결핵 환자의 치료경과 중 혈청 내 Cytokine 분비와 변화 (Circulating Cytokine Levels and Changes During the Treatment in Patients with Active Tuberculosis in Korea)

  • 연구배경 : 결핵균 항원으로 세포 매개성 면역반응이 활성화되면 여러 종류의 cytokine이 분비되고 각기 다른 cytokine과 network system으로 작용하여 여러 병태생리적인 과정을 조절한다. 결핵 환자에서 염증반응, 조직파괴 및 질환의 중증도는 proinflammatory cytokine과 suppressive cytokine의 균형과 조합에 의하여 결정되며, 결핵 진행의 제한과 악화에 중요한 역할을 할 것으로 생각되고 있다. 따라서 결핵의 이환과정에서 cytokine의 분비 및 변화와 역할을 파악하는 것이 질환의 병태생리를 이해하는데 크게 도움이 될 수 있다. 대상 및 방법 : 활동성 폐결핵 83명, 기관지 결핵 10명의 치료전과 정상 대조군 20명에서 말초혈액을 채취하여 혈청을 분리하여 $-70^{\circ}C$에 보관하였고, sandwich ELISA 방법을 이용하여 혈청 IL-$1{\beta}$, IL-2, IL-4, IL-6, IL-10, IL-12(p40), TNF-${\alpha}$, IFN-${\gamma}$, TGF-${\beta}$를 측정하였다. 폐결핵 환자 83명을 ATS guideline에 따라 중증도를 분류하였고, 추적관찰 중 탈락자를 제외한 45명에서 초치료 2개월과 6개월 후 각각 혈청 sIL-$1{\beta}$, IL-2, IL-4, IL-6, IL-10, IL-12(p40), TNF-${\alpha}$, IFN-${\gamma}$, TGF-${\beta}$를 재측정하였다. 결 과 : 1) IL-$1{\beta}$, TNF-${\alpha}$, IFN-${\gamma}$는 폐결핵 환자에서 대조군에 비하여 증가된 경향을 보였고(p>0.05), IL-6는 폐결핵군에서 통계적으로 유의하게 증가되었다(p<0.05). TGF-${\beta}$는 폐결핵과 기관지 결핵환자에서의 분비가 대조군에 비하여 감소된 경향을 보였으며(p>0.05), IL-2, IL-12(p40), IL-4, IL-10은 대조군과 폐결핵 환자에서 별다른 차이를 보이지 않았다. 2) 기관지 결핵 환자에서 IL-6, TNF-${\alpha}$는 대조군에 비해 증가되고 TGF-${\beta}$는 감소된 경향을 보였다(p>0.05). 폐결핵에 비해 IL-12(p40)의 분비는 증가된 경향을 보였다. 3) 결핵의 중증도가 심할수록 IFN-${\gamma}$와 IL-6가 유의하게 증가하였고(P<0.05), 특히 중증군에서 현저하였다. 4) 폐결핵 환자에서 치료전 측정한 IL-$1{\beta}$, IL-6, TNF-${\alpha}$ 간 및 IL-2, IL-4, IL-12 간에는 강한 양의 상관관계를 보였다(p<0.01). 5) 폐결핵 환자 45명에서 측정한 IL-6와 IFN-${\gamma}$는 치료후 2개월과 6개월에 각각 통계적으로 유의하게 감소되었다(p<0.05). 결 론 : 이상의 결과로 결핵의 병태생리에 있어서 여러 cytokine간의 균형과 조합의 변화가 숙주의 염증, 조직파괴 및 결핵의 중증도와 관련되어 있을 것으로 생각되지만 결핵의 형태나 면역반응 정도에 따른 다양성을 보여서 결과의 해석과 cytokine 측정의 임상적 이용에 대한 연구가 더욱 필요할 것으로 사료된다.

Schizandrin에 의한 호염구와 비만세포의 Th2 관련 사이토카인 발현 억제 (Suppression of Th2-associated Cytokines Expression by Schizandrin in Basophils and Mast Cells)

  • Schizandrin (SZ), was isolated from the fruit of Schizandra chinensis, has been reported to have many biological properties, including anti-inflammatory and antitumor activities. However, its anti-allergic effects are not completely elucidated. We focused on the anti-allergic effects of SZ in PMA/ionomycin (PI)-induced rat basophilic leukemia (RBL-2H3) cells and P815 mast cells. Cytokines (IL-4, IL-13), synthesized by basophils and mast cells, are implicated in pathological conditions such as asthma and allergy. The production of IL-4 and IL-13 was quantified by ELISA and the mRNA expression was detected by using RT-PCR assay. In this study, we found that SZ did not show cytotoxic effect at up to 100 ${\mu}M$ on RBL-2H3 cells and mast cells. In addition, SZ inhibited the production of IL-4 and IL-13 and also decreased the level of mRNA in PI-induced RBL-2H3 cells and mast cells in a dose-dependent manner. Thus, we suggest that SZ may have the effect on preventing allergic disorders by inhibiting IL-4 and IL-13 cytokines.

길경약침(桔梗藥鍼)의 천식억제(喘息抑制) 및 면역조절효과(免疫調節效果)에 대(對)한 실험적(實驗的) 연구(硏究) (The Experimental Study on the Suppression Effect of Asthma and Immune Response Improvement of Platycodi Radix Herbal-acupuncture)

  • Objectives : The aim of this study was to investigate the effect of Asthma-depression and Immunoregulation with PR-HAS(Herbal-acupuncture with Platycodi Radix infusion solution) injection at Joksamni(St36) on ovalbumin-induced asthma in mice. Methods : C57BL/6 mice were sensitized and challenged with OVA(ovalbumin) for 12 weeks(once a week). The experimental group(OVA-PR-HA) wase treated with concentrations(1%) of PR-HAS at Joksamni(St36) for the later 8 weeks(3times/week). The second experimental group(OVA-Needle prick) was treated with Needle-Prick at Joksamni(St36) for the later 8 weeks(3times/week). Results : 1. The weight and total cells in the mice lung treated with PR-HA decreased significantly compared with that of control group. 2. Total leukocytes and eosinophils in BALF of the mice group treated with PR-HA decreased remarkably compared with those of control group. 3. The sticking of collagen on histological analysis of lung sections, the mice group treated with PR-HA decreased significantly compared with those of control group. 4. The concentrations of IL-4, IL-5, IgE in BALF, and IL-4, IL-5, IL-13 in serum of the mice group treated with PR-HA decreased significantly compared with those of control group. 5. The number of $Gr-1^+/CD11b^+\;and\;CD11b^+$ cells in the lungs of the mice group treated with PR-HA decreased significantly compared with that of control group. 6. The numbers of $CCR3^+\;cells,\;CD4^+\;cells\;and\;CD8^+\;cells$ in the lungs, and $CD3e^+/CD69^+$ in the lungs of the mice group treated with PR-HA decreased significantly compared with those of control group. 7. The mRNA expression of ${\beta}-actin,\;TNF-{\alpha}$, IL-4, IL-5,IL-13 in the mice group treated with PR-HA with RT-PCR decreased significantly compared with those of control group. Conclusion : The concentrations of IL-4, IL-5, IgE in BALF, and IL-4, IL-5, IL-13 in serum of the mice group treated with PR-HA decreased significantly compared with those of control group. The number of $Gr-1^+/CD11b^+\;and\;CD11b^+$ cells in the lungs of the mice group treated with PR-HA decreased significantly compared with that of control group. 창출(蒼朮)이 천식 관련 Th1/Th2 세포 분비 cytokine에 미치는 영향 (Studying of the Effects of Atractylodes Japonica Extract on Th1/Th2 Cell-derived Cytokines)

  • Background and Objective : Atractylodes japonica (AJ) is a commonly-used herbal medicine in Asian countries such as Korea, China and Japan. The present study was designated to evaluate the direct effects of AJ on helper T cell activities and on Th1/Th2 lineage development in vitro. Materials and Methods : Spleen cells from 8-week BALB/c mice were cultured in CR extracts containing medium without activation for 24 hours and with activation for 48 hours. CD4+ T cells were isolated and analyzed for mRNA expression levels of INF-$\gamma$, IL-4, T-bet and GATA-3 by RT-PCR and secretion cytokines levels of INF-$\gamma$, IL-2, IL-4, IL-5 and IL-10 by ELISA. Results : The results demonstrated that AJ had no mitogenic effects on unstimulated CD4+ T cells, but augmented CD4+T-cell proliferation upon activation with anti-CD3/anti-CD28 antibodies in a dose-dependent manner. AJ treatment significantly increased CD4+ T cell population and IFN-$\gamma$ expression was significantly enhanced, while IL-4 expression significantly decreased. In addition, in vitro Th1/Th2 polarization experiments revealed that AJ enhanced IFN-$\gamma$ secretion in Th1 cells, but reduced the IL-4 in Th2 cells in dose-dependent manner. Conclusion : These results suggest that AJ treatment could be a desirable alternative therapy for the prevention or correction of Th2 dominant pathological disorders, such as allergy and asthma.

Cross-talk between STAT6 and Ras/MAPK Pathway for the IL-4-mediated T Cell Survival

  • As a prototypic Thl vs Th2 cytokine, IFN-$\gamma$ and IL-4 activate distinct STAT proteins, STAT1 and STATE, respectively. In cytokine-producing Jurkat T cells, IL-4 is effectively rescued from cell death that is induced by dexamethasone, but IFN-$\gamma$ failed to do so. Since the Ras/MAPK pathway is known to play an important role in cytokine-induced cell survival, we investigated the mechanism of T cell survival through the analysis of functional cross-talk between Ras/MAPK and distinct STAT proteins that are activated by IL-4 and IFN-$\gamma$. Although IL-4 and IFN-$\gamma$ each induced the activation of STATE and STATI. in Jurkat T cells, respectively, only IL-4 was capable of inducing MAPK. Along with tyrosine kinase inhibitors, MEK/MAPK inhibitors also caused a significant suppression of the IL-4-induced STATE activity. This suggests a positive regulation of STATE by MAPK during IL-4 signal transduction. Furthermore, transfection studies with dominant active (da) vs dominant negative (dn) Ras revealed that daRas, but not dnRas, selectively up-regulated the expression and activity of STATE with a concomitant increase in MAPK activity. These results, therefore, suggest that there is a functional cross-talk between the Ras/MAPK and Jak/STAT6 pathways, which may have a role in the IL-4-induced T cell survival.

Vaccination with an Ovalbumin/Interleukin-4 Fusion DNA Efficiently Induces Th2 Cell-Mediated Immune Responses in an Ovalbumin-Specific Manner

  • to more effectively drive immune responses toward antigen-specific T helper type 2 (Th2) cell-mediated responses, we constructed a mammalian expression vetor (oPVA/IL4) carrying a fused gene in which the ovalbumin (OVA) cDNA was covalently linked to murine interleukin-4 (IL-4) cDNA. A biologically active OVA/IL4 DNA, as demonstrated by Wes tern blotting and cytokine bioassay. In tramuscular injection of BALB/c mice with the pOVA/IL4 DNA increased both the production of OVA-specific IL-4 by CD$4^{+}$ T cells and the ratio of anti-OVA lgG1 to anti-OVA lgG2a isotypes, while the injection with the pOVA DNA alone, or with the mixture of the pOVA and pIL4 DNA did no or little increase. furthermore, the OVA-specific, Th2 cell-mediated immune responses were significantly enhanced by multiple injections with the pOVA/IL4 DNA. These studies indicate that the direct linkage of an OVA gene to an IL-4 gene in the expression plasmid confines the effects of IL-4 to the OVA-specific cells, efficiently driving the immune response toward OVA-specific, Th2 cell-mediated responses.

Role of Interleukin-4 (IL-4) in Respiratory Infection and Allergy Caused by Early-Life Chlamydia Infection

  • Chlamydia pneumoniae is a type of pathogenic gram-negative bacteria that causes various respiratory tract infections including asthma. Chlamydia species infect humans and cause respiratory infection by rupturing the lining of the respiratory which includes the throat, lungs and windpipe. Meanwhile, the function of interleukin-4 (IL-4) in Ch. pneumoniae respiratory infection and its association with the development of airway hyperresponsiveness (AHR) in adulthood and causing allergic airway disease (AAD) are not understood properly. We therefore investigated the role of IL-4 in respiratory infection and allergy caused by early life Chlamydia infection. In this study, Ch. pneumonia strain was propagated and cultured in HEp-2 cells according to standard protocol and infant C57BL/6 mice around 3-4 weeks old were infected to study the role of IL-4 in respiratory infection and allergy caused by early life Chlamydia infection. We observed that IL-4 is linked with Chlamydia respiratory infection and its absence lowers respiratory infection. IL-4R α2 is also responsible for controlling the IL-4 signaling pathway and averts the progression of infection and inflammation. Furthermore, the IL-4 signaling pathway also influences infection-induced AHR and aids in increasing AAD severity. STAT6 also promotes respiratory infection caused by Ch. pneumoniae and further enhanced its downstream process. Our study concluded that IL-4 is a potential target for preventing infection-induced AHR and severe asthma.

Role of IL-23 and Th17 Cells in Airway Inflammation in Asthma

  • Asthma is characterized by chronic airway inflammation with intense eosinophil and lymphocyte infiltration, mucus hyperproduction, and airway hyperresponsiveness. Accumulating evidence indicates that antigen-specific Th2 cells and their cytokines such as IL-4, IL-5, and IL-13 orchestrate these pathognomonic features of asthma. In addition, we and others have recently shown that IL-17-producing $CD4^+$ T cells (Th17 cells) and IL-23, an IL-12-related cytokine that is essential for survival and functional maturation of Th17 cells, are involved in antigen-induced airway inflammation. In this review, our current understanding of the roles of IL-23 and Th17 cells in the pathogenesis of allergic airway inflammation will be summarized.

Cytokine Pattern is Affected by Training Intensity in Women Futsal Players

  • To find the relation between exercise and cytokines, we examined the effect of the training intensity on the levels of cytokines, including interferon-gamma (IFN-γ), interlukine-4 (IL-4) and interlukine-4/interferon-gamma ratio (IL-4/IFN-γ ratio) in female Futsal players. Twelve well-trained female college Futsal players aged 19~22 participated in this study. The athletes completed 30-min of running at 60~65% maximal heart rate [moderate-intensity exercise], and 30-min of running at 75~80% maximal heart rate [high-intensity exercise]. peripheral blood samples were collected 24 h before and 24 h and 48 h after each of the exercise bouts. finding showed that The 30-min bout of moderate-intensity exercise induced a significant increase in IFN-γ (p=0.01) and significant decreases in IL-4 (p=0.001) and IL-4/IFN-γ ratio (p=0.003). And also, 30-min of running at 75~80% maximal heart rate induced increase in IFN-γ (p=0.07) and decreased in IL-4 (p=0.01) and IL-4/IFN-γ ratio (p=0.06) that these changes not significantly. In summary, exercise intensity can effect on the magnitude of changes in cytokines. It seems that moderate intensity exercise enhances cytokine pattern in female college Futsal players.

소 난관 상피 세포의 체외 배양시 IL-4의 첨가에 의한 배양액내 호르몬 변화 (Hormonal Changes in Cultured Medium on In Vitro Culture of Bovine Oviduct Epithelial Cells (BOEC) Supplemented with UK-4)

  • 본 연구는 소 난관 상피 세포를 채취 체외 배양을 실시하고, 이에 착상과 관련이 있은 IL-4를 첨가하여 배양액내의 임신에 관련된 호르몬들(P4, E2, TGF-$\beta$)의 변화를 관찰함으로써, 소 난관 상피 세포와 착상과의 관계를 구명하고자 실시하였으며, 그에 따른 결과는 다음과 같다. 소 난관 상피 세포의 체외 배양시 IL-4 첨가에 의한 배양액내의 P4의 생산은 0.001 ng/ml의 IL-4를 첨가한 배양액의 P4의 농도는 배양 시간이 경과할수록 증가하는 경향을 보였으며, 24시간보다 120시간에서는 약 2배의 생산을 보여 유의적인 차이를 나타냈다(P<0.05). 0.01 ng/ml의 경우에도 0.001의 경우와 유사한 경향을 보였으나, 0.001 ng/ml의 경우보다는 다소 생산량이 낮았다. 0.1이나 1 ng/ml의 경우는 배양 시간에 따른 생산량은 다른 두 가지의 농도와 같이 배양시간 96시간까지는 증가하였으나, 배양 시간 120시간에서는 감소하였다. 소 난관 상피 세포의 체외 배양시 IL-4 첨가에 의한 배양액내의 E2의 생산은 0.001, 0.01 ng/ml 첨가시는 P4의 경우와 같이 배양 시간 72시간까지 배양 시간에 따라 생산량이 증가하여 유의적인 차이를 나타내었으며(P<0.05), 0.1 및 1 ng/ml의 경우는 배양 시간 96시간까지 증가하는 경향을 보였다. 그러나 배양 시간 120 시간에는 IL-4의 첨가 농도에 관계없이 배양 시간 24시간째의 생산량과 유사한 경향을 나타냈다. 소 난관 상피 세포의 체외 배양시 IL-4 첨가에 의한 배양액 내 TGF-$\beta$의 생산은 IL-4의 첨가 농도 및 배양 시간에 대하여 차이를 나타내지 않았으며, 유의성도 나타나지 않았다. 배양 초기에 비하여 배양시간 120시간에는 약간 생산이 낮아지는 것으로 나타나 IL-4에 의한 TGF-$\beta$의 생산은 배양 시간 96이후에는 활성이 저하하는 것으로 나타났다. 이상의 결과로 소 난관 상피 세포의 체외 배양시 IL-4 첨가는 P4 및 E2의 생산에 영향을 미치는 것으로 나타났으며, TGF-$\beta$의 생산에는 영향을 미치지 않는 것으로 나타나, IL-4는 소의 임신의 성립에 중요한 역할을 하며, 난관 상피 세포 이외의 자성 생식 기도 내에 있어서 IL-4와 관련된 기전에 대하여 더 많은 연구가 요구된다.
