Gorham-Stout Syndrome with Focal Segmental Glomerulosclerosis: A Case Report

  • Gorham-Stout syndrome is a rare bone disorder characterized by progressive massive osteolysis and proliferation of vascular and lymphatic vessels. A 15-year-old boy was initially diagnosed with Gorham-Stout at the age of 8 years based on clinical and radiological findings. Following diagnosis, he was treated with pamidronate, interferon alfa, propranolol, oral corticosteroids, and sirolimus. He developed proteinuria at the age of 15 and progressed into the nephrotic range 2 years later. A renal biopsy revealed focal segmental glomerulosclerosis, not otherwise specified variant. The sequential increase in proteinuria associated with medications suggested that the focal segmental glomerulosclerosis may be caused by pamidronate and sirolimus, but cannot completely rule out the possibility of kidney involvement of GSS itself.

Large Focal Extrapleural Hematoma of Chest Wall: A Case Report

  • Although hemothorax and pneumothorax are common complications seen in rib fractures, focal extrapleural hematoma is quite rare. We report a 63-year-old female patient that developed large focal extrapleural hematoma after falling off a second floor veranda. The patient had sustained 3, 4, 5th costal cartilage rib fractures and a sternum fracture. She had developed suspected empyema with loculations with small amount of hemothorax. She underwent a planned early decortication/adhesiolysis by video assisted thoracoscopic surgery at the 12th post-trauma day due to failed drainage. Unexpectedly, she had no adhesions or any significant retained hematoma mimicking a mass, but was found with the focal extrapleural chest wall hematoma. She was discharged on postoperative 46th day for other reasons and is doing fine today.

  • Posterior reversible encephalopathy syndrome (PRES) is a rare condition manifested by inflammation in certain areas of the brain. Rhabdomyolysis with acute kidney injury (AKI) complicated by PRES is rarely reported. A 26-year-old female presented with neurological symptoms, high blood pressure, and AKI. Her symptoms improved with blood pressure control, anticonvulsant drug medications, and renal replacement therapy. This case demonstrates that PRES should be considered in the differential diagnosis of patients who have rhabdomyolysis with AKI accompanied by neurological symptoms, including headaches and convulsions.

Study on the Trend of Medical Departments in Hospitals (중소병원의 개설 진료과목의 추세분석)

  • This study analyzed the trend of relationship between hospital departments. Hospitals directory issued by Korean Hospital Association in 2009 was used to collect the study sample data. The study sample used 188 hospitals established during the four years from 2005 to 2008, and bed size, establishment year was identified in the directory. Population variable in 2009 was taken from the website of Statistics Korea. SAS 9.2 was used for the statistical analysis. In regression analysis, year of establishment (2008) was statistically significant in explaining the number of hospital departments. Hospitals established in 2008 showed significant positive number of department than other years. Study hospitals showed increasing number of hospital departments during four years. Further study will be needed to analyze the effects of hospital departments on their performances.

  • Background: About 3% of all cancer patients suffer from cancer of unknown primary origin. Generally, carcinoma of unknown primary (CUP) carries a grave prognosis, but primary tumor presented to the neck is exception to this. The aims of study are to determine the role of chemotherapy and to find the prognostic factors in unknown primary tumor presented to the neck. Method and Material: Eighty-four patients were diagnosed with unknown primary tumor presented to the neck between January 1996 and June 2002. Among 84 patients, 43 patients (52%) received chemotherapy, radiation or surgery were performed in 20 patients (23%), 21 patients (25%) had no treatment. Results: The response rates to chemotherapy were 87.5% in CUP only localized to the neck and 44.0% in CUP systemically involved (p=0.012). A median follow-up duration was 6.4 years and overall median survival time was 9 months. The median overall survival time of patients treated with chemotherapy were 17 months and that of patients who received surgery or radiation were 20 months (p=0.3548). The important prognostic factors were performance status and the number of involved organ. Conclusion: The prognosis of patients with CUP presented to the neck is more favorable than that of patients with CUP of other localization. The effectiveness of chemotherapy for CUP only localized to the neck was similar to that of surgery or radiation. The important prognostic factors were performance status and the number of involved organ.

  • To determine the accuracy of magnetic resonance (MR) imaging for discrimination between intervertebral disk extrusion versus protrusion. MR images of 80 patients who had MR imaging of the spine and confirmed as intervertebral disk extrusion or protrusion were retrospectively reviewed by an experienced musculoskeletal radiologist. A 1.5-T scanner was used. After review of medical records, MR findings of disk extrusion and protrusion were compared using the chi-square test. Intraobserver agreement for differentiation of disk extrusion from protrusion was calculated by using coefficient.

  • Primary Epstein-Barr virus (EBV) infection can manifest with a broad spectrum of neurological complications. There are only rare reports of Guillain-$Barr{\acute{e}}$ syndrome (GBS) supervening on meningitis in patients with primary EBV infection. Clear evidence of central nervous system infection makes it difficult for the clinicians to consider a diagnosis of GBS. We present a patient with GBS supervening on meningitis in primary EBV infection.

  • Trichuris trichiura, commonly referred to as a whipworm, has a worldwide distribution, particularly among countries with warm, humid climates. In Korea, trichuriasis was a highly prevalent soil-transmitted helminthiasis until the 1970s. However, the nationwide prevalence decreased to 0.02% in 2004 as a result of national control activities and improvement in the socioeconomic status of Koreans. Most infected individuals have no distinct symptoms, if lightly infected. The diagnosis is typically confirmed by detection of T. trichiura eggs on examination of a stool sample; few reports have described detection of the parasite during colonoscopy. Recently, we managed 4 patients with trichuriasis who were diagnosed by detection of the parasite on colonoscopy, and we reviewed the literature on the colonoscopic diagnosis of T. trichiura in Korea. We suggest that colonoscopy might be a useful diagnostic tool, especially when infected by only a few male worms with no eggs in the stool.

  • A 38-year-old man, who was diagnosed nasopharyngeal carcinoma (stage T4N2Mx) had rapidly growing lower neck mass (at level IV area) with local inflammation sign during concurrent chemoradiotherapy. After we performed Color Doppler sonography and fine needle aspiration biopsy of the neck mass, we differentiated neck soft tissue infection from tumor extension. Size of the mass decreased after antibiotics therapy. It was difficult to differentiate neck infection from tumor progression during treatment in naspharyngeal carcinoma.

  • The recently proposed Bethesda system for cervical/vaginal cytology has made a standardization related to "atypia". In cellular changes due to inflammation or repair, the word "benign cellular change" has been suggested as a substitute for atypia. Terminology related to atypical cells may become standardized, but the cytologic criteria has not been well defined yet. We evaluated 160 cases of atypical squamous cells of undetermined significance(ASCUS) by the Bethesda Sys4em(TBS). Among 30,428 cases screened, a cytologic diagnosis of ASCUS was made in 498 cases(1.6%) and 160 cases were histologically verified ASCUS was diagnosed based on nuclear enlargement and nonclassical signs of condyloma. The results are as follows: One hundred and twenty three cases(76.9%) revealed chronic cervicitis. Thirty seven cases(23.1%) demonstrated squamous intraepithelial lesion. Among intraepithelial lesions, condyloma and mild dysplasia were 28 cases(75.7%). Moderate and severe dysplasia were 5 cases(13.5%) and 4 cases(10.8%), respectively. it is concluded that patients with ASCUS should be colposcopically examined.
