This study is about a comparison of controlled and uncontrolled hypertension groups regarding comprehensive diagnosis of Qi blood water and quality of life. We surveyed "controlled and uncontrolled hypertension patients" using questionnaires for comprehensive diagnosis of Qi blood water, SF-36 and HTN QoL (Measurement Scale for the quality of life in hypertensive patients). There was no difference in comprehensive diagnosis of Qi blood water between the controlled and uncontrolled hypertension groups. Within the controlled hypertension group, the patients diagnosed with a Qi deficiency, Qi stagnation, Qi counterflow, blood deficiency, and water retention received lower total scores in SF-36 and HTN QoL than in undiagnosed patients. Within the uncontrolled hypertension group, the patients diagnosed with Qi deficiency, blood deficiency, and water retention got lower total scores in SF-36 and HTN QoL than in undiagnosed patients. These results were statistically significant. These results are insufficient that we and use comprehensive diagnosis of Qi blood water for a diagnosis tool of hypertension. But if we have better studies that make up for weak points, these results will help to make a diagnosis tool for hypertension.

Pulmonary hypertension (PH) is a condition of increased blood pressure in the pulmonary arteries and is diagnosed with an increased a mean pulmonary artery pressure ≥25 mm Hg. This condition may be associated with multiple clinical situations. Based on pathophysiological mechanisms, clinical presentation, hemodynamic profiles, and treatment strategies, the patients were classified into five clinical groups. Although there have been major advances in the management of PH, it is still associated with significant morbidity and mortality. The diagnosis and treatment of PH have been performed mainly by following European guidelines, even in Korea because the country lacks localized PH guidelines. European treatment guidelines do not reflect the actual status of Korea. Therefore, the European diagnosis and treatment of PH have not been tailored well to suit the needs of Korean patients with PH. To address this issue, we developed this guideline to facilitate the diagnosis and treatment of PH appropriately in Korea, a country where the consensus for the diagnosis and treatment of PH remains insufficient. This is the first edition of the guidelines for the diagnosis and treatment of PH in Korea, and it is primarily based on the '2015 ESC/ERS Guidelines for the diagnosis and treatment of pulmonary hypertension.' with the acceptance and adaptation of recent publications of PH.

Objectives: Controlling blood pressure is a key step in reducing cardiovascular mortality in older adults. Gender differences in patients' attitudes after disease diagnosis and their management of the disease have been identified. However, it is unclear whether gender differences exist in hypertension management among older adults. We hypothesized that gender differences would exist among factors associated with hypertension diagnosis and control among community-dwelling, older adults. Methods: This cross-sectional study analyzed data from 653 Koreans aged ${\geq}60years$ who participated in the Korean Social Life, Health, and Aging Project. Multiple logistic regression was used to compare several variables between undiagnosed and diagnosed hypertension, and between uncontrolled and controlled hypertension. Results: Diabetes was more prevalent in men and women who had uncontrolled hypertension than those with controlled hypertension or undiagnosed hypertension. High body mass index was significantly associated with uncontrolled hypertension only in men. Multiple logistic regression analysis indicated that in women, awareness of one's blood pressure level (odds ratio [OR], 2.86; p=0.003) and the number of blood pressure checkups over the previous year (OR, 1.06; p=0.011) might influence the likelihood of being diagnosed with hypertension. More highly educated women were more likely to have controlled hypertension than non-educated women (OR, 5.23; p=0.013). Conclusions: This study suggests that gender differences exist among factors associated with hypertension diagnosis and control in the study population of community-dwelling, older adults. Education-based health promotion strategies for hypertension control might be more effective in elderly women than in elderly men. Gender-specific approaches may be required to effectively control hypertension among older adults.
Objectives : The objective of this study was to evaluate the correlation between earlobe crease(ELC) and Hypertension(HTN) in the auricular point diagnosis. Methods : 140 patients who were admitted the Jeongeup City Geriatric Hospital were examined for the presence or absence of ELC and HTN. The one examined the ELC of patients and the other examined the presence of hypertension. The presence of right ELC, left ELC, right or left ELC, and right and left ELC were respectively compared with the presence of HTN. Chi-square test was used to analyze the correlation between ELC and HTN. Results : The presence of right ELC was significantly related to presence of HTN(p=0.001). The sensitivity, specificity, and positive and negative predictability of ELC for diagnosis of HTN were 0.755, 0.524, 0.787, and 0.478. The presence of left ELC was significantly related to presence of HTN(p=0.002). The sensitivity, specificity, and positive and negative predictability of ELC for diagnosis of HTN were 0.745, 0.524, 0.785, and 0.468. The presence of right or left ELC was significantly related to presence of HTN(p<0.001). The sensitivity, specificity, and positive and negative predictability of ELC for diagnosis of HTN were 0.806, 0.500, 0.790, and 0.525. The presence of right and left ELCs was significantly related to presence of HTN (p=0.007). The sensitivity, specificity, and positive and negative predictability of ELC for diagnosis of HTN were 0.694, 0.548, 0.782, and 0.434. Conclusions : These results show that the presence of ELC is significantly associated with hypertension.

An expert system for the diagnosis and indication of hypertension is implemented through HTML-based backward inference. HTML-based backward inference is performed using the hypertext function of HTML, and many HTML files, which are hyperlinked to each other based on the backward rules, should be prepared beforehand. The development and maintenance of the HTML files are conducted automatically using the decision graph. Still, the drawing and input of the decision graph is a time consuming and tedious job if it is done manually. So, automatic generator of the decision graph for the diagnosis and indication of hypertension was implemented. The HTML-based backward inference ensures accessibility, multimedia facilities, fast response, stability, easiness, and platform independency of the expert system. So, this research reveals that HTML-based inference approach can be used for many Web-based intelligent site with fast and stable performance.
Childhood hypertension (HTN) has become a significant public health issue because of the increased risk of cardiovascular disease in adulthood. However, childhood HTN is underrecognized and underdiagnosed in clinical practice. The European Society of Hypertension in 2016 and the American Academy of Pediatrics (AAP) in 2017 published updated guidelines for the screening, prevention, and management of pediatric HTN. There were notable differences between the two guidelines as well as many similarities. The updated AAP guidelines have clarified and simplified the recommendations for screening, diagnosis, and treatment of childhood HTN based on current evidence. This review highlights the important developments in both guidelines, focusing on recent advances in the classification and treatment of childhood HTN.
Background : Little work has been carried out regarding quality assessment research in a primary care setting, comparing with that of hospitals. This study aims to evaluate the process of diagnosis and management of hypertension by public health doctors on the basis of pre-established clinical guideline, and to identify several modifying factors associated with them. Methods : Hypertension was selected as the target disease, because it is a chronic disease which is of great public health importance. Self-administered questionnaires were mailed to public health doctors practicing at health centers and health subcenters across the nation. The response rate was 20.9%. The questionnaire included the diagnosis and management process such as measuring blood pressure, history taking, physical examinations, and treatment approches and potentially modifying factors such as level of training, duration of practice as a public health doctor, and education on management of hypertension. Results : Public health doctors pay little attention in measuring BP, hypertension related history taking, performing physical examination and laboratory examination. But they devoted much effort in diagnosing hypertension exactly and giving nonpharmacological treatment. Among various antihypertensive drugs, calcium-channel blockers were the most preferred agent(50.9%). Level of training, duration of practice ad a public health doctor, and education on management of hypertension made no difference on quality of care(p>0.05). Conclusion : These public health doctors showed poor compliance with the pre-established clinical guidelines, which leaves much to be desired in diagnosing and managing hypertensive patients by public health doctors. This study might be able to contribute to develop some strategies, such as educational programs, which would be able to improve the process of care in hypertensives.
Accessory hepatic lobe is noted as and considered a rare disease in children. It can manifest with various symptoms and complications depending on the location, volume, type and position of the disease as presented on a child. The patient presented as a 14-month-old girl who was seen with a notable hepatosplenomegaly and portal hypertension. A diagnosis was made after taking an extensive medical history, observation and radiological examinations. The formal diagnosis was a prehepatic portal hypertension associated with accessory hepatic lobe.

Midaortic syndrome (MAS) is a rare vascular disease that commonly causes renovascular hypertension. The lumen of the abdominal aorta narrows and the ostia of the branches show stenosis. MAS is associated with diminished pulses in the lower extremities compared with the upper extremities, severe hypertension with higher blood pressure in the upper rather than lower extremities, and an abdominal bruit. The clinical symptoms are variable, and recognition in children with hypertension can aid early diagnosis and optimal treatment. Hypertension with MAS is malignant and often refractory to several antihypertensive drugs. Recently, radiologic modalities have been developed and have led to numerous interventional procedures. We describe the case of a 3-year-old boy presenting with left ventricular hypertrophy whose severely elevated blood pressure led to the diagnosis of idiopathic MAS. This case highlights the importance of measuring blood pressure and conducting a detailed physical examination to diagnose MAS. This is the first reported case of idiopathic MAS diagnosed in childhood in Korea.
Hypertension is a major risk factor of atherosclerosis which results in cardiovascular disease, and remains a major health problem worldwide. While children are more likely to have secondary hypertension, recent studies support the theory that the prevalence of essential hypertension in children and adolescents is increasing with the global epidemic of childhood obesity, and close attention is needed. Evaluation of hypertension in the pediatric age group should be guided by the age at presentation, and renal diseases must be considered in every child with hypertension, because of the prevalence of renovascular and renal parenchymal disorders as the etiology in any age group. The majority of children with chronic kidney disease are hypertensive, and many have associated end organ damage. Thus, once hypertension has been confirmed, end organ care as well as pharmacologic therapy must be continued. In renovascular hypertension, as cure could be gained with surgical/endovascular intervention, accurate diagnosis is important and it is recommended that every suspected child should undergo angiography.
