A Mediastinal Mass (종격동 종괴)

  • A 32-year-old female was admitted for evaluation of known mass on right suprahilar area. Chest PA showed $4{\times}3cm$ round and homogenous mass on suprahilar area. No abnormal findings were found in PFT, cytology, bacterial study, and fine needle aspiration biopsy(FNAB). On chest computed tomography, solitary mass was on right suprahilar area and no evidence of intrapulmonary metastasis or lymphnode metastasis was seen. Right upper lobectomy of lung was performed and Castleman's disease of hyaline vascular type was diagnosed based on the histologic findings of multiple and large lymphoid follicles with prominent vascular proliferation and hyalization in the central portion.

  • Lymphoid hamartomas are a rare benign disease which can be easily treated by complete surgical excision. They developed most often in the thorax, and can be discovered usually on routine chest X-ray.p But some of them can also be found because of pressure symptoms or the presence of a palpable mass if outside the thorax. We experienced a case of the hyaline-vascular type of lymphoid hamartoma in the left hilum of a 29 year-old Korean male in March, 1979. He was well except intermittent cough and left chest discomfort of a year duration and was treated by resection of the left upper lobe including nodular tumor masses. The histological characteristics were an aggregation of lymphoid follicles composed of concentrically arranged mature lymphocytes with centrally placed thick walled arterioles showing endothelial proliferation and some of them were hyalinized. Between the follicles, there was extensive capillary proliferation and infiltration of numerous lymphocytes, scanty plasma cells and eosinophils.

A Taxonomic Study on Korean Allium L. Based on the Morphological Characters (형태학적 형질에 기초한 한국산 부추속의 분류학적 연구)

  • For 20 taxa of Korean Allium, including 16 species and 5 varieties, examined were morphological characters (i.e. sexuality, structure and shape of underground part, types and growing patterns of leaf and scape, and shapes of perianth, filament and pistil). The specialization and the evolutionary trends of taxonomic characters were inferred from morphological examination. Taxonomic relationships and system of Korean Allium were also studied. The characters showing evolutionary trends were the structure and shape of underground part including rhizome and bulb, leaf, scape, inflorescens, filament and ovary. It seemed that thin and short rhizome developed from thick and long one, and the membranous simple bulb tunic evolved into fibrous reticulate one. The presence of hyaline sheath in A. monanthum of sect. Microscordum was apomorphic. Both angular leaf blade with 2-rowed vascular bundle and flat blade with 1-rowed vascular bundle were developed from terete one with 2-rowed vascular bundle. The base of filament have differentiated from entire to toothed, and 2-ovuled ovary as well as erect scape before flowering was plesiomorphic type. In addition, sexuality, structure of underground part, the presence of hyaline sheath and cross-section structure of leaf were taxonomic characters with the level of the subgenus in this genus. Shape of rhizome, bulb, leaf, scape and pedicel discriminated each section from other ones in the subgenera easily. The shape and arrangement of perianth and filaments were the diagnostic characters for species level along with shape of ovary and stigma.

  • Castleman's disease is a rare non-neoplastic lymphoproliferative disorder of unknown etiology. It is divided into three histologic subtypes; hyaline-vascular(HV), plasma cell(PC) type and mixed type (HV-PC). It has two clinical expressions. The localized form, which presents as a slow growing mass, has a relatively benign clinical course. The multicentric form is multilocated and holds significant morbidity. The mainstay of treatment of the localized form is surgical resection. The multicentric form requires medical treatment comprising prednisolone and other immunosuppressor drugs. The disease in children seems to have a more favorable course than in adults. We report a 13-year-old boy with Castleman's disease of multicentric form who was successfully treated with prednisolone and intravenous immunoglobulin.

Treatment for Fracture of Neck of the Talus (거골 경부 골절의 치료)

  • The neck of the talus is its most vulnerable and fragile segment, because of narrow diameter, devoid of hyaline padding and honeycombed internally by vascular channels etc. Talar neck fractures comprise 50% of all major to the talus. The majority occurs as a result of high-energy injuries, such as motor vehicle accidents or fall from a height. Anatomically, talar surface is covered mainly with articular cartilage and blood supply to the talus is very poor. So, complications, such as non-union, avascular necrosis and post traumatic arthritis, are frequent. The authors reviewed fourteen cases of talar neck fractures treated in our clinics from Jan. 1992 to Mar. 1997, and average follow-up period was over 15 months. The results obtained were as follows; 1. Patients' average age was 31.2 years. 2. The most common cause was traffic accident(9/14, 64%), and hyperdorsiflexion injury of the ankle was common mechanism of the fractures. 3. According to the modified Hawkins classification, type I was four cases, type II was nine cases, type III was one case and type IV was no case. 4. Hawkins sign of subcortical radiolucency was found in 64% (9/14) of the fractures. 5. Avascular necrosis was occurred in 21% (3/14) of the fractures(in two cases of type II fractures, and in one of type III). 6. According to the Hawkins criteria, four cases in type I, five in type II were an excellent result. Two cases, one in type II and one in type III were good result, and two in type II were fair. One in type II was poor result.

  • Castleman's disease is an uncommon lymphoproliferative disorder. The disorder can be classified based on histological subtype, such as hyaline vascular type, plasma cell type, and mixed type, and can also be clinically divided into either unicentric or multicentric type. Its exact pathophysiology is not clearly identified. The unicentric type is able to be treated by surgical resection. However, there is no standard treatment modlity for the multicentric type. Treatment of multicentric type includes anti-cancer chemotherapy and radiation therapy. Recently, authors have experienced a rare case of unicentric type of Castleman's disease accompanying a mucoepidermoid carcinoma of parotid gland and report a case which is discussed with references.

  • Giant lymph node hyperplasia(Castleman's disease) is a rare disease, which represents a peculiar form of lymph node hyperplasia. Generally, it has been considered as benign and localized disease but recently, revealed malignant transformation in some cases of multicenteric form. It usually occurs on the mediastinum and occasionaly neck, lung, axilla, mesentery, broad ligament, retroperitoneum or soft tissue of extremities. Histopathologically, it is divided into hyaline vascular or plasma cell type and the former is characterized with prominent vascular proliferation and hyalinization in the central portion and tight concenteric layering of lymphocytes at the periphery of the fillicles(mantle zone) and the latter is characterized by a diffuse plasma cell proliferation in the interfollicular area. From the point of view of clinical presentation, it has been divided into solitary form, which presents as a localized mass located most commonly in the mediastinum, and multicenteric form, which occurs multiple location and has systemic manifestation arid transformation into malignancy. Herein we report a case of Giant lymph node hyperplasia occuring in the left spermatic cord in a 58-year old male with brief review of literatures.

Castleman's Disease of the Neck and Mediastinum (경부와 종격동에 발생한 캐슬만씨 병(Castleman's Disease))

  • Objectives: Castleman's disease(CD) is a lymphoproliferative disorder of unknown etiology. To elucidate the clinicopathologic characteristics of CD, we retrospectively reviewed our experience. Methods: Fifteen patients with CD of the neck and mediastinum were identified. Patients were divided into two groups: group I had an unicentric CD and group II had multicentric CD. The histology of CD was divided into 3 subtypes: hyaline-vascular(HV), plasma cell(PC), and mixed. Results: The study included 12 cases of group I, 3 cases of group II in the clinical aspect and 10 cases with HV, 3 cases with PC, 2 cases with mixed type in the histologic aspect. Of group I patients who underwent complete surgical excision, all are currently free of disease. The clinical course of group II patients was variable. Of two patients with multicentric plasma cell CD who were treated, one remain free from disease and the other had a local recurrence in the neck. One patient with multicentric mixed CD died of disease after 30 months of presentation. Conclusion: Surgical resection is recommend for patients with the unicentric CD, regardless of histologic subtype. Patients with multicentric disease do not benefit from surgical resection and should be candidates for multimodality therapy.

Neutrophilic Respiratory Burst Contributes to Acute Lung Leak in Rats Given N-nitroso-N-methylurethane (N-nitroso-N-methylurethane으로 유도된 급성 폐손상에서 호중구에 의한 산화성 스트레스의 역할)

  • As is well known that N-nitroso-N-methylurethane (NNNMU) causes acute lung injury (ALI) in experimental animals. And ALI caused by NNNMU is very similar to ARDS in human being in its pathology and progress. In its context, we investigated the pathogenetic mechanism of ARDS associated with oxidative stress by neutrophils in Sprague-Dawley rat model of NNNMU-induced ALI. NNNMU had increased lung weight/body weight ratio (L/B ratio), lung myeloperoxidase (MPO) activity, protein content and number of neutrophils in bronchoalveolar fluid (BALF) compared with those of control rat (p<0.001, respectively). In contrast, the amount of pulmonary surfactant in BALF was decreased by NNNMU (p<0.001). Morphologically, light microscopic examination denoted pathological findings such as formation of hyaline membrane, infiltration of neutrophils and perivascular cuffing in the lungs of NNNMU-treated rats. In addition, ultrastructural changes such as the necrosis of endothelial cells, swelling and vacuolization of lamellar bodies of alveolar type II cells, and the degeneration of pulmonary surfactant were identified after treatment of NNNMU. Very interestingly, cerium chloride electron microscopic cytochemistry showed that NNNMU had increased the production of cerrous-peroxide granules in the lung, which signified the increased production of hydrogen peroxide in the lung. Collectively, we conclude that NNNMU causes acute lung leak by the mechanism of neutrophilic oxidative stress of the lung.
