대시호탕이 배양신경세포를 이용한 저산소증에 미치는 영향 (The Effects of Daesiho-tang Extracts on Hypoxic E18 Cortical Neuroblast)

  • Objectives : In acute stage of CVA, many patients experience Bugibultong as a symptom. Daesiho-tang is one of the most useful prescriptions for constipated stroke patients. This study investigated the effects of Daesiho-tang extracts on hypoxic E18 cortical neuroblast. Methods : To evaluate the effect of Daesiho-tang extracts on enuronal death caused by hypocia the neuronal viability and protein expressions of NMDA receptor subunits, ${\alpha}CaMK$ II PDE2 PSD95 densin-180, $eEF-l{\alpha}$ and GFAP were investigated. Results : We guessed that Daesiho-tang extracts worked to protect against cell damage through effecting on the protein expression of PSD(post synatpic density), especially increase of ${\alpha}CaMK$ II, $eEF-l{\alpha}$ PDE2, PSD95 and densin-180 Conclusions : According to the above results, it is supposed that Daesiho-tang is clinically applicable to stroke patients in the acute stage.

인삼(人蔘)이 허혈성 중추신경 손상으로 발현 증가된 CD81 및 GFAP에 미치는 영향 (The Effect of the Radix Ginseng on Expression of CDSI and GFAP Following Hypoxic Injury on Central Nervous System)

  • Objectives : Gliosis disturbs recovery of damaged astrocytes following central nervous system(CNS) injury. Gliosis relates to up-regulation of CD81 and GFAP. In glial cells at injured CNS, the expression of CD81 and GFAP is increased. It is possible that when the gliosis formation is suppressed, regeneration of oxons can occur. According to the recent study, the treatment with anti CD81 antibodies enhanced functional recovery in rats with spinal injury. So, the author studies the effect of water extract of Radix Ginseng on regulation of CD81 and GFAP with CNS injury. Methods : In the cell study, hypoxic damage was induced by CoC12. And according to Longa et al, cerebral ischemia was made by middle cerebral artery occlusion in the rat. Cross sections of rat brain were examined under microscope. MTT analysis was performed to examine cell viability, cell based ELISA, Western Blot and PCR were used to detect the expression of CD81 and GFAP. Results : The following results were obtained. 1. We found that CD81 and GFAP were decreased in hypoxic injured cells following Radix Ginseng administration. 2. We injected the extract of Radix Ginseng to the middle cerebral artery occlusion in rats, and the immunohistochemistry analysis showed that CDS1 and GFAP were decreased. Conclusions : These results show that the extract of Radix Ginseng could suppress the gliosis formation and prevent cell death, by controlling the expression of CDS1 and GFAP. Therefore, Radix Ginseng could be a useful to regenerate CNS injury.

Analysis of the global gene expression profiles in genomic instability-induced cervical cancer cells

  • Preserving intact genetic material and delivering it to the next generation are the most significant tasks of living organisms. The integrity of DNA sequences is under constant threat from endogenous and exogenous factors. The accumulation of damaged or incompletely-repaired DNA can cause serious problems in cells, including cell death or cancer development. Various DNA damage detection systems and repair mechanisms have evolved at the cellular level. Although the mechanisms of these responses have been extensively studied, the global RNA expression profiles associated with genomic instability are not well-known. To detect global gene expression changes under different DNA damage and hypoxic conditions, we performed RNA-seq after treating human cervical cancer cells with ionizing radiation (IR), hydroxyurea, mitomycin C (MMC), or 1% O2 (hypoxia). Results showed that the expression of 184-1037 genes was altered by each stimulus. We found that the expression of 51 genes changed under IR, MMC, and hypoxia. These findings revealed damage-specific genes that varied differently according to each stimulus and common genes that are universally altered in genetic instability.

저산소성 뇌손상의 자기공명영상 소견: 유병기간 및 예후와의 연관성 (MR Findings of Hypoxic Brain Damage: Relation to Time Elapse and Prognosis of Patients)

  • 목적 : 저산소성 뇌손상의 자기공명영상(MR) 소견을 유병기간 및 예후와 연관 지어 알아보고자 하였다. 대상 및 방법 : 임상적으로 저산소성 뇌손상으로 진단된 환자 18명의 19예의 MR을 대상으로 분석하였다. MR은 저산소증 발생 후 1일에서 20일(평균 8.6일)사이에 시행되었으며, 대상환자의 T1 및 T2 강조영상에서 비정상 고신호강도 병변의 위치, 뇌부종의 동반 유무 및 시간경과와 예후에 따른 병변의 양상을 분석하였다. 결과 : 전체 19예의 MR영상에서, T2 강조영상에서의 고신호강도 병변은 기저핵(15예, 78.9%), 뇌피질(13예, 68.4%), 뇌백질(9예, 47.4%), 시상(6예, 31.6%), 소뇌(4예, 21.1%), 뇌간(1예, 5.3%)의 순으로 관찰되었다. 뇌피질의 병변은 모든 경우 심부회색질에도 이상소견을 동반 하였으며 양측성, 미만성의 병변을 보였고, 뇌피질 대부분 또는 두정 후두엽에 국한되어 나타났다. 뇌피질 및 심부회색질의 T2 강조영상에서의 고신호는 급성기(6일이내)부터 아급성기(6일이후)에 걸쳐 1예를 제외한 전 예에서 다양한 정도로 있었다. 뇌간이나 소뇌의 침범은 비교적 드물었고 모두 뇌피질의 병변이 동반되어 관찰되었다. 뇌백질의 병변도 대부분 아급성기에 뇌피질과 심부회색질의 병변이 있을 때 발생했지만 양측성 분수경계역 뇌경색의 소견으로 단독 침범한 경우도 1예 있었다. T1 강조영상에서의 고신호강도 병변은 뇌피질 및 심부회색질에 주로 아급성기에 관찰되었으나, 급성기에도 일부 관찰되었다. 뇌부종은 11예에서 급성기, 아급성기에 걸쳐 관찰되었다. 의식이 회복된 환자의 MR에서 뇌피질의 침범이나 뇌부종의 빈도가 적었다. 결론 : 저산소성 뇌손상의 MR소견은 다양하지만, 거의 대부분 뇌피질과 심부 뇌회색질을 침범하는 양측성 미만성의 병변으로 나타났다. 뇌백질의 병변은 아급성기 이후에 주로 나타나나 급성기의 혹은 단독의 병변으로도 나타날 수 있다. 뇌피질의 침범정도가 경미하거나 뇌부종이 미약한 경우 예후가 좋았다.

  • Hypoxic ischemic encephalopathy (HIE) is an important cause of permanent damage to central nervous system cells, most cases with oxygen deprivation in the nenonate due to birth asphyxia. Survival children with HIE develop problems such as cerebral palsy, mental retardation, learning difficulties, and other permanent neuro-developmental disabilities. This article report two cases of the children with HIE who had be traumatized on their teeth from oropharyngeal airway (OPA) in the emergency situation. The patients with the disease accompanying seizure or convulsion, needs special consideration for the prevention from dental trauma in emergency airway management.

Sertad1 Induces Neurological Injury after Ischemic Stroke via the CDK4/p-Rb Pathway

  • SERTA domain-containing protein 1 (Sertad1) is upregulated in the models of DNA damage and Alzheimer's disease, contributing to neuronal death. However, the role and mechanism of Sertad1 in ischemic/hypoxic neurological injury remain unclear. In the present study, our results showed that the expression of Sertad1 was upregulated in a mouse middle cerebral artery occlusion and reperfusion model and in HT22 cells after oxygen-glucose deprivation/reoxygenation (OGD/R). Sertad1 knockdown significantly ameliorated ischemia-induced brain infarct volume, neurological deficits and neuronal apoptosis. In addition, it significantly ameliorated the OGD/R-induced inhibition of cell viability and apoptotic cell death in HT22 cells. Sertad1 knockdown significantly inhibited the ischemic/hypoxic-induced expression of p-Rb, B-Myb, and Bim in vivo and in vitro. However, Sertad1 overexpression significantly exacerbated the OGD/R-induced inhibition of cell viability and apoptotic cell death and p-Rb, B-Myb, and Bim expression in HT22 cells. In further studies, we demonstrated that Sertad1 directly binds to CDK4 and the CDK4 inhibitor ON123300 restores the effects of Sertad1 overexpression on OGD/R-induced apoptotic cell death and p-Rb, B-Myb, and Bim expression in HT22 cells. These results suggested that Sertad1 contributed to ischemic/hypoxic neurological injury by activating the CDK4/p-Rb pathway.

저산소 심장의 산소 재공급에 따른 심근 손상에 있어서 항산화제의 보호 효과 (Protective Effect of Antioxidants on the Reoxygenation Injury in Hypoxic Myocardium of Rat)

  • 저산소 심근의 산소 재공급시에 보이는 심근 손상(oxygen paradox) 기전을 규명하고, 이의 예방법을 찾기 위한 연구의 일환으로 유독성 산소 대사물인 산소 라디칼의 관련성과 지질과산화활성화 및 항산화제의 심근 보호 효과를 검토하였다. 흰쥐 적출 심장을 Langendorff 심장관류법 으로 산소 및 glucose 공급을 중단한 cardioplegic 용액으로 관류 ($37^{\circ}C$, 90분)하여 저산소 상태를 만든 후, 계속해서 산소재공급 관류(20분)를 시행하여 저산소-산소 재공급 심근 손상을 유도 하였다. 심근 손상의 지표로 creatine phosphokinase(CPK), lactic dehydrogenase(LDH)의 관상관류액으로의 유출을, 그리고 지질과산화 척도로는malondialdehyde(MDA) 생성을 측정하였으며, 이에 대한 산소 라디칼 제거물질과 항산화제 ${\alpha}-tocopherol$ 및 butylated hydroxytoluene(BHT)의 효과를 검토하여 다음과 같은 성적을 얻었다. 1. 세포질 효소인 CPK 및 LDH의 유출과 지질과산화산물의 하나인 MDA의 생성은 산소 재공급과 더불어 급격히 증가하였다. 2. 산소 재공급시 세포질 효소의 유출과 MDA 생성은 높은 상관관계를 보였다. 3. Superoxide anion$(O_2)$의 제거 호소인 superoxide dismutase (10,000U), $H_2O_2$ 제거 효소인 catalase (25,000 U) 그리고 hydroxyl radical (OH) 제 거 물질인 dimethylsufoxide(10%)는 세포질 효소의 유출 증가와 MDA 생성 증가를 현저히 억제하였다. 4. 생리적 항산화물질인 ${\alpha}-tocopherol$.of (4.5 uM)과 합성 항산화제인 butylated hydroxytoluene(2 uM)은 산소 공급에 따른 MDA 생성 증가와 세포질 효소의 유출 증가를 용량의존적으로 억제하였다. 5. 항산화제들의 심근 보호 효과는 산소 재공급시 투여할 때보다는 저산소 관류시부터 투여한 경우에 더욱 현저하였다. 이상의 결과에서 저산소 심근의 산소 재공급은 유독성 산소 대사물인 산소 라디칼의 생성을 증가시키며, 그에 따른 지질성분의 과산화가 심근 손상을 일으키는데 관여할 것으로 여겨졌으며, 저산소-산소 재공급 심근 손상은 지질과산화 반응을 억제하는 항산화제에 의하여 방지될 것으로 사료되었다.

하악골 부분절제술을 받은 환자에서 발생한 기도 관리 실패 (Failed Airway Management in a Patient with Wound Hematoma After Partial Mandibulectomy and Reconstruction with Free Flap)

  • We experienced failed airway management in a patient who had partial mandibulectomy and reconstruction with free-flap. 40 year-old man (height: 164 cm, body weight: 59 kg) with malignant melanoma on #38 tooth area of mandibular body was scheduled for partial mandibulectomy and reconstruction with free flap. Approximately fifteen-hours after surgery, the patient was extubated without complication. Seven hours after extubation, we experienced respiratory failure andfailed airway managementdue to airway edema and neck. We failed orotracheal intubation with direct laryngoscopy andlaryngeal mask airway, thus we tried tracheostomy but the patient was hypoxic state for more than 30 minutes. The patient had got hypoxic brain damage in whole cerebral cortex and basal ganglia. We should have the policy of airway management of the patients who have massive oro-maxillo-facial surgery and all medical personnel who treat these patients should be educated the policy and airway management methods.

Effects of carnosine and hypothermia combination therapy on hypoxic-ischemic brain injury in neonatal rats

  • Background: Carnosine has antioxidative and neuroprotective properties against hypoxic-ischemic (HI) brain injury. Hypothermia is used as a therapeutic tool for HI encephalopathy in newborn infants with perinatal asphyxia. However, the combined effects of these therapies are unknown. Purpose: Here we investigated the effects of combined carnosine and hypothermia therapy on HI brain injury in neonatal rats. Methods: Postnatal day 7 (P7) rats were subjected to HI brain injury and randomly assigned to 4 groups: vehicle; carnosine alone; vehicle and hypothermia; and carnosine and hypothermia. Carnosine (250 mg/kg) was intraperitoneally administered at 3 points: immediately following HI injury, 24 hours later, and 48 hours later. Hypothermia was performed by placing the rats in a chamber maintained at 27℃ for 3 hours to induce whole-body cooling. Sham-treated rats were also included as a normal control. The rats were euthanized for experiments at P10, P14, and P35. Histological and morphological analyses, in situ zymography, terminal deoxynucleotidyl transferase-mediated dUTP nick end-labeling (TUNEL) assays, and immunofluorescence studies were conducted to investigate the neuroprotective effects of the various interventional treatments. Results: Vehicle-treated P10 rats with HI injury showed an increased infarct volume compared to sham-treated rats during the triphenyltetrazolium chloride staining study. Hematoxylin and eosin staining revealed that vehicle-treated P35 rats with HI injury had decreased brain volume in the affected hemisphere. Compared to the vehicle group, carnosine and hypothermia alone did not result in any protective effects against HI brain injury. However, a combination of carnosine and hypothermia effectively reduced the extent of brain damage. The results of in situ zymography, TUNEL assays, and immunofluorescence studies showed that neuroprotective effects were achieved with combination therapy only. Conclusion: Carnosine and hypothermia may have synergistic neuroprotective effects against brain damage following HI injury.

The role of cytokines in seizures: interleukin (IL)-$1{\beta}$, IL-1Ra, IL-8, and IL-10

  • Brain insults, including neurotrauma, infection, and perinatal injuries such as hypoxic ischemic encephalopathy, generate inflammation in the brain. These inflammatory cascades induce a wide spectrum of cytokines, which can cause neuron degeneration, have neurotoxic effects on brain tissue, and lead to the development of seizures, even if they are subclinical and occur at birth. Cytokines are secreted by the glial cells of the central nervous system and they function as immune system mediators. Cytokines can be proinflammatory or anti-inflammatory. Interleukin (IL)-$1{\beta}$ and IL-8 are proinflammatory cytokines that activate additional cytokine cascades and increase seizure susceptibility and organ damage, whereas IL-1 receptor antagonist and IL-10 act as anti-inflammatory cytokines that have protective and anticonvulsant effects. Therefore, the immune system and its associated inflammatory reactions appear to play an important role in brain damage. Whether cytokine release is relevant for the processes of epileptogenesis and antiepileptogenesis, and whether epileptogenesis could be prevented by immunomodulatory treatment should be addressed in future clinical studies. Furthermore, early detection of brain damage and early intervention are essential for the prevention of disease progression and further neurological complications. Therefore, cytokines might be useful as biomarkers for earlier detection of brain damage in high-risk infants.
