No-Touch Radiofrequency Ablation for Early Hepatocellular Carcinoma: 2023 Korean Society of Image-Guided Tumor Ablation Guidelines

  • Radiofrequency ablation (RFA) has been widely used to manage hepatocellular carcinomas (HCCs) equal to or smaller than 3 cm. No-touch RFA has gained attention and has recently been implemented in local ablation therapy for HCCs, despite its technical complexity, as it provides improved local tumor control compared to conventional tumor-puncturing RFA. This article presents the practice guidelines for performing no-touch RFA for HCCs, which have been endorsed by the Korean Society of Image-Guided Tumor Ablation (KSITA). The guidelines are primarily designed to assist interventional oncologists and address the limitations of conventional tumor-puncturing RFA with describing the fundamental principles, various energy delivery methods, and clinical outcomes of no-touch RFA. The clinical outcomes include technical feasibility, local tumor progression rates, survival outcomes, and potential complications.

Assessment of Local Tumor Progression After Image-Guided Thermal Ablation for Renal Cell Carcinoma

  • Focal enhancement typically suggests local tumor progression (LTP) after renal cell carcinoma is percutaneously ablated. However, evaluating findings that are false positive or negative of LTP is less familiar to radiologists who have little experience with renal ablation. Various imaging features are encountered during and after thermal ablation. Ablation procedures and previous follow-up imaging should be reviewed before determining if there is LTP. Previous studies have focused on detecting the presence or absence of focal enhancement within the ablation zone. Therefore, various diagnostic pitfalls can be experienced using computed tomography or magnetic resonance imaging examinations. This review aimed to assess how to read images during or after ablation procedures, recognize imaging features of LTP and determine factors that influence LTP.

마스크-보조 어텐션 기법을 활용한 항공 영상에서의 퓨-샷 의미론적 분할 (Few-shot Aerial Image Segmentation with Mask-Guided Attention)

  • The goal of few-shot semantic segmentation is to build a network that quickly adapts to novel classes with extreme data shortage regimes. Most existing few-shot segmentation methods leverage single or multiple prototypes from extracted support features. Although there have been promising results for natural images, these methods are not directly applicable to the aerial image domain. A key factor in few-shot segmentation on aerial images is to effectively exploit information that is robust against extreme changes in background and object scales. In this paper, we propose a Mask-Guided Attention module to extract more comprehensive support features for few-shot segmentation in aerial images. Taking advantage of the support ground-truth masks, the area correlated to the foreground object is highlighted and enables the support encoder to extract comprehensive support features with contextual information. To facilitate reproducible studies of the task of few-shot semantic segmentation in aerial images, we further present the few-shot segmentation benchmark iSAID-, which is constructed from a large-scale iSAID dataset. Extensive experimental results including comparisons with the state-of-the-art methods and ablation studies demonstrate the effectiveness of the proposed method.

Current status of stereotactic body radiotherapy for the treatment of hepatocellular carcinoma

  • Stereotactic body radiotherapy (SBRT) is an advanced form of radiotherapy (RT) with a growing interest on its application in the treatment of hepatocellular carcinoma (HCC). It can deliver ablative radiation doses to tumors in a few fractions without excessive doses to normal tissues, with the help of advanced modern RT and imaging technologies. Currently, SBRT is recommended as an alternative to curative treatments, such as surgery and radiofrequency ablation. This review discusses the current status of SBRT to aid in the decision making on how it is incorporated into the HCC management.
