종합병원에 근무하는 요통 간호사의 간호업무활동, 신체역학원리 및 직무 스트레스와의 관련성 (Relationships among Nursing Activities, the Use of Body Mechanics, and Job Stress in Nurses with Low Back Pain)

  • Purpose: This study was conducted to identify relationships among nursing activities, the use of body mechanics, and job stress in nurses with low back pain. Methods: Participants were 225 nurses with low back pain working at an university hospital. The data were collected with self-reported questionnaires at March, 2012 and analyzed by descriptive statistics, t test, ANOVA, Scheff$\acute{e}$e test and multiple regression analyses. Results: There was significantly a higher degree of pain in subjects who working at special units including intensive care units and emergency room than at general wards. Low back pain was negatively correlated with the use of body mechanical principles while it was positively associated with the degree of job stress. According to results of multiple regression analyses, low back pain was significantly associated with the use of body mechanics and job stress in nurses working at general ward and special units including intensive care units and emergency room. Conclusion: The use of the principles of body mechanics and reducing job stress are important to prevent low back pain in nurses. It is necessary to develop and apply stress management and education program about the use of the principles of body mechanics.

혈액종양병동 간호사의 소진에 영향을 미치는 요인 (Factors Associated with Burnout for Nurses Working in Hematology-oncology Wards)

  • Purpose: The purpose of this study was to examine factors related to burnout of nurses at hemato-oncology wards. Methods: The participants were 134 nurses working at hematology-oncology wards in three tertiary general hospitals in Seoul and Gyeonggi Province. Data were collected using a structured questionnaire that consisted of personal characteristics, occupational stress, satisfaction and burnout. Using a SPSS/WIN 12.0 program, descriptive statistics, t-test, ANOVA, and stepwise multiple regression procedures were employed. Results: The mean score of burnout reported by the participants was $3.15{\pm}0.50$ (range 1~5), showing the highest scores in the area of physical burnout ($3.53{\pm}0.57$). The satisfaction with nursing profession was found to be the most impact on the burnout of the hematology-oncology ward nurses (${\beta}$=-.533), followed by occupational stress (${\beta}$=.351) and years of clinical experiences (${\beta}$=-.154). Conclusion: Given the findings, the sorts of strategies to increase the satisfaction with nursing profession and decrease their occupational stress are required, particularly for newly employed nurses.

간호사의 SBAR 환자보고가 의사소통의 명확성과 간호사와 의사의 협력관계에 미치는 영향: 단일군 전후실험 설계 (The Effect of SBAR Reports on Communication Clarity and Nurse-Physician Collaborative Relationships: A One Group Pretest-Posttest Design)

  • Purpose: The purpose of this study was to investigate the effect of Situation-Background-Assessment-Recommendation (SBAR) reports on communication clarity and nurse-physician collaborative relationships. Methods: SBAR forms in one group were measured at baseline, at 2 weeks, and at 4 weeks postintervention. The subjects were 30 nurses who worked in a general ward of a general hospital in B city, Korea. A 45-minute SBAR education was provided to all subjects. Data was collected between September to October 2015. The collected data was analyzed using a repeated measure ANOVA. Results: After SBAR reports, levels of nurse's communication clarity and nurse-physician collaborative relationships were significantly higher than levels before SBAR reports. Conclusion: SBAR reports improved the communication clarity and nurse-physician collaborative relationships. It is recommended to use a lot of SBAR in clinical practice.

암환아 어머니의 돌봄 적응에 관한 일상생활기술적 연구 (Ethnographic Research on Adjustment of Mothers Caring for their Cancer Children in Korea)

  • Purpose: The purpose of this study is to provide the basic data to develop the effective nursing intervention for the parent who have children with cancer by acquiring the deeper understanding of the mothers' adjustment of caring for their children with cancer. Methods: The ethnographic research method was used to find out the pattern of caring adjustment in Korean cultural context. Informants consisted of 12 mothers who were caring for their children with cancer. The data were collected using in-depth interviews, participant observation, and telephone interviews by maximum variation purposive sampling. The data were analyzed following Spradley's methodology. Results: The mothers' caring adjustment were organized into one cultural theme, four categories, and twelve properties. The cultural theme was 'standing alone as a mother with sin'. The four categories were 'blaming for falling illness', 'overcoming with motherhood', 'desperate struggling with side effects', and 'establishing new network as a dependent'. Conclusion: For the mothers who are caring children with cancer, the supportive nursing intervention based on the deeper understanding of mothers' pattern of caring adjustment for their children and centered on facilitating effective adjustment in each cultural context especially from the very early stage of caring in the hospital ward is extremely required.

일반병동간호사의 사회심리적 스트레스, 이직의도와 간호 업무수행 (A Study of Psychosocial distress, Intention to Quit and Nursing Performance in General Ward Nurses)

  • Purpose: The purpose of this study was to examine the relationship of psychosocial distress, intention to quit and nursing performance. Methods: The data were collected through structured questionnaires from 210 registered nurses in a general hospital. They were analyzed by descriptive statistics, t-test, ANOVA, scheffe test, Pearson's correlation coefficient and multiple regression with the SPSS WIN program. Results: The results of the analysis showed that the mean of the psychosocial distress was $25.38{\pm}7.26$, intention to quit was $3.51{\pm}0.78$, and nursing performance was $3.67{\pm}0.46$. In the correlation analysis, the nursing performance had negative correlation with psychosocial distress(r=-.371, p=.000) and intention to quit(r=-.211, p=.002). There were statistically significant differences in nursing performance depending on age, marital status, position and work experience. The psychosocial distress and age explained 15.1% of nursing performance. Conclusion: This study showed psychosocial distress and intention to quit affects the nursing performance. Therefore, nursing executives and unit managers need to concern on the significance of the stress management programs so that these can be organizational support.

임상간호사의 일터영성과 감성지능이 조직시민행동에 미치는 영향 (The Influences of Workplace Spirituality and Emotional Intelligence on the Organizational Citizenship Behavior for Clinical Nurses)

  • Objectives : This study was done to identify the influences of workplace spirituality and emotional intelligence on the organizational citizenship behavior of clinical nurses. Methods : The subjects were 230 clinical nurses, working over 6 months in four general hospitals in three cities. Data were obtained from March 20 to March 30, 2016, with a self-report questionnaire, and analyzed using the IBM SPSS 21.0 program. Results : The factors related to the organizational citizenship behavior of nurses were workplace spirituality(${\beta}$=.36), emotional intelligence(${\beta}$=.29), 40 years of age or higher(${\beta}$=.26), working in a special ward(${\beta}$=.11), and turnover intention(${\beta}$=.10). These variables explained 61.6% of the variance in organizational citizenship behavior of nurses. Conclusions : In order to improve the quality of nursing services and business performance in hospital organizations, it is necessary to enhance workplace spirituality and emotional intelligence. This would lead to promoting organizational citizenship behavior through the development and application of intervention programs from a nursing organization perspective.

웃음치료가 구안와사환자의 안면마비 회복정도, 통증 및 스트레스에 미치는 효과 (Effects of Laughter Therapy on Recovery Status from Facial Paralysis, Pain and Stress in Bell's palsy Patients)

  • Purpose: This study was done to identify the effects of Laughter therapy on recovery status from facial paralysis, pain, and stress in Bell's palsy patients. Methods: This research was a nonequivalent control group non-synchronized pre-posttest quasi-experimental research design. Data were collected from August 8, to October 15, 2013 at D university Oriental Medical Center in B city. Participants were 60 patients with Bell's palsy who were assigned to one of two groups: an experimental group of 30 patients and a control group of 30 patients. t-test, $x^2-test$, Fisher's exact test and Welth-Aspin test were used to test the homogeneity between the two groups and t-test, Welth-Aspin test and ANCOVA were used to test the research hypotheses. SPSS program was used for statistical analysis. Results: There were significant increases in recovery status from facial paralysis and decreases in stress after Laughter therapy in the experimental group. Conclusion: The results indicate that Laughter therapy is effective in facilitating recovery status from facial paralysis and decreasing stress. Therefore, Laughter therapy can be used as an effective nursing intervention for patients with facial paralysis and to relieve stress in these patients.

간호사-의사 협력, 직무자율성과 조직몰입의 관계 (The Relationship among Nurse-Doctor Collaboration, Job Autonomy and Organizational Commitment)

  • Purpose: This study aimed to investigate and analyze the state of the relationship among nurse-doctor collaboration, job autonomy and organizational commitment. Method: The 304 participants were obtained who were working at a General ward, Intensive care unit and Operation room in three university hospitals located in Seoul and Kyunggi-do. The data were collected using a structured questionnaire from March 2d to April 10th, 2009. The collected data were analyzed with t-test, ANOVA, Scheff$\acute{e}$ test and Pearson's correlation on SPSS Win 16.0. Result: There was a significant relationship among nurse-doctor collaboration, job autonomy and organizational commitment. The level of appointment, clinical experience and current hospital experience of nurses affected significantly nurse-doctor collaboration, job autonomy and organizational commitment. The age of nurse had the relation nurse-doctor collaboration and organizational commitment. The relationship between the nurse's working area and job autonomy had positive correlation. Conclusions: The findings of study suggest that the program enhancing the collaborated relationship between nurses and doctors is important to improve nurse's job autonomy and organizational commitment under the situation of citizen's demanding more advanced medical service.

Comparison of Psychotropic Prescriptions between Oncology and Cardiology Inpatients: Result from a Pharmacy Database in a Teaching Hospital in Malaysia

  • Objective: To examine the prescription rates in cancer patients of three common psychotropic drugs: anxiolytic/hypnotic, antidepressant and antipsychotic. Materials and Methods: In this retrospective cohort study, data were extracted from the pharmacy database of University Malaya Medical Center (UMMC) responsible for dispensing records of patients stored in the pharmacy's Medication Management and Use System (Ascribe). We analyzed the use of psychotropics in patients from the oncology ward and cardiology from 2008 to 2012. Odds ratios (ORs) were adjusted for age, gender and ethnicity. Results: A total of 3,345 oncology patients and 8,980 cardiology patients were included. Oncology patients were significantly more often prescribed psychotropic drugs (adjusted OR: anxiolytic/hypnotic=5.55 (CI: 4.64-6.63); antidepressants=6.08 (CI: 4.83-7.64) and antipsychotics=5.41 (CI: 4.17-7.02). Non-Malay female cancer patients were at significantly higher risk of anxiolytic/hypnotic use. Conclusions: Psychotropic drugs prescription is common in cancer patients. Anxiolytic/hypnotic prescription rates are significantly higher in non-Malay female patients in Malaysia.

상급 종합병원 간호사의 직무만족도와 조직몰입이 환자안전관리 활동에 미치는 영향 (Impact of Nurses' Job Satisfaction and Organizational Commitment on Patient Safety Management Activities in Tertiary Hospitals)

  • Purpose: The purpose of this study was to examine the impact of job satisfaction and organizational commitment on patient safety management activities of nurses. Methods: A cross-sectional design was adopted. The sample included 307 nurses working in three tertiary university hospitals. Data were collected during November and December, 2012, with a response rate of 93%. A 40-item patient safety management activities scale was used to measure patient safety activities. Job satisfaction and organizational commitment were also measured with established instruments and included general characteristics. Data were analyzed with PASW 18.0 program. Results: The mean score for patient safety management activities was 3.98 out of 5. Stepwise multiple linear regression indicated that organizational commitment (${\beta}$=.20, p<.001) and job satisfaction (${\beta}$=.18, p=.013) respectively predicted patient safety management activities. Especially, the addition of organizational commitment as the strongest predictor of patient safety management activities increased the associated explained variance ($R^2$ change=.11, p<.001). Conclusion: It is important for managers to create an organizational climate that promotes the organizational commitment and job satisfaction of nurses. Future studies are encouraged to replicate the findings and also to better understand patient safety management activities and personal emotional stability.
