A Clinical Analysis of Abdominal Stab Injuries (복부 자상 환자에 대한 임상적 고찰)

  • Purpose: This study was done to evaluate the incidence, type of injury, medical consequences, mortality, and prognostic factors associated with stab wounds in patients who visited Kangnam Sacred Heart Hospital, Hallym University, Seoul, Korea. Methods: A retrospective analysis of the clinical data of 40 patients with abdominal stab wounds who visited Kangnam Sacred Heart Hospital, Hallym University, Seoul, Korea from January 1, 2000 to December 31, 2004. Results: 1) The most prevalent age group was patients in their the twenties to fourties (77.5% of all patients), and average age of the patients was 39.3 years. The male-to-female ratio was 2.07:1. 2) The external site of stab wounds was most commonly the periumbilcal area of the abdomen (14 cases, 33.3%). 3) The most commonly injured organs were the liver and the stomach (10 cases each, 16.9%). 4) Operations were performed on all 40 patients, with 9 (22.5%) negative operation findings. 5) Death occurred in 5 cases (12.5%). 6) The trauma indices of the death group were TRISS 51.9%, RTS 3.6 points, and APACHE II 23.0 points. 7) The average transfusion amount of the death group was 13.8 pints Conclusion: These data suggest that the transfusion amount and the trauma index of abdominal stab injuries may be statistically significant factors for predicting mortality.

The Radial Artery Superficial Palmar (RASP) Branch Free Flap for Finger Soft Tissue Reconstruction (요골 동맥 표재 수장 분지 유리 피판술을 이용한 수지 연부 조직의 재건)

  • The radial artery superficial palmar branch free flap is based on the perforators of the superficial palmar branch of the radial artery and its venae comitantes. This flap can be used as a sensible flap including palmar cutaneous branch of the median nerve. Forty radial artery superficial palmar branch free flaps were performed at Centum Institute during October 2010 to December 2011. There were 32 males and 8 females and their mean age were 48 years (range 30 to 66 years). The thumb injured in 13 patients, the index finger in 16 patients, the middle finger in 4 patients, the ring finger in 2 patients, and the little finger in 5 patients. The mean size of the flap was $2.5{\times}3.5$ cm(range $2{\times}2.5$ to $3{\times}7$ cm). The donor site was always closed primarily. The overall survival rate was 90.2 percent. The flaps showed well-padded tissue with glabrous skin. All patients have touch sensation and showed 12 mm two point discrimination in an average(range 8 to 15 mm). Donor site morbidity was conspicuous. One patient showed unsightly scar. Early postoperative range of motion of the affected thumb showed slightly limited radial and palmar abduction. But it improved after postoperative 2 months, and patients did not complaint limitation of motion. In conclusion, the radial artery superficial palmar branch free flap can be used as an option for soft tissue reconstruction of finger defects where local or island flaps are unsuitable.

The Pro and Post Effects of Soshiho-tang on Rat's Liver Damage induced by $CCl_4$ (소자호탕이 $CCl_4$로 유발된 Rat의 간 장해 전후에 미치는 영향)

  • In studying the specific effects of some drugs, animals under experiments get some stress through laboratory environments, drug injection, and adaptation period. These stimuli do harms on liver function. Nowadays studies on liver intoxication and its protection are under research, but the function of dissolution is rarely under studies. It is widely accepted that Soshiho-tang has function of clearing away low spirits, and that it enables liver bloods to move stronger, and to have calm mind. So I injured rats liver by injectioning CCI₄. And the rats took in Soshiho-tang solution. I made a comparison between the functions before and after rat's liver damage. There are many representative serums used to note an index on liver damage. I used total protein, albumin, ALP, GOT, GPT activity, P450, SOD, Catalase, GST, GR, and GPx. I got the following results. When Soshiho-tang was injected after CCI4 intoxication, total protein and albumin decreased. When Soshiho-tang was injected, ALP decreased, compared with control group. When Soshiho-tang was injected after CCI₄ intoxication, AST and ALT decreased. When Soshiho-tang was injected before CCI₄ intoxication, P450 was restrained. When Soshiho-tang was injected, LPO was all restrained. When Soshiho-tang was injected, SOD, Catalase, GST, GR, and, GPx increased. These results show that blood test reveals that it is good to inject Soshiho-tang after CCI₄ intoxication, but that it is good to inject Soshiho-tang before CCI₄ intoxication in case of P450, LPO, SOD, Catalase, GST, GR, and GPx. It is estimated that the medication period and time of liver damage by CCI₄ have counter results, and that it needs more modified study.

Pharmaceutical Studies on Microencapsulated Pivampicillin Hydrochloride (염산피밤피실린의 마이크로캅셀에 관(關)한 약제학적(藥劑學的) 연구(硏究))

  • Pivampicillin hydrochloride is a kind of broad spectrum antibiotics with bactericidal action, and is used in many countries, although it has bitter taste, unpleasant odour and side effects of irritating gastric mucosa, nausea, penicillin allergy, etc. For the improvement of such side effects of pivampicillin hydrochloride, microcapsules, with wall of ethylcellulose, have been prepared by coacervation method. The shape was observed through the scanning electron microscope, the release of the drug into an aqueous medium was studied and the effects of core: ethylcellulose ratio were interpreted as well as making sensory evaluation of taste and odour. There was decreasing trend in dissolution rate of the drug with the increase of core: ethylcellulose ratios, and the smaller microcapsules released their contents more rapidly. A linear relationship was established between the amount of ethylcellulose and the time for 60% release of the drug, and the release pattern was found to have similar characteristics to the release of the drug from an insoluble porous matrix. The release of the drug in the artificial intestinal fluids (pH 6.8) was found to be similar to that in water, while the release in the artificial gastric juice (pH 1.2) was slightly slower. Bioavailability of microcapsule was compared with that of pivampicillin hydrochloride in rabbits using serum concentration and urinary excretion measurements. Microcapsule gave showed slightly higher serum level than pivampicillin hydrochloride from 2 hours after administration, while no significant difference was observed in the accumulated urinary excretion rate between pivampicillin hydrochloride and microcapsule. The ulcer index of pivampicillin hydrochloride administered group was 2.6, and microcapsule administered group was 1.5, while control group was 0.8. Therefore it may be concluded that microencapsulation of pivampicillin hydrochloride is a useful pharmaceutical approach to protect the gastrointestinal tract from being injured by direct contact of pivampicillin hydrochloride without any significant difference of bioavailability.

The Clinical Study of Electroacupuncture Treatment at Hua-Tuo-Jia-Ji-Xue on Spondylolisthesis (협척혈(夾脊穴)에 전침(電鍼)을 이용한 척추전방전위증 환자에 대한 임상적 고찰)

  • Objectives : The study was performed to evaluate the effect of treatment for spondylolisthesis by using electroacupuncture at Hua-Tuo-Jia-Ji-Xue is located about 0.5Cun(寸) at both sides of spinous process of each vertebra. Methods : This clinical study was carried out 5 case with spondylolisthesis, who had bean treated from December, 2006 to November, 2007, in the department of acupuncture and moxibustion, Dongshin University Oriental Medical Hospital. We treated the patient who was diagnosed as spondylolisthesis by simple X-ray or MRI. Results : 1. Degenerative type is 3 cases, and isthmic type is 2 cases. 2. In the distribution of injured level was between 4-5th lumbar vertebra the most(3 cases). 3. The improvement index showed 0.29, 0.56, 0.43 points in degerative type, and 0.39, 0.36 points in isthmic type. 4. Therapeutic efforts above "good" by Kim's criteria was 4 cases. 5. Good improvement was showed in VAS about average 3.4. Conclusions : There was reports about Hua-Tuo-Jia-Ji-Xue electronic acupuncture treatment of patients with spondylolisthesis. It is very effective to improve condition of patients with spondylolisthesis.

  • Despite the movement of safe traffic by the Korean government to reduce deaths in traffic accidents, the casualties increase year by year. In particular, more and more accidents and casualties are reported from car collisions from the back of the vehicles parked for managing traffic accidents on the road, cleaning main roads and medial strips, repairing roads. In order to response to these accidents, the government should take protective measures for road users. In the last decade, seventy-one cases have been reported to occur during highway repair and maintenance. As a result, eight persons were killed and seventy-six were injured, showing the high death rate of 11.3 percent. Therefore, it seems urgent to take some actions against it. The United States and European countries legislate that vehicles of road repair and maintenance should be mandatorily equipped with shock absorber at the back. Korea, however, does not have such legislative measures, which are needed at this time to protect workers on the road. This study compares the performance of the traditional shock absorber for road maintenance vehicles with that of the rear safety guard using air bag, manufactured in accordance with related laws in Korea. Based on the results of the 60km/h rear collision test, this paper proposes improvements in related laws and regulations in an attempt to reduce casualties.

Flexor Tenorrhaphy Using Absorbable Suture Materials

  • Background Nonabsorbable sutures are favorable for repairing flexor tendons. However, absorbable sutures have performed favorably in an animal model. Methods Two-strand sutures using the interlocking modified Kessler method with polydioxanone absorbable sutures 4-0 were used to repair completely ruptured flexor tendons in 55 fingers from 41 consecutive patients. The medical records of average 42 follow up weeks were analyzed retrospectively. The data analyzed using the chi-squared test, and Fisher's exact test was used for postoperative complications. The results were compared with those of other studies. Results Among the index, middle, ring, and little fingers were injured in 9, 17, 16, and 13 fingers, respectively. The injury levels varied from zone 1 to 5. Of the 55 digits in our study, there were 26 (47%) isolated flexor digitorum profundus (FDP) injuries and 29 (53%) combined FDP and with flexor digitorum superficialis injuries. Pulley repair was also conducted. Concomitant injuries of blood vessels and nerves were found in 17 patients (23 fingers); nerve injuries occurred in 5 patients (10 fingers). Two patients had ruptures (3.6%), and one patient had two adhesions (3.6%). Using the original Strickland criteria, all the patients were assessed to be excellent or good. Also, fibrosis and long-term foreign body tissue reactions such as stitch granuloma were less likely occurred in our study. Compared to the Cullen's report that used nonabsorbable sutures, there was no significant difference in the rupture or adhesion rates. Conclusions Therefore, this study suggests that appropriate absorbable core sutures can be used safely for flexor tendon repairs.

Effect of simulated Acid rain on Foliar Structural of Changes of Ginkgo biloba and Pinus thunbergii (은행나무와 곰솔에 처리된 인공산성비에 의한 잎의 형태변화)

  • Visible injury symptoms such as necrosis, chlorosis and premature senscence in the leaves of Ginkgo bloba and Pinus thunbergii treated with acid rain of pH 3.2 or below were observed. The epicuticular wax erosions were observed by SEM after exposure to acid rain of pH 2.4 and 3.2 in G. biloba and pH 4.0 below in P. thunbergii. The adaxial epidennal cells and sponge parenchyma cells were compressed, and those were distorted in the leaves of G. biloba treated with simulated acid rain of pH 3.2 or below. However, vascular tissue was intact. With increase of acidity, mesophyll cells were smaller than those of control while intercellular space in mesophyU was increased. In P. thunbergii, sponge parenchyma cells and vascular tissue except epidennis were distorted after exposure to acid rain of pH 2.4. The size change of stomata in foliar injury was not observed, but the stomatal index and size of stomatal aperture in leaves treated with acid rain increased. The stomata of injured leaf were opened in both species examined.amined.

Traumatic degloving injuries: a prospective study to assess injury patterns, management, and outcomes at a single center in northern India

  • Purpose: This study investigated the epidemiology, management, outcomes, and postoperative disabilities of degloving soft tissue injuries (DSTIs) treated at a tertiary care center in northern India. Methods: A prospective study of patients with DSTIs was conducted over 15 months. The type of degloving injury, the mechanism of injury, and any associated injuries were analyzed using the World Health Organization Disability Assessment Schedule (WHODAS) 2.0 along with the management, outcomes, and disabilities at a 3-month follow-up. Results: Among 75 patients with DSTIs, the average age was 27.5 years, 80.0% were male, and 76.0% had been injured in traffic accidents. The majority (93.3%) were open degloving injuries. Lower limbs were affected most often (62.7%), followed by upper limbs (32.0%). Fractures were the most commonly associated injuries (72.0%). Most patients required more than two procedures, including secondary debridement (41.3%), split skin grafting (80.0%), flap coverage (12.0%), or vacuum-assisted closure (24.0%), while five patients underwent conservative management for closed degloving injuries. Postoperative complications included surgical site infections (14.7%) and skin necrosis (10.7%). Two patients died due to septic shock and multiple organ dysfunction syndrome. The mean length of hospital stay was 11.5±8.1 days, with injuries affecting the lower limbs and perineum requiring longer hospital stays. The mean WHODAS 2.0 disability score at 3 months was 19. Most patients had mild disabilities. Time away from work depended largely upon the site and severity of the injury. Approximately 75% of patients resumed their previous job or study, 14% changed their job, and 8% stopped working completely due to residual disability. Conclusions: DSTIs are common injuries in trauma and management is challenging. Although open DSTI are clinically evident at secondary survey, closed degloving injuries may be missed in the primary survey, necessitating a high index of suspicion, thorough clinical examination, and protocol-based management. Primary preventive strategies (e.g., road safety protocols, preplacement training, and proper protective equipment in industries) are also needed to reduce the incidence of these injuries.

Musculoskeletal Injuries by Weapons in Korean Soldiers: Four-Year Follow-Up (총기 및 폭발물에 의한 군인의 근골격계 손상: 최근 4년간 분석)

  • Purpose: To date, studies of firearm and explosive injuries in the Korean military have been limited compared to its importance. To overcome this, this study examined the characteristics of musculoskeletal damages in soldiers who have suffered firearm and explosive injuries over the past four years. Materials and Methods: From January 2015 to July 2019, military forces who had suffered musculoskeletal injuries from firearms or explosive substances were included. The medical records and radiographs were reviewed retrospectively, and telephone surveys about Short Musculoskeletal Functional Assessment (SMFA) for this group were conducted. To compare the functional outcomes, statistical analysis was performed using a t-test for the types of weapons, and ANOVA for others. Results: Of the 61 patients treated for firearms and explosives injuries, 30 patients (49.2%) were included after undergoing orthopedic treatment due to musculoskeletal injury. The average age at injury was 26.4 years old (21-52 years old). The number of officers and soldiers was similar. Eleven were injured by gunshot and 19 by an explosive device. Sixteen were treated in the Armed Forces Capital Hospital and 10 at private hospitals. More than half of the 16 patients (53.3%) with a fracture had multiple fractures. The most common injury site was the hand (33.3%), followed by the lower leg (30.0%). There were 14 patients (46.7%) with Gustilo-Anderson classification 3B or higher who required a soft tissue reconstruction. Fifteen patients agreed to join the SMFA survey for the functional outcomes. Between officers and soldiers, officers had better scores in the Bother Index compared to soldiers (p=0.0045). Patients treated in the Armed Forces Capital Hospital had better scores in both the Dysfunction and Bother Index compared to private hospitals (p=0.0008, p=0.0149). Conclusion: This is the first study to analyze of weapons injuries in the Korean military. As a result of the study, the orthopedic burden was high in the treating patients with military weapon injuries. In addition, it is necessary to build a military trauma registry, including firearm and explosive injuries, for trauma treatment evaluation and development of military trauma system.
