A Case of Showing Granuloma Following a Sea Urchin Sting (성게 가시에 의하여 육아종이 발생한 1례)

  • The case was described of a 31 year old female who developed fusiform swelling of the right third finger at the site of puncture by sea urchin spine. We observed a radiopaque linear material suggesting a remnant spine which the size was 2.4 mm in sonography and mammography. We confirmed that it exhibits a urchin spine and distinct caseous necrosis with central calcification. Sea urchins(Echinoidea) are ubiquitous marine creatures that resemble pincushions and are covered by calcareous skeletons surrounded by numerous movable calcium carbonate spines. The sharp spines are brittle and easily detach when touched by the victim. Contact with sea urchin spines produces intense pain, some localized erythema and edema. Inflammation develops in response to retained fragments. Granulomas 1-5 mm in diameter develop rarely 2-12 months after envenomation. In treatment, hot water immersion and thorough wound cleansing are advisable. Analgesics may, be necessary after hot water immersion. Attempts to remove these spines should be performed with good lighting sources, preferably with radiological direction. Chronic granulomas usually require complete excision. Acetone has been suggested for rapid resolution of pain. No antidotes are available.

  • Systemic lupus erythematosus is a severe cutaneous-systemic disorder of unknown etiology, It is represented with erythematous patches on the face in a so-called butterfly distribution, and characteristically classified as an autoimmune disease with antinuclear antibodies. The autoimmune diseases such as systemic lupus erythematosus, $Sj{\ddot{o}}gren$ syndrome, rheumatoid arthritis have been associated with lymphoid malignancy - leukemia, malignant lymphoma - which could involve various organs(spleen, liver, brain, mediastinal lymph node, supraclavicular lymph node, inguinal lymph node, cervical lymph node etc.). Many authors have studied about the association of systemic lupus erythematosus and malignant lymphoma, but exact etiology is still unknown. A common viral etioloty for systemic lupus erythematosus has been suggested since virus-like particles have been found in the glomerular endothelium of patients with systemic lupus erythematosus. These oncogenic viruses may be responsible for the higher frequency of malignant lymphoma in patients with systemic lupus erythematosus. In the other theory, the causes of malignant lymphoma are the defect of immune system due to systemic lupus erythematosus and the long-term use of therapeutics for treatment of systemic lupus erythematosus. When the cellular immune system(delayed hypersensitivity) is impaired by immunosuppressive drugs, it is likely that the body is no longer able to recognize and reject malignant cells as they arise; they continue to grow and divide unhindered. The impairment of the cellular immune system may allow growth of oncogenic virus or the survival of neoplatic tissues. 47-year old female patient treated systemic lupus erythematosus with steroid and immunosuppressive drugs for 5 years visited to our hospital due to elevated mass on left upper anterior maxilla area. By performing biopsy, we diagnosed this lesion as malignant lymphoma and referred to oncologist for chemotherapy. So we report a case of malignant lymphoma due to systemic lupus erythematosus with review of literatures.

The Immunoadjuvant Activity of The Water-Extract of Korean mistletoe (Viscum album var. coloratum) Fruit (한국산 겨우살이 열매 추출물의 Immunoadjuvant 효과)

  • To evaluate the immunomodulatory activity of a water extract (KMF-WE) of Korean mistletoe (Viscum album var. coloratum) fruit, we examined its ability to induce humoral and cellular immune response against keyhole limpet hemocyanine (KLH). Immunized mice with KLH admixed with KMF-WE (KLH/KMF-WE) showed significant induction of KLH-specific antibodies compared to mice immunized with KLH alone. The assay for determining isotypes of antibodies revealed that KMFWE augmented KLH-specific-IgG1 and -IgG2a production. In vitro T lymphocyte proliferation analysis against KLH revealed that the splenocytes of mice immunized with KLH/KMF-WE showed a significantly higher proliferative ability than those from mice immunized with KLH alone. The culture supernatants of splenocytes, which were harvested from mice immunized with KLH/KMF-WE, showed higher levels of both Th-1 type (IL-2, IFN-${\gamma}$) and Th-2 type (IL-4) cytokines in response to KLH stimulation compared to those from mice immunized with KLH alone. Also, in delayed-type hypersensitivity (DTH) assay, mice immunized with KLH/KMF-WE showed a significantly higher reaction to KLH than mice treated with KLH alone. These results suggest that KMF-WE possess immunoadjuvant activity to enhance both antigen-specific humoral and cellular immune responses against protein antigens (KLH).

Protective Immune Reponses Induced by Non-infectious L-particles of Equine Herpesvirus Type-1: Implication of Cellular Immunity

  • Mice immunized with equine herpesvirus type-1(EHV-1) L-particles skewed a significant increase (p<7.75) in serum antibody titers. Upon a booster dose four weeks lateral antibody titers increased significantly. Interestingly, immunization via intravenous or intramuscular route induced significantly higher (p<0.75) antibody titers. However, mice iummunized with UV-treated L-particles, visions or immunization via intranasal route induced lower antibody titers. Upon challenge inoculation with wildtype EHV-1, our data showed there was a poor correlation between antibody titers and protection against virus replication. Therefore, the role of cell-mediated immunity Inwards protection was investigated. As predicted, the strongest cell-mediated immunity, as measured by delayed-hypersensitivity test, was detected in mice immunized with live virus particles. The magnitude of cell-mediated immune response correlated with the efficacy of L-particles as immunizing agent. The highest efficacy, as indicated in mice immunized via intranasal routed was highly correlated with cell-mediated immunity. A similar phenomenon was also demonstrated in mice immunized intranasally with UV-treated L-particles. However, the degree of protection was reduced when mice immunized intravenously or intramuscularly with UV-treated L-particles. In conclusion, protection conferred in these animals was highly implicated by immune cells and the least by antibodies. The route of immunization and the nature of the antigen also contributed to the efficacy of L-particles as immunizing agent. In contrast to that of herpes simplex virus type 1, our data showed EHV-1 non-infectious L-particles are highly suitable for immunization of the host against EHV-1 disease.

Immunobiological Studies on Route of Administration of Amygdalin (아미그달린의 투여경로에 따른 면역생물학적 연구)

  • Experiments were performed on male Sprague-Dawley rats to investigate the immunobiological effects on route of administration of amygdalin(AM). Rats were administered orally at 12.5, 25, or 50mg/kg/day of AM or injected wtih 25,50, or 100mg/kg/day of AM intravenously for 2 weeks. Rats were immunized and challenged with sheep red blood cells(SRBC). The results of this study were summarized as follows;(1) In oral administration of AM, body weight gains were significantly increased by 50mg/kg AM as compared with controls, the relative weights of liver and thymus also were significantly increased by 12.5 and 25mg/kg AM. However, 2-mercaptoethanol-resistant hemagglutination titier (2-MER HA), Plaque forming cells (PFC) and rosette forming cells (RFC) were non-dose dependently decreased. Phagocytic activity and delayed-type hypersensitivity (DTH) reaction also were significantly decreased by 50mg/kg AM. (2) In intravenous injection of AM, body weight gains, hemagglutination titer (HA), 2MER-HA, DTH reaction, PFC, RFC and circulating leukocytes were not influenced by AM. However, the relative weights of liver, spleen and thymus were significantly enhanced 100mg/kg AM. These results indicated that oral administration of AM non-dose dependently suppresses humoral and cell-mediated immunity in SD rats, and that intravenous injection of AM is unaffected humoral and cell-mediated immunity, however, the high dose of it significantly enhances phagocytic activity.

  • The purpose of this study was to investigate the effect of nutritional status on the cell-mediated and humoral immunity in female college students. The nutritional status of twenty subjects was determined by six-days food records, anthropometric measurements, and biochemical assessments of serum nutrients. Cell-mediated and humoral immunity of the subjects was analyzed by in vivo and in vitro assessments. The results were summerized as follows : First, The average daily energy intake was 1437Kcal(CHO : PRO : FAT = 61:13:26), which corresponds to 71.9% of RDA. Anthropometric measurements showed that 50% of the subjects was under-weight(BMI<20), only 5% was over-weight(25

A Study on the Effects of Sopunghwalhyeoltang-gamibang on the Inhibition of Arthritis Induced by Collagen on the Mouse (소풍활혈탕가미방이 Collagen으로 수발된 생쥐의 관절염 억제에 관한 연구)

  • This study was carried out to know the effects of sopunghwalhyeoltang-gamibang(hereinafter referred to SPHHT) on the inhibition of arthritis induced by collagen on the mouse. Various experimental were performed in vivo (in DBA/1 J mice which are experimental model of arthritis induced by collagen) to analyse the immunomodulatory effects of SPHHT. The results were obtained as follows: 1. The cytotoxicity against mLFC was not measured in all concentration. 2. The arthritis index, incidence, hind paw edema, delayed-type hypersensitivity reaction, spleen weight were reduced in SPHHT treated group. 3. The expression of CD3 ε +/CD4+, CD3 ε +/CD8+ and CD19+ in peripheral blood mononuclear were reduced in SPHHT treated group. 4. The production of pro-inflammatory cytokines such as IL-6 and TNF-α were significantly reduced in SPHHT treated group. 5. The formation of new bones and synivium were stimulated in SPHHT treated group. Comparison of the results for this study showed that Sopung-hwalhyeoltang-gamibang had immunomodulatory effects. So we expect that Sopunghwalhyeoltang-gamibang should be used as a effective drugs for not only rheumatoid arthritis but also another auto-immune disease. Therefore we have to survey continuously in looking for the effective substance and mechanism in the future.

Haematological and Immunological Response in Lambs Fed on Raw and Variously Processed Cottonseed Meal

  • An experiment was conducted with twenty crossbred male lambs to assess the effect of cotton (Gossypium) seed meal (CSM) on blood constituents and immunity. Lambs were randomly assigned to a reference diet (30% deoiled peanut meal, DPNM) and four test diets containing 40% of either raw, 45 minutes cooked, 1% $Ca(OH)_2$ and iron (1 free gossy-pol, FG : 0.3 Fe) treated CSM (replacing approximately 50%, reference concentrate mixture). These isonitrogenous and isocaloric concentrate mixtures were fed to meet 80% of protein requirements (NRC, 1985) along with ad lib maize hay for 180 days. Blood was collected at 60, 120 and 180 days post feeding. The lambs were sensitized with Brucella abortus S99 antigen after 140 days and were subjected to ELISA and delayed type hypersensitivity. Blood haemoglobin, erythrocyte count, leucocyte count, total protein, total albumin, total globulin, urea, creatinine concentration and aspartate aminotransferase activity in lambs fed on raw or processed CSM were comparable to the values of reference lambs. The higher (p<0.01) blood glucose levels observed in CSM fed lambs at 60 days of feeding was latter reduced to the levels comparable with those on reference diet at 120 and 180 days of feeding. The alanine amino transferase activity was lower in lambs fed raw and cooked CSM containing diets at 120 and 180 days of feeding. A marginal increase in serum iron and alkaline posphatase activity was observed in iron treated group and raw CSM fed lambs, respectively. The humoral immune response and DTH reactivity was lower (p<0.05) in lambs fed raw CSM (consuming 302.83 mg FG/day). Cooking, $Ca(OH)_2$ and iron treatment of raw CSM showed a positive response in alleviating the suppression of immune response owing to the reduced consumption of FG by 40.19, 17.40% and 26.73%, respectively in these diets. The present study thus indicated that consumption of 40% raw CSM (302.83 mg FG/day) though did not affect majority of the haematological and blood biochemical parameters, but markedly suppressed the immune mechanism of lambs.

Chemically-induced delayed cutaneous hypersensitivity in dogs infected with Demodex canis (Demodex canis 감염이 화학적으로 유발된 지연형 피부과민증에 미치는 영향)

  • To observe the effect of Demodex canis infection on the cellular immune response and hematological profile, 8 Doberman pinschers experimentally infected with D cains and 4 uninfected control dogs were sensitized with 2, 4-dinitro-chlorobenzene(DNCB) on the skin and were challenged with DNCB 14 days after the initial sensitization to elicit allergic contact dermatitis. Histological and hematological changes of these dogs were then observed. Macroscopic changes of skin challenged with DNCB in D canis-infected dogs included significantly reduced area of allergic reaction(p<0.05) than in uninfected control group. Infiltration of inflammatory cells in the D canis-infected group was also significantly reduced(p<0.05) than in the uninfected control group. These changes indicated that the cell-mediated immune response of the animals was suppressed by the infection with D canis. Total white blood cell count in dogs infected with D canis was increased when dogs were sensitized with DNCB (p<0.01). The result appeared to be caused by stress due to D canis infection, secondary bacterial infection and decreased efficacy of general body defense system. Blood eosionophils were increased in D canis-infected dogs which appreared to be caused by the allergic contact dermatitis. Blood chemistry analysis revealed that total protein and globulin were increased(p<0.05), while albumin level was decreased. This result appeared to be caused by secondary bacterial infection.

Generation of Renal Cell Carcinoma-specific CD4+/CD8+ T Cells Restricted by an HLA-39 from a RCC Patient Vaccinated with GM-CSF Gene-Transduced Tumor Cells

  • Background: Granulocyte-macrophage colony-stimulating factor (GM-CSF) gene-transduced tumor cell vaccines induce very potent systemic anti-tumor immunity in preclinical and clinical models. Our previous phase I clinical trial in patients with metastatic renal cell carcinoma (RCC) has demonstrated both immune cell infiltration at vaccine sites and T cell-mediated delayed-type hypersensitivity (DTH) response to whole tumor cell vaccines. Methods: To investigate the immune responses to autologous genetically- modified tumor cell vaccines, tumor-specific $CD8^+$ T cell lines were generated from peripheral blood lymphocytes (PBL) of a RCC patient 1.24 by repeated in vitro stimulation with either B7.1-transduced autologous RCC tumor cells or B7.1-transduced autologous tumor cells treated with interferon gamma ($IFN{\gamma}$), and cloned by limiting dilution. Results: Among several RCC-specific cytotoxic T lymphocytes (CTLs), a $CD4^+/CD8^+$ double positive T cell clone (17/A2) appeared to recognize $IFN{\gamma}$-treated autologous RCC restricted by HLA-B39. The 17/A2 also recognized other HLA-B39 positive RCC tumor cells after $IFN{\gamma}$ treatment. Conclusion: These results demonstrate that autologous RCC vaccination successfully generates the tumor-specific CTL 17/A2, and suggest that the presentation and recognition of the tumor antigen by the 17/A2 might be upregulated by $IFN{\gamma}$.
