A Case Report on Heat Sensation in Both Feet Caused by Restless Leg Syndrome Using Conservative Korean Medical Treatment (양 족부 열감을 호소하는 하지불안증후군 환자 치험 1례)

  • Objectives: The purpose of this study is to evaluate the effectiveness of electroacupuncture on heat sensation of the feet of a 61-year-old female patient with restless leg syndrome. Methods: Electroacupuncture was performed at four acupoints (KI3 (太谿), KI6 (照海), LR3 (太衝), LR2 (行間)) selected to reduce heat sensation in both feet. The patient's symptoms were assessed using the VAS (Visual Analogue Scale), the SF-MPQ (short-form McGill Pain Questionnaire), and the IRLS (International Restless Legs Scale). The treatment was performed from April 20, 2010 to April 24, 2010. Results: During five sessions of electroacupuncture treatments, the VAS, SF-MPQ, and IRLS scores improved throughout the treatment period and afterwards. Conclusions: The results suggest that electroacupuncture at KI3(太谿), KI6(照海), LR3(太衝), LR2(行間) can be effective for reducing hot flush in both feet.

A Case Report of migraine and a case report of restless legs syndrome treated with Osuyu-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 오수유탕(吳茱萸湯)을 투여한 편두통 증례 및 하지불안증후군 증례 각 1례)

  • Objective : This study was intended to evaluate if treatment with Osuyu-Tang following the disease pattern identification diagnostic system based on Shanghanlun provisions (DPIDS) worked analyzing the progress of patients with migraine and restless legs syndrome and to reinterpret the provision of 309 Soyinbing Osuyu-Tang based on the analysis of whether it worked. Methods : Two cases treated with Soyinbing Osuyu-Tang were analyzed using DPIDS. The migraine and restless legs syndrome in both cases were evaluated using the migraine disability assessment (MIDAS) and the Korean versions of the international restless legs scale (K-IRLS), respectively. Results : In the first case, during 45 days of treatment, the MIDAS decreased from 10 days to 3 days. In the second case, during 30 days of treatment, the K-IRLS decreased from 30 to 9. In both cases, Soyinbing was associated with the disease, although the activity was less; and Leng (冷) was associated with numbness in the kneeling position. Conclusions : It was confirmed that treatment with Osuyu-Tang was effective against migraine and restless legs syndrome and that Leng could be associated with numbness in the kneeling position. The etymological analysis of the Shanghanlun characters suggested the possibility of further clarifying the clinical significance and interpretation of Shanghanlun.

  • Objective: To report the effect of oriental medical treatment on post spinal surgery syndrome (PSSS) in restless leg syndrome (RLS). Methods: We treated this patient with oriental medicine and measured his progress using a Visual Analogue Scale (VAS), the Oswestry Disability Index (ODI), and the International Restless Leg Scale (IRLS). Results: After treatment, most symptoms had decreased: the VAS score dropped from 6 to 3, ODI from 22 to 14, and IRLS from 34 to 20. Conclusions: Oriental medicine may be effective in alleviating PSSS and RLS. However, more rigorous studies are required to identify exactly what treatment is more efficient for PSSS and RLS.

Polysomnographic Characteristics and Prescription Status of Restless Legs Syndrome Patients in Naturalistic Setting (임상현장에서 하지불안증후군 환자의 수면다원검사결과의 특징과 약물처방현황)

  • Objectives: The aim of this study is to evaluate the polysomnographic characteristics and prescription status of restless legs syndrome (RLS) patients in naturalistic setting. Methods: We reviewed medical record of the patients over 18 years olds who (i) satisfied the clinical RLS diagnostic criteria and (ii) had the polysomnography and got treatment related thereto. As a baseline, we evaluated the four diagnostic criteria of the International Restless Legs Syndrome Study Group (IRLSSG) and the International Restless Legs Scale (IRLS) of the subjects. Then the polysomnography and the suggested immobilization test (SIT) were conducted and, after one month of pharmacotherapy using dopamine agonist, the IRLS was evaluated again. Results: A total of 211 subjects participated in this analysis and 94 (44.5%) of them were male and the other 117 (55.5%) were female and the average age of the 211 subjects was $46.9{\pm}14.2$. Out of such 211 subjects, 136 subjects (64.5%) also had the obstructive sleep apnea (OSA), and 53 subjects (25.1%) also had the periodic limb movement disorder (PLMD). 185 subjects (87.7%) out of the 211 subjects had some other sleep disorders except RLS. The results of the polysomnography were as follows : 78.0% of sleep efficiency, 86.8 min of wake after sleep onset, and 3.4% of N3. More specifically, 12.4/h of the average apnea hypopnea index, 14.8/h of the periodic limb movement during sleep (PLMS), 41.2/h of the periodic limb movement during wake during SIT and 21.6/h of total arousal index during sleep. Out of the total subjects, 149 (70.6%) of them took the ropinirole and 47 (22.3%) of them took the pramipexole, and the average dosage of ropinirole was 0.9mg(dosage range 0.125-5 mg) while the average dosage of pramipexole was 0.5 mg (dosage range 0.125-4 mg). The dosage of the ropinirole showed a significant positive correlation with the age (r=0.25, p=0.002) and also with the IRLS (r=0.23, p=0.038). The IRLS at the baseline was 24.9 while the same was decreased down to 13.4 after one month. Conclusions: Analyzing the result of this study, a majority of clinical RLS subjects demonstrated comorbidity with some other sleep disorder such as the OSA or PLMD. 25.1% of the subjects showed a PLMD, which was less than in previous researches and the average PLMS was not very high as 14.8/h. The dosage of dopamine agonist taken was often a bit more than the amount recommended in Korea. A prospective research using a large scale controlled subjects will be necessary with respect to this topic.

A Case of a Patient with Pontine Infarction Complaining of Discomfort in Both Legs (양 하지 불편감을 호소하는 교뇌 경색 환자에 대한 한의 치험 1례)

  • Objective: This study examined the effectiveness of Korean medical treatment for an 81-year-old female patient with pontine infarction complaining of discomfort in both legs. Methods: The patient was hospitalized for 45 days and treated with herbal medicine (mainly Bohyulanshin-tang [補血安神湯], pharmacopuncture, acupuncture, electroacupuncture, and moxibustion). We used the International Restless Legs Scale (IRLS) and Numerical Rating Scale (NRS) and checked the duration of the symptoms to evaluate the clinical effects of the treatment. Results: After 45 days of hospitalization, the IRLS score decreased from 33 to 19, with the lowest score of 8 recorded on the twenty-seventh day after discharge. The NRS score and the duration of symptoms also improved, and the treatment effect continued following discharge. Conclusions: This study suggests that Korean medical treatment may be effective for the treatment of restless legs.

  • HCI, 비전 기반 사용자 인터페이스 또는 제스쳐 인식과 같은 많은 분야에서 3 차원 얼굴 모션을 추정하는 것은 중요한 작업이다. 연속된 2 차원 이미지로부터 3 차원 모션을 추정하기 위한 방법으로는 크게 외형 기반 방법이나 모델을 이용하는 방법이 있다. 본 연구에서는 동영상으로부터 3 차원 실린더 모델과 Optical flow를 이용하여 실시간으로 얼굴 모션을 추정하는 방법을 제안하고자 한다. 초기 프레임으로부터 얼굴의 피부색과 템플릿 매칭을 이용하여 얼굴 영역을 검출하고 검출된 얼굴 영역에 3 차원 실린더 모델을 투영하게 된다. 연속된 프레임으로 부터 Lucas-Kanade 의 Optical flow 를 이용하여 얼굴 모션을 추정한다. 정확한 얼굴 모션 추정을 하기 위해 IRLS 방법을 이용하여 각 픽셀에 대한 가중치를 설정하게 된다. 또한, 동적 템플릿을 이용해 오랫동안 정확한 얼굴 모션 추정하는 방법을 제안한다.

Online training and education from the VR-1 reactor-Lessons learned

  • Hands-on education and training is a key part of fixing and developing technology knowledge and is an inherent part of many engineering and scientific curricula. However, access to large complex training facilities, such as nuclear reactor, could be limited by various factors, such as unavailability of those facilities in the region, high traveling costs or harmonization of the schedules of hands-on E&T with theoretical lectures and with the operational schedule of the facility. To handle the issue, several success stories have been reached with the introduction of the Internet Reactor Labs (IRL). The Internet Reactor Labs can strongly contribute to accessibility of training at research reactors and can contribute to improvements in their utilization. The paper describes the development of the Internet Reactor Lab at the VR-1 reactor of the Czech Technical University in Prague. Contrary to single-purpose IRLs, it presents various modalities of online teaching and training in experimental reactor physics and reactor operation in general as well as outreach activities that have been developed in recent years.

  • Restless leg syndrome is a nervous system disorder that causes an overpowering urge to move one's legs. Symptoms of restless leg syndrome usually worsen when one tries to fall asleep and can prevent sufficient sleep. Restless leg syndrome is common in patients with chronic kidney failure and can be caused or worsened by chronic kidney failure and hemodialysis. Various medications can treat restless leg syndrome, though the long-term use of medications can cause augmentation and adverse effects. In addition, the use of dopamine agonists is limited in patients with chronic kidney failure. This is because the dose of administration should be controlled for patients with chronic kidney failure, and the treatment effect has not been clearly proven. This study reports the case of a 56-year-old male diagnosed with chronic kidney failure complaining of uncomfortable leg sensations. The patient underwent Korean medicine treatment using Jakyakgamcho-tang. The IRLS, NRS, and AIS scores were evaluation tools during treatment. This study suggested significantly improved symptoms through the individual interventions of Jakyakgamcho-tang in a restless leg syndrome patient with chronic kidney failure.

Restless Legs Syndrome and Quality of Life in Hemodialysis Patients (혈액투석을 받는 만성신부전 환자에서 하지불안증후군과 삶의 질)

  • Objective: Restless legs syndrome (RLS) is known to be associated with chronic renal failure (CRF) patients on hemodialysis, however the prevalence of RLS in CRF patients on hemodialysis is variable due to different diagnostic criteria or dialysis technique. A few reports have indicated the association between RLS symptom and lower life quality in CRF patients on hemodialysis. This study aims to investigate the prevalence of RLS and its association with the quality of life in CRF patients of a single dialysis unit in Korea. Methods: A total of 83 Korean CRF patients on hemodialysis in the Korea University Hospital were examined. International Restless Legs Syndrome Study Group (IRLSSG) criteria and International Restless Legs Scale (IRLS) were used to determine the diagnosis and severity of RLS. Questionnaires including Athens Insomnia Scale (AIS), Epworth sleepiness scale (ESS), and Medical Outcome Study Form-36 (SF-36) were administered to all the patients for the assessment of sleep and quality of life. Hamilton Depression Rating Scale (HDRS) and Clinical Global Impression (CGI) were also measured for depression and status of mental illness by psychiatrist. Results: Of the 83 patients, 31 (37.3%) patients were found to have RLS and 43 (51.8%) patients met at least one of the RLS diagnostic criteria. The AIS (t=2.40, p=0.019), ESS (t=2.41, p=0.018), HDRS (t=3.85, p<0.001) and CGI (t=3.52, p=0.001) were higher in the subjects with RLS compared to other subjects. The SF-36 scores were significantly lower in the patients with RLS except physical functioning and bodily pain. Total (p=0.005), physical component (p=0.019), and mental component scores (p=0.019) of SF-36 were significantly lower in patients with more severe RLS symptoms. Conclusion: There was significant relationship between RLS and poor quality of sleep and life. More severe RLS symptom was proven to be an important factor to make a quality of life worsen.
