A Design of Cooperation Coordinator using Band-Cloud

  • The Internet of Things(IoT) market is expected to grow from 15.5billion to 75.4 billion by 2015-2025. As the number of IoT devices increases, the amount of data that is sent to the cloud is increasing. Today's Cloud Computing models are not suited to handle the vast amount of data generated by IoT devices. In this paper, we propose a Cooperation Coordinator System that reduces server load and improved real-time processing capability under specific circumstances by using Band-Cloud. The cooperation coordinator system dynamically forms the cloud when cooperation is needed between mobile devices located near. It is called Band-Cloud. Band-Cloud provides services entrusted by Central Cloud. This paper describes the proposed system and shows the cooperation process using the Android-based mobile devices and Wi-Fi Direct technology. Such a system can be applied to cases where real-time processing is required in a narrow area such as a hospital ward or a school classroom.

유방암 환자의 화학요법 치료시기에 따른 체중변화 (Weight Changes according to the Period of Chemotherapy Treatments for Breast Cancer)

  • Purpose: This study aimed to evaluate weight changes in women during the period of TAC (docetaxel-doxorubicin-cyclophosphamide) chemotherapy after breast surgery and the differences in weight changes by disease characteristics among Korean women with breast cancer. Methods: The design of this study was retrospective survey research. The subjects of this study were 130 patients with fully completed TAC chemotherapy between January 2012 and April 2015. Body weight before their operation, and at 3 weeks, 6 weeks, 9 weeks, 12 weeks, 15 weeks, and 18 weeks after surgery, as well as general and disease characteristics, were reviewed via their medical charts. Results: The mean age of the patients was 51.2 years. The preoperative (baseline) mean weight was $59.4{\pm}8.44$. Significant weight losses were observed in the initial (1st 3 week) period of TAC chemotherapy compared to the baseline and significant weight losses were observed with the exception of the period 6 weeks after breast surgery. There were significant differences in mean weight changes according to the stage of breast cancer, radiotherapy, and hormone therapy among disease characteristics. Conclusion: It is necessary to study weight changes after breast surgery for the long periods of TAC chemotherapy. Through the accumulated results of studies, nursing programs for breast cancer patients must be developed for the prevention of weight gain in the periods of TAC chemotherapy.

정신과 환자의 치료적 활동에 관한 문헌적 고찰 (Literary Review of Therapeutic Activity of Psychiatric Patients)

  • More and Tore emphasis is put on moral treatment in the care of psychiatric patients. Methods employed for moral treatment rake use of therapeutic activity for the mentally ill patient. In some psychiatric hospitals all of the therapeutic activities are categorized as occupational therapy, while in others these activities are departmentally categorized -recreational, musical, occupational and bibliotherapy. All of these constitute separate departmental therapies. By research, it is known at this time, that the above activities have been of great help in personality integration, reeducation and early recovery of patients, as well as in their social adjustment after discharge. The goal of these therapeutic activities incorporate the goals set by healthier interpersonal relationship and psychotherapy. Put the therapeutic activities require a prolonged time of therapy for effectiveness, economic support, special trained personnel, careful planning, guidance and proper evaluation. Therefore this treatment is a very complicated one. But it may be the most profitable therapy for certain patients. One true fact about this treatment is, that simple recreation, occupation, music, reading, etc., make the patients life in the ward more interesting and have a good effect on the interpersonal relationship between patients. Thus it has been proved that this kind of treatment demands the active and full participation of the personnel and the full support of the hospital administration.

간호사의 이미지와 호감도 분석 (Analysis of the Image and Likability of the Nurse)

  • Purpose: The purpose of this study was to investigate nurse images and likability that were perceived by various groups in the general population. Methods: Using cross-sectional descriptive design, six groups of people were invited to participate in the survey. The participants included nurses, physicians, nurse aids/technicians, patients/caregivers, college students, and the general public. A total of 1,530 individuals were drawn from 42 hospitals nationwide. Bhargava's likability questionnaires and the Ward Nurse Image tool were used. Results: Compared with other groups, patients and caregivers had higher scores for likability and better nurse image than other groups. Most respondents perceived nurses are clean and caring, but many individuals still had the image that nurses are submissive. Conclusion: To improve nurse image, perceptions from various groups of people need to be considered and programs develop to show the positive aspects of nursing and nurses.

노인 뇌졸중환자의 수분섭취교육 프로그램의 적용 효과 (Effects of Fluid Therapy Education Program for Aged Stroke Patients)

  • Purpose: This study was to verify the effects of a fluid therapy education program for aged stroke patients for preventing dehydration, pyuria and bacteriuria. Methods: The study employed a non-equivalent control group pretest-posttest design in quasi experimental basis. Subjects were 38 stroke patients (20 in the experimental group and 18 in the control group) older than 65, hospitalized in a senior care center in S city, Korea. Results: The amount of daily average fluid intake, normal ratio of blood urea nitrogen-creatinine ratio, serum $Na^+$ and urine white blood cells were increased significantly in the experimental group. Normal ratio of bacteriuria increased in the experimental group, but not significantly. Conclusion: The education program is considered to be an effective nursing intervention tool for preventing dehydration and urinary tract infections which related to the secondary wellness of aged stroke patients. However, longer term study is necessary for better quality of nursing and developing more specific education programs for aged stroke patients usually hospitalized for a long period of time.

신규간호사의 재직의도 영향요인 (Affecting Factors of New Nurse's Intention to Retention in Hospitals)

  • Purpose: The purpose of this study is to investigate relationship among intention to retention, role conflicts, psychosocial job conditions and job satisfaction of new nurses, and identify affecting factors for intention to retention. Methods: Data were collected from 136 new nurses who were graduated at 2016 and working now in general hospitals by self-reported questionnaire. The relationship among variables were analyzed with Pearson's coefficient correlation and affecting factors for intention to retention were identified by using multiple linear regression analysis. Results: The mean score of intention to retention was $2.63{\pm}0.28$. Intention to retention was significantly different by the working ward, hope period for work, and reason for retention. Intention to retention had positive relationships with work organization and job contents, and job satisfaction, but negative relationships with frequency of role conflict, demands at work. Job satisfaction and meaning of work identified as influencing factors inn intention to retention. Conclusion: It is needed a positive hospital environment to improve adaptation in clinical nursing practice and to emphasize professionalism including value of nursing and meaning of nurses' work for new nurses.

경락마사지가 편마비 환자의 손부종과 손기능에 미치는 효과 (The Effect of the Meridian Massage on the Hand Edema and Function of the Hemiplegic Patient)

  • Purpose: The objective of the study was to identify the effects of the meridian massage therapy on hand edema and hand functions in patients with hemiplegia. Method: The experiment was conducted in the Oriental Medical ward of "K" hospital during the period of 2000. 12. 15 - 2001. 03. 15. Fifty five subjects with hemiplegia following a stroke participated in the study. Volumetric size of the hand and the circumference of the index finger of each patient was measured and functional points were recorded for pre and post experiment. Result: 1. Apparent decrease in volumetric size of the hand and the circumference of the index finger(p=.022. p=.021), and higher functional points(p=.003) in the experimental group in comparison to the control group. 2.Volume of hands & their functions appeared to be irrelevant(r=-.195, p=.154). 3. Circumference of the index finger & their functions also turned out to be irrelevant (r=-.110, p=.424). Conclusion: Meridian massage is an effective nursing interventional therapy to relieve hand edema and improve hand functions in hemiplegic patients and has great potential for use in a wide range of medical fields as an efficient supplementary treatment for stroke rehabilitation.

가족구성원의 입원으로 인한 가족의 스트레스와 대처방안에 관한 연구 (A study on perception of stress and coping patterns of family members of the hospitalized in patients)

  • The purpose of this study was to assess the degree of stress and patterns of coping for that Stressful events on family members because of the hospitalization of the patients. Stress and coping were measured with a tool on the basis of Volicer and Bell's questionnaire. In data collection, the modified 38 items of Volicer's stress scale and Bell's 18 item coping scale were administered. The subjects consisted of 259 family members of general ward-patients in Seoul National University Hospital during April in 1990. They were randomly selected on the basis of relationship of patients; patient's spouse, patient's daughters or sons. The stressors of the family members were ranked as follows; The first rank Stressful events was related to the patient's diseases and pain, the second ones was related ·to caring of their patients and family's psychosocial life. Families used long term coping method significantly more than short term ones. The results indicated that there was no difference in use of coping method between pre and post hospitalization. Finding out more about situation and optimism were the most common coping methods, and the least frequent coping method was the use of drugs. In conclusion, the identification of perceived stress and coping patterns of family members provides useful information for family nursing and aimes at better nursing care for the hospitalized in patients.

입원 환자의 욕창예방과 중재를 위한 알고리즘 개발 (Development of an Algorithm for the Prevention and Management of Pressure Ulcers)

  • Purpose: The purpose of this study was to develop an algorithm for preventing and managing of pressure ulcer and to verify the its appropriateness. Methods: The first step was development of a pre-algorithm through a literature review and expert opinion. The second step was to establish content validity by submitting the algorithm questionnaires about the content to 12 experts. The third step was the revision of the algorithm. The fourth and last step was to establish the clinical validity of the algorithm with 25 experienced nurses. Results: For the ease of the practitioner the algorithm for prevention and the management of pressure ulcers was confined to one page depicting the main algorithm pathway and seven stepwise guidelines. The guidelines included skin care of pressure ulcer prevention, mechanical loading care, support surface care, reposition care of pressure ulcer, and Stages II, III and IV explanations along with debridement/wound irrigation and infection control. Most of all algorithm courses chosen more than 80% of agreement by expert index of content validity. The usefulness, appropriateness, and convenience of the algorithm were demonstrated through clinical validity with intensive care unit and ward nurses. Conclusion: The algorithm will improve the quality of pressure ulcer nursing care as it provides a model for decision making for clinical nurses as well as providing consistent and integrated nursing care for patients with pressure ulcer throughout an institution.

Follow-Up Study of Survival of Patients with Advanced Cancer in a Hospice Setting

  • Objective: This study was to present the survival of advanced cancer patients and explore the influence of various factors on survival time as well as survival rate. The results provide guidelines for clinical practice of cancer treatment. Methods: Follow-up of 674 advanced cancer patients was performed in a hospice. The median survival time and survival rate were calculated, and survival analysis was carried out. Results: The median survival time of all patients dying from cancer was 12.0 months and the average survival time was 25.1 months. The 1-year cumulative survival rate was $0.518{\pm}0.020$ and the 5-year cumulative survival rate was $0.088{\pm}0.012$. The following factors showed significant impacts on survival rate: gender, age, primary diagnosis, surgery and the time when pain appeared. Conclusions: The survival time of patients with advanced cancer was relatively short. Major approaches to extend the survival time include early detection, early diagnosis, effective surgical treatment, pain control, reasonable supply of nutrients and multiple interventions.
