Effect of Kamisamul-tang on Hypertension and Free Radical (가미사물탕(加味四物湯)이 고혈압 병태 모델과 활성산소에 미치는 영향)

  • Various kinds of related parameters on hypertension such as anti-oxygen effect, ACE, weight of body, hwart and kidney, blood pressure, heartbeat rates, contents of aldosterone, catecholamine, change rates, of plasma constituents, uric acid, BUN, creatinine were determined to verify the effects on hypertension by Kamisamul-tang (KSMT). And the results are concluded as follows. KSMT did not show any cytotoxicity at the range of concentration (1-250 ${\mu}g/m{\ell}$) on the human fibroblast cell (hFCs). KSMT decreased the production of reactive oxygen species (ROS) and DPPH generation depending on the concentration. KSMT significantly inhibited angiotensin converting enzyme(ACE) activity depending on the concentration compared with control. KSMT maintained body weight of body, heat and kidney nearly normal group in hypertensive rat induced by DOCA-salt. KSMT significantly blood pressure and heart beat rate compared with control in hypertensive rat induced by DOCA-salt. KSMT significantly decreased aldosterone, dopamine, norepineph- rine, epinephrine compared with control in hypertensive rat induced by DOCA-salt. KSMT significantly decreased the level of potassium and cloride compared with control wheareas increased that of calcium significantly in hypertensive rat induced by DOCA-salt. KSMT significantly decreased the level of uric acid and BUN compared with control in hypertensive rat induced by DOCA-salt. It is verified experimentally tat Kamisamultang(KMST) which has been used broadly as a clinical therapeutics in oriental medicine is effective for anti-hypertension mechanism. And it could be applied to develope the reliable prescriptions for anti-hypertension in the future.

  • This study is to conduct a 12-month f/u survey on changes of metabolic syndrome (MetS) components according to the resting heart rate (RHR) in patients with essential hypertension (HTN) without cardiovascular disease (CVD) complication. We used the KorHR (Korean Hypertension Registry) data and a total of 951 subjects were analyzed using the R statistical program. As a result of this study, it was found that the risk and changes of MetS components differs according to the RHR group with first and revisit hypertensive patients. Therefore, guidelines for customized RHR control for first and revisit hypertensive patients should be prepared. Also, it is necessary to prepare various interventions to control the RHR. Through this, it is intended to prevent the occurrence of CVD caused by HTN, which is a representative chronic disease, to improve the quality of life of patients and their families as well as to contribute to increasing the efficiency in the health care.

Effect of Chungyeoldodam-tang on Hypertension (청열도담탕이 고혈압에 미치는 영향)

  • To access the safety and efficacy of Chungyeoldodam-tang(CDT), a traditional herbal medicine prescription, on hypertension we examined various parameters involved in the pathogenesis of hypertension. CDT seems to be safe because CDT at the concentrations lower that 250 ug/ml showed no toxic effects in cultured human fibroblast and no toxic effects on liver function. The production of reactive oxygen species (ROS) were greatly decreased in CDT treated group compared with control, and angiotensin converting enzyme activities were reduced by CDT in a dose dependent manner. There was no differences in weight of hearts between control and CDT treated group. The blood pressure and pulse rate were significantly decreased. CDT greatly reduced the levels of plasma hormones including aldosterone, dopamine, and norepinephrine, but not epinephrine, and serum electrocytes including Na$^+$ and Cl$^-$, but not K$^+$. were also decreased. The levels of uric acid, BUN and creatinine were significantly decreased compared with control. These results suggested that CDT has suppressive effects on various pathologic factors in hypertension, and CDT has potential as a safe and effective therapeutics for hypertension.

Accessible Strategy of the Registration & Management of Hypertension and Diabetes Mellitus Patients through the Public-Private Partnership: Policy Implications Fron the Hongcheon-gun Case (민관협력을 통한 고혈압.당뇨병 등록.관리사업의 접근 전략: 홍천군 사례의 정책적 함의)

  • Objectives: This study aimed to share with experiences of a demonstration program based on a community for prevention and management of hypertension and diabetes mellitus, and to supply the evidence of accessible strategies within the community through the public-private partnershipin the near future. Methods: This study case was "the program of registration and management of hypertension and diabetes mellitus patients" which was conducted in Hogncheon-gun in Gangwon-province, 2012. Results: The infrastructure of this center was constructed with the public-private sector partnership according to the basic model of demonstration program since November, 2012. So, the total registered rate of hypertension and diabetes mellitus patients were 26.6% in comparison with suspected patients (to the result of 2011 Korea National Health and Nutrition Examination Survey), 37.8% in comparison with the real number of outpatient (to the claims data of 2011 Branch Honcheon-gun, National Health Insurance Corporation), and 107.8% in comparison with the project goal, sequentially. To the patients who were not treated for 30 and 60 days among the registered patients, a recall service was conducted. Through this intervention, it was monitored that this program has enhanced the consecutive treatment rate of the registered patients. Conclusions: To improve the continuous management of hypertension and diabetes mellitus patients, we are gotten to know that the community need the joint participation and mutual cooperation with public-private sector partnership.

Prevalence, Awareness, Treatment, and Control of Hypertension, Diabetes, and Dyslipidemia Among Patients with Rheumatoid Arthritis (여성 류마티스관절염 대상자의 고혈압, 당뇨, 이상지질혈증의 유병률, 인지율, 치료율 및 조절률)

  • Purpose: The purpose of this study was to evaluate the prevalence, awareness, treatment and control of hypertension, diabetes, and dyslipidemia among women with rheumatoid arthritis. Methods: This cross-sectional study of 288 women with rheumatoid arthritis was conducted in a university-affiliated hospital. Data were collected by reviewing the women's medical charts and self-report questionnaires. Results: The prevalence of hypertension, diabetes and dyslipidemia was 27.8%, 5.9% and 48.3%, respectively. Despite the high prevalence of dyslipidemia, the subjects'awareness of it and its treatment rates were the lowest, which resulted in a low control rate. Of the 288 subjects, 158 (55%) had at least one of the three conditions. The prevalence of the combination of cardiovascular risk factors increased with age, and 32.7% of those aged 60 or older had two or three of the cardiovascular risk factors. Combinations of cardiovascular risk factors were significantly related with severe pain and fatigue. Conclusion: Cardiovascular risk factors were highly prevalent among the women with rheumatoid arthritis, but the rates of awareness, treatment, and control were suboptimal. There is a pressing need to facilitate the prevention, early detection and appropriate management of hypertension, diabetes and dyslipidemia among patients with rheumatoid arthritis in order to prevent cardiovascular disease.

Effects of Aroma inhalation on Blood Pressure, Pulse Rate, Sleep, Stress, and Anxiety in Patients with Essential Hypertension (아로마 흡입이 본태성고혈압 환자의 혈압, 맥박, 수면, 스트레스, 불안에 미치는 효과)

  • Purpose: This experimental research explored the effects of aroma inhalation on blood pressure, pulse, sleep, stress, and anxiety in patients with essential hypertension who are taking antihypertensive medication. Methods: A pilot survey, pre-test and post-test were conducted. The data were collected from August to October, 2009. The participants were 44 patients randomly assigned to an experimental group (22 people) and a control group (22 people). The experimental group received individual instruction for aroma inhalation, which lasted for 3 minutes. They inhaled blended oils of Lavender, Majoram and Ylang Ylang (The mixed ratio was 4:3:3), three times a day, for two weeks. Data from 36 patients were used for the final analysis (20 from the experimental group, 16 from the control group). Data were analyzed using SPSS version 13.0. Results: Sleep disturbance decreased in the experimental group (t=-2.258, $p$=.030). Blood pressure, pulse, stress, and anxiety were not significantly different between the two groups ($p$ >.05). Additional analysis of the experimental group showed that physical stress and psychological stress were significantly lowered. Conclusion: Aroma inhalation is an effective paramedical intervention for patients with essential hypertension.

Hypertension Management Status in Vulnerable Rural Areas and the Role of Community Health Practitioners (농어촌 의료취약지역에서의 고혈압관리 실태와 보건진료전담공무원의 역할)

  • Purpose: This study was a descriptive study to investigate correlations between self-care behaviors and quality of hypertension management by hypertensive patients being cared by community health practitioners. Method: Participants were 583 people who were prescribed antihypertensive medications at a health clinic located in South Chungcheong Province. The survey was done from June 1 to July 30, 2018. A self-report questionnaire was administered, and data analysis was performed using descriptive statistics and Pearson correlation coefficients with the SPSS 24.0 program. Results: The rate of awareness, treatment, and control of hypertension were 97.9%, 99.1% and 92.8%, respectively. The number of hypertensive self-care behaviors was 1.82 (${\pm}0.36$) out of 3 points. The quality of hypertension management was 3.22 (${\pm}0.46$) out of 4 points. There was a moderate correlation between hypertensive self-care behaviors and quality of hypertension management (r=.340, p<.001). Conclusions: Results of this study confirm that the quality of hypertension management by community health practitioners is related to self-care behaviors of hypertensive patients. Therefore, it is necessary to improve the quality of hypertension management by health care specialists for self-management of hypertension patients. In addition, a systematic program to improve the quality of hypertension management by community health practitioner is needed.

The Relationship between Patients' Expectations of Treatment Effect and Clinical Outcome in a Trial of Acupuncture for Hypertension (고혈압 침 임상연구에서 환자의 기대치와 치료효과 간의 상관관계)

  • Objectives : To identify potential correlation between patients' expectations and clinical outcome in a randomized control study of acupuncture. Methods : In a clinical trial of acupuncture for hypertension, 60 participants with pre and mild hypertension were randomized into an acupuncture group and a control group. After randomization, all participants were asked to rate their expectation for the intervention on a scale of 0 to 10. To analyze the effect of expectation on clinical outcome, change of blood pressure was compared between high and low expectation groups. Results : There was no significant difference of baseline blood pressure between low expectation group and high expectation group. Proportion of acupuncture group and control group was also not different between low and high expectation groups (p = 0.638). The change of systolic blood pressure was -1.55 mmHg in low expectation group and -3.07 mmHg in high expectation group, and it was not significantly different (p = 0.54). There was no significant difference in the change of diastolic blood pressure between two groups (p = 0.58), with -3.24 mmHg in low expectation group and -2.34 mmHg in high expectation group. Conclusion : In this study, the expectation of intervention (including acupuncture treatment) was not associated with the effect of intervention.

Effect of Hyeonsamdansameum on Hypertensive Rat Induced Monocrotaline (현삼단삼음(玄蔘丹蔘飮)이 Monocrotaline으로 유발된 고혈압 흰쥐에 미치는 영향)

  • This experiment was performed to investigate the effects of Hyeonsamdansameum(HDE) on hypertension. For the study of HDE, we had divided Sprague-Dawley rats to three groups-normal, control, HDE. The control group was injected subcutaneous with monocrotaline(50 mg/kg). The treatment group was injected subcutaneous with monocrotaline(50 mg/kg) and orally administered with HDE extract for 4 weeks(once a day, 208 mg/kg). Then we measured blood pressure, heart rate, on the plasma aldosterone, catecholamine, electrolyte, uric acid, BUN, creatinine, and observed the lung, cardiac muscle. liver. etc. The results of these were as follows: 2,2-diphenyl-1-picrylhydrazyl (DPPH) scavenging activity and superoxide dismutase(SOD) - like activity were increased. reactive oxygen species (ROS) was decreased. Angiotensin converting enzyme(ACE) inhibitory activity was increased in a concentration-dependent by HDE. HDE significantly increased body weight in monocrotaline hypertensive rat, so supported nearly normal group. HDE significantly decreased blood pressure and heart rate in monocrotaline hypertensive rat. HOE significantly decreased aldosterone in adrenocortical hormones. HDE significantly decreased dopamine. norepinephrine, epinephrine. Na+. Cl- were intended to decrease. K+ was decreased significantly by HDE. Uric acid. BUN were significantly decreased and creatinine was intended to decrease by HDE. HDE inhibited lung, liver and heart injury connected with hypertension. These results suggest that HDE is usefully applied in treatment and prevention of hypertension.

Effect of exercise on cystatin C as a risk factor for renal failure and hypertension

  • This study was conducted to investigate the effect of a 24-week combined exercise training program in older women with hypertension. Women with hypertension who were 70 years and older were randomized into two groups: combined exercise group (CE; n = 15) and a control group (n = 15). The CE group performed a combined exercise training program four times per week for 24 weeks and the control group did not. Five factors, including body composition (percent body fat and skeletal muscle mass), health-related physical fitness, adipocytokines (interleukin-6 [IL-6] and tumor necrosis factor-alpha [TNF-α]), kidney risk factors (glomerular filtration rate [GFR] and cystatin C), and systolic and diastolic blood pressure were measured before and after the program. The findings showed that total muscle mass, health-related physical fitness factors, and GFR increased significantly in the CE group compared to those in the control. Additionally, systolic and diastolic blood pressure and IL-6, TNF-α, and cystatin C levels in the CE group decreased significantly after the intervention. In contrast, total muscle mass decreased significantly and blood pressure remained unchanged in the control group. These results suggest that CE training may positively impact circulating levels of adipocytokines and cystatin C and improve physical fitness levels in elderly women with hypertension. Therefore, CE training helps to prevent renal disease and improve health-related physical fitness, eventually leading to a better quality of life.
