고중성지방혈증을 동반한 지방간 환자 치험 1례 (Case Report of a Patient Diagnosed with Fatty Liver Accompanied by Hypertriglyceridemia)

  • Objectives: This study aimed to identify whether herbal medicine improves the clinical symptoms and abnormal blood tests of a patient with fatty liver accompanied by hypertriglyceridemia. Methods: A 44-year-old man with fatty liver and hypertriglyceridemia was prescribed Saenggangunbi-tang from July 20, 2020, to November 11, 2020, to reduce fatigue and to improve laboratory findings, such as liver enzymes and the lipid profile. We observed changes in symptoms, serum levels of liver enzymes, and the lipid profile during about 4-month treatment. We also recorded changes in the bioelectrical impedance analyzer findings during that time. Results: In this case study, an approximately 4-month treatment with Saenggangunbi-tang led to considerable improvement in laboratory findings and visceral fat area. In particular, the patient experienced a noticeable decrease in triglyceride levels compared with the baseline parameters of the first visit. In addition, no side effects-including weight gain and liver enzyme increases-were observed during treatment. Conclusion: This study suggests that appropriately prescribed herbal medicine is a therapeutic option to manage fatty liver combined with hypertriglyceridemia.

고지방/고콜레스테롤 식이 랫트 모델에서 홍삼에 의한 고중성지방혈증 개선 효과 (Effect of Korean Red Ginseng on Hypertriglyceridemia in High Fat/high Cholesterol Diet Rat Model)

  • Korean Red Ginseng (RG) are used as a traditional treatment for improve blood circulation. This experimental study was designed to investigate the inhibitory effects of Korean red ginseng on lipid metabolism in high fat/cholesterol diet (HFCD)-induced hypertriglyceridemia. Sprague Dawley rats were fed the HFCD diet with/without fluvastatin (Flu, positive control) 3 mg/kg/day, and RG 125 or 250 mg/kg/day, respectively. All groups received regular diet or HFCD diet, respectively, for 13 weeks. The last three groups treatment of Flu and RG 125, and RG 250 orally for a period of 9 weeks. Group 1, reular diet; group 2, HFCD diet; group 3, Flu + HFCD diet; group 4, RG 125 + HFCD diet; group 5, RG 250 + HFCD diet. As a result, treatment with low or high doses of RG markedly attenuated plasma levels of triglycerides and augmented plasma levels of high-density lipoprotein (HDL) in HFCD-fed rats. RG and Flu also led to an increase in lipoprotein lipase activity in the HFCD group. On the other hand, RG and Flu led to an decrease in fatty acid synthase and free fatty acid activity in the HFCD group. Treatment with RG suppressed increased expressions of $PPAR-{\alpha}$ and AMPK in HFCD rat liver or muscle. In addition, the RG attenuated triglyceridemia by inhibition of $PPAR-{\gamma}$ and FABP protein expression levels and LXR and SREBP-1 gene expression in liver or muscle. The RG significantly prevented the development of the metabolic disturbances such as hypertriglyceridemia and hyperlipidemia. Taken together, the administration of RG improves hypertriglyceridemia through the alteration in suppression of triglyceride synthesis and accentuated of triglyceride decomposition. These results suggested that RG is useful in the prevention or treatment of hypertriglyceridemia.

40대 이상 당뇨환자의 당화혈색소 조절 상태에 영향을 미치는 요인: 2012 국민건강영양조사결과를 바탕으로 (Factors Associated with Hemoglobin A1c among Patient Aged 40 years over with Diabetes Mellitus: 2012 Korea Health and Nutrition Examination Survey)

  • Purposes: This study was done to identify factors associated with uncontrolled hemoglobin A1c among patient with diabetes mellitus. Method: The sample was 412 Koreans with diagnosed diabetes aged 40 years or older who participated in the Fifth Korea Health and Nutrition Examination survey, which was a nationwide, cross-sectional survey. Data was analyzed using logistic regression. Uncontrolled hemoglobin A1c rate was 55%. Results: There was a difference between controlled and uncontrolled group in hemoglobin A1c as follows: duration of diabetes, BMI(Body Mass Index), hypertriglyceridemia, vitamin D, sleep duration. After adjusting for confounding factor, longer duration of diabetes (>7 years vs. ${\leq}7$)(Adjusted OR=2.277, 95% CI [1.277-4.060]), presence of hypertriglyceridemia (Adjusted OR=4.019, 95% CI [1.871-8.634]), lower vitamin D level (<20ng/mL vs. ${\geq}20$)(Adjusted OR=2.487, 95% CI[1.411-4.381]), longer sleep duration (6-8 hours vs >8 hours)(Adjusted OR=6.831, 95% CI [1.877-24.855]) were significantly associated with increased odds of uncontrolled hemoglobin A1c. Conclusions: Results show that duration of diabetes, hypertriglyceridemia, vitamin D and sleep duration are significantly related to hemoglobin A1c. Therefore, considering these factors it would be helpful to develop strategies to improve blood glucose control in patients with diabetes.

Canola Oil Influence on Azoxymethane-induced Colon Carcinogenesis, Hypertriglyceridemia and Hyperglycemia in Kunming Mice

  • Azoxymethane (AOM) is a potent genotoxic carcinogen which specifically induces colon cancer. Hyperlipidemia and diabetes have several influences on colon cancer development, with genetic and environmental exposure aspects. Here, we investigated plasma lipid and glucose concentrations in Kunming mice randomized into four groups; control (no AOM or oil exposure), AOM control, AOM + pork oil, and AOM + canola oil. Aberrant crypt foci (ACF), plasma cholesterol, plasma triglyceride, plasma glucose and organ weight were examined 32 weeks after AOM injection. Results revealed that AOM exposure significantly increased ACF number, plasma triglyceride and glucose level. Further, male mice displayed a much higher plasma triglyceride level than female mice in the AOM control group. Dietary fat significantly inhibited AOM-induced hypertriglyceridemia, and canola oil had stronger inhibitory effect than pork oil. AOM-induced hyperglycemia had no sex-difference and was not significantly modified by dietary fat. However, AOM itself not change plasma cholesterol level. AOM significantly increased liver and spleen weight in male mice, but decreased kidney weight in female mice. On the other hand, mice testis weight decreased when fed canola oil. AOM could induce colorectal carcinogenesis, hypertriglyceridemia and hyperglycemia in Kunming mice at the same time, with subsequent studies required to investigate their genome association.

Jinan red ginseng extract inhibits triglyceride synthesis via the regulation of LXR-SCD expression in hepatoma cells

  • Hypertriglyceridemia is one of the metabolic syndrome that is often observed as a result of lipid abnormalities. It is associated with other lipids, metabolic disorders, cardiovascular disease and liver disease. Korean red ginseng is known to affect obesity, dyslipidemia, liver disease and liver function, but the mechanism of its effect is not clear. This study examined the beneficial effects of hypertriglyceridemia and the mechanism of action of Jinan red ginseng extract (JRG) in hepatoma cells. To measure the levels of triglyceride accumulation, we studied the expression of proteins and mRNAs related to lipidogenesis in hepatoma cells (Huh7 and HepG2). JRG decreases the lipidogenic markers, peroxisome proliferator-activated receptor γ (PPARγ), CCAAT-enhancer-binding proteins α (C/EBPα) and C/EBPβ which are major regulators of triglyceride synthesis in hepatoma cells. We also found that JRG reduced sterol regulatory element binding proteins 1c (SREBP-1c), C/EBPα and C/EBPβ by regulating liver X receptor (LXR) and stearoyl CoA desaturase (SCD) expressions. In addition, the first-limited step of synthesis triglyceride (TG), glycerol-3-phosphate (G3P) is decreased by JRG. These results suggest that the anti-hypertriglyceride effect of JRG in hepatoma cells could be accompanied with the inhibition of lipidogenic transcription factors by regulating LXR and SCD expression.

Inhibitory Effects of Lactobacillus plantarum Q180 on Lipid Accumulation in HepG2 Cells

  • Recently, the prevalence of hyperlipidemia has been increasing, and consequently, the need to identify safe and effective treatments to control this chronic disease has also increased. The beneficial effects of probiotics have been revealed by several studies over the past few years, including their effects on hypertriglyceridemia. However, the mechanisms of action of probiotics are still unclear. The anti-obesity effects of Lactobacillus plantarum Q180 on lipid accumulation have already been demonstrated using an in vitro HepG2 cell model, and therefore, we investigated its efficacy and mechanism of action. Lipid accumulation was induced in HepG2 cells by palmitic acid treatment and then the cells were incubated with L. plantarum Q180 lysate or supernatant to investigate changes in lipid accumulation and expression of lipid metabolism-related genes. The results showed that the L. plantarum Q180-treated group exhibited significantly lower levels of lipid accumulation and mRNA expression of lipid synthesis- and adipogenesis-related genes than the palmitic acid-treated group did. These results indicate that L. plantarum Q180 may contribute to alleviating hypertriglyceridemia by inhibiting lipid synthesis.

안면 정보를 이용한 나이브 베이즈 기반 고중성지방혈증 예측 모델 (Prediction Model for Hypertriglyceridemia Based on Naive Bayes Using Facial Characteristics)

  • 최근에 이르러, 기계학습 및 데이터마이닝은 수많은 질병 예측 및 진단에 활용되고 있다. 만성질환은 전체 사망률의 약 80%를 차지하는 질병으로, 점점 증가하는 추세이다. 만성질환 관련 예측 모델을 연구한 기존 연구들은 예측 모델을 구성하는 데이터로 혈당, 혈압, 인슐린 수치 등의 건강검진 수준의 데이터를 이용한다. 본 논문은 만성질환의 위험 요인인 이상지질혈증과 안면 정보의 연관성을 검증하고, 기계학습 기반 안면 정보를 이용한 이상지질혈증 예측 모델을 세계 최초로 개발한다. 본 연구는 5390명의 임상 데이터 중 안면 정보와 중성지방혈증 정보를 바탕으로 수행하였다. 중성지방혈증은 이상지질혈증을 판단하는 척도이다. 연구의 결과로 얼굴의 하악(mandibular) 간의 거리를 나타내는 FD_43_143_aD(p<0.0001, Area Under the receiver operating characteristics Curve(AUC)=0.652) 와 고중성지방혈증이 매우 높은 연관성을 가진 것을 밝혀냈고, 이를 기반으로 구축한 모델은 0.662의 AUC값을 획득하였다. 이러한 연구결과는 향후 질병 역학 및 대중 보건 영역의 스크리닝 단계에서 안면정보만으로 다양할 질병을 예측할 수 있는 기반을 제공할 수 있을 것이다.

  • The effect of feeding various diets on plasma lipids, liver lipids, fecal excretion of lipids and triglyceride secretion rate was studied in Sprague-Dawley rats fed for 1 month diets containing high fat(40% of energy ; HF) high cholesterol(1% ; HC), low fat/high sucrose(70% ; HS) or control diet(CT). Weight gain in HS group but there were no differences in TG contents of HF or HC groups compared to CT group. Fecal excretion of lipid was higher in HC than in HF, HS and CT groups. Hepatic TG secretion was not significantly different in four dietary groups. Accordingly, hypertriglyceridemia shown in HS group does not seem to be resulted from the increased TG secretion rate.

Fenofibrate Inhibits Visceral Adiposity by Inhibiting UCPs in C57BL/6J Mice Fed on a High Fat Diet

  • We investigated to verify whether the $PPAR{\alpha}$ agonist fenofibrate regulates adipose tissue metabolism and to determine the molecular mechanism involved in this regulation. After male mice (C57BL/6J) received a high fat diet with or without fenofibrate for 6 weeks, the effects of fenofibrate on not only adipose tissue weight, visceral adipocyte size, serum lipid and glucose levels, but also the expression of uncoupling proteins (UCPs). Mice given a fenofibrate-supplemented high fat diet showed reduced both visceral and subcutaneous adipose tissue weights versus high fat diet-fed animals. The size of visceral adipocytes was significantly decreased by fenofibrate treatment. The administration of fenofibrate resulted in decreased serum levels of triglycerides, free fatty acids, and glucose. Moreover, fenofibrate up-regulated mRNA levels of visceral adipose tissue UCP2 and skeletal muscle UCP3. Therefore, our results suggest that the increases in the expression of UCPs by fenofibrate seem to suppress diet-induced visceral adiposity as well as severe hypertriglyceridemia and hyperglycemia in male mice.
