A Study on the Sufficiency of Anticipated Effect of Order Communication System Introduced to Medium-Sized Hospitals (Focused on the case of S Hospital) (중소병원의 처방전달시스템 도입효과분석에 관한 연구 (S병원의 사례를 중심으로))

  • The purpose of this study is to ascertain whether the effect of introduction of OCS(Order Communication System) to the hospital is satisfied or not comparing the anticipated effect with the actual effect. For this purpose, a domestic hospital which has introduced and has been operating OCS for several years was chosen. Based on the internal data of S Hospital prepared before introducing OCS, researcher has analyzed the basic direction, design standard and status of operation after the introduction of OCS, etc. After analyzing the status of operations of several departments using OCS and interviewing with the chiefs of pertinent departments, a survey form was designed. Actual survey and interviews were conducted by the researcher for weeks to know whether doctors, nurses, medical technicians and clerks of the patient management dept. were satisfied with OCS and to find if they have any recommendations to improve OCS. Based on the analysis of survey, the effect of OCS was evaluated whether it has satisfied the anticipated effectiveness. For the question if they feel convenient in using OCS, doctors, nursing staffs in charge of ward and the staffs of billing dept. has answered that they were all satisfied(100%). The answers for the same question were relatively high in the case of nurses in charge of outpatient and staffs of radiography. Of course, there have been some nurses and staffs who complained for the inconvenience. However, overall satisfaction was high on the average. Some common problems occurred after the introduction of OCS were frequent errors due to instability of OCS system, paralysis of function of hardware on data back-up system and redundant investment due to erroneous choice of DB program in setting DB. It was also pointed out that lack of computer education and low participation of medical staffs has resulted in failure of developing effective software. As a result, it has lowered the efficiency of OCS. For example, some works have to be done by hands even after OCS. Based on the result of this research, recommendations to maximize the effect of OCS were presented as follows. First, strong leadership of CEO and active cooperation of doctors are mandatory. Second, all the process of hospital work should be analyzed and be redesigned in more efficient ways. Third, OCS should be designed to be user-based system which can be used efficiently by all staffs of the hospital. Forth, prior to the operation of OCS, proper tests of the program and trainings of the pertinent staff are required. Fifth, prior to the selection of hardware, BMT(Bench Marking Test) should be conducted. Sixth, before introducing OCS, staffs in charge of OCS should visit many hospitals operating the OCS system and take their cases into account.

  • This study is to analyze the reason of Breakdown of collective bargaining caused by the difference of main issue about bargaining and other factors influencing collective bargaining from the viewpoint of both labor and capital in Korean medical system. Korean medical system is facing the huge change of medical policy, so it should pursue institutional change following the change of medical system, On the contrary, the activity of labor union in medical system is more organized, activated, so understanding and cooperating on the related policy between labor and management are necessary. As the method of administration participation is collective bargaining, main issues between labor and management may lead Brwakdown of bargaining, so the author will analyze it from three viewpoints. First, the difference of recognition between labor and management due to the characteristics of hospital Secondly, the difference of recognition between labor unions due to the characteristics of hospital Thirdly, the difference of recognition between the characteristics of hospital and labor union.

  • This study was purposed to find out the difference of the accounting of practical cost between the ABC system and the traditional costing system applied in a hospital, to verified general effect of ABC. Methods: This case study deals with the method of calculation, the cost information that is produced at K hospital in Busan. To examine ABC system and traditional costing system, applying them to the clinical pathology, radiology, physics in K hospital. Results: As a result of costing analysis, it is showed maximum difference of 50% between ABC and traditional cost. compared in revenue center, it occurs the difference of 15% of them. considering the result, it is confirmed that ABC could be used as a means to offer more precise information. therefore, ABC makes possible to produce precise costing information and grasp the driver of cost, and it is possible to reduce cost effectively. Conclusion: ABC provide six benefits: (1) more accurate of service delivered (2) inproved pricing and contracting strategies (3) improved management decision making capability (4) greater ease of determining relevant costs (5) reduced nonvalue added costs.

  • Some of the large sized companies have taken parts in a hospital business with a view or justification to improve medical care regadless of the disadvantageous fee-for-service medical insurance reimbursement system controlled by authorities related. This gradually brought about the financial difficulties to university hospitals as well as general hospitals that were less competitive. In this circumstance the hospital administrators are called for preparing and implementing proper financial strategies by analyzing external circumstances and internal abilities of their hospitals. In this aspect, an effective cost-analysis system in the hospital has been needed for years. K-University hospital developed the practical cost-analysis system and applied it to the hospital management. The effects of cost analysis system are as belows: first, the trend of the monthly revenue per medical specialist from March to July in 1996 showed increasing pattern which is different from that in past years. second, it turned out that the department of functional laboratory in relation to medical treatment enlarged the medical revenue very sharply. third, the intensive care units were being operated at the state of deficit, while other general wards were lucrative.

  • This study is a proposal of quality scale and measurement for the Mobile Hospital Information System Usability used to the Chungnam National University Hospital in Daejeon Korea. This study is to provide decision- making guideline for hospital managers and to provide feedback for the users' information needs in Mobile Hospital Information System. The target people of the study were medical doctors in Chungnam National University Hospital. The service contexts of usability were request of medical Care, Vital sign check of Patient, Care Agreement of Patient, Blood management, Check of care state, Sign of choice doctors, Query of doctor order and patient measurement result. The Usability factors were the efficiency, effectiveness and satisfaction of quality model ISO9241-11. This study shows that the Mobile HIS is used for the communication and education between doctor and patients. Especially, The Care Agreement of Patient is a part of the most utilized in the Mobile HIS.

The Relationship between Hospital Specialization and Operational Performance: Focusing on Diseases of the Musculoskeletal System and Connective Tissue (병원의 전문화 전략과 운영성과 간의 관계: 근골격계 및 결합조직 질환을 중심으로)

  • This study is aimed at investigated and compared the differences in the affect of hospital specialization according to hospital size using claims data of the Health Insurance and Review Assessment National Inpatient Sample in 2018 for diseases of the musculoskeletal system and connective tissue. To this end, we used multivariate hierarchical linear models(a.k.a., multi-level models) using two-tier data from 106,599 patients discharged after diseases of the musculoskeletal system and connective tissue from 734 hospitals. Multivariate results indicate that patients who were discharged with diseases of the musculoskeletal system and connective tissue from specialized hospitals with 200 beds or less stayed shorter and paid less inpatient charge than those who were discharged from less specialized hospitals. But for hospitals with 201-300 beds, no positive impact relationship was found between hospital specialization and operational performance. This finding may be limited evidence that the affect of a hospital's specialization strategy may vary depending on the size of the hospital. We discussed several managerial and health policy implications below.

  • In Korea's distribution and production field establishment of logistics information system and automation of logistics were greatly developed. But there is no characteristic and professionalism about hospital logistics, and distribution center is located in outside warehouse, logistics system is very important. Since hospital has human power to receive product, system is not as much required, but outside warehouse requires system for arrival from medicine and medical supplies wholesale dealer and vender, warehousing, storage, and automation facilities, and system for various works such as peaking instruction, classification, packing, cargo-working, delivery, order of medicine from hospital and non-medicine in hospital is essential. Therefore, This study is about development of automatic identification of slide which cannot be overlooked in efficiency of hospital logistics, establishment of database, and information interlocking between automatic storage system and outside warehouse.

  • In this paper many discussions are raised regarding the outside director system in order to establish the position of the outside director within institutional organizations and to review ways to activate the outside director system in for-Profit Hospital. it seems to be more desirable in Korea for each company to establish an efficient system by itself in accordance with suggested guidelines suitable for each for-Profit Hospital management circumstances rather than to apply a unilateral outside director system to for-Profit Hospital through legalization. In this regard, while keeping the current outside director system for the time being, it is necessary to review and improve the management status of companies and the usefulness of the system, and various recommendations from lawyer associations, Hospital business associations, and academia should be allowed rather than just for-Profit Hospital recommendation by the government.

Perception and Evaluation of Quality of Hospital Information System (병원정보시스템 품질에 대한 인식 및 평가)

  • The purpose of this study is to investigate the perception of quality property of hospital information system, the quality level, and its effects. The participants were 730 employees who are using hospital information system in hospitals. In order to analyze the relationship, the quality property regulates the consideration of user's taste and the state of user's favor for the design of hospital information system, and the quality level regulates user's convenience and properness. The main results from this study can be summarized as follows, First, the perception of quality property for hospital information system showed few significance level between occupation, but revealed significance level between position. The evaluation of quality level showed significance level depending on the occupation and position. Second, quality property which generally affects to the quality level of the hospital information system were different between types of occupation and quality factor.
