일부지역 치과의 기관, 감염관리자, 환자의 감염관리에 관한 연구 (The Study on Organization, Infection Controller, Patient Infection Control of Dental Clinic in Certain Areas)

  • 치과의료기관의 감염관리 수행 조사를 위해 2014년 6~7월 기간 동안 일부 지역사회 치과위생사 158명을 대상으로 기관, 감염관리자, 환자의 감염관리 수행을 자기기입식 설문조사하였다. 치과병원은 감염관리지침서 구비, 감염관리 담당자 지정, 정기감염관리 교육지원, 1인 멸균핸드피스 사용 비율은 57~74%였으나 치과의원은 13~20% 수준이었다(p<0.05). 감염관리자의 감염관리지침 인지는 치과병원 17.4%, 치과의원 5.9%였고 정기예방접종 시행은 치과병원 60.9%, 치과의원 34.1%였다(p<0.05). 환자의 손 위생 수행, 진료 전 칫솔질, 교차 감염 교육경험은 20% 미만이었고, 병력조사 협조는 90% 이상이었다(p<0.05). 감염관리 실천 상관분석 결과 기관 지원은 치과위생사 감염관리자실천, 환자감염관리협조, 개인방호실천, 진료실감염관리실천, 폐기물관리실천과 양의 상관관계를 나타내었으며 통계적으로 유의하게 분석되었다(p<0.05). 감염관리 실천을 위해 의료진, 환자, 기관의 지원과 협조가 요구되며 감염관리 관리자는 지속적인 교육을 통해 감염관리지침을 인지하고 실천할 것이 요구된다.

물리치료사의 병원감염에 대한 인식도 및 실천도 연구 (study on the level of recognition and performance of the physical therapist about the management of nosocomial infection)

  • 본 연구의 목적은 물리치료사들의 감염에 대한 개인 위생과 치료실 위생의 인식도 및 실천도를 조사하고 이에 영향을 미치는 요인을 분석하여 물리치료실 내 병원감염 관리 프로그램과 정책을 수립하기 위한 기초자료를 제공하고자 한다. 본 연구는 320명의 물리치료사를 대상으로 자료를 수집하여 분석하였다. 연구 도구는 자기 기입식 설문지를 사용하여 일반적 특성과 병원감염의 인식도와 실천도를 조사하였다. 리커트 5점 척도 설문지로 데이터는 백분율, 독립 T-test, 일원배치분산분석을 사용하여 분석하였다. 본 연구의 결과 감염관리부서가 있는 곳이 17.8%로 대부분 존재하지 않는다고 하였고, 41.6%의 물리치료사들이 병원감염과 관련된 교육을 받지 못했다고 하였다. 또한, 치료 시에 필요한 보호 장구를 충분히 제공받는 물리치료사는 25.3%로 매우 낮았다. 이러한 이유로 인하여 병원 내 물리치료사들의 병원 감염에 대한 인식도와 실천도가 매우 낮았음을 알 수 있었다. 따라서 앞으로 병원 감염의 인식도와 실천도를 높이기 위하여 병원 내에서 치료 시 필요한 보호 장구가 충분히 제공이 되어야 하고 병원 내에서 자체적으로 물리치료사의 병원 감염 교육을 정기적으로 실시해야 할 것으로 사료된다.

일개 대학 병원 건강 검진 수검자에서 진동 노출과 골격근 지수의 관련성 (Association between Vibration Exposure and Skeletal Muscle Mass Index in a Single University Hospital Health Check-up)

  • Objectives: The aim of this study is to evaluate the association between vibration exposure and skeletal muscle mass index through a single university health check-up. Methods: We used data from 134,067 male subjects who received a general health check-up or vibration exposure health check-up out of the 1,515,322 people who underwent medical check-up at a local university hospital from 2002 to 2018. Pearson correlation analysis was conducted for comparing the association between skeletal muscle mass index and demographic and hematological variables in both groups. Mixed linear model analysis after controlling demographic and hematological variables was used to analyze the differences in skeletal muscle mass index between groups at every visit over 10 years. Results: In the Pearson correlation test, the variables that showed different results when comparing the two groups were C-reactive protein (p=0.001) and glycated hemoglobin (p=0.002) in the vibration exposure group and erythrocyte sedimentation rate (p<0.001) and vitamin D (p<0.001) in the general group. After the adjustment of demographic and hematologic variables, the skeletal muscle mass index at every visit was markedly decreased in the vibration exposure group (p<0.001). Conclusions: In the vibration exposure group, the skeletal muscle mass index showed a tendency to decrease markedly over time compared to the general health check-up group, which showed that C-reactive protein and glycated hemoglobin would have an influence on skeletal muscle index in male workers exposed to vibration.

금속제품 제조 산업장내 공기중 금속농도에 관한 연구 (A Study on Metal Concentrations in the Air of Metal Products Manufacturing Industry)

  • This study was conducted for the purpose of obtaining the fundamental data on improvement of working environment and contributing to health improvement of workers who dealed with metal by assessing the metal concentration in air of industries located in Chang-Won Industrial Complex. Authors measured the concentration of metals(Al, Cd, Cr, Cu, Mn, Ni, Pb, Sn and Zn) is the air to 25 working processes of 73 industries by flame atomic absorption spectrometry from February to December 1994. Personal air sampler was used for air sampling with mixed cellulose-ester membrane filter. The results were as follows : 1. The geometric means(range) of metal concentration; 1) Al: $0.1505mg/m^3$ ($0.0147-18.6100mg/m^3$) 2) Cd: $0.0077mg/m^3$ ($0.0003-7.0710mg/m^3$) 3) Cr: $0.0163mg/m^3$ ($0.0013-1.1510mg/m^3$) 4) Cu: $0.0097mg/m^3$ ($0.0009-0.4950mg/m^3$) 5) Mn: $0.0412mg/m^3$ ($0.0006-4.7877mg/m^3$) 6) Ni: $0.0088mg/m^3$ ($0.0001-1.0170mg/m^3$) 7) Pb: $0.0152mg/m^3$ ($0.0015-0.4499mg/m^3$) 8) Sn: $0.0486mg/m^3$ ($0.0037-0.1500mg/m^3$) 9) Zn: $0.1911mg/m^3$ ($0.0122-8.2920mg/m^3$) 2. 종합병원 간호사의 업무 및 작업자세 분석결과 특징 (Features of Work and Posture Analysis Outputs in General Hospital Nurses)

  • Objectives: This study was to examine the features of work and posture analysis outputs in assessment of exposure to musculoskeletal disorder (MSD) risk factors in general hospital nurses. Methods: Work and posture analyses were carried out using observational approaches for nurses at general hospitals across Korea. With development of a taxonomy for assessing exposure to MSD risk factors, nursing tasks were documented in frequency (%time) for 8 hours a day in work analyses. Rapid Entire Body Assessment (REBA) scores were obtained for mode and maximum risk levels, respectively, during posture analyses. Results: A total of 27 nurses were observed while conducting 7 nursing tasks at 6 general hospitals. For both the work analyses and posture analyses, the taxonomy was developed and used. In the work analyses, 'Video display terminal task' and 'Nursing examination/ treatment' were the highest as 25%time for 8 hours a day, followed by 'Patient care' and 'Room rounding' as 13%time in order. In the posture analyses, the mode REBA scores were 2 or less for all nursing tasks while the maximum REBA scores were 7 for upper limbs at 'Room rounding' and 6 for trunk/neck/legs at 'Patient care'. Conclusions: The results showed the study nurses are occasionally at a risk for MSD, a medium level as designated in the REBA risk level, suggesting that it is important to control awkward posture at the nursing tasks such as 'Room rounding' and 'Patient care', in priority, for preventing MSD in the hospital sector including the study general hospitals.

대학병원 간호사의 교대근무 형태에 따른 사회심리적 스트레스 (The Effect of the Shift Work by Types on Sociopsychological Stress in the University Hospital Nurses)

  • Objectives: To determine the effect of the various type and number of shift work on sociopsychological stress in the university hospital nurses in Seoul, South Korea. Methods: Data was collected through a self-administered questionnaire that included general characteristics, the type and number of shift work in the past 4 weeks and the an sociopsychological stress based on PWI-SF. Logistic regression analysis was conducted to analyze the effect of shift work by types on sociopsychological stress of university hospital nurses using SAS 9.4 and it adjusted general characteristics by the additional models. Results: When the number of night shift work 3 or 4 compared to 0, sociopsychological stress was significantly high(OR=2.16, 95% CI=1.29-3.65). When 5 or more, it was higher(OR=2.39, 95% CI=1.55-3.72). Two continuous night shift work compared to 0 increased sociopsychological stress(OR=2.37, 95% CI=1.49-3.81) and 3 or more even more(OR=2.70, 95% CI=1.72-4.27). If the shift work type changes 14 or 15 times compare to below 11, sociopsychological stress is increased and even higher when 15 or more times but it's not significant after adjusting for covariates. Conclusions: It suggests that workplace with nurse expose to shift work should be on lookout for more than three night shift work, more than two continuous night shift work and more than 14 shift work change which can lead to burnout, absenteeism, depression and decreased productivity.

Review of a Case of Chronic Obstructive Pulmonary Disease in Workers Exposed to Synthetic Fibers

  • Objectives: Objectives of this study were: 1) to introduce industrial situation and health hazards of synthetic fiber, 2) to review a case of chronic obstructive pulmonary disease in a worker exposed to synthetic fiber reported to the Korea Occupational Disease Surveillance Center, and 3) to suggest supplementary measures for the occupational health system for workers exposed to synthetic fibers. Methods: Respiratory exposure, health hazards, and exposure standards for synthetic fiber dust in Korea and other countries were reviewed. In addition, a case of chronic obstructive pulmonary disease due to exposure to nylon dust reported to the Korea Occupational Disease Surveillance Center was reviewed and summarized. Results: The worker was a 53-year-old non-smoking male who had been involved in the nylon weaving process for 26 years. He had shortness of breath from three years ago. He was diagnosed with chronic obstructive pulmonary disease. PM1.0, PM2.5, and PM10 were measured at 26.6 ㎍/m3, 48.2 ㎍/m3, and 91.7 ㎍/m3, respectively. Fiber components estimated as nylon fiber were detected in the microscopic examination of a solid sample. Conclusions: For workers exposed to synthetic fiber dust, special health examinations of the respiratory system, regular work environment measurement, and work environment management through workplace health management should be performed. It is necessary to research on health effects of synthetic fibers.

우리나라 일부 여성근로자에서 초과근무가 피로증상에 미치는 영향 (Impact of overtime work on fatigue symptom in selected Korean female workers)

  • Objectives : To investigate the effect of overtime work on fatigue symptoms among Korean female workers. Methods : A total of 1,875 subjects were analyzed using the structured questionnaire, which was composed of overtime work, fatigue symptom, and background characteristics. Data analysis included multiple regression to test for a relationship of overtime work with fatigue symptom while statistically controlling for demographic and occupational variables. Results : Overtime work was more common in those with shorter work years of 3 or less than in those with longer work years. Overtime work was more popular in day workers than in shift workers. No significant difference of smoking and drinking state was observed between two groups. Subjective fatigue symptoms were significantly higher in overtime workers than in workers without overtime work. In a multiple regression analysis with significant control variables, overtime work was associated with 0.31 more fatigue symptom(p<0.001). Conclusion : Our findings suggest that overtime work may increase risks for fatigue symptom. Therefore, preventive strategies will be needed to control the negative effect of overtime work on health status of female workers.

치과위생사의 근무환경 및 조직 내 갈등수준이 이직에 미치는 영향 (Work Environment and Conflict Level of Dental Hygienists Affect Turnover)

  • Objectives : The purpose of this study was to understand how work environment and conflict level of dental hygienists affect turnover for the efficive manpower management of dental hygienists. Methods : Dental hygienists in dental (hospital)clinics near Daegu were randomly selected as the subjects of this study, and the followings analysis results are from 249 questionnaires: which were collected from March 2 to March 31 2011. Results : Among the 249 dental hygienists surveyed, 33.7% experienced turnover and 59.0% had turnover intention. From the analysis of the demographic, working environment and levels of conflict, dental hygienists had significantly higher turnover experiences based on following factors: older age, employment at a dental clinic rather than a dental hospital, longer working hours, longer total career as a dental hygienist, and longer of length of time at their current job. Furthermore, dental hygienists had significantly higher turnover intentions based on the following factors: younger age, more ignorant and higher level of discomfort due to conflict levels. Conclusions : Therefore, the working environment of dental hygienists has to be improved to inspire motivation, and conflict management has to be taughht within an organization to allow free communication and active participation in decision-making.

간호대학생의 병원감염관리 표준주의에 대한 인지도와 수행도 (Nursing Students' Awareness and Performance on Standard Precautions of Infection Control in the Hospital)

  • Purpose: The purpose of this study was to investigate nursing students' awareness and performance on standard precautions and to provide meaningful information for nursing students' education regarding hospital infection control. Method: Four hundred forty seven nursing students at 6 universities in D-city participated in this study. Data collection was conducted from March to April 2011. Students' awareness and performance on standard precautions of infection control in hospitals were measured using the modified 2007 CDC standard precautions guidelines. Data analysis was performed using the SPSS WIN 18.0 program, descriptive statistics, t-test, and ANOVA. Results: The level of students' awareness in the standard precautions was higher than performance. The higher levels of students' awareness and performance on standard precautions included patient care equipment, safe injection practices, and worker safety. The lower levels of students' awareness and performance on standard precautions included hand hygiene and personal protective equipment. There was no difference in the students' awareness and performance of standard precautions according to their characteristics. Conclusions: Based on the findings of this study, hand hygiene and personal protective equipment need to be stressed more within the program to improve nursing students' infection control techniques.
