This study aims at providing necessary informations for management decision-making to the hospital manager, such as ratios of fixed and variable cost to total operating expense, and variable cost ratio to operating revenues, and determinant factors affecting cost behavior. A study model and related hypotheses were established, data were collected from 41 private university hospitals for the 6years from 1998 to 2003, and regression analyses were performed to test the hypotheses. The results of the analyses and conclusions are as follows; First, labor cost and administration cost within the same number of beds have not only fixed quality of the cost, but variable quality of that. Also, the ratio of the variable costs to operating revenue of the metropolitan was estimated 76.9% and that of other area hospitals was 80.1%. Second, the major factors affecting the increase rate of the operating expense were the increase rates of the number of inpatients, the number of employee, and the number of hospital operating bed. This result implies that maintaining a optimal hospital bed size and efficient operation of the beds are important strategic factors of hospital management.

Large hospitals such as medical centers provide not only medical services, but also carry the responsibilities for emergency refuges, medical researches and education. The function of large hospitals is as important as other infrastructure systems such as highways, bridges, and utilities. When disasters occur, the hospitals have to provide emergency medical services for victims and support the patient's needs of health. In order to keep a smooth operation of hospitals, the maintenance management of hospital buildings should be carefully investigated. However, there are few researches focused on maintenance management issues of hospital buildings. This paper investigated the National Taiwan University Hospital (NTUH) and established a maintenance cost database. The NTUH is the best-known and most high-renowned medical center in Taiwan in which more than 4,000 employees serving approximately 2,000 in-patients and 7,000 out-patients daily. The data were collected from the NTUH which consisted of 16,228 maintenance records in the past ten years. This paper analyzed these data to obtain various characteristics of maintenance records, and revealed the key items of maintenance cost for large hospital buildings, which can provide the facility manager of hospital buildings to execute a proper maintenance policy for hospital buildings.
Objectives: This study was conducted to provide basic data for developing a professionalism improvement program for dental hygienists in preparation for the 4th industrial revolution era. Methods: A survey was conducted in 216 dental hygienists from May 3 to June 14, 2021, and statistical processing was performed using frequency analysis, t-test, one-way ANOVA, Pearson's correlation analysis, and multiple regression analysis. Results: Communication competence were at higher levels for dental hospital workers (3.98) and university/graduate school (3.94); professionalism was at higher levels for dental hospital workers (3.64), university/graduate school (3.55), and 7 or more dental hygienists at workplace (3.53); self leadership was at higher levels for dental hospital workers (4.11), university/graduate school (4.04), and intermediary manager (4.00); job performance was at higher levels for 11 years or more (4.08), university/graduate school (4.04), intermediary manager (4.04), and 30-34 years of age (4.04). There was positive correlation among communication competence, professionalism, self-leadership, and job performance. Factors affecting job performance were self-leadership (β=0.461), communication competence (β=0.353), 30-34 years of age (β=0.141), and total experience of 8-10 years (β=-0.149). Conclusions: In the era of the 4th industrial revolution, it is necessary to develop a program to strengthen the professional competence of dental hygienists.
Despite of increasing importance of management in healthcare administration and demand for the professional managers in hospitals, there have been few studies on the issues of competencies and qualifications of mid-managers for effective role performance. This article uses hospital mid-managers' judgements to discuss them in Korean hospital settings. Based on 67 questionnaires completed through the mail survey, the data were analyzed by chi-square test, t-test, ANOVA, and logistic regression. The result shows that knowledge of management skills in healthcare field is the most necessary area in general. Specifically, 'problem-solving ability' is the most important competency, followed by communication and planning capabilities. The mid-managers tend to consider the hospital employment history more important than the academic level. They also present the willingness to recruit the candidates with bachelor degrees rather than those with master or Ph.D. degrees. It is suggested that the candidate with bachelor degree and hospital employment over 6 to 10 years is the most qualified for a mid-manager in hospitals. The study results presented in the paper will provide the direction to improve the educational program and also the insights toward a advising strategy for job consultation for students majored in health service administration.
Background: Interleukin-33 (IL-33) has recently been implicated in tumor development. Methods: Data was obtained from PubMed, EMBASE, Clinical trial, Cochrane Library, Web of Science, CNKI and Wanfang databases. After quality assessment and data extraction, a meta-analysis was performed using Review Manager 5.2 software. Results: There were eight documents included in this meta-analysis. The results showed IL33 levels to be higher in tumor patients than that in health people, but no correlations tumor stage, metastasis and survival time of tumor patients were evident. Conclusion: IL33 may be useful as an alarm factor in tumor detection and prognosis.
It is important how the leader leads the organizational members with leadership appropriate to environment changes as the leader has great influence on the management of hospital organization. These data were collected from the questionnaire surveyed, from February 14 through February 28, 2003, to 218 people among those in the field of doctorship, management administration, nursing, technology, medical engineering, management engineering, and general affairs, working for three hospitals under S medical foundation in Pusan. The principal findings of this research are as below : 1. Type of leadership and job satisfaction had positive correlation in terms of both transformational and transactional leadership, with former having more relation than the latter. 2. Type of leadership and organizational commitment had positive correlation in terms of intellectual stimulus and situational rewards, having little relation statistically. 3. Type of leadership and turnover intentions had negative correlation only in individual consideration, situational rewards, and exceptional management, showing that transactional leadership had more relation than transformational leadership. 4. In terms of leadership and its effect on the organizational performance and turnover intentions, transformational leadership had more correlation than transactional leadership. But transactional leadership also turned out to have effect on organizational performance. When transformational leadership used, turnover intentions were lower than when transactional leadership used. However, both transformational and transactional leadership had negative correlation to turnover intentions. 5. Effect that type of leadership according to job classification, status, age, and academic career has on organizational performance and turnover intentions was differed by those factors; especially, in the occupation of doctor, both transformational and transactional leadership had positive correlation to turnover intentions. 6. Regression analysis on the factors of organizational performance and turnover intentions showed that higher age had positive correlation to the organizational performance in terms of socio-demographical features and that the more leadership was used for charismatic and situational rewards and the less leadership was used for intellectual stimuli, the higher organizational performance was likely. In short, hospital operators and managers should examine each manager's qualification, type, and style of leadership in the hospital organization, and they must develop lasting programs for effective education and training for developing characteristics of leadership are in needed to have managers with appropriate managerial mind to respective post and status. It should be kept in mind that manager's leadership especially has great effect on members' goal-setting, goal-achievement, organizational performance, and turnover intentions as it is a decisive factor of organizational members' attitude and motivation.
As healthcare environment being more complex and turbulent, strategic approach of hospitals became more important. This study was to investigate strategic orientation of Korean hospitals and their related characteristics. We surveyed managers of 360 hospitals randomly selected from all hospitals in Korea. For typology of strategic orientation, we used that developed by Miles and Snow, and results are as follows. Firstly, major types of organizational strategic orientation of Korean hospitals were analyzer(42.0%) and prospector(34.0%). Secondly, characteristics affecting to hospitals' strategic orientation significantly were hospital ownership and sex of managers. Medical corporation had a high tendency of prospector strategic orientation by 2.7 times compared to personal ownership. Female managers had a low tendency of prospector strategic orientation by 0.2, which was statistically significant. Though 60+ age group (compared to twenties and thirties) and middle managers (compared to CEO) had a tendency of being more prospector strategic orientation, but insignificant. The study result that majority of strategic orientation were prospector and analyzer reflects Korean hospital environment are complex and unstable. Hospital managers need to more focus on environment and boundary spanning function for maintenance and survival of their organizations.
Objectives : The purpose of this study was to survey the opinion and attitude of hospital managers toward the hospital evaluation program. Method : Managers of 157 hospitals which had participated in the hospital evaluation program were requested to respond to structured self-administered questionnaire. The questionnaire was composed with five categories: the preparation for the hospital evaluation program, the expertise levels of surveyors, the process and contents of the hospital evaluation program, the applying strategies for the result of the hospital evaluation to manager's work at their hospitals, and the main points to improve the hospital evaluation program. Result : 135 out of the 157 subjects completed the questionnaire, and the overall response rate was 86%. The hospital managers answered that they didn't have enough information such as the scoring rule of the standards, the process of the evaluation, and how to ask and get an answer to prepare the hospital evaluation. Furthermore, they estimated that the surveyors weren't specialized enough and didn't give a chance of checking over the result of the evaluation. In addition, they experienced that the result and feedback of the evaluation weren't enough information to be used as a guideline to improve in hospital management. Managers of the hospitals responded that the standards and method of survey in the hospital evaluation system should be reformed. Conclusion : Most of the responded managers seemed to have negative opinion on the hospital evaluation system, even though they were pushed up for interest in quality. Further studies and extensive evaluation of the hospital evaluation program are needed to bring up various information such as receptiveness and effectiveness.
Korean labor market has showed remarkable change of the increase in the amount of unemployment and contingent employment since IMF bailout agreement. There is a theoretical position to explain this increase in contingent employment at hospitals with the notion of flexibility. The high flexibility of employment due to the increase of contingent employees is becoming very important part in new business strategy of hospitals. The types of contingent employment of the nurse are part-time employment temporary employment, fixed-term employment, and internship which was introduced in early 1999. Recently, Korean health care industry managers have paid attention to the customer oriented service, rationalization of business administration, service quality control so that they can adjust their business to outer environment. Especially their efforts concentrate on the wage reduction through efficient and scientific control of man power because wage shares about 40% of total cost. This dissertation aims at verifying the phenomena of the contingent employment of the nurse and analyzing the related factors and problems. To rephrase these aims in ordinal: First, verifying the phenomena of contingent employment of the nurse. Second, verifying the problems of that phenomena. Third, analyzing the related factors of the contingent employment of the nurse. To accomplish these research goals, a statistical survey was executed. in which 384 questionnaires-66 for manager nurses, 318 for contingent nurses - were given to nurses working at 66 hospitals-which have at least 100 beds-in Seoul. Among them, 187 questionnaires-38 from manager nurses, 149 from contingent nurses'- 'were returned. Then, the data coded and submitted to T-test, $X^2$ -test, variance analysis(ANOVA), correlation analysis, multiple regression analysis, Logistic Regression with SAS program. The research results of the contingent nurses are followings: 1. The average career term at the present hospital 8.4 months: duty-on days per month are 24.2 days: working time per day is 7.9 hours. These results showed little difference from regular nurses. 2. Their wage level is about 70% of regular nurses except for internship nurses whose wage level is 41% of regular nurses. To break down the wage composition, part-time nurses and internship nurses get few allowance and bonus. And contingent nurses get very low level of additional pay except for fixed-term nurses who are under similar condition of employment to regular nurses. These results show that hospital managers are trying to reduce the labor cost not only through the direct way of wage reduction but through differential treatment of bonus, retirement allowance, and other additional pay. 3. The problem of contingent employment: low level of pay; high level of turn-over rate: weakening of union; low level of working condition: heavy burden of work; inhuman treatment. The contingent nurses consider these problems more seriously than manager nurses do. What manager nurses regard problematic is the absence of feeling-belonged and responsibility of the contingent nurses. 4. The factors strongly related with the rate of the number of contingent nurses for the number of regular nurses; gross turn-over nurses; average in-patients per day; staring wage of graduate from professional college: the type of hospital ownership; the number of beds; the gap between gross newcomer nurses and gross turn-over nurses. The factors related with their gross wage per month; the number of beds; applying of health insurance; applying of industrial casualty insurance; applying of yearly-paid leave; the type of hospital ownership; average out-patients per day; gross turn-over nurses. The meaningful factors which make difference by employment type: monthly-paid leave; physiological leave. The logistic regression analysis using these two factors shows that monthly-paid leave is related with the type of hospital ownership; the number of beds; average out-patient per day, and physiological leave is related with the gross newcomer nurses; gross turn-over nurses; the number of beds.
Background : This study is to identify the inappropriate hospital services for elderly inpatients over 65 years in general hospital with acute care functioning. Consequently elderly inpatient care and the management of long-term care facilities are key issues for current government health policy. Method : The survey was conducted for two months for all inpatients over 65 in 7 general hospitals, 6 work sampling days randomly selected. In each survey day, the subjective judgement by medical staff on the degree of acute care needs and by nursing unit manager on hospital services of each inpatients was also conducted. Result : The total number of cases collected are 2,541 elderly inpatients, according to subjective judgements by medical staff on inpatient condition. However 46.8% of cases are turned out to be non-acute care group. The frequency of medical services provided to non-acute group are 2~3 vital sign checks per day 78.2%, IV injection 40.1%, antibiotics medication 20.2%. Conclusion : Lots of elderly patients' who are staving in acute hospitals, at present need to be transferred to long-term care facilities. However, there was been shortage of long-term care facilities. It is expected to identify the need of elderly inpatients and therefore, to provide cost-effective, appropriate and good quality health services to elderly inpatients depending on their needs.
