical examination of the TMJ, according to the Research Diagnostic Criteria for TMDs. MR imaging was performed and the LPM was segmented using the ITK-SNAP 1.4.1 software, which calculates the volume of each segmented structure in voxels per cubic millimeter. The chi-squared test and the Fisher's exact test were used to relate the TMD variables obtained from the MR images and clinical examinations to the presence of migraine. Logistic binary regression was used to determine the importance of each factor for predicting the presence of a migraine headache. Results: Patients with TMDs and migraine tended to have hypertrophy of the LPM (58.7%). In addition, abnormal mandibular movements (61.2%) and disc displacement (70.0%) were found to be the most common signs in patients with TMDs and migraine. Conclusion: In patients with TMDs and simultaneous migraine, the LPM tends to be hypertrophic. LPM segmentation on MR imaging may be an alternative method to study this muscle in such patients because the hypertrophic LPM is not always palpable.

전자포탈영상장치(EPID)를 이용한 선형가속기의 기하학적 QC/QA System (On-line Quality Assurance of Linear Accelerator with Electronic Portal Imaging System)

  • 전자포탈영상장치 (EPID)를 의료용 선형가속기의 QC/QA로 이용할 수 있게 하고 이 시스템의 유용성을 평가할 목적으로, 의료용 선형가속기의 기하학적 QC/QA 항목 중 빛-광자선 조사면 일치, 콜리메이터 회전축, 캔트리 회전축 등을 분석할 수 있고 임상응용에의 유용성을 실용적으로 수행할 수 있는 GUI(Graphic User Interface) 방식의 소프트웨어를 개발하였다. 상용 전자포탈영상장치에서 수집한 영상을 네트워크를 통해 수집하고, 이 영상을 조작할 수 있는 OQuE (On-line quality assurance system using electronic portal imaging) 시스템을 PC 상에서 구현하였다. EPID는 Portal Vision(Varian, USA)을 사용하였고, 시스템의 유용성 검증을 위해 CL/2100/CD (Varian, USA)의 의료용 선형가속기를 이용하여 기하학적 QC/QA 성능과 임상응용에의 유용성을 평가하였다. 빛-광자선 조사면 일치를 ~1mm의 정밀ㆍ정확도로 평가할 수 있었다. 콜리메이터 또는 갠트리, 카우치 회전축 역시 4 개의 방사선 방향에 대한 영상을 중첩시켜서 중첩된 영상이 이루는 교점을 분석하여 구할 수 있었다. 빛-광자선 조사면 일치는 조사면별로 각각 영상을 얻고, 이들의 확대 영상과 중첩 영상을 보면서 중력장 중심 (center of gravity) 을 계산한 결과 5$\times$5 $cm^2$에서 0.2 mm, l0$\times$10, 15$\times$15, 20$\times$20 $cm^2$에서 0.3 mm내에 들어옴을 알 수 있었고 Upper Jaw 회전축은 $\pm$0.2 mm, Lower Jaw 회전축은 $\pm$0.1mm, 갠트리 회전축은 앞뒤 방향으로 $\pm$0.3 mm, 좌우 방향으로 $\pm$0.7 mm 내에 들어옴을 알 수 있었다. 카우치 회전측은 $\pm$0.2 mm 이내에 들어옴을 알 수 있었다. 영상수집 시간은 필름에 비해 훨씬 짧았으며, 그 분석 또한 객관적이고 정량적으로 이룰 수 있어서 전자포탈영상장치가 의료용 선형가속기의 QC/QA 도구로서 적당함을 알 수 있었다. 그리고 기준영 상과 포탈영상의 필드경계와 해부학적 모양을 분석함으로써 임상응용에의 유용성을 평가할 수 있었다. 적절한 사용자 편의를 보완하면 이 소프트웨어는 의료용 선형가속기의 기하학적인 성능평가 및 임상에 적극 활용할 수 있음을 알 수 있었다.

측두하악관절 장애 환자의 임상증상과 자기공명영상에 관한 비교 연구 (A comparison of clinical symptoms and magnetic resonance images in temporomandibular joint disorders)

  • Purpose : To determine the relationship between clinical symptoms and magnetic resoncance (MR) images in patients presenting with temoporomandibular joint (TMJ) disorders. Materials and Methods: This study was based on 172 joints in 86 patients presenting with TMJ disorders. Joint pain and sound during jaw opening and closing movements were recorded, and the possible relationship between disc positions and bony changes of the condylar head and the articular fossa in MR images in the oblique sagittal planes were examined. Data were analyzed by Chi-square test. Results : There was no statistically significant relationship between clinical symptoms and MR images in the patients with TMJ disorders. Conclusion: In the patient with TMJ disorders, joint pain and sound could not be specific clinical symptoms that are related with MR image findings, and asymptomatic joints did not necessarily imply that the joints are normal according to MR image findings.

골격성 III급 부정교합을 가진 양악 수술 환자의 술후 측모 예측을 위한 Video imaging (Power $Ceph^{\circledR}$ Ver 3.3)의 정확도에 관한 연구 (A study on the accuracy of profile change Prediction by video imaging (Power Ceph $^{\circledR}Ver$ 3.3) in Class III two jaw surgery patients)

  • 악교정 수술의 계획에 있어서 수술 후 경조직의 이동에 따른 연조직 변화 양상을 보다 정확히 예측하는 것은 매우 중요하다. 최근에는 측모 두부 규격 방사선 사진과 실제 측모 사진을 병합하여 수술 후 측모 변화에 대한 가시성을 높인 여러 종류의 video imaging 프로그램들이 개발되어 널리 사용되고 있다. 그러나 이들 프로그램의 정확도를 판단하는 연구는 아직 미비한 실정이며 특히 국내에서 많이 시행되고 있는 골격성 III급 부정교합 환자의 수술 후 변화 양상에 대한 연구가 임상적인 활용에 앞서 선행되는 것이 필요하다. 이에 저자는 video imaging프로그램의 하나인 Power $Ceph^{\circledR}$ (Power Soft Co. Ver 3.3)을 이용하여 국내에서 양악수술을 시행 받은 골격성 III급 부정교합 환자 30명(남 14명, 여 16명)을 대상으로 예측도와 실제 술후의 측모를 비교하여 정확도를 평가하였으며 정확도에 영향을 미치는 요인을 분석하기 위해 이동량 및 술식별로 분류 비교하여 다음과 같은 결론을 얻었다. 1. 예측치와 실제 계측치의 비교에서 24개의 계측항목 중 14개 항목에서 유의성 있는 차이를 보였다. 2. 계측항목 중 유의성 있는 차이를 보이는 항목은 대부분 상악에 관련된 연조직으로 하악에 관련된 연조직에 비해 다양한 차이를 나타내었다. 3.상악 및 하악의 이동량에 따른 비교에서 이동량이 적은 군이 이동량이 많은 군에 비하여 예측치와 실제 계측치 비교에서 유의성 있는 차이를 보인 항목이 적게 나타났다. 4. 이부 성형술 여부에 따른 비교에서 이부 성형술을 시행하지 않은 군에서 이부 성형술을 시행한 군에 비하여 예측치와 실제 계측치 비교에서 유의성 있는 차이를 보인 항목이 적게 나타났다. Video imaging(Power $Ceph^{\circledR}$ Ver 3.3)프로그램에 의한 예측치와 실제 계측치 비교시 그 차이는 대부분 2mm 이내로써 임상적으로 수용할 수 있는 수준이었다.

  • Squamous cell carcinoma arising in an odontogenic cyst is uncommon. The diagnosis of carcinoma arising in a cyst requires that there must be an area of microscopic transition from the benign epithelial cyst lining to the invasive squamous cell carcinoma. We report a histopathologically proven case of squamous cell carcinoma arising in a residual mandibular cyst in a 54-year-old woman.

Central odontogenic fibroma (simple type) in a four-year-old boy: atypical cone-beam computed tomographic appearance with periosteal reaction

  • Central odontogenic fibroma (COF) is a rare benign tumor that accounts for 0.1% of all odontogenic tumors. A case of COF (simple type) of the mandible in a four-year-old boy is described in this report. The patient showed asymptomatic swelling in the right inferior border of the lower jaw for one week. A panoramic radiograph showed a poorly-defined destructive unilocular radiolucent area. Cone-beam computed tomography showed expansion and perforation of the adjacent cortical bone plates. A periosteal reaction with the Codman triangle pattern was clearly visible in the buccal cortex. Since the tumor had destroyed a considerable amount of bone, surgical resection was performed. No recurrence was noted.

Three types of ossifying fibroma: A report of 4 cases with an analysis of CBCT features

  • Ossifying fibroma is a slow-growing benign neoplasm that occurs most often in the jaws, especially the mandible. The tumor is composed of bone that develops within fibrous connective tissue. Some ossifying fibromas consist of cementum-like calcifications, while others contain only bony material; however, a mixture of these calcification types is commonly seen in a single lesion. Of the craniofacial bones, the mandible is the most commonly involved site, with the lesion typically inferior to the premolars and molars. Ossifying fibroma of the jaw shows a female predominance. Some reports of ossifying fibroma have been published in the literature; however, this report continues the research on this topic by detailing 3 types of ossifying fibroma findings on panoramic radiographs and cone-beam computed tomographic images of 4 patients. The radiographs of the presented cases could help clinicians understand the variations in the radiographic appearance of this lesion.

CBCT findings of periapical cemento-osseous dysplasia: A case report

  • Periapical cemento-osseous dysplasia (PCOD) is a subtype of cemento-osseous dysplasia that usually occurs in middle-aged black women. This report described a case of a 45-year-old Iranian woman who was diagnosed with PCOD on the basis of cone beam computed tomographic (CBCT) findings. CBCT enabled detailed visualization of the bone changes. This report described the special radiographic characteristics of PCOD, including discontinuity of the lingual cortex on the CBCT sectional and three-dimensional images.

Supernumerary teeth in non-syndromic patients

  • Hyperdontia or supernumerary teeth without associated syndrome is a rare phenomenon, as supernumerary teeth are usually associated with cleft lip and palate or other syndromes such as Gardner's syndrome, cleidocranial dysplasia, and so on. Five patients with supernumerary teeth visited our department. They had no familial history or other pathology, certain treatment protocols was modified due to the presence of supernumerary teeth. Non-syndromic supernumerary teeth, if asymptomatic, need to have periodical radiographic observation. If they showed no variation as they impacted in the jaw, careful examination is necessary because they may develop into pathological status such as dentigerous cysts. The importance of a precise clinical history and radiographic examination for patients with multiple supernumerary teeth should be emphasized.
