Background Skin defects in the hands are common injuries, and autologous skin grafting is the ideal treatment. However, complications can occur at the donor and recipient sites. This study compares the "Swing-door" technique with conventional skin grafting. Methods From August 2019 to February 2023, 19 patients with skin defects of hand underwent the "Swing-door" split-thickness skin graft (STSG) technique. The thin epithelial layer was elevated with proximal part attached. Skin graft was harvested beneath. Donor site was then closed with epithelial flap like a "Swing-door". The outcomes were evaluated in terms of healing time, scar formation, and pain at the donor and recipient sites. The data were compared with the conventional STSG. Results The "Swing-door" group had lower graft take percentages, but complications did not significantly differ between the two groups. The "Swing-door" technique resulted in better cosmetic outcomes, as evidenced by lower Vancouver Scar Scale scores, faster donor site epithelialization, and reduced pain and discomfort during the early postoperative period, as measured by Visual Analog Scale. Conclusion The "Swing-door" STSG is a useful alternative for treating hand skin defects.

Restoration of severly carious, malformed or traumatically fractured primary incisors is one of the most difficult challenges in restorative dental care for preschoolers. In restoring primary anterior teeth, four types of complete coverage crowns have been attempted over the years. Stainless steel crowns, open-faced stainless steel crowns, acid-etched crowns(strip crowns), and preformed polycarbonate crowns have been the choices. While these restorations have been acceptable for many patients, they all have very distinct limitations. The ideal full coronal restoration for a primary incisor requires durability, retention, and esthetics. This paper reported veneered stainless steel crown restoration of primary anterior teeth by silicoater technique. Veneered stainless steel crown restoration by silicoater technique is considered to be a durable, retentive, and esthetic restoration.
Ideal rotator cuff repair is to maintain high fixation strength and minimize gap formation for optimizing the environment of biologic healing of tendon to bone. Among the current repair techniques, the suture bridge technique is superior to single- or double-row repair in ultimate load to failure, gap formation, restoring anatomical footprint and achieving pressurized contact area. The suture bridge technique also minimizes gap formation and has rotational and torsional resistances allowing early rehabilitation. However, despite superior biomechanical characteristics of the suture bridge technique, there is no evidence that these mechanical advantages result in better clinical outcomes. Furthermore, there is no difference in failure rates between the double-row repair and suture bridge techniques. An appropriate repair technique should be determined based on tear size and pattern and tendon quality.

Background Nipple-areolar complex (NAC) reconstruction is the final stage of breast reconstruction. Ideal reconstruction of the NAC requires symmetry in position, size, shape, texture, pigmentation, and permanent projection, and although many technical descriptions of NAC reconstruction exist in the medical literature, there is no gold standard technique. The technique devised by the authors is very versatile, with excellent results, and it enables 1-step reconstruction with optimal results in terms of shape and nipple projection. Methods Our technique consists of a combination of modified local flaps and a full-thickness skin graft. Patients were observed for 18 months to estimate the amount of retraction. This procedure was performed in 40 patients, four of them bilaterally. The duration of the follow-up was 30 months. Complications occurred in 10% of patients, and included infections (5%), ischemia (2.5%), and hematoma (2.5%). Results No cases of total nipple necrosis were reported. The NAC shape remained optimal in all cases, with a very small reduction of the vertical and horizontal diameters of the areola, which maintained its designed round shape well, and negligible retraction in the diameter and projection of the nipple. Conclusions The oval technique represents a major step forward, involving a combination of existing techniques, such as the C-V flap and the cutaneous graft, to achieve excellent results regarding areola shape and nipple projection, significantly reducing the cases of nipple ischemia. These results were substantially obtained through subcutaneous equatorial sutures, skin grafting, and flattening of the apexes of the flap.
Stereotactic radiosurgery (SRS) is a technique that delivers a high dose to a target legion and a low dose to a critical organ through only one or a few irradiations. For this purpose, many mathematical methods for optimization have been proposed. There are some limitations to using these methods: the long calculation time and difficulty in finding a unique solution due to different tumor shapes. In this study, many clinical target shapes were examined to find a typical pattern of tumor shapes from which some possible ideal geometrical shapes, such as spheres, cylinders, cones or a combination, are assumed to approximate real tumor shapes. Using the arrangement of multiple isocenters, optimum variables, such as isocenter positions or collimator size, were determined. A database was formed from these results. The optimization procedure consisted of the following steps: Any shape of tumor was first assumed to an ideal model through a geometry comparison algorithm, then optimum variables for ideal geometry chosen from the predetermined database, followed by a final adjustment of the optimum parameters using the real tumor shape. Although the result of applying the database to other patients was not superior to the result of optimization in each case, it can be acceptable as a plan starling point.

Discussions for water resources vulnerability and index development with sustainable concept are actively being made in recent years. Based on such index, water resources vulnerability of present and future is determined and diagnosed. This study calculated the water resources vulnerability rankings by 152 nations, using indicator related to water resources assessment that can be obtained from World Bank, VRI (Vulnerability Resilience Indicator), ESI (Environmental Sustainability Index). In order to quantitatively assess of water resources vulnerability based on this indicator, TOPSIS (Technique for Order Preference by Similarity to Ideal Solution) technique was applied to index water vulnerability and to determine the rankings by nations. As a results, South Korea was ranked as the 88th among the 152 nations including Korea. Among the continents, Oceania was the least vulnerable and Afirica was the most vulnerable in continents. WUnited State, Japan, Korea and China were vulnerable in order among the major countries. Therefore, water resources vulnerability rankings by nations in this study helps us to better understand the situation of South Korea and provide the data for water resources planning and measure.

In the past, researchers mainly used the supervised learning technique of machine learning to analyze power data and investigated the identification of patterns through the data mining technique. Data analysis research, however, faces its limitations with the old data classification and analysis techniques today when the size of electric power data has increased with the possible real-time provision of data. This study thus set out to propose a clustering architecture to analyze large-sized electric power data. The clustering process proposed in the study supplements the K-means algorithm, an unsupervised learning technique, for its problems and is capable of automating the entire process from the collection of electric power data to their analysis. In the present study, power data were categorized and analyzed in total three levels, which include the row data level, clustering level, and user interface level. In addition, the investigator identified K, the ideal number of clusters, based on principal component analysis and normal distribution and proposed an altered K-means algorithm to reduce data that would be categorized as ideal points in order to increase the efficiency of clustering.

Functional residual capacity (FRC) is an important diagnostic parameter measured using $N_{2}$ analyzer. Since $N_{2}$ analyzer is expensive as well as cumbersome for use of noisy vacuum pump, the FRC measurement becomes possible only in large well-equipped hospitals. The present study introduced a new $TN_{2}$ wash-out technique to measure FRC by $O_{2}/CO_{2}$ analysis, which is relatively cheaper and much simpler to apply. Slower $O_{2}$ response was compensated for high frequency to be coincided with $CO_{2}$ response, thereby enabled indirect, but accurate $N_{2}$ concentration measurement. FRC was estimated by continuous integration of expired $N_{2}$ volume obtained with air flow signal. Experiment with 3 L syringe, a standard calibration device recommended by the American Thoracic Society, demonstrated less than 1% error at 0, 1, and 2 L. Correlation coefficient was almost ideal, guaranteeing linear estimation of FRC. The present technique is inexpensive and simple to apply, thus should he of great convenience.

The communication system using multiple antennas improves link reliability or system capacity using tx & rx diversity, spatial multiplexing, and beamforming technique with services and characteristics of channel environment. This system is sensitive to spatial channel environment. In case of diversity, the lower correlation among links as a LoS environment, the better performance is acquired. In practical channel environment, However, there is high correlation, and there is high performance difference between ideal case and practical case. On the contrary, in case of beamforming, the higher correlation among links, the better performance is acquired. If we use the spatial adaptive transmission technique with spatial channel characteristics, we can get the system that maintains minimum link reliability and guarantees the overall system performance. In this paper, we propose the adaptive transmission and reception technique which use diversity or beamforming technique with channel characteristics.

Background The forehead, which occupies about one third of the face, is one of the major determinants of a feminine or masculine look. Various methods have been used for the augmentation of the forehead using autologous fat grafts or alloplastic materials. Methylmethacrylate (MMA) is the most appropriate material for augmentation of the forehead, and we have used an injection-molding technique with MMA to achieve satisfactory results. Methods Under local anesthesia with intravenous (IV) sedation, an incision was made on the scalp and a meticulous and delicate subperiosteal dissection was then performed. MMA monomers and polymers were mixed, the dough was injected into the space created, and manual molding was performed along with direct inspection. This surgery was indicated for patients who wanted to correct an unattractive appearance by forehead augmentation. Every patient in this study visited our clinics 3 months after surgery to evaluate the results. We judged the postoperative results in terms of re-operation rates caused by the dissatisfaction of the patients and complications. Results During a 13-year period, 516 patients underwent forehead augmentation with MMA. With the injection-molding technique, the inner surface of the MMA implant is positioned close to the underlying frontal bone, which minimizes the gap between the implant and bone. The borders of the implant should be tapered sufficiently until no longer palpable or visible. Only 28 patients (5.4%) underwent a re-operation due to an undesirable postoperative appearance. Conclusions The injection-molding technique using MMA is a simple, safe, and ideal method for the augmentation of the forehead.
