Machine Learning-Based Prediction of COVID-19 Severity and Progression to Critical Illness Using CT Imaging and Clinical Data

  • Objective: To develop a machine learning (ML) pipeline based on radiomics to predict Coronavirus Disease 2019 (COVID-19) severity and the future deterioration to critical illness using CT and clinical variables. Materials and Methods: Clinical data were collected from 981 patients from a multi-institutional international cohort with real-time polymerase chain reaction-confirmed COVID-19. Radiomics features were extracted from chest CT of the patients. The data of the cohort were randomly divided into training, validation, and test sets using a 7:1:2 ratio. A ML pipeline consisting of a model to predict severity and time-to-event model to predict progression to critical illness were trained on radiomics features and clinical variables. The receiver operating characteristic area under the curve (ROC-AUC), concordance index (C-index), and time-dependent ROC-AUC were calculated to determine model performance, which was compared with consensus CT severity scores obtained by visual interpretation by radiologists. Results: Among 981 patients with confirmed COVID-19, 274 patients developed critical illness. Radiomics features and clinical variables resulted in the best performance for the prediction of disease severity with a highest test ROC-AUC of 0.76 compared with 0.70 (0.76 vs. 0.70, p = 0.023) for visual CT severity score and clinical variables. The progression prediction model achieved a test C-index of 0.868 when it was based on the combination of CT radiomics and clinical variables compared with 0.767 when based on CT radiomics features alone (p < 0.001), 0.847 when based on clinical variables alone (p = 0.110), and 0.860 when based on the combination of visual CT severity scores and clinical variables (p = 0.549). Furthermore, the model based on the combination of CT radiomics and clinical variables achieved time-dependent ROC-AUCs of 0.897, 0.933, and 0.927 for the prediction of progression risks at 3, 5 and 7 days, respectively. Conclusion: CT radiomics features combined with clinical variables were predictive of COVID-19 severity and progression to critical illness with fairly high accuracy.

Analytical Psychological Interpretation of the Book of Revelation Focused on Main Visions (요한 계시록의 분석심리학적 해석 : 주요 환상을 중심으로)

  • The Testament is 'the repository of the psyche' which can understand the images revealed from human life, mind, and numinous experiences. When the Scripture is psychologically interpreted, not only does it offer an abundance in biblical exegesis, but is also incredibly valuable in understanding the actual dimensions of life. This study examined the meaning of main visions in the Book of Revelation from the perspective of analytical psychology. The core contents of these visions are 1) the image of Christ represented as One like the Son of Man and Apocalyptic lamb, 2) Sun and Moon Woman and Dragon, 3) Whore Babylon and Bride New Jerusalem, 4) Hieros Gamos and Descending New Jerusalem. Such archetypal images lead to become conscious of an individual, and at the same time the visions of Revelation as a drama of archetype present the transion of civilization, as ultimately penetrating the history of the time period. This article contemplated on the characteristics of the archetypal image emerging in the visions and categorized them into the father archetype, mother archetype, or anima archetype. The ultimate purpose of all the visions can be understood as the ascent, conjunction and descent. This will mean to become conscious of human and incarnation of God, i.e. the individuation process. In our time suffering from masculine one-sidedness, the vision of new Jerusalem presents how the feminine can redeem an individual and this world.

Expert Opinion Questionnaire About Chest CT Scan Using A Negative Pressure Isolation Strecher in COVID-19 Patients: Image Quality and Infection Risk (COVID-19 환자에서 음압격리들것을 이용한 흉부 CT 검사에 대한 전문가 의견 설문: 영상품질과 감염위험)

  • Purpose To survey perceptions of certified physicians on the protocol of chest CT in patients with coronavirus (COVID-19) using a negative pressure isolation stretcher (NPIS). Materials and Methods This study collected questionnaire responses from a total of 27 certified physicians who had previously performed chest CT with NPIS in COVID-19 isolation hospitals. Results The nine surveyed hospitals performed an average of 116 chest CT examinations with NPIS each year. Of these, an average of 24 cases (21%) were contrast chest CT. Of the 9 pulmonologists we surveyed, 5 (56%) agreed that patients who showed abnormalities in serum D-dimer required contrast chest CT. All 9 surveyed radiologists agreed that the image quality of the chest CT with NPIS was sufficient for CT image interpretation regarding pneumonia or pulmonary embolism. Furthermore, in our 9 surveyed infectionologists, 5 (56%) agreed that a risk of secondary infection in the CT room after temporary opening of NPIS could be prevented through a process of disinfection. Conclusion Experienced physicians considered that the effects of NIPS on chest CT image quality was minimal in patients with COVID-19, and the risk of CT room contamination was easily controlled.

  • Multiple myeloma (MM) is a malignant hematologic disease caused by the proliferation of clonal plasma cells in the bone marrow, and its incidence is increasing in Korea. With the development of treatments for MM, the need for early diagnosis and treatment has emerged. In recent years, the International Myeloma Working Group (IMWG) has been constantly revising the laboratory and radiological diagnostic criteria for MM. In addition, as whole-body MRI (WBMR) has been increasing used in the diagnosis and treatment response evaluation of patients with MM, the Myeloma Response Assessment and Diagnosis System (MY-RADS) was created to standardize WBMR image acquisition techniques, image interpretation, and response evaluation methods. Radiologists need to have a detailed knowledge of the features of MM for accurate diagnosis. Thus, in this review article, we describe the imaging method for MM according to the latest IMWG guidelines as well as the image acquisition and response evaluation technique for WBMR according to MY-RADS.

CNN Model-based Arrhythmia Classification using Image-typed ECG Data (이미지 타입의 ECG 데이터를 사용한 CNN 모델 기반 부정맥 분류)

  • Among cardiac diseases, arrhythmias can lead to serious complications such as stroke, heart attack, and heart failure if left untreated, so continuous and accurate ECG monitoring is crucial for clinical care. However, the accurate interpretation of electrocardiogram (ECG) data is entirely dependent on medical doctors, which requires additional time and cost. Therefore, this paper proposes an arrhythmia recognition module for the purpose of developing a medical platform through the analysis of abnormal pulse waveforms based on Lifelogs. The proposed method is to convert ECG data into image format instead of time series data, apply visual pattern recognition technology, and then detect arrhythmia using CNN model. In order to validate the arrhythmia classification of the CNN model by image type conversion of ECG data proposed in this paper, the MIT-BIH arrhythmia dataset was used, and the result showed an accuracy of 97%.

  • Preoperative perforator marking for deep inferior epigastric artery perforator flaps is vital to the success of the procedure in breast reconstruction. Advances in imaging have facilitated accurate identification and preselection of potentially useful perforators. However, the reported imaging accuracy may be lost when preoperatively marking the patient, due to 'mapping errors', as this relies on the use of 2 reported vectors from a landmark such as the umbilicus. Observation errors have been encountered where inaccurate perforator vector measurements have been reported in relation to the umbilicus. Transcription errors have been noted where confusing and wordy reports have been typed or where incorrect units have been given (millimetres vs. centimetres). Interpretation errors have also occurred when using the report for preoperative marking. Furthermore, the marking process may be unnecessarily time-consuming. We describe a bespoke template, created using an individual computed tomography angiography image, that increases the efficiency and accuracy of preoperative marking. The template is created to scale, is individually tailored to the patient, and is particularly useful in cases where multiple potential suitable perforators exist.

  • This study is to analyze a case of US Bravo TV program, Interior therapy with Jeff Lewies, based on analytical psychology by Carl Jung, to understand characteristics of interior therapy in psycho-therapeutic viewpoint. Case study among descriptive observation approaches was chosen as a study method while the study range was set as 8 episodes in 2012 and 8 episodes in 2013. As a result of study, future application of therapy to domestic interior design was suggested as 3 types the following; a) Paranoid and storage disorder type that pursues space design by horizontal elements with a sense of comfortable, starting from throwing things away, and suggests whiteness based on desire to be cleaned; b) Individual taste type starting from give and take between members and recognizing one another. e.g., aligning the U-shaped sofa and giving a sense of comfort by using the achromatic colors; and c) Workroom type involved with the face-to-face type of space design and arrangement of mild grey and brown colors. Finally, this study has its significance in suggesting the direction for new occupational clusters in the field of domestic space design (interior, architecture, environment design, etc.), which could be utilized as a basic data for training program development to foster interior therapy professionals.

  • The knots have been used practically and functionally in close relation to people's daily life, and it shows the beauty of its ornamental purpose. This study was to discover a new formativeness, such as the natural and symbolic beauty of knots, based on the interpretation of knots' basic image, recreating Korean beauty and characteristics within a modern sense. A bowknot is one of the beautiful Korean traditional knots and it is considered as a symbol of transformation. In this study, the formativeness of bowknots was shown and used, resulting in developing textile patterns that symbolize peace, love, joy, and hope. It was believed that the symbolism of bowknots could be suitable for a pattern of kindergarten uniforms which the children would experience for the first time. Considering the functional characteristics without discomfort while the children participate in various activities and movements, we made two pairs of boys' uniforms and two for girls.

  • A rectangle-based relative localization method is proposed for a mobile robot based on a novel geometric formulation. In an artificial environment where a mobile robot navigates, rectangular shapes are ubiquitous. When a scene rectangle is captured using a camera attached to a mobile robot, localization can be performed and described in the relative coordinates of the scene rectangle. Especially, our method works with a single image for a scene rectangle whose aspect ratio is not known. Moreover, a camera calibration is unnecessary with an assumption of the pinhole camera model. The proposed method is largely based on the theory of coupled line cameras (CLC), which provides a basis for efficient computation with analytic solutions and intuitive geometric interpretation. We introduce the fundamentals of CLC and describe the proposed method with some experimental results in simulation environment.

Effect of Printing Conditions on Fluting in Heatset Web Offset Printing (Heatset 윤전 오프셋 인쇄에서 인쇄주름에 대한 인쇄조건의 영향)

  • A printing defect known as fluting (or waviness) of the web printed by heatset web offset printing process is one of the chronically serious problems deteriorating print quality. In this paper, fluting occurrence on uncoated papers was explored in terms of many printing conditions including drying temperature, fountain solution amount, ink supply, and press configurations. For this purpose, fluting on prints from real press runs was appraised in a quantitative manner. As results, ink supply was a distinctive factor for fluting such that the lower ink amount, the milder fluting. However increase in fountain solution seemed to make fluting severer while the effect of drying temperature was inconsistent for each paper. This result might indicate variable drying requirements for each paper. Thereby it was suggested that the optimum drying conditions related to the printabilities of each paper need to be established to minimize fluting potential. A press with short dryer and drastic cooling unit produced higher fluting. Suggestions for future work were given along with interpretation for the results.
