의료기관조제실제제의 현황분석을 통한 개선방안에 관한 연구 (Analysis and Strategy to Improve Hospital on-site Preparations)

  • Hospital on-site preparations (HOP) have long been produced in most institutions since enactment of the Ministry of Health and Welfare Notification No. 92-12 in 1992. However, due to the unclear definition of the HOP, most institutions have trouble in production and reporting of the HOP. Therefore, in this study, the definition and range of HOP are examined by questionnaire for 34 institutions, mainly teaching hospitals, which reported the items of HOP among 84 institutions having 500 or more beds. It was found that 472 items and 1,116,888 jesu of the HOP were produced at 34 institutions in 2006. Most institutions prepared $21{\sim}30$ items of HOP, and the number of institutions producing $5,000{\sim}10,000$ forms was most prevalent. Accordingly, for HOPs whose need was identified by this study, pharmaceutical companies should be pressed for commercialization.

토끼 실험적 골 결손부에 이식한 이종 이식골편의 처리방법에 따른 신생골 형성능력 비교 (Comparison of Efficacy of New Bone Formation According to Implant Treatment in Xenograft Transplanted for Experimental Bone Defects of Rabbits)

  • Bone graft had been widely investigated for reconstruction of bone defects or acceleration of bone healing in orthopedics, neurosurgery and dental surgery. Autograft is the golden standard of bone graft but it is associated with donor site morbidity and is restricted in quantity. Xenograft has been researched an alternative method for autograft. The purpose of this study was to investigate the efficacy of new bone formation according to three different preparations of implants on rabbit xenograft. Cortical bone xenografts which made from bovine femoral cortical bone were treated by freezing, freeze-drying or defat-freezing implant preparations. They were transplanted into proximal diaphyseal shaft of bifibulae of 15 rabbits which were divided into three groups according to their implant preparation method. The fibulae transplantations were evaluated radiographically and examined osteoblast activity by bone alkaline phosphatase (BALP) biweekly for 16 weeks to observe new bone formation and union of the experimental defected region. New bone formation was observed in 7 cases in freeze-drying and defat-freezing group, respectively. Union of proximal and distal end of defected region, which was considered as success of bone graft, was observed in 4 cases (40%; 4 of 10 cases), respectively. In freezing group, new bone formation was observed in 6 cases but, there is no union observed. BALP value was increased over twice after two weeks of graft procedure in all union cases of freeze-drying and defat-freezing group (two of five animals, respectively) then gradually decreased to 16th week. In non-union cases, there is no significant variation in BALP value. Defat-freezing or freeze-drying preparations of implants are more efficacious in new bone formation than freezing method on rabbit xenograft. While it is difficult to propose which is superior between defat-freezing and freeze-drying, defatting of implants may enhance new bone formation in xenograft.

The Anconeus Muscle Free Flap: Clinical Application to Lesions on the Hand

  • Background It can be difficult to select an appropriate flap for various defects on the hand. Although defects of the hand usually must be covered with a skin flap, some defects require a flap with rich blood supply and adequate additive soft tissue volume. The authors present their experience with the anconeus muscle free flap in the reconstruction of various defects and the release of scar contractures of the hand. Methods Ten patients underwent reconstruction of the finger or release of the first web space using the anconeus muscle free flap from May 1998 to October 2013. Adequate bed preparations with thorough debridement or contracture release were performed. The entire anconeus muscle, located at the elbow superficially, was harvested, with the posterior recurrent interosseous artery as a pedicle. The defects were covered with a uniformly trimmed anconeus muscle free flap. Additional debulking of the flap and skin coverage using a split-thickness skin graft were performed 3 weeks after the first operation. Results The average flap size was $18.7cm^2$ (range, $13.5-30cm^2$). All flaps survived without significant complications. Vein grafts for overcoming a short pedicle were necessary in 4 cases. Conclusions The anconeus muscle free flap can be considered a reliable reconstructive option for small defects on the hand or contracture release of the web space, because it has relatively consistent anatomy, provides robust blood supply within the same operative field, and leads to no functional loss at the donor site.
