Climate and Human coronaviruses 229E and Human coronaviruses OC43 Infections: Respiratory Viral Infections Prevalence in Hospitalized Children in Cheonan, Korea

  • The study of climate and respiratory viral infections using big data may enable the recognition and interpretation of relationships between disease occurrence and climatic variables. In this study, real-time reverse transcription quantitative PCR (qPCR) methods were used to identify Human respiratory coronaviruses (HCoV). infections in patients below 10 years of age with respiratory infections who visited Dankook University Hospital in Cheonan, South Korea, from January 1, 2012, to December 31, 2018. Out of the 9010 patients who underwent respiratory virus real-time reverse transcription qPCR test, 364 tested positive for HCoV infections. Among these 364 patients, 72.8% (n = 265) were below 10 years of age. Data regarding the frequency of infections was used to uncover the seasonal pattern of the two viral strains, which was then compared with local meteorological data for the same time period. HCoV-229E and HCoV-OC43 showed high infection rates in patients below 10 years of age. There was a negative relationship between HCoV-229E and HCoV-OC43 infections with air temperature and wind-chill temperatures. Both HCoV-229E and HCoV-OC43 rates of infection were positively related to atmospheric pressure, while HCoV-229E was also positively associated with particulate matter concentrations. Our results suggest that climatic variables affect the rate in which children below 10 years of age are infected with HCoV. These findings may help to predict when prevention strategies may be most effective.

ESKAPE Pathogens in Oral and Maxillofacial Infections

  • Purpose: Most cases of oral and maxillofacial infections are usually easily treated by proper diagnosis, elimination of causative factors, and antibiotic therapy. However, the emergence and the increase of multidrug-resistant bacteria make treatment challenging. "ESKAPE" pathogens are the most common opportunistic organisms in nosocomial infections and have resistant to commonly used antibiotics. There are many medical reviews of ESKAPE pathogens, but few in dentistry. This study focuses on oral and maxillofacial infection especially with ESKAPE pathogens. The purpose of this study is to prepare feasible data about tracing and treatment of infection related to pathogens that may be beneficial to clinicians. Methods: A total of 154 patients with oral and maxillofacial infections were reviewed by analyzing retrospectively hospitalized data in the Department of Oral and Maxillofacial surgery, Chosun University Hospital, Korea, past 5 years from January 2014 to December 2018. Based on the medical records and microbiological tests, the results were divided into two groups: infections with ESKAPE pathogens and other bacteria. Results: A total of 22 species were isolated from 154 patients. The proportion of ESKAPE pathogens among all bacterial isolates collected from infected patients was 39.6%. Causative factors, especially in post-operative infection, showed a statistically significant correlation to ESKAPE infections (29 cases). And average of treatment period in ESKAPE group was longer than non-ESKAPE groups. Overall, Klebsiella pneumoniae (60.7%) was the most frequently isolated ESKAPE pathogen. And high antibiotic resistance rates had been detected in the ESKAPE during the five-year period. Conclusions: Infections with ESKAPE pathogens are now a problem that can no longer be overlooked in Dentistry. Based on results of this study, ESKAPE pathogens were highly associated with post-operative or opportunistic infections. Clinicians should be careful about these antibiotic resistant pathogens and use appropriate antibiotics to patients while having dental treatments.

급성 호흡기 감염으로 입원한 소아에서 분리된 보카바이러스의 계통분석 (Phylogenetic Analysis of Human Bocavirus in Hospitalized Children with Acute Respiratory Tract Infection in Korea)

  • 목적: 보카바이러스는 2005년 소아의 비인두 흡입물에서 처음으로 발견된 호흡기 바이러스로서 현재 전세계적으로 발견되고 있고 국내에서도 보카바이러스 호흡기 감염증이 보고되고 있다. 이번 연구는 중합효소 연쇄 반응(PCR)을 이용한 보카바이러스의 분리 및 유전자 분석을 통하여 2010년 국내에서 유행하였던 보카바이러스의 계통 및 유전적 변이를 알아보고자 하였다. 방법: 2010년 1월부터 2010년 12월까지 세브란스 어린이병원에 하기도 감염증으로 입원한 소아에서 비인두 흡입 검체를 채취하였다. 다중 중합효소 연쇄 반응을 이용하여 12종의 바이러스에 대한 분자진단이 이루어졌고 이 중 보카바이러스 양성인 검체에 대하여 보카바이러스 1-4형에 특이적인 VP1/2 유전자 염기서열을 기반으로 하는 시발체를 이용한 중합효소 연쇄 반응을 시행하여 얻은 증폭산물의 염기서열분석을 확인하여 계통 분석을 시행하였다. 결과: 전체 953명의 소아 중 141명(14.8%)에서 보카바이러스가 검출되었고, 61.7%에서 다른 바이러스와의 중복감염을 보였다. 염기서열분석 결과 모두 보카바이러스 1형으로 확인되었고 보카바이러스 2, 3, 4형은 검출되지 않았다. 검출된 모든 보카바이러스의 염기서열들은 98% 이상의 염기유사도를 보였다. 결론: 2010년 국내에서 유행한 보카바이러스 호흡기 감염은 유전학적으로 모두 보카바이러스 1형에 의한 것이었고, 동정된 균주들은 높은 염기유사도를 보였다. 아직까지 국내에서 보카바이러스의 변이는 미미한 상태이지만 향후 보카바이러스 호흡기 감염의 유행시 지속적인 유전자형의 추적관찰이 필요하다.

  • Purpose: The purpose of this study was to investigate the knowledge and practice level of infection managements in child guardians with respiratory infections after H1N1 diffusion of 2009. Methods: The data were collected during February and March 2010 using a self-report questionnaire. The participants were 228 child guardians. Data were analyzed using frequency, percentage, means, SD, t-test, F-test, Pearson correlation coefficients, and Cronbach's alpha with the SPSS 17.0 program. Results: Mean age of the guardians and children were 37.8 years, 4.1 years respectively. For knowledge level of respiratory infections, hand washing method had the highest score and snuffles prevention method, the lowest. For practice level for respiratory infections, environmental hygiene management had the highest score and symptom management, the lowest. Infection management knowledge and practice level had a positively significant correlation in every area. Conclusion: The results indicate the necessity of continual education on infection management, and that the most efficient timing for the education appears be for participants during their first pregnancy or after the woman has delivered her child.

The First Neonatal Case of Panton-Valentine Leukocidin-Positive Staphylococcus aureus Causing Severe Soft Tissue Infection in Korea

  • 황색 포도알균은 흔한 피부 및 연조직 감염의 원인이며, panton-valentine leukocidin (PVL) toxin 생산 황색 포도알균은 전 세계적으로 발견되고 있으며 심각한 감염을 일으키는 것으로 알려져 있다. 그러나, PVL 양성 황색 포도알균으로 인한 신생아 감염은 매우 드물다. 저자들은 생후 7일 신생아에서 발견된 PVL 양성 황색 포도알균 감염으로 인한 심한 피부, 연조직 감염 사례를 보고하고자 한다. 환자는 당일 발생한 발열, 2일 전 시작된 둔부 열감, 부종, 압통으로 응급실로 내원하였다. 내원 당일, 발열, 빈맥, 전신 컨디션 저하, 둔부의 열감과 압통 진행이 확인되었다. 초음파 및 자기공명영상검사에서 피부 및 연조직, 일부 근육을 침범한 괴사성 근막염이 확인되었다. 둔부 병변에서 배농한 검체에서 PVL 양성 메티실린내성 황색포도알균 (MRSA)이 배양되었고, 균혈증은 없었다. 환자는 한달 간의 정맥 항생제 투여와 외과적 배액 수술 후 회복되었다. 퇴원 한달 후, 환자는 외이도염으로 재입원, 피부에서 동일한 MRSA가 확인되었다. 병변은 정맥 항생제 투여 및 소독만으로 호전되었다. 환자는 0.5% 클로르헥시딘 목욕을 통해 탈집락화(decolonization)를 시행하였고, 회복 후 재감염은 없었다. 본 사례는 PVL 생산 황색 포도알균 감염에 대해 적극적인 배액 수술 및 항생제 치료가 필수적이며, 재발 및 지역사회 확산을 방지하기 위해 추가적인 탈집락화가 필요함을 시사한다.

Elucidation of Bacterial Pneumonia-Causing Pathogens in Patients with Respiratory Viral Infection

  • Background: Bacterial pneumonia occurring after respiratory viral infection is common. However, the predominant bacterial species causing pneumonia secondary to respiratory viral infections other than influenza remain unknown. The purpose of this study was to know whether the pathogens causing post-viral bacterial pneumonia vary according to the type of respiratory virus. Methods: Study subjects were 5,298 patients, who underwent multiplex real-time polymerase chain reaction for simultaneous detection of respiratory viruses, among who visited the emergency department or outpatient clinic with respiratory symptoms at Ulsan University Hospital between April 2013 and March 2016. The patients' medical records were retrospectively reviewed. Results: A total of 251 clinically significant bacteria were identified in 233 patients with post-viral bacterial pneumonia. Mycoplasma pneumoniae was the most frequent bacterium in patients aged <16 years, regardless of the preceding virus type (p=0.630). In patients aged ${\geq}16years$, the isolated bacteria varied according to the preceding virus type. The major results were as follows (p<0.001): pneumonia in patients with influenza virus (type A/B), rhinovirus, and human metapneumovirus infections was caused by similar bacteria, and the findings indicated that Staphylococcus aureus pneumonia was very common in these patients. In contrast, coronavirus, parainfluenza virus, and respiratory syncytial virus infections were associated with pneumonia caused by gram-negative bacteria. Conclusion: The pathogens causing post-viral bacterial pneumonia vary according to the type of preceding respiratory virus. This information could help in selecting empirical antibiotics in patients with post-viral pneumonia.

신생아 중환자실에서 포도알균 감염에 대한 임상적 고찰과 위험 요인 조사 (Clinical characteristics and risk factors for staphylococcal infections in neonatal intensive care unit)

  • 목 적 : 신생아 중환자실에서 포도알균 감염이 증가하고 있어서 감염시 사망률과 유병율이 높기 때문에 신생아 중환자실에서 포도알균 감염의 임상적인 특징과 위험인자에 대하여 조사하였고 최근 늘어가고 있는 Methicilline 저항성 포도알균과 관련하여 항생제에 대한 감수성을 조사하였다. 방 법 : 2000년 1월부터 2004년 12월까지 한일병원과 경희의 대부속병원 신생아 중환자실에 입원한 세균 배양 검사상 포도알균에 양성을 보인 환아 240명을 대상으로 하여 의무 기록을 후 향적으로 조사하였다. 포도알균을 MRSA, MSSA, S.epidermidis, S.epidermidis 외 CNS로 나누어 감염에 대한 임상적 특징과 여러요인과 발생빈도에 있어서의 연관성에 대한 조사를 시행하였다. 포도알균에 양성을 보인 240명의 환아중 MRSA는 88명, MSSA는 41명, S.epidermidis는 63명, S.epidermidis 외 CNS는 48명이었다. 결 과 : 5년동안 신생아실에 입원한 3,593명 중에 배양검사를 시행한 환아수는 7,481명이었으며 배양검사상 양성 환아수는 2000년도에 23명, 2001년도에 109명, 2002년도에 53명, 2003년도에 35명, 2004년도에 20명이었다. 포도알균 감염의 위험요인은 제태연령 37주 미만, 5분 아프가 점수가 7점 미만, 입원후 총정맥영양 시행, 입원후 인공호흡기의 사용, 제대정맥관 또는 제대동맥관 삽입, 입원후 비강 또는 구강용 영양튜브의 사용 등이었다. 항생제 감수성에 있어서는 모두 vancomycin에 감수성이 100%였으며 teicoplanin, trimethoprim-sulfamethoxazole, chloramphenicol, clindamycin에 감수성이 높았으며 gentamycin과 erythromycin에는 감수성이 낮았다. 결 론 : 포도알균 감염의 빈도를 줄이기 위해서는 철저한 산전 관리 등을 통해 미숙아 출생을 낮춰야 하며 신생아실에 입원하는 환아에게 총정맥영양의 시행, 인공호흡기 사용, 중심정맥도관의 삽입, 비강 또는 구강용 영양튜브의 사용을 최소한도로 해야 하겠으며 최근 신생아실에서 증가하고 있는 methicillin 저항성 포도알균에 대해서는 조기에 적절한 항생제를 사용하는 것이 MRSA 감염으로 인한 유병률과 사망률을 낮출수 있을 것으로 사료된다.

A Case-Study of Implementation of Improved Strategies for Prevention of Laboratory-acquired Brucellosis

  • Background: In 2012, the Alaska Section of Epidemiology investigated personnel potentially exposed to a Brucella suis isolate as it transited through three laboratories. Methods: We summarize the first implementation of the United States Centers for Disease Control and Prevention 2013 revised recommendations for monitoring such exposures: (1) risk classification; (2) antimicrobial postexposure prophylaxis; (3) serologic monitoring; and (4) symptom surveillance. Results: Over 30 people were assessed for exposure and subsequently monitored for development of illness. No cases of laboratory-associated brucellosis occurred. Changes were made to gaps in laboratory biosafety practices that had been identified in the investigation. Conclusion: Achieving full compliance for the precise schedule of serologic monitoring was challenging and resource intensive for the laboratory performing testing. More refined exposure assessments could inform decision making for follow-up to maximize likelihood of detecting persons at risk while not overtaxing resources.

  • Identification of the pathogens causing infection is important in terms of patient's health management and infection control. Synovial fluids could be used as clinical samples to detect causative pathogens of prosthetic joint infections (PJIs) using molecular diagnostic assays, therefore, normalization and validation of clinical samples are necessary. Microbial culture is considered the gold standard for all infections, including PJIs. Recently, molecular diagnostic methods have been developed to overcome the limitation of microbial culture. Therefore, guideline for validating clinical samples to provide reliable results of molecular diagnostic assays for infectious diseases is required in clinical field. The present study aimed to develop an accurate validating method of synovial fluid clinical samples using GAPDH gene as an internal control to perform the quantitative PCR TaqMan probe assay to detect pathogens causing PJIs.

Serologic Tests of IgG and IgM Antibodies and IgG Avidity for Diagnosis of Ocular Toxoplasmosis

  • This prospective study was aimed to detect acute and chronic ocular toxoplasmosis by comparison of anti-Toxoplasma gondii IgM and IgG antibody levels and IgG avidity test. One hundred and seventeen patients with ocular toxoplasmosis (OT) who referred to the Farabi Eye Hospital, Tehran, Iran were included in this study. Of the patients, 77 cases were positive for anti-T. gondii IgG, and 8 cases were positive for anti-T. gondii IgM. IgG avidity test revealed 11, 4, and 102 cases were low, intermediate, and high, respectively, and 6.8% and 9.4% of cases were positive for IgM and IgG avidity tests, respectively (P=0.632). Agreement (Kappa value) between paired tests IgG-IgM, IgG-IgG avidity, and IgM-IgG avidity was 0.080, 0.099, and 0.721, respectively (P<0.05). This study showed that conventional serologic tests (IgM and IgG levels) and IgG avidity correlate well each other and can be used to differentiate recent infections from old OT. It seems that reactivated old infections rather than recently acquired infections are majority of Iranian OT patients.
