It is generally accepted that the delivery of health care is undergoing many changes specially those related to acute, contagious disease care and to the increase of chronic illnesses which can not be cured but are controlable. The health care practitioner can not be soley responsible for the control of their clients' care. Because the clients will play a vital role in controlling their illnesses, long term participation by both the health care provider and the client is necessary. Since most individuals with hypertension do not experience signs or symptoms, the disease is difficult to detect and even when diagnosed, clients do not comply well with their hypertension regimens. The noncompliant client is at increased risk for compliants involving the heart, brain, kidney and other organs. In an effort to explore methods of increasing patient participation in and adherence to treatment programs for hypertension, the researcher used health contracting to promote self care. The research questions are; 1) Will the health contracting increase compliance in health behavior and reduce the blood pressure\ulcorner 2) If clients comply with their regimens will this reduce their blood pressure\ulcorner The research design utilized in this study was a quasi-experimental design. A purposive sample, was abtained from two churches in the 1. area, consisting of 64 clients with hypertension. The data was collected from the middle of January to the 1st of September 1985. Randomization was only of the two church groups into experimental and control groups. Compliance with health behavior related to the hypertensive regimen, blood pressure and body weight were measured, compared and analyzed. In the experimental group measurements were made 6 times; one month before the education program after education program when health contracting was done and 4 more times once a month for 4 months. In the control group measurements were made 3 times; one month before the education program after the education program, and once 4 months later. There was no health contracting. The data were analyzed by t-test, Pearson correlation and ANOVA according to purpose of the study. The result of this study may be summarized as follows: The result related to the hypothesis on the effect of health contracting are as follows: H$_1$; “The hypothesis that the experimental group, with a health contractual agreement will demonstrate increased compliance levels for health behavior than the control group” was supported(t=-5.29, df=62, p=.000). H$_2$; “The hypothesis that the experimental group, with a health contractual agreement, will demonstrate a greater reduction in blood pressure than the control group” was supported (for systolic blood pressure t=2.72, df=62, p=.009, for diastolic blood pressure t=1.95, df=62, p=.050). H$_3$; The hypothesis that the greater the compliance of the client with health behavior the lower the client's blood pressure will be was partially supported (for systolic pressure r=-.2981, p=.008, for diastolic pressure r=-.1720, p=.087). From the examination of the results of this study it can be concluded that the interaction between the nurse and the client, contracting to define goals and reinforcing compliant behavior, leads to improved compliance with health care behaviors and thus to an increase in the effectiveness of nursing care. Further consideration need to be given to the inclusion of the concept of health contracting in primary nursing and to further research in this area.
본 연구는 장애인 복지관을 이용하는 지체장애인 및 뇌병변장애인 중 고혈압 환자를 대상으로 복지관 간호사와 간호학생 자원봉사자를 통해 제공된 8주 전화상담 고혈압 자가관리 프로그램의 효과를 파악하는데 목적이 있다. 비동등성 대조군 전후 유사실험설계 연구로, 2019년 12월부터 2020년 5월까지 사전, 사후 조사를 완료한 대조군 28명, 실험군 29명의 자료를 SPSS 프로그램을 이용하여 분석하였다. 주요 연구결과로, 프로그램 적용 후 실험군이 대조군에 비해 고혈압 관련 지식, 건강신념 중 심각성과 민감성, 삶의 질 EQ-VAS에서 통계적으로 유의한 차이를 나타나며 높아진 것으로 나타났다. 따라서 장기적인 건강관리가 필수적인 장애인 고혈압 환자를 대상으로 다양한 자가관리 프로그램을 적용하여 관련 지식, 건강신념 및 삶의 질 등을 높이는 노력이 계속적으로 필요할 것이다.
Purpose: This study was a descriptive study to investigate correlations between self-care behaviors and quality of hypertension management by hypertensive patients being cared by community health practitioners. Method: Participants were 583 people who were prescribed antihypertensive medications at a health clinic located in South Chungcheong Province. The survey was done from June 1 to July 30, 2018. A self-report questionnaire was administered, and data analysis was performed using descriptive statistics and Pearson correlation coefficients with the SPSS 24.0 program. Results: The rate of awareness, treatment, and control of hypertension were 97.9%, 99.1% and 92.8%, respectively. The number of hypertensive self-care behaviors was 1.82 (${\pm}0.36$) out of 3 points. The quality of hypertension management was 3.22 (${\pm}0.46$) out of 4 points. There was a moderate correlation between hypertensive self-care behaviors and quality of hypertension management (r=.340, p<.001). Conclusions: Results of this study confirm that the quality of hypertension management by community health practitioners is related to self-care behaviors of hypertensive patients. Therefore, it is necessary to improve the quality of hypertension management by health care specialists for self-management of hypertension patients. In addition, a systematic program to improve the quality of hypertension management by community health practitioner is needed.
본 연구는 범이론모형에 기반하여 운동행위강화 프로그램을 개발하고 이를 고혈압 노인에게 적용하여 혈압 및 신체활동량에 미치는 효과를 규명하고자 하였다. 연구대상자는 서울시 소재 노인복지관에 내원하며 연구에 참여를 허락한 65세 이상 고혈압 노인 41명으로 실험군 20명, 대조군 21명이었다. 실험군에게는 범이론모형에 기반한 운동행위강화 프로그램을 8주 동안 적용하였다. 연구결과 실험군은 대조군에 비하여 운동행위 변화단계, 전체 변화과정 점수, 행위적 변화과정 점수, 운동 자기효능감, 신체활동량이 유의하게 증가하였으나, 경험적 변화과정 점수, 의사결정균형, 혈압 등은 유의한 차이가 없었다. 이러한 결과를 바탕으로 운동행위강화 중재는 비활동 고혈압 노인의 운동행위로의 이행과 신체활동량을 증진시키는데 효과가 있음을 확인하였다. 본 프로그램이 지역사회에 거주하는 고혈압 노인에게 적용된다면 운동습관을 형성하게 하여 고혈압 합병증을 예방하는데 기여할 것으로 기대된다.
Purpose: This study examined socio-demographic and health status factors associated with hazardous drinking in male and female elderly with hypertension. Methods: The sample consisted of 2,322 seniors with hypertension and was obtained from the Seventh Korean National Health and Nutrition Examination Survey (VII-1, VII-2, VII-3). Complex sample analysis was performed using the independent t-test, the Chi-square test and multiple logistic regression in SPSS/WIN 24.0 program. Results: The factors associated with hazardous drinking in the male hypertensive seniors were age (OR=2.13, CI=1.37-3.32), weekly drink number (OR=0.11, CI=0.07-0.16) and weight gain control (OR=0.44, CI=0.21-0.94), and corresponding factors in female hypertensive seniors were age (OR=4.56, CI=2.37-8.80), income level (OR=1.88, CI=1.05-3.37), weekly drink number (OR=0.09, CI=0.04-0.17), current smoking (OR=6.88, CI=2.29-20.64), weight loss control (OR=1.99, CI=1.14-3.47), hypertension treatment (OR=35.62, CI=2.02-629.43), and hypertension drug (OR=0.06, CI=0.01-0.61). Conclusion: Risk drinking in elderly with hypertension was found to be related to personal characteristics and health status. Therefore, to manage hypertension effectively in the elderly, drinking patterns should be improved and active participation in the practice of healthy activities such as weight control, smoking cessation, and decreased alcohol intake, should be encouraged.
Purpose: The purpose of this study was to develop a self-management program applying Dongsasub training based on self-efficacy theory, and to verify the program effectiveness on self-esteem as well as self-efficacy, self-management behaviors, and blood pressure. Methods: The study design was a non-equivalent, pre-post controlled quasi-experiment study. Thirty-eight patients aged 65 and older from a senior welfare center in Seoul participated in this study (20 patients in the experimental group and 18 patients in the control group). The self-management program applying Dongsasub training consisted of eight sessions. After development was complete the program was used with the experimental group. Outcome variables included self-efficacy, self-esteem, self-management behaviors measured by questionnaires, and blood pressure measured by electronic manometer. Results: Self-efficacy (t=2.42, p=.021), self-esteem (t=2.57, p=.014) and self-management behaviors (t=2.21, p=.034) were significantly higher and systolic blood pressure (t=-2.14, p=.040) was significantly lower in the experimental group compared to the control group. However, diastolic blood pressure (t=-.85, p=.400) was not significantly different between the two groups. Conclusion: The results indicate that the self-management program applying Dongsasub training can be used as a nursing intervention in community settings for improving self-management behaviors for older adults with hypertension.
Purpose: This study was conducted to compare health-related characteristics and self-care behavior between a controlled group and a non-controlled group of hypertension patients in a customized home visiting health service. Methods: This study was conducted as cross-sectional research. The subjects were 1,317 hypertension-controlled patients and 555 non-controlled ones. Data were collected using a structured questionnaire and analyzed by descriptive statistics, $x^2$-test, and t-test using the SPSS WIN 15.0 program. Results: Sociodemographic characteristics and self-care behavior were not significantly correlated between the groups. According to health-related characteristics, the health status was significantly higher in the hypertensioncontrolled group than in the non-controlled group. The cholesterol level was significantly lower in the hypertension-controlled group than in the non-controlled group. Conclusion: Attention should be paid to patients who are not in good health status in order to control their hypertension. Also, it is necessary to educate the patients in order for them to manage their hypertension and cholesterol.
Purpose : This study was to investigate the effects of aerobic exercise and resistive exercise program on hypertension in the elderly. Methods : Subjects were allocated into one of three groups (aerobic exercise group, resistive exercise group, control group). Blood pressure was measured pre- and post-exercise in each exercise group. Data were analyzed with paired t-test and one-way ANOVA to determine blood pressure differences. Results : The results of this study were as follows. 1) The systolic blood pressure of the aerobic exercise group decreased significantly after exercise (p<.05). 2) The diastolic blood pressure of the resistive exercise group decreased significantly after exercise (p<.05). 3) There was a significant decrease in systolic blood pressure of aerobic exercise group compared with control group following the exercise. 4) There was no significant differences in diastolic blood pressure among three groups following the exercise (p>.05). Conclusion : Aerobic exercise and resistive exercise program were effective in reducing systolic blood pressure and diastolic blood pressure of the hypertension in the elderly. Thus, this program can be recommended as an effective intervention for the elderly. Therefore, regular and continued those exercises will be the solution for decreasing systolic blood pressure and diastolic blood pressure.
This study was conducted to identify a effect of the comprehensive self-regulation program for hypertensives as a nursing intervention on self-care performance and the physiologic parameters in rural areas. For this purpose, a nonequivalent control group and a pre and post-test quasi- experimental design was used. Fifty-four were evaluated subjects from two Community Health Practitioner Posts in the suburbs of Taejon City. The subjects of the control and experimental groups were matched for age and sex. The self-regulation program developed by author given to the experimental group. The program consisted of group education on hypertension and self-care, self-regulation including the blood pressure self-monitoring, recording of self-care activities, and encouraging and reinforcing self-efficacy. The whole program was carried out from September to November of 1999. The data were analyzed by repeated measure ANCOVA, t-test, and ANCOVA. The results were as follows ; There was significant improvement in the scores on knowledge (F=.68, P=.004), perceived self-efficacy (F=26.39, P=.000), self-care performance (F=26.11, P=.000) of the experimental group compared with those of the control group. There was no significant change on the score of locus of control, perceived benefits and perceived barrier, blood cholesterol level, body weight between two groups (P>.05). From these results, it can be concluded that the self-regulation program is an effective nursing strategy to promote self-care performance of hypertensives in rural area.
Purpose: This study was to analysis the effect of self-foot reflexology(SRM) program on, level of hypertension. Method: A nonequivalent control group pretest-posttest design was used. For the experimental group, SRM was applied three times a week for 8wks. Test was done by measured x2-test, t-test, repeated measures ANOVA test. Result: There were significant decreases in systolic, diastolic blood pressure and difference of pretest and post test and between the experimental group and control group. Conclusion: Findings indicate that this study will contribute to develop nursing strategies for the regulation of the blood pressure.
