A Case of Acute Intermittent Porphyria -A case report- (급성 간헐성 포르피리아 1례)

  • We report a case of acute intermittent porphyria presenting with variable symptoms and signs such as hypertention, polyneuropathy, syndrome of inappropriate secretion of antidiuretic hormone and cerebral infarction. A 47 year-old female patient entered hospital with abdominal pain followed by generalized seizure. She was diagnosed to have acute intermittent porphyria in consequence of Watson-Schwartz test and ${\delta}$-ALA in 24 hours urine, but died of respiratory failure.

Influencing and Mediating Factors in Stroke: Based on 2007-2012 Korea National Health and Nutrition Examination Survey (뇌졸중의 영향 요인과 매개요인)

  • This study conducted influencing and mediated effect on stroke to lead correct health behavior of stroke patients and tried to provide preliminary data of stroke prevention. It used stage 4 and 5 data of a national health and nutrition examination survey, analysis method was frequency analysis, Chi-square test, multiple logistic regression and structural equation modeling. In case of male, factors affecting to stroke were age, job, self-related health, alcohol, hypertention and diabetes. In case of female, age, job, self-related health, stress level and hypertention affected to stroke. In tested results on whether or not mediated effect of preceding disease exists, 5.80 difference in ${\chi}^2$ between partial mediated modeling and full mediated modeling was statistically significant(p<0.01). Therefore, partial mediated modeling was adequate to this study. We need preventive health education for changing wrong health behaviors and policy that strengthens health care network. If someone has preceding disease, they need long-term diagnosis for health condition and continuous improvement in life style.

Characteristics of Individuals Seeking Comprehensive Health Check-ups (종합 건강검진을 원하는 피검진자의 건강특성에 관한 연구)

  • This study was carried out to identify basic data for more efficient operation of comprehensive health check-up centers in the national health care system. The researcher reviewed and analyzed the main symptoms of the subjects according to sex, age, occupation and positive rate in the screening tests of a comprehensive health check-up, performed at one comprehensive health check-up center in located in a hospital. The subjects were 512 persons who had a check-up at the comprehensive health check-up, center in K university hospital in Seoul from October 2, 1996 to March 30, 1997. A questionaire developed by the researcher to obtain subject's general characteristics, main symptoms, medical examination, diagnosed disease was used as the tool for the study. The researcher analyzed the data with SPSS PC/sup +/ program. : the distribution of the subject's general characteristics and main symptoms was described with percentages and the difference between main symptoms and screening tests analyzed with X²-test, t-test, ANOVA. The result of this study are as follows : 1. For the distribution of diagnosis in males. liver disease was most frequent and hypertension was second : in females, hypertention and liver disease were not common in that order. 2. The most common complaints were indigestion in the digestive system, sputum in the respiratory system, irregular heart beat in the cardiovascular system, frequent urine in the urinary system vertigo in the hematologic system. 3. The main symptoms according to sex were for females, a statistically siguificant higher rate of complaints in the digestive system, cardiovascular system, urinary system, hematologic system, musculoskeletal, mental & nervous system as compared to males. 4. The main symptoms according to age were that in the the second decade there wara higher rate of complaints in the digestive system, and cardiovascular system than in the older age groups : and in the fifth decade more in the musculoskeletal system. 5. The main symptoms according to occupation were that formers and fishers had a higher rate of complaints in the mental and nervous system as compared to other occupation groups : and office workers, in gynecology. 6. The main symptoms according to diagnosis, were symptoms of the digestive system, frequent in liver disease : symptoms of the cardiovascular system in allergic disease. 7. The screening tests which showed the highest rates were UGI /FGS, PFT : In males a higher rate of abnormality was seen in abdominal ultrasono- graphy and in, females, in mammography. 8. In screening tests according to sex, males showed a higher positive rate for chest X-rays, and LFT, and females, in mammography, pap smear, CBC. 9. In screening tests according to age, higher positive rates of obesity, high blood pressure, chest X-ray, mammography, pap smear, FBS, CBC, urine test were seen in the fifth decade : PFT and abdominal ultrasonography in the third decade. The subjects who wanted comprehensive health check-up were not asymptomatic but had an illness with several symptoms. It is found that the most frequent diagnosed diseases were hypertention and liver disease. So proper observations must be done for them. Comprehensive health check-ups should be changed to a comprehensive examination including treatment beyond examination for those subjects with positive results.

A Study on the Educational Effectiveness of Chronic Diseases Among University Students (일부 대학생들의 성인건강 교육의 효과에 관한 연구)

  • Korea has been recently reducing the quality of life as well as rising medical cost because of the increase of chronic diseases. But we can prevent those chronic diseases through the improvement of environment or life style. We evaluated the educational effectiveness of chronic diseases(hypertention, diabetes, cancer, stroke and other chronic diseases) designed to increase the knowledge, attitude and practice of chronic diseases among university students. Between August 1994 and November 1994, we implemented chronic diseases prevention instruction in intervention students; unmatched control students were selected in same university. We conducted pre- and post-intervention surveys both intervention and control students with self-reported questionnaires(50 items). We assigned score(0-4 points) to items and conducted a analysis of covariance(ANCOVA) with sex, grade and economic status as the covariate, using the SAS PC computer statistical package. And we culculated odds ratio with safety scores between intervention and control students. The results of this study were followed. 1. In demographic characteristics of subjects both pre- and post intervention, we found no significant differences in intervention and control students at religion, father's education, mother's education, mother's occupation and type of residence(p>0.05), but we found significant differences at sex(p<0.001), grade(p<0.001) and economic status(p<0.05). 2. The sex, grade and economic status-adjusted mean prevention knowledge scores for diabetes and stroke increased from the pre- to post-survey in the intervention students(p<0.001), but control students did not increased(p>0.05). As odds ratios in knowledge were below 1, the knowledge of intervention students were higher than control students. 3. The attitudes for general adult health increased from the pre- to post-survey in the intervention students(p<0.05), but control students did not increased(p>0.05). As odds ratios in attitudes were approximately 1, we can not say effectiveness in intervention students 4. The pratices for cancer and stroke increased from the pre- to post-survey in both the intervention and control students(p<0.001). Also odds ratio of hypertention was 0.91, and that of stroke was 1.14. 5. Health related behaviors did not increased from the pre- to post- survey in both the intervention and control students(p>0.05). But odds ratio of drinking was 0.76 and that of body weight was 1.21. 6. Health status did not increased from the pre- to post- survey in both the intervention and control students(p>0.05). As odds ratio of health status was 1.09, prevention education was not effect in intervention students We would like to recommend as follows; 1. University students must learn about prevention of chronic diseases. Because the knowledge of invetervention students was higher than that of control students. 2. The prevention education of chronic diseases should be taught from primary school. 3. Adult health education for university students must be practiced continuously. Education period(l5 weeks) in this study was not complete. 4. The evaluation of chronic diseases was conducted real measurement(such as BP check) as well as self reported-survey. 5. Educational materials(video tape, pamphlet) related the prevention of chronic diseases should be developed at national level. And we must easely use those materials. 6. The prevention education of chronic diseases should be made through mass media as well as school education.

  • The final purpose of oral & maxillofacial trauma is functional & esthetic repair. Nowadays, severe trauma involving with the head & neck trauma is increasing. After these trauma occurs, the patients develop similar signs & symptoms with the postoperative healing period, as like thurst, hypertention, excitability, disorientation, convulsion, et al. Because SIADH which is one of important complications after head trauma, shows similar clinical features after operation, we should pay attention to detect it. SIADH shows characteristic laboratory findings, as like hyponatremia, urine hyperosmolality, increased plasma ADH level, continued renal excretion of sodium, so we can easily distinguish it from postoperative conditions. This paper reports two cases, one was the case of the mandibular fracture and cerebral contusion, which included permanent SIADH. The other was the case of the multiple teeth injury and cerebral contusion, which was transient SIADH. We treated them with water restriction, hypertonic saline, and diuretics.

Comprehensive Measures for Disease Prevention and Health Promotion (질병예방 및 건강증진 종합 대책)

  • While the threat from traditional communicable diseases have been decreasing non communicable chronic diseases are increasing due to the aging of population and change in life pattern of the people such as over intake of cholesterol and lack of physical exercise etc. On the other hand, since 1980s, AIDS is spreading rapidly throughout the globe and environmental pollution, accidents, addictive diseases such as drug abuse and alcoholism are becoming serious factors to hinder the health promotion of the people. In order to improve general public health and promote individual health status, existing program for communicable disease control by the government such as tuberculosis, leprosy, STD and acute communicable diseases should be effectively continued. In principle, effort should be placed on eradication of source of infection, reduction of communicability of source in infection, treatment of source of infection as well as increase of individual registance to the diseases through immunization and improvement of physical status. Since the pattern of illness is being shifted from communicable diseases to non communicable chronic diseases such as cancer, cerebral hemorrhage, heart disease and hypertention etc., special emphasis should be placed on the prevention and control of those adult diseases. Early detection of the patients, registration and treatment of patients and health education should be systematically developed for effective control of the diseases. In addition, program activities on MCH, nutrition, dentistry, mental health, environmental health, accident prevention. medical delivery system, health insurance. and all other health issues should be improved in order to achieve our goal of health promotion.

  • Metabolic syndrome consists of several risk factors of metabolic origin like elevated blood pressure, abdominal obesity, elevated triglyceride concentrations, decreased HDL-cholesterol concentrations, and elevated fasting glucose. It appear to directly increase the development of atherosclerotic cardiovascular diseases which leading to cardiovascular diseases in later life. This 39 years female patient was diagnosed schizophrenia at the age of 17. but she didn't take reasonable treatments. during 20 years, she stayed in home and lived unsuited life with her family. As a result she gains heavy weight and centeral obesity, diabetic mellitus and hypertention, and developed cerevral vascular accident twice. This patient was treated with herb medication, acupuncture, western medication, psychotherapy primarily in focusing of pre-illness state(未病) as well as the stagnation of phelm. During 140 days of treatment from admission, before 2nd attack tbe patient showed improvement of Lt. hemiparesis but after second attack improved a little. And symptom of schizophrenia improved considerably. Also diet habit changed.

  • Acute Myocardica Infarction has high mortality and bad prognosis. We treated a 68 year-old female patient of acute myocardiac infacrtion to follow cerebral infarction. The chief complainment is right hemiplegia, dyspnea, chest discomfort, palpitation, anxiety, insomnia. The prognosis cannot be good, Because she has anterior wall infarction, cardiomegaly, conduction block, hypertention and she is 68 years old. We treated her with chiefly Nogyongdaebo-tang and Gongjin-dan, and gradually increased momentum. also we used self west medicine. The chief complainment was improved. She became to do cane walking by herself.

The Reaserch of Dochangbup (도창법(倒倉法)의 연혁(沿革)과 현대적 응용)

  • Objective : "Dochangbup" is one of the therapies that remove pathogenic qi[邪氣] from the Emetic Method in the Miscellaneous Disorders part of Dongeuibogam東醫寶鑑. It is, in particular, the method of treatment that removes phlegm[痰], the cause of various disorders. Method : Application of "Dochangbup" had a long history in Korea as well. The meaning and application of "Dochangbup" are mentioned in various texts from early Chosun dynasty to the post liberation. Result : In China, there are a lot of medical texts by physicians throughout Ming and Qing Dynasty, with GeZhiYuLun格致餘論 at the top of the list, dealing with the meaning and applicable scope of "Dochang" method and pharmacy of "Hacheongo霞天膏". Most of the physicians are affiliated with Dan Xi School, regarding ZhudanXi朱丹溪 as a suzerain. In "Dongeuibogam", it is mentioned that "Dochangbup" can treat various disorders caused by phlegm. Though, when there is a possibility of harming original qi[元氣] during the treatment or grave deficiency in patients, it is requested to consider tonifying while purging. Conclusion : "Dochangbup" can be applied not only to the disorders mentioned in classical medical texts, like heart pain[心痛], leg disease[脚氣], urine turbidity[小便濁], involuntary discharge of semen[遺精], cough[咳嗽], blood spitting[喀血], but also to metabolic syndrome such as obesity, hypertention and diabetes, commonly found in the modern era.

  • Obestity, characterized by an excess accumulation of fat, is a detriment to good health and wellbeing. It is easy for individuals to take on excess fat as soon as enough food and leisure are available in a society, causing an imbalance between energy intake and energy expenditure. Although there has been disagreement as to which side of this energy equation is more important in th epidemic of obesity, both sides are certainly involved. I think the prevalence of obesity begins with infancy. It seems to be relatively constant throughout childhood. Some retrospective studies have suggested that there is a direct progression from a fat chile to a fat adult. Obesity has been associated with excess mortality in many studies. The effect of obesity on cardiovascular disease has not always been an independent one, but has generally been through exacerbation of other rish factors such as hypertention, diabetes, hyperlipedmia, galbladder disease, gout and cancer of the grease and endometrium. The weights of identical twins raised in separate homes have been reported to be similar, thereby suggesting that heredity contributes significantly to weight. Overweight and obesity are by far greater health problems for many people in our society. Therefore in this paper we consider the relationship between body energy balance and problems of obesity. Whatever the weight of the person, the goal of the nutritionist and health care workers is to find solutions that will allow persons to feel better about themselves.
