Two Cases of Psoriasis treated with Sasang Constitutional Therapy and Fasting Therapy (사상처방과 단식요법을 활용한 건선(乾癬) 치험 2례)

  • 1. Objectives The purpose of this study was to give an account of the effects of Sasang constitutional medicine and fasting therapy for the patient who has suffered from various signs or symptoms caused by psoriasis. 2. Methods The cases were one 50-year old man diagnosed as Taeeumin and one 29-year old man diagnosed as Soyangin. They all have the symptoms caused by psoriasis, such as rash, erythema, scaly skin, itching, etc. So we've recorded the progress of their symptoms as a score at the tables to check their conditions throughout the hospitalized period. We've diagnosed them as Soyangin, Taeeumin have prescribed Yangdokbaecho-tang and Galgeunhaegi-tang and have carried out fasting therapy. 3. Results and Conclusions The general condition of the patient have improved incredibly, where the each scores at the tables dropped from 4 to 0,1 and their symptoms like rash, erythema, scaly skin, itching, edema have almost disappeared at the end of the hospitalization period. This indicates that Yangdokbaecho-tang and Galgeunhaegi-tang can be used to treat main symptoms of psoriasis and fasting therapy can be used as a subsidiary therapy for the psoriasis.

Study on Characteristics of Acute Stroke Patient who took Chungpyesagan-tang (청폐사간탕 복용한 급성기 중풍 환자에 대한 제반 특성)

  • Objective : The aim of this study was to examine the characteristics of the acute stroke patient who took Chungpyesagan-tang, and provide the basis of Chungpyesagan-tang prescription. Method : We studied hospitalized patients within 4 weeks after their occur who were admitted at Kyunghee University Oriental Medical Center, Kyunghee University East-West Neo Medical Center, Kyungwon University Oriental Medical Center, Semyung University Oriental Medical Center from February 2010 to July 2010. We compared the general characteristics of acute stroke patient with herbal medicine. Result : The patient who took Chungpyesagan-tang showed significant difference for age, family history of hypertension, ALT, coffee drinking, fastfood eating, stool, Oriental Medical Diagnosis. Conclusion : The above result show that Chungpyesagan-tang can be prescribed to stroke patient whose stool is hard, whose Oriental Medical Diagnosis is fever type. Further studies will be needed to better understand the difference between Chungpyesagan-tang group and Other herbal medicine among acute stroke patients.

  • In acquired immunodeficiency syndrome (AIDS) patients, immune reconstitution inflammatory syndrome (IRIS) due to Mycobacterium avium complex (MAC) infection is one of the most difficult IRIS types to manage. We report an unusual case of MAC-associated IRIS. At first the patient was diagnosed human immunodeficiency virus (HIV) infection after he was admitted with pneumocystis pneumonia. After starting antiretroviral therapy he presented unmasked IRIS with MAC infection. Next, he was hospitalized with continuous loose stools and new-onset fever. Investigation included computed tomography (CT), which showed homogeneous enhancement and enlargement of the lymph nodes (LN), elevation of ferritin (>1,650 ng/mL) and lactate dehydrogenase (306 IU/L) levels, and F- fluorodeoxyglucose positron emission tomography/computed tomography (FDG PET/CT) scan, which showed increased FDG uptake. These findings were highly indicative of lymphoma. We performed laparoscopic biopsy of the mesenteric LN, and the biopsy culture grew MAC. So we made a diagnosis of MAC-associated. Therefore, IRIS must be considered as a possible diagnosis when AIDS patients develop new symptoms or exhibit exacerbations of existing symptoms. Furthermore the biopsies should be conducted.

Factors Affecting Emotional Status and Suicidal Risk in Hospitalized Medical Patients (일 종합병원 입원 환자의 자살 위험도 및 정서 상태에 영향을 미치는 요인 연구)

  • Objectives : This study aimed to identify the group with high risk of suicide in general hospital inpatient by comparing the depression, anxiety, and distress and their clinical characteristics. Methods : The study included all patients admitted to a general hospital between January 2016 and December 2017, and the final 41,249 patients were analyzed. Distress thermometer (DT) and hospital anxiety-depression scale (HADS) were used for emotional state evaluation. Group with high risk of suicide was defined as showing more than 4 in the DT and more than 8 in one of the two subscales (anxiety and depression) of the HADS. Results : Among the subjects, 3,603 patients (8.7%) met the high-risk criteria for suicide. The group with high risk of suicide experienced more severe pain than the control patient group, and it was found that there was more underlying disease. Patients who reported pain showed higher scores than those who reported no pain on the DT and HADS subscales. Conclusions : Group with high risk of suicide showed more severe pain and more underlying disease. Therefore, suicide prevention program specifically targeted for the group with high risk of suicide should be developed according to the risk of the patient evaluated at the time of the admission.

A Review of Cardioembolic Stroke Patients Hospitalized in Hospital of Korean Medicine (한방병원에 입원한 심인성 뇌색전증 환자에 대한 고찰)

  • ■ Objectives Atrial fibrillation is the most common cause of cardioembolic stroke. Of the 44 ischemic stroke patients with atrial fibrillation who were hospitalized in hospital of Korean Medicine from July 1, 2014 to June 30, 2017, we selected 39 patients who have had Magnetic Resonance Angiography. We divided them into Atrial Fibrillation group with no stenosis or less than 50% stenosis in the ipsilateral artery of the lesion and Artery to Artery Embolism group with more than 50% stenosis or occlusion in the ipsilateral artery of the lesion. ■ Methods Clinical characteristics, examination and evaluation tools were collected from the patient's electronic medical records. CHADS2, Initial National Institutes of Health Stroke Scale, 8-item Stroke Scale and Improved 8-item Stroke Scale Number were checked. ■ Results & Atrial Fibrillation group showed differences in age, brain lesion location, vascular lesion, Conclusion initial National Institutes of Health Stroke Scale, initial 8-item Stroke Scale and progress compared to Artery to Artery Embolism group.

  • Objective : The aim of this study was to examine the characteristics of the acute stroke patient who take Gwakhyangjeonggisan, and provide the basis of Gwakhyangjeonggisan prescription Method : We studied hospitalized patients within 4 weeks after their ictus who were admitted at Kyunghee University Oriental Medical Center, Kyunghee University East-West Neo Medical Center, Kyungwon University Oriental Medical Center, Semyung University Oriental Medical Center from February 2010 to July 2010 We compared the general characteristics of acute stroke patient according to herb medicine Result : The patient who take Gwakhyangjeonggisan show significant difference for sex, family history of Stroke, Face color, Tongue color, Pulse condition, HDL Cholesterol, Fastfood eating, Waist circumference. Conclusion : The above result show that Gwakhyangjeonggisan can be prescribed to stroke patient whose complaining gastrointestinal symptoms, & Oriental Medical Diagnosis is Cold & Deficiency type. Further studies will be needed to better understand the difference between Gwakhyangjeonggisan group and Other herb medicine among acute stroke patients.

  • On December 31, 2018, an incident occurred where a doctor was attacked and killed by a patient carrying a lethal weapon in the outpatients' clinic of the psychiatric department of a tertiary general hospital. The suspect was diagnosed with bipolar affective disorder (manic depressive disorder) and has been hospitalized and cared for in the psychiatric ward of this hospital. This incident illustrates the necessity of more active cures and therapeutic intervention for mental patients with intellectual developmental disorders who require treatment considering the fact that a radical outcome has been caused by such a patient. However, on the other hand, there is also a need for an approach and analysis from the perspective of crime prevention for all medical departments. The reason for this is that even a tertiary general hospital equipped with the largest human resources, medical devices, facilities, and so forth, is susceptible to violence. As for illegal actions perpetrated against health and medical service personnel in medical institutions, such as verbal abuse, assault, injury, etc. there have neither been understanding shown for the current extent of damage in detail, nor discussions of active institutional improvement related to the seriousness of the act. It can be said that violence in the field of medical treatment is a realm requiring serious discussion and appropriate remedial actions. This is because when such incidents take place, if a patient who is supposed to get treatment from the damaged health care provider is in an urgent situation or on the waiting list of serious cases, he or she could suffer serious damage caused by deprivation of treatment opportunity, or secondary damage might be caused to the patient and/or a guardian who can hardly have an opportunity to take action. Accordingly, in this review, we would like to help create the necessary conditions for both health and medical service personnel and patients/guardians, respectively, to provide and receive medical treatment in a more secure environment. Therefore, objective assessment of the institution and issues relating to this aforementioned incident and general cases of violence occurring in medical institutions, and by suggesting legal and institutional improvements and solutions.

  • Objectives: This study reports on the efficacy of using Korean medical treatment with suspected Meniere's Disease. Methods: We treated a patient with suspected Meniere's disease using Korean medical treatments (herbal medicines, acupuncture, herbal acupuncture therapy). To evaluate the results, we used the visual analogue scale (VAS) to measure dizziness, tinnitus, ear fullness, and headache. The degree of tinnitus was also evaluated using the Korean Tinnitus Handicap Inventory (K-THI). The generic health status was evaluated using the European Quality of Life-5 Dimensions (EQ-5D) scale. Results: Improvements in the VAS and EQ-5D scale scores were observed after the treatments. The change in the K-THI scores showed that the patient's tinnitus handicap was relieved. Conclusions: The results of this case study suggest that Korean medicine therapies may be effective in the treatment of suspected Meniere's Disease.

A Case Report on Korean Medical Treatment for a Patient Hospitalized with Acute Compression Fracture (한방병원에 입원한 압박 골절 환자에 대한 한방 치료 1례)

  • Objectives: This study aimed to determine the effects of Korean medical treatment on a patient with compression fractures. Methods: The patient was treated with herbal medicine and acupuncture for 51 days. Symptom changes were measured using the Numerical Rating Scale (NRS), Range of Motion (ROM), Oswestry Disability Index (ODI), and European Quality of Life Five Dimensions (EQ-5D) Scale. Results: The NRS score for lower back pain decreased from 7 to 3, and the ODI score decreased from 51.11 to 22.22. Range of motion (ROM) increased meaningfully in flexion(from 30 to 60), Left/Right lateral bending(from 10/30 to 30/45), Left/Right rotation (from 10/30 to 30/45), and the EQ-5D score also increased from 0.506 to 0.677. Conclusion: The results indicate that Korean medical treatment may be effective for managing patients' acute compression fractures.

  • Objectives: This study investigated the efficacy of Korean medical treatment for a headache (suspected CPSP) patient with a history of cerebral infarction. Methods: We treated the patient using Korean medical treatments (herbal medicines-Kamiseokyong-tang, acupuncture, pharmacopuncture, and chuna). The treatments were measured using the Numerical Rating Scale (NRS) and the Neck Disability Index (NDI) for headache and post neck pain. General health status was evaluated using the European Quality of Life Five Dimensions (EQ-5d) Scale. Results: We observed improvements in the NRS, NDI, and EQ-5d scores after the treatments. Conclusions: This study suggests that Korean medicine may effectively treat headaches (suspected CPSP) with accompanying cerebral infarction.
