A1 Study on the Possibility of Surface Electromyography as a Clinical Assessment Scale for Facial Nerve Palsy

  • Background: The aim of this study is to determine the correlation between clinical assessment scales, self-assessment scales, and surface electromyography (SEMG) for facial nerve palsy. Methods: This study assessed 32 cases of facial nerve palsy on the first visit, 11 cases on the second visit, and 9 cases on the third visit to the Korean medicine hospital, university. This study was conducted from October 22, 2022, to December 22, 2022. The patients were evaluated using SEMG, clinical assessment scales, and self-assessment scales 3 times. The House-Brackmann grading systems (HBGS), Yanagihara unweighted grading system (Y-score), facial disability index, numerous rating scale, and accompanying symptoms of facial nerve palsy were used for assessment. Moreover, statistical correlation was analyzed using Pearson correlation. Results: On Visit 1, Significant correlations were observed between the results of SEMG and other clinical assessment scales as well as between SEMG-F (frontalis) and different parts of the Y-score. On Visit 2, significant correlations were observed between the results of SEMG and HBGS as well as between SEMG-F and the detailed parts of the Y-score. On Visit 3, significant correlations were observed only between SEMG-F and the detailed parts of the Y-score. A significant correlation was also observed between the changes in the clinical assessment scales on Visits 1 and 3 and between the changes in SEMG-F and those in the patient self-assessment scales. Conclusion: These findings suggest that SEMG can be used to evaluate facial nerve palsy in conjunction with the use of other clinical assessment scales.

  • Objectives : This study was done for reporting the effect of bloodletting therapeutics on peripheral facial paralysis patients with back of the affected ear pain. Methods : We investigated 37 cases of peripheral facial paralysis patients with postauricular pain, and devided patients into two groups : We treated one group by complex oriental medical treatment with bloodletting therapeutics, and did the other group by complex oriental medical treatment without bloodletting therapeutics. Visual analogue scale(VAS) and gross grading system of House-Brackmann(H-B grade) and at baseline and final were used for evaluating the effect of the treatment. Results : 1. In bloodletting therapeutics group and non-bloodletting therapeutics group, compared with baseline, at final, VAS and H-B grade was significantly decreased. 2. At final, bloodletting therapeutics group showed significant decrease on VAS and H-B grade compared with non-bloodletting therapeutics group. Conclusions : Bloodletting therapeutics showed significant improvement in peripheral facial palsy with postauricular pain.

  • Objectives : The purpose of this study is to report a case of bilateral facial palsy in a patient with Guillain-Barre Syndrome after coronavirus disease 2019(COVID-19) vaccination. Methods : The patient diagnosed as Guillain-Barre Syndrome and Bilateral facial palsy after COVID-19 vaccination. We applied Korean medical treatment including acupuncture and herbal medicine for bilateral facial palsy. We used HBGS(House-Brackmann Grading System), VAS(Visual Analog Scale) to estimate the symptoms. Results : After the treatment, both facial palsy showed big improvement. Compared to the onset, HBGS improved by Grade 1/2 and VAS by 2. Conclusions : This case report shows effect of Korean medicine on Bilateral facial palsy and suggests a possibility of Korean medicine to treat adverse events following COVID-19 vaccination.

  • Objectives : The purpose of this study is to report a case of erythematotelangiectatic type rosacea and peripheral facial palsy improved by Korean medicine treatment with Soshioho-tang gagambang. Methods : The patient visited our clinic due to facial flushing and peripheral facial palsy symptoms. Based on Sanghallon provision, we treated this case with Soshiho-tang gagambang. The result of treatment was evaluated by Visual Analogue Scale(VAS), Dermatology Life Quality Index(DLQI) and House-Brackmann Grading System(HBGS). Results : After 12 days of taking Soshiho-tang gagambang, VAS of rosacea was decreased from 9 to 4 and DLQI was decreased from 22 to 5. After treatment, rosacea with facial flushing was improved and HBGS was reduced from 3 to 1. Conclusions : This study shows the possibility of taking Soshiho-tang gagambang for erythematotelangiectatic type rosacea and peripheral facial palsy derived from stress, fatigue and sleep disturbance according to Sanghallon provision.

발병초기 Bell's Palsy 환자의 안면부 피부전기활동성 차이에 관한 연구 (Differences Between Facial Electrodermal Activities of Paralyzed Side and Those of Normal Side in Acute Stage of Bell's Palsy Patients)

  • Background and purpose: Bell‘s Palsy is a condition that causes the facial muscles to weaken or become paralyzed. It's caused by trauma to the 7th cranial nerve, and is not permanent. The aim of this study is to be convinced of differences between facial electrodermal activities of paralyzed side and those of normal side in acute stage of Bell's Palsy patients Methods: Electrodermal activity (EDA) was performed within 1 week after the onset of facial palsy and facial nerve electromyography (EMG) at 2 weeks after the onset. The recovery of facial nerve function was documented by House and Brackmann grading. All the patients were followed up weekly until recovery or up to 6 weeks. Results: There was significant differences (conductivity A: t=3.319, p=0.002; conductivity C: t=2.699, p=0.010) between facial electrodermal conductivities of paralyzed side and those of normal side in acute stage of Bell's Palsy patients (N=45). And the result showed that logarithmic scale of electrodermal conductivity A value ratio obviousely decreased with logarithmic scale of EMG zygomatic branch amplitude ratio (r=-0.472, p=0.143); logarithmic scale of capacitance B, logarithmic scale of EMG temporal branch amplitude ratio (r=-0.422, p=0.133); logarithmic scale of conductivity C, logarithmic scale of EMG buccal branch amplitude ratio (r=-0.545, p=0.083) (N=12). Conclusion: Electrodermal conductivities increased in paralyzed facial side in acute stage of Bell's Palsy patients.

안면 성형침을 이용한 안면신경마비 치험 2례 (Clinical Case Study of Facial Nerve Palsy Using Facial Acupuncture)

  • Facail nerve palsy is a common disease in the face. This disease is treated by many methods. But it is not treated by Facial Acupunture. Therefore we performed to evaluate the effect of Facial Acupuncture with oriental dedical treatment in facail nerve palsy. This clinical study was carried out 2 caese of facail nerve palsy which had been admitted our hospital. In this study the patients were treated by Facail Acupuncture. Other treatment were herbal medication, regabilitative therapy. H-B scale(House-Brackmann facial nerve grading system) and Digital Infrared Thermal Imaging were measured from admission to discharge. After being treated by our methods, Patients' H-B scale and Inequality of Facial DITI were improved in cases compared to existing. In the study, The treatment of facial nerve palsy using Facial acupuncture was more effective, especially H-B scale, Digital Infrared Thermal Imaging and treatment period, compared to existing.

안면성형침을 이용한 안면신경마비 후유증의 치험례 보고 (The Treatment of Sequela of Facial Nerve Palsy Using Facial Rejuvenation Acupuncture : Case Report)

  • Objectives : This study was performed to evaluate the effect of facial rejuvenation acupuncture on cases of sequela of facial nerve palsy. Methods : This study was carried out in respect of 5 cases of sequela of facial nerve palsy patients which had been treated in Dong-Shin university oriental hospital from 2009 year with House-Brackmann grading system(HBGS) above Grade IV. In this study the patients were treated by facial rejuvenation acupuncture. The effect of the treatment were evaluated via scales and picture. Results : After treatment with facial rejuvenation acupuncture, 5 patients improved H-B scale and symptoms of the sequela of facial nerve palsy. The average of H-B scale was increased from 3.2 to 6.4. There also formed forehead folds, better blinking of the eyes and improved asymmetry of the left and right face. Conclusions : In the study, facial rejuvenation acupuncture showed marked improvement in sequela of facial nerve palsy. Randomized-controlled trial for further evaluation will be needed in the future.

  • Objectives : The purpose of this case is to report the improvement of two patients diagnosed with eight and a half syndrome after combination therapy of oriental and western medicine. Methods : We treated the patients with combination therapy of oriental and western medicine such as acupuncture, moxibustion, herbal medication, antithrombotic agents and steroid therapy. Changes of peripheral facial paralysis were evaluated using House-Brackmann facial grading system and the degree of dizziness, tenderness and pain of upper abdomen were assessed using numerous rating scale. Changes in motor grade of upper and lower extremities were evaluated using medical research council scale. Results : We have recently experienced two cases of eight and a half syndrome - a syndrome characterized by the coexistence of one and a half syndrome, a rare ophthalmoparetic syndrome characterized by a conjugate horizontal gaze palsy in one direction and an internuclear ophthalmoplegia in the other, and cranial nerve VII palsy. The one and a half syndrome was caused by acute cerebral infarction. Two patients in this report were improved through combination therapy of oriental and western medicine. Conclusions : We report the clinical course and treatment methods of eight and a half syndrome.

Comparative Study of Multimodal Therapy in Facial Palsy Patients

  • Introduction In chronic facial palsy, synkinetic muscle overactivity and shortening causes muscle stiffness resulting in reduced movement and functional activity. This article studies the role of multimodal therapy in improving outcomes. Methods Seventy-five facial palsy patients completed facial rehabilitation before being successfully discharged by the facial therapy team. The cohort was divided into four subgroups depending on the time of initial attendance post-onset. The requirement for facial therapy, chemodenervation, or surgery was assessed with East Grinstead Grade of Stiffness (EGGS). Outcomes were measured using the Facial Grading Scale (FGS), Facial Disability Index, House-Brackmann scores, and the Facial Clinimetric Evaluation scale. Results FGS composite scores significantly improved posttherapy (mean-standard deviation, 60.13 ± 23.24 vs. 79.9 ± 13.01; confidence interval, -24.51 to -14.66, p < 0.0001). Analysis of FGS subsets showed that synkinesis also reduced significantly (p < 0.0001). Increasingly, late clinical presentations were associated with patients requiring longer durations of chemodenervation treatment (p < 0.01), more chemodenervation episodes (p < 0.01), increased doses of botulinum toxin (p < 0.001), and having higher EGGS score (p < 0.001). Conclusions This study shows that multimodal facial rehabilitation in the management of facial palsy is effective, even in patients with chronically neglected synkinesis. In terms of the latency periods between facial palsy onset and treatment initiation, patients presenting later than 2 years were still responsive to multimodal treatment albeit to a lesser extent, which we postulate is due to increasing muscle contracture within their facial muscles.

베게너 육아종으로 진단된 좌안면신경마비 환자에 대한 한의복합치료 1례 (A Case Report of Korean Medicine Treatment for a Patient with Left Facial Palsy Diagnosed with Wegener's Granulomatosis)

  • Background: The study reports the effect of Korean medicine treatment on a patient with left facial palsy caused by Wegener's granulomatosis. Case information: The patient was receiving steroid treatment for facial palsy and was hospitalized for 21 days. He received acupuncture, electroacupuncture, bee venom pharmacopuncture, and moxibustion treatment along with Ssanghwa-tang and Cheongansohabwon herbal medicine treatment. During hospitalization, the Sunnybrook facial grading scale, House-Brackmann scale and length of time for the eye to completely close were evaluated at 11/16, 11/21, 11/27, 12/1, and 12/6. Post treatment, the degree of the movement in the left eyelid was improved, it took less time for the eye to close completely, the movement of the forehead wrinkle muscles improved, but that of the facial muscles under the eyes and left orbicularis oculi muscle showed little improvement. Conclusion: Korean medicine treatment for facial palsy caused by Wegener's Granulomatosis helps restore mobility of the forehead and orbicularis oculi muscles.
