MR Findings of Papillary Neoplasms of the Breast (유두 종양의 자기공명 영상소견)

  • Purpose : To review MR imaging finding of papillary lesion identified as additional suspicious lesion on MR image in women with biopsy-proven breast cancer and to evaluate upgrading rates after subsequent surgical histopathological diagnosis. Materials and Methods: Among 1729 preoperative MR image of women with biopsy proven breast cancer, US-guided CNB-proven 22 papillary lesions from 21 patients, which showed additional suspicious contrast enhancement other than index cancer on MR image, were subjected to the study. Some of these lesions underwent surgery, thus the comparisons between the histopathologic results were able to be compared to the results of US-guided CNB. Also retrospective analysis was done for MR findings of these lesions by BI-RADS MRI lexicon. Results: On MR imaging, 8 mass lesions, 7 non-mass lesions, 7 focus lesions were detected. All of the focus lesion (100%, 7/7) was diagnosed as benign lesion and showed plateau and washout pattern in dynamic MR image. After excisional biopsy, one of 9 benign papilloma (11.1%), 3 of 3 papillary neoplasm with atypia component (100%), 3 of 5 papillary neoplasm (60%) were upgraded to malignancy such as ductal carcinoma in situ (DCIS), invasive ductal carcinoma (IDC), invasive lobular carcinoma (ILC). Conclusion: The MR images of papillary lesions diagnosed by US-guided CNB exhibit no significant differences between malignancy and benign lesion. Also 41.2% of the lesion (7/17) was upgraded after subsequent surgery. Thus all of the papillary lesions require excisional biopsy for definite diagnosis and the MR imaging, it's just not enough by itself.

  • Synthetic dynamic infrared image generation from the given virtual environment is being the primary goal to simulate the output of the infra-red(IR) camera installed on a vehicle to evaluate the control algorithm for various search & reconnaissance missions. Due to the difficulty to obtain actual IR data in complex environments, Artificial intelligence(AI) has been used recently in the field of image data generation. In this paper, CycleGAN technique is applied to obtain a more realistic synthetic IR image. We added the Structural Similarity Index Measure(SSIM) loss function to the L1 loss function to generate a more realistic synthetic IR image when the CycleGAN image is generated. From the simulation, it is applicable to the guided-missile flight simulation tests by using the synthetic infrared image generated by the proposed technique.

Diagnosis of Iliotibial Band Friction Syndrome and Ultrasound Guided Steroid Injection

  • A 64-year-old woman visited our pain clinic with the pain of right lateral side of thigh for one year. Her pain always started from knee and was radiated to buttock area when symptom was severe. She showed significant tenderness at knee lateral side and local tightness at lateral thigh. Magnetic resonance image of the knee was performed and we could identify high signal intensity of iliotibial band through coronal and axial view. In spite of medication and physical stretching exercise of iliotibial band for one month, she did not show any improvement of pain. To alleviate her symptom, ultrasound guided local corticosteroid injection targeting beneath the iliotibial band was performed. After the procedure, the reduction of pain was significant and there was no need for further management.

  • We developed an machine vision method for navigation control of a traveling automatic guided vehicle(AGV) on desired trajectory with guided marks. The formulated EDF accumulates the edge magnitude for edge directions. The EDF has distinctive peak points at the vicinity of trajectory directions due to the directional and the positional continuities of desired trajectory. Examining the EDF by its shape parameters of the local maxima and symmetry axis results in identifying whether or not change in traveling direction of an AGV has occurred. Simulation results show that the presented method is useful for navigation control of AGV.

  • In modern-war campaign, precision-guided missiles are dominantly used to minimize the collateral damage. Imaging infrared seekers are widely applied for the precise guidance. Due to the high cost of the infrared detector, the cost for the one-shot weapon's test is a burden for the development. To reduce the test cost, a simulation method including imagery tracking is required, which is so-called integrated-flight simulation(IFS). The synthetic image generation(SIG)-based simulation method is typically used, which however cannot represent various environmental and target conditions. In this paper, a new IFS method is proposed using on-sight measured image to overcome the limitations of the SIG-based IFS(SIIFS). The target image acquired at the launching sight has been used only for checking the performance criteria of the image tracker and has not been tried for IFS since it has low resolution and little information. The study described in this paper, however, shows that the on-sight image-based IFS can predict the pre- and mid-course flight performance quite similarly and is very useful for the flight test analysis.

  • Stereotactic radiosurgery for intracranial lesion is well established since the Lars Leksell first introduced radiosurgery concept in 1951 Its use in the treatment of spinal lesion has been limited by the availability of effective immobilization devices. The first clinical experience of the spinal stereotactic radiosurgery technique was reported by Hamilton AJ. in 1995. Recently, Optic-guided patient positioning technique for extracranial stereotactic radiosurgery was developed and reported. This study is for assess the target positioning accuracy of the optic guided patient positioning system Exactrac (BrainLab., Inc, Germany). We have designed phantom for assess the accuracy of spinal stereotactic radiosurgery The infrared reflective body markers attached to the relatively immobile part of the body and a series of 2 mm CT images was taken. The image sets were transferred to the planning computer. During the radiosurgery treatment, we measure the real-time display showing the positioning values from Exactrac computer. And we compare the isocenter deviation from irradiated center point of the film which was mounted on the lesion site of the phantom and pin hole site of that film. The accuracy of the ExacTrac system in positioning a target point shows enough for the clinical applications.

Sparse reconstruction of guided wavefield from limited measurements using compressed sensing

  • A wavefield sparse reconstruction technique based on compressed sensing is developed in this work to dramatically reduce the number of measurements. Firstly, a severely underdetermined representation of guided wavefield at a snapshot is established in the spatial domain. Secondly, an optimal compressed sensing model of guided wavefield sparse reconstruction is established based on l1-norm penalty, where a suite of discrete cosine functions is selected as the dictionary to promote the sparsity. The regular, random and jittered undersampling schemes are compared and selected as the undersampling matrix of compressed sensing. Thirdly, a gradient projection method is employed to solve the compressed sensing model of wavefield sparse reconstruction from highly incomplete measurements. Finally, experiments with different excitation frequencies are conducted on an aluminum plate to verify the effectiveness of the proposed sparse reconstruction method, where a scanning laser Doppler vibrometer as the true benchmark is used to measure the original wavefield in a given inspection region. Experiments demonstrate that the missing wavefield data can be accurately reconstructed from less than 12% of the original measurements; The reconstruction accuracy of the jittered undersampling scheme is slightly higher than that of the random undersampling scheme in high probability, but the regular undersampling scheme fails to reconstruct the wavefield image; A quantified mapping relationship between the sparsity ratio and the recovery error over a special interval is established with respect to statistical modeling and analysis.

Interfraction variation and dosimetric changes during image-guided radiation therapy in prostate cancer patients

  • Purpose: The aim of this study was to identify volume changes and dose variations of rectum and bladder during radiation therapy in prostate cancer (PC) patients. Materials and Methods: We analyzed 20 patients with PC treated with helical tomotherapy. Daily image guidance was performed. We re-contoured the entire bladder and rectum including its contents as well as the organ walls on megavoltage computed tomography once a week. Dose variations were analyzed by means of Dmedian, Dmean, Dmax, V10 to V75, as well as the organs at risk (OAR) volume. Further, we investigated the correlation between volume changes and changes in Dmean of OAR. Results: During treatment, the rectal volume ranged from 62% to 223% of its initial volume, the bladder volume from 22% to 375%. The average Dmean ranged from 87% to 118% for the rectum and 58% to 160% for the bladder. The Pearson correlation coefficients between volume changes and corresponding changes in Dmean were -0.82 for the bladder and 0.52 for the rectum. The comparison of the dose wall histogram (DWH) and the dose volume histogram (DVH) showed that the DVH underestimates the percentage of the rectal and bladder volume exposed to the high dose region. Conclusion: Relevant variations in the volume of OAR and corresponding dose variations can be observed. For the bladder, an increase in the volume generally leads to lower doses; for the rectum, the correlation is weaker. Having demonstrated remarkable differences in the dose distribution of the DWH and the DVH, the use of DWHs should be considered.

  • The goal of this work is to develop and test a robot-assisted surgery system for spinal fusion. The system is composed of a robot, a surgical planning system, and a navigation system. It plays the role of assisting surgeons for inserting a pedicle screw in the spinal fusion procedure. Compared to conventional methods for spinal fusion, the proposed surgical procedure ensures minimum invasion and better accuracy by using robot and image information. The robot plays the role of positioning and guiding needles, drills, and other surgical instruments or conducts automatic boring and screwing. Pre-operative CT images intra-operative fluoroscopic images are integrated to provide the surgeon with information for surgical planning. Some experiments employing the developed robotic surgery system are conducted. The experimental results confirm that the system is not only able to guide the surgical tools by accurately pointing and orienting the specified location, but also successfully compensate the movement of the patient due to respiration.

Automatic Change Detection Using Unsupervised Saliency Guided Method with UAV and Aerial Images

  • In this paper, an unsupervised saliency guided change detection method using UAV and aerial imagery is proposed. Regions that are more different from other areas are salient, which make them more distinct. The existence of the substantial difference between two images makes saliency proper for guiding the change detection process. Change Vector Analysis (CVA), which has the capability of extracting of overall magnitude and direction of change from multi-spectral and temporal remote sensing data, is used for generating an initial difference image. Combined with an unsupervised CVA and the saliency, Principal Component Analysis(PCA), which is possible to implemented as the guide for change detection method, is proposed for UAV and aerial images. By implementing the saliency generation on the difference map extracted via the CVA, potentially changed areas obtained, and by thresholding the saliency map, most of the interest areas correctly extracted. Finally, the PCA method is implemented to extract features, and K-means clustering is applied to detect changed and unchanged map on the extracted areas. This proposed method is applied to the image sets over the flooded and typhoon-damaged area and is resulted in 95 percent better than the PCA approach compared with manually extracted ground truth for all the data sets. Finally, we compared our approach with the PCA K-means method to show the effectiveness of the method.
