  • In a hospital, there are many clinical departments. In general, the hospital management often uses the index of cure rate (i.e. the rate of patients recovered by the cure of this hospital) to evaluate the medical quality of each clinical department. But, since the cure rate varies significantly with the rate of patient with severe disease, the evaluation by cure rate is unreasonable and incomplete. This paper proposes a new evaluation method called the cause-selecting evaluation method proposed by Prof. Gongxu Zhang (1990). Besides, this method can also be used to diagnose the abnormality of cure rate. The practical case studies check with the theory very well.

Methods : 1,135 copies of survey have been sent to the group of people who have claimed the dental recuperation income expense to dental recuperation institutions in Daejeon, Chungcheong Do that are registered to the health insurance evaluation and estimation office as of the May 2010 and 207 surveys that were regarded to be sincere for answering were analyzed. Results : Majority of respondence were belonged to the dentist institutions with more than 5 years of claim experiences as well as 10~50% of claim rate. The recognition of medical fee evaluation guideline was normal level, and negative recognition was higher to the health center with daily charge policy compare to the dental hospital and university affiliated dental center with treatment charge policy, Highest opinion for inappropriateness of dentist with significance was found (p<0.05). The openness of evaluation cases are regarded to be discharged through the transparent evaluation and most of the opinions for insurance claim evaluation adjustment are within the both 'Do not understand the evaluation guideline and program error of disease category, code and program' with significance(p<0.05). The reaction after the evaluation adjustment was high in reflection on the claim process after examining the reason for the evaluation adjustment through the evaluation and estimation office and university affiliated dental institution and dental center was regarded to be most active and deputy reclaimment was seemed to be most actively discharge the objection registration task (p<0.05). The claim error improving resolution recognition was highly prioritized to the accurate charting for the disease title and treatment description, improving the setting of claim program, and most highly recognized by the university affiliated dental hospital/dental center and comparably low by health center(p<0.05). and although the most of the responds of treatment description and browsing the medical fee was positive, 50% of dentists disagreed the idea so that this was creating a significant discrepancy with other groups(p<0.05). Conclusions : From this research, the recognition of medical fee evaluation guideline for dental (university) hospital and dentists were negative and high adjustment experience was examined as lacking of evaluation guideline understanding and error of disease name, code and programs and deputy reclaimment, university affiliated dental hospital/dental center were most actively handle the objection registration tasks and dentists have objection on the treatment description and browsing the treatment fee so that if these indexes can be referred to implement into the recuperation income claim process, this can be regarded to be a opportunity to create mutual credibility between recuperation institution, treatment pensioner and the evaluation institutions.

  • Objective : Hospital workers' mental health has deteriorated because of the ongoing COVID-19 pandemic. The purpose of this study was to investigate the impact of the prolonged COVID-19 pandemic on the mental health of hospital workers and its determinants. Methods : Two surveys were conducted among employees working in a hospital that received COVID-19 patients from the early phase of the pandemic in South Korea. Data on demographics, perceived threat, workplace evaluation, resilience, and mental health status were collected using the Korean General Health Questionnaire-20 in the initial phase (February 2020) and during the third wave of COVID-19 (December 2020) for 467 and 545 workers, respectively. The mental health of hospital workers in the two phases was compared, and the risk and protective factors during the third wave were investigated. Results : The proportion of patients in the psychiatric high-risk group increased from 2.8% in the initial phase to 11.4% during the third wave. The perceived threat, workplace evaluation, and resilience of respondents deteriorated. Risk factors for mental health during the third wave included the perceived threat items of job stress, loss of control, and considering resignation. Protective factors included presence of children, workplace satisfaction, and hardiness in resilience. Conclusion : Hospital workers' mental health deteriorated as the pandemic progressed. General stress and tension such as job stress, loss of control, considering resignation rather than COVID-19-specific stress had negative effects on mental health of hospital workers. Therefore, care for work stress itself can be helpful to maintain the mental health of hospital workers. Also, governance to improve workplace satisfaction or hardiness in resilience can be a potential protective factor for hospital workers' mental health during the prolonged pandemic.

  • This study was initiated to develop a uniform design suitable for child inpatients. The experimental hospital inpatient uniform design was developed based on the results of the prior studies. The panel was composed of 5 experienced nurses and 5 clothing specialists. They evaluated the function of the experimental inpatient uniforms developed in this study. The experimental hospital gown design was reformed based on the panel's evaluation.1'hen the child inpatients performed the wear test for the reformed experimental hospital gown. The results of the study are as follows; 1. The child inpatient hospital gown design of the most hospitals were similar to the adult patient's uniform : the V-neckline shirts with full length set-in sleeves. The bottom was full length pull-on pants. The fabric was white cotton with blue hospital logo and stripe print in most cases. 2. The panel's specialty affected the evaluation of the uniform design. The nurses concerned about the durability after washing and ease for medical treatment. The clothing specialists cared about the features related to the clothing construction and ease for physical movement of body. 3. The most preferred hospital inpatient uniform design by the panels was the shirts or one- piece gown with three-quarter length sleeve and overarm seam opening. 4. The researchers developed one-piece dress gown for the children under two years old. Three different size pajamas were developed for children 2∼5 years old,5∼8 years and 8∼12 years old.
