Chest Wall Lipogranuloma after Hydrogel Implant Rupture: Case Report

  • We present a 53-year-old woman with a large chest wall mass in the interpectoral space, which was eventually confirmed as a lipogranuloma resulting from hydrogel implant rupture. Ultrasonography (US) showed reduced implant volume with surrounding peri-implant fluid collection, suggesting the possibility of implant rupture. A heterogeneously hypoechoic mass was found between the pectoralis major and minor muscles adjacent to the ruptured implant. On magnetic resonance imaging (MRI), there was a large mass in the left interpectoral space of the upper inner chest wall. The mass showed slightly high signal intensity (SI) on pre-contrast T1-weighted image (WI) with mixed iso and high SI on T2-WI. The signal of the mass was suppressed using the water suppression technique but not with the fat suppression technique on T2-WI. The mass showed diffuse enhancement upon contrast enhancement. The enhancing kinetics showed persistent enhancement pattern. US-guided core needle biopsy revealed a lipogranuloma and removal confirmed a ruptured PIP hydrogel implant.

A pre-stack migration method for damage identification in composite structures

  • In this paper a damage imaging technique using pre-stack migration is developed using Lamb (guided) wave propagation in composite structures for imaging multi damages by both numerical simulations and experimental studies. In particular, the paper focuses on the experimental study using a finite number of sensors for future practical applications. A composite laminate with a surface-mounted linear piezoelectric ceramic (PZT) disk array is illustrated as an example. Two types of damages, one straight-crack damage and two simulated circular-shaped delamination damage, have been studied. First, Mindlin plate theory is used to model Lamb waves propagating in laminates. The group velocities of flexural waves in the composite laminate are also derived from dispersion relations and validated by experiments. Then the pre-stack migration technique is performed by using a two-dimensional explicit finite difference algorithm to back-propagate the scattered energy to the damages and damages are imaged together with the excitation-time imaging conditions. Stacking these images together deduces the resulting image of damages. Both simulations and experimental results show that the pre-stack migration method is a promising method for damage identification in composite structures.

  • Thoracic pedicle screw fixation techniques are still controversial for thoracic deformities because of possible complications including neurologic deficit. Methods to aid the surgeon in appropriate screw placement have included the use of intraoperative fluoroscopy and/or radiography as well as image-guided techniques. We describe our technique for free hand pedicle screw placement in the thoracic spine without any radiographic guidance and present the results of pedicle screw placement analyzed by computed tomographic scan in two human cadavers. This free hand technique of thoracic pedicle screw placement performed in a step-wise, consistent, and compulsive manner is an accurate, reliable, and safe method of insertion to treat a variety of spinal disorders, including spinal deformity.

  • Heavy ion particle, represented carbon ion, radiotherapy is currently most advanced radiation therapy technique. Conventional radiation therapy has made remarkable changes over a relatively short period of time and leading various developments such as intensity modulated radiation therapy, 4D radiation therapy, image guided radiation therapy, and high precisional therapy. However, the biological and physical superiority of particle radiation, represented by Bragg peak, can give the maximum dose to tumor and minimal dose to surrounding normal tissues in the treatment of cancers in various areas surrounded by radiation-sensitive normal tissues. However, despite these advantages, there are some limitations and factors to consider. First, there is not enough evidence, such as large-scale randomized, prospective phase III trials, for the clinical application. Secondly, additional studies are needed to establish a very limited number of treatment facilities, uncertainty about the demand for heavy particle treatment, parallel with convetional radiotherapy or indications. In addition, Bragg peak of the heavy particles can greatly reduce the dose to the normal tissues front and behind the tumor compared to the photon or protons. High precision and accuracy are needed for treatment planning and treatment, especially for lungs or livers with large respiratory movements. Currently, the introduction of the heavy particle therapy device is in progress, and therefore, it is expected that more research will be active.

  • This review was described to investigate the feasibility of using ultrasound as an image tool for interventions of lumbar spine. This article will first provide an overview of lumbosacral spine surface anatomy and sonoanatomy. A detailed understanding of anatomy is critical for interpretation of ultrasound and procedural performance at spine. Fluoroscopy is most commonly used in interventional spine procedures, but radiation exposure is the major concern when obtaining fluoroscopic images. Ultrasound is radiation-free, is easy to use, and can provide real-time images with high accuracy. Also this device can be used in virtually any clinical setting. Ultrasound guidance offers a reliable alternative to fluoroscopy or computed tomography for lumbar interventions and can be safely performed without radiation exposure.

This treatise studied about techniques to encipher videotex to prevent variation and outward flow of burn information, peculation etc., except general encryption notation such as user certification to have drawn problem about stability of general video conference system, and is used present as countermeasure about here. Used improved Vernam's encryption techniques to encrypt videotex.

A Smart Setup for Craniospinal Irradiation

  • Our purpose is to present a novel technique for delivering craniospinal irradiation in the supine position using a perfect match, field-in-field (FIF) intrafractional feathering, and simple forward-optimization technique. To achieve this purpose, computed tomography simulation was performed with patients in the supine position. Half-beam, blocked, opposed, lateral, cranial fields with a collimator rotation were matched to the divergence of the superior border of an upper-spinal field. Fixed field parameters were used, and the isocenter of the upper-spinal field was placed at the same source-to-axis distance (SAD), 20 cm inferior to the cranial isocenter. For a lower-spinal field, the isocenter was placed 40 cm inferior to the cranial isocenter at a constant SAD. Both gantry and couch rotations for the lower-spinal field were used to achieve perfect divergence match with the inferior border of the upper-spinal field. A FIF technique was used to feather the craniospinal and spinal-spinal junction daily by varying the match line over 2 cm. The dose throughout the target volume was modulated using the FIF simple forward optimization technique to obtain homogenous coverage. Daily, image-guided therapy was used to assure and verify the setup. This supine-position, perfect match craniospinal irradiation technique with FIF intrafractional feathering and dose modulation provides a simple and safe way to deliver treatment while minimizing dose inhomogeneity.

  • Infrared(IR) guided heat-seeking missiles uses IR emissions from aircraft to detect and track a target. Due to passive characteristic of the IR guidance, early detection of the missile is difficult and it is significant threat to aircraft survivability. Therefore, IR signature prediction of the aircraft is an important aspect of the stealth technology. In this study, we simulated IR signature of the aircraft in real atmospheric conditions. Aircraft surface temperature distribution was calculated by using RadthermIR code. Based on temperature distribution, IR radiance and BRDF(Bidirectional Reflectance Distribution Function) image were simulated for different weather(seasonal) and background(sky/soil) conditions. The IR contrast tendencies are not aligned with surface temperature or magnitude of target IR radiance. Therefore, it is essential to simulate IR signature with various conditions and background to acquire reliable database.

  • 최근 촬영 기기의 기술발전으로 인해 디지털 영상의 해상도가 증가함에 따라 선명한 디지털 영상에 대한 요구가 증가하고 있다. 이러한 요구에도 불구하고 디지털 영상 내 가우시안 잡음 (gaussian noise)은 촬영기기를 통해 영상 획득 및 처리 과정에서 발생하여 화질을 열화 시킨다. 디지털 이미지에서 발생하는 가우시안 잡음을 제거하기 위해서 기존의 저대역 통과 필터 (low-pass filter: LPF)를 사용하면 잡음은 제거되지만, 블러링 현상 (blurring phenomenon)이 나타난다. 이러한 문제점을 개선하기 위해 소벨 연산자 (sobel operator)를 사용하여 영상 내 에지 맵 (edge-map)을 생성하여 에지 영역과 동질 영역을 구분한다. 에지영역에서는 약한 저역 필터 (weak low-pass filter)를 사용하고, 그 외의 이미지 영역에서는 강한 저역 필터 (strong low-pass filter)를 사용하는 알고리듬을 제안하였다. 그리고 다양한 이미지에 대하여 기존 알고리듬과 제안한 알고리듬의 적용한 결과를 통해 주관적 화질 비교하였고 객관적 지표로 최대 신호 대 잡음비 (peak signal-to noise ratio: PSNR)와 구조 유사성 (structural similarity: SSIM)을 사용하여 성능을 평가하였다. 실험결과를 통해 제안된 알고리듬이 잡음 제거 및 외곽선 보존의 우수함을 확인하였다.
