Study on Clinical Effects of Homnis Placenta Herbal Acupuncture on Osteoarthritis of Knee Joint (자하거(紫河車) 약침(藥針)의 퇴행성(退行性) 슬관절염(膝關節炎) 치료(治療)에 대한 임상적 고찰(考察))

  • Objectives : The study was peformed to evaluate the effect of treatment for Osteoarthritis of knee joint by using Homnis Placenta Herbal Acupuncture. Methods : We investigated 60 cases of patient with Osteoarthritis of knee joint from 1st February 2005 to 31st January 2005, 60 cases of patient with Osteoarthritis in knee joint treated at the department of acupuncture & moxibustion of Kwang-Dong Oriental Medicine Hospital were randomly selected for two group. One group was treated by Homnis Placenta Herbal Acupuncture therapy, the other group was treated by Filiform Acupuncture therapy. We studied Lyshoim score, Ahlaback Grade, Nine-point scale and improvement degree of the grade of Ahlaback. Results : 1. In the Lyshoim score, Homnis Filiform Acupuncture was more effective than Filiform Acupuncture. But, there was no significance between two groups statistically in the Lyshoim score. 2. In the Nine point score, Homnis Placenta Herbal Acupuncture was more effective than Filiform Acupuncture. 3.In the improvement degree of the grade of Ahlaback., Homnis Placenta Herbal Acupuncture was as effective as Filiform Acupuncture. Conclusion : In the treatment of Osteoarthritis of knee joint, Homnis Placenta Herabl Acupuncture was not more effective than Filiform Acupuncture statistically. But both Homnis Placenta Herbal Acupuncture and Piliform Acupuncture was effective in Osteoarthritis of knee joint.

The Effect of Homnis Placenta Herbal Acupuncture on Reducing Expression of LPS-induced Arthritis Model as an Anti-inflammatory Agent (자하차약침(紫何車藥針)의 항염증능(亢炎症能)이 LPS 유발 관절염(關節炎) 치료(治療)에 미치는 영향(影響))

  • Objectives : Rheumatoid arthritis(RA) is a systemic & a chronic inflammatory autoimmune disease . A chronic , locally destructive inflammmatory reaction in human is examplified by the synovitis present in some connective tissue disorder. The presence of a number of cytokines, $TNF-{\alpha}$, iNOS & expression of nitric oxide, NF-kB p65 activation implies an important role of cellular immune response in RA inflammatory reaction. This study was designed to evaluate on the effects of the Homnis Placenta herbal acupuncture on EX-LE201 & ST 35 reducing expression of LPS-induced arthritis model in mice. Materials and Methods : Homnis Placenta herbal acupuncture was inserted into 10 rats induced rheumatoid arthritis. The acupunctures were injected into the EX-LE201 and ST35 points. Such indexes were measured the inhibition of inducible nitric oxide synthase(iNOS) expression, nitric oxide(NO) production in vitro experiment and Tumor Necrosis $Factor-{\alpha}(TNF-{\alpha})$ & Nuclear Factor kappa $B(NF-{\kappa}B)$ p65 activation, synovial hyperplasia, angiogenesis and fibrosis in synovial membrane of knee joint of mice in vivo experiment. Results : 1.Homnis Placenta Herbal acupuncture inhibited iNOS mRNA and NO in RAW 264.7 cell of LPS-induced rheumatoid arthritis in a dose dependent manner. 2.Homnis Placenta Herbal acupuncture also showed significant inhibition of $TNF-{\alpha}$ & $NF-{\kappa}B$ p65, activation, synovial hyperplasia, angiogenesis and fibrosis in synovial membrane of knee joint of mice. Conclusion : These results suggest that Homnis Placenta Herbal acupuncture has an therapeutic effects on LPS induced-rheumatoid arthritis by inhibiting $TNF-{\alpha}$ activation.

Effects of Pharmacopuncture and Danggwisu-powder for Lateral Malleolus Avulsion Fracture: a Case Report (족외과 견열골절 환자에서 약침치료, 당귀수산을 통해 호전된 1례 증례보고)

  • Objectives : The purpose of this study is to report the clinical effect of Korean medical treatment, specifically Homnis Placenta pharmacopuncture, bee venom injection and Danggwisu-powder, on a patient suffering from a lateral malleolus avulsion fracture. Methods : A 63-year-old female patient suffering from right ankle pain due to lateral malleolus avulsion fracture was treated with Homnis Placenta pharmacopuncture, bee venom injection, and herbal medicine(Danggwisu-powder). from May 20th to June $16^{th}$ of 2015. The change in patientcondition was measured with the American Orthopedic Foot and Ankle Society Ankle-Hindfoot Scale, the verbal numerical rating scale, and via the ankle's range of motion. Results : After treatment, all obtained results showed improvement. The American Orthopedic Foot and Ankle Society Ankle-Hindfoot Scale increased from 18 to 71, the verbal numerical rating scale changed from 8 to 3, and the ankle's range of motion improved. Conclusions : This study suggests that Korean medical care focused on Homnis Placenta pharmacopuncture, bee venom injection and Danggwisu-powder can be effective to treat lateral malleolus avulsion fracture.

  • Objective : This study was designed to find out whether NS(normal saline) is able to be constituted as an appropriate control group for HHT(Hwangryunhaedoktang herbal acupuncture) and HP(Homnis Placenta herbal acupuncture). Methods : NS and two herbal acupuncture were inserted into Quchi(LI 11) of the subjects. After 5 minutes the subjects completed a questionnaire rating the intensity of 21 kinds of acupuncture sensation; hurting, penetrating, sharp, aching, intense, spreading, radiating, tingling, pricking, stinging, pulling, heavy, dull, numb, electric, shocking, hot, burning, cool, pulsing, and throbbing. We compared the subjective acupuncture sensation between the two groups. Results : As for HHT seven items of the acupuncture sensation were significantly different from NS. In case of HP just two items were significantly different from NS. Conclusion : We found that NS may be able to be an placebo herbal acupuncture for HP. Further study is needed for the appropriate placebo herbal acupuncture.

Research Trends on the Treatment of Knee Osteoarthritis in Korean Medicine (퇴행성 슬관절염의 한의학적 치료에 관한 연구동향)

  • Objectives : To research trends of studies on treatments of knee osteoarthritis in Korean medicine. Methods : Domestic theses, published from 2000 to 2010, mentioning the treatments of knee osteoarthritis in Korean medicine were reviewed and analyzed. This study researched 79 theses and classified again according to the fields of the treatment. Results : The results were obtained as below ; 1. Studies on pharmacoacupuncture account for 41% of the total (32cases/79cases). The target of treatments had changed from pain alleviation and anti-inflammation to cartilage-protection since 2006. 2. Herbal Drug, Bee Venom, Cervi Cornu Parvum pharmacoacupuncture, Taping Therapy and Tai-Chi Exercise program were more effective than others. Moreover research on exercise program such as Tai-Chi has been increasing. 3. Some Herbal Drug (Glycyrrhizae Radix, Angelicae Gigantis Radix, Achyrantis Radix, Paeoniae Radix, Eucommiae Cortex and so on) were mainly used to investigate the effect on cartilage protection, pain relief and anti-inflammation in laboratory test. 4. Acupuncture including individualized, local acupoints and constitution acupuncture were effective than others in pain alleviation and symptom relief. 5. Pharmacoacupuncture Bee Venom were effective in recovery of joint function and pain relief while Buthus martensii Karsch, Homnis Placenta and Cervi Cornu Parvum were effective in cartilage protection and recovery. 6. Moxibustion was efficient to reduce joint pain and partly increase ROM (range of motion) of knee joint, but was not efficient to recover the function of knee joint. Conclusions : These results indicate that research trends on knee osteoarthritis were changed from symptomatic treatment (pain alleviation and anti-inflammation) to conservative treatment (cartilage protection and recovery).
