서울시 거주자의 주택상담과 자료 요구도에 관한 연구 (Needs for the Housing Consultation and Information about Housing in Seoul Area)

  • This study aimed to aimed to obtain some basis data for the housing consultation and information about housing through survey of needs for the housing consultation. Respondents were housewives from 395 households living in Seoul area. Questionnaire was used for survey and the collected data were classified by two items such as types of residence and age of housewives. The survey was performed during two weeks in September 1997 and analyzed by frequency mean and percentage using SAS program The finding are as follows; (1) According to the actual conditions of housing consultation the main source of information related to housing was a newspaper Those who had experience detached house had much more opportunities than those of multi-family house. The most frequent agency for housing consultation was a realty dealer showing 65.8%. However the percentage of visiting permanent housing exhibition centers was high among the people aged 20 to 30, noted 20.4%. (2) Level of understanding housing consultation showed 3.45 points among 5. This result indicated that the service system of housing consultation were required. Architects and interior designers were most preferred as specialists whom the respondents wanted to consult with . As to the contents of consultation the most required item was changing of interior space(22.6%). In case the housing information leaflet is sold the respondents were willing to pay 523 won for each of it. (3) Among leaflets related to various information leaflets about housing purchase were most preferred(21.0%) According to the result of the survey the need for the information about housing purchase indicated higher preference than about housing management. The fact could be thought that consumers' consciousness of housing should be changed. Therefore it is desirable to lead consumers to lengthen the expected life span of their house and value happiness of the house by providing them with consulting systems about housing management throug housing consultation materials and development of its programs.

사회복지전담공무원의 일가정갈등이 이직의도에 미치는 영향 - 소진과 직무만족의 매개효과를 중심으로 - (Effects of Work-Family Conflict among Public Social Welfare Officers on Turnover Intentions - Focus on the Mediating Effects of Exhaustion and Job Satisfaction -)

  • 본 연구는 사회복지전담공무원의 일가정갈등이 이직의도에 미치는 영향을 살펴보고 소진과 직무만족의 매개효과를 검증하였다. 연구자료는 노원 구정연구단에서 찾아가는 동주민센터 복지 업무 담당 인력의 실태를 파악하기 위해 실시한 설문조사 데이터를 분석자료로 활용하였으며, 노원구 19개 동주민센터의 사회복지전담공무원 146명을 최종분석 대상으로 하였다. 분석 결과, 일가정갈등은 소진에 영향을 주는 것으로 나타났으나, 이직의도에 미치는 영향은 직접적이지 않다. 소진은 직무만족에 영향을 미치며, 이직의도에 직접적·간접적 영향을 미치는 것으로 나타났다. 분석결과를 바탕으로 사회복지전담공무원의 소진을 낮추고 직무만족을 높이기 위해 일가정 양립 지원 정책에 대한 정책적·실천적 함의를 제시하였다.

산업재해 장애인의 일상생활 수행능력, 우울 및 삶의 질 (Ability of Daily Living, Depression and Quality of Life of the Industrial Accident Disabled)

  • The purpose of this study is to Identify the amount of ability of daily living, depression and quality of life and to examin the relationship among ability of daily living, depression and quality of life of the Indestrial Accident Disabled. Three hundred and forty five industrial accident disabled were randomly selected among who have stayed in Seoul, Incheon, and Gyeonggi area and who were confirmed as the disabled classified according to its severity from the first to fourteenth grade between January, 1998 to December, 2000. Data were collected for one month between August 1, 2001 to August 31, 2001. Each study team was consisted of two members of researcher among 20 members and interviewed the disabled individually by visiting their home. Collected data were analyzed from the frequency, percentages, Correlation, etc. The mean score of ability of daily living was 136.3 out of possible 147. The mean score of depression was 31.5 out of possible 64. The mean score of quality of life was 20.1 out of possible 32. The relationship between ability of daily living and depression was significantly correlated and between depression and quality of life was significantly correlated and between ability of daily living and quality of life was significantly correlated. The industrial accident disabled had been in bad health or faced by difficulties in daily activities even after the completion of therapy for the disability. Especially, they are depressed and perceive lowly their quality of life. So, the rehabilitation program for the disabled should be psychosocial oriented program and a comprehensive service.

3차 진료기관 외래약국 투약대기시간 단축을 위한 택배 제도 도입 타당성 조사 (The study on appropriateness of adapting door-to-door delivery system for the reducong of waiting time at outpatient pharmacy department in Tertiary care hospital)

  • Background : Because of introduction of nationwide health care system in 1989 and the improvement of socioeconomic status of population the number of outpatient visiting university hospital has good facilities and manpower has increased. So the waiting time for medical service at university hospital are lengthened. Particularly outpatients complain that waiting for prescribed drugs at pharmacy depart are long. Reducing waiting time at pharmacy depart door-to-door delivery system that the patients applying for door-to-door delivery receive prescribed drug at home without waiting at pharmacy depart were studied. The objective of this study is to analysis the opinion of outpatients for door-to-door delivery system, to study the appropriateness of adopting the system and to produce ideal model for the system. Method : Outpatients waiting drug at pharmacy depart were questioned about door-to-door delivery system. to find the factors affect utilizing the system the logistic regression was used. Result : 83.3% of the patients want to utilize the system without charging, and 72.9% of the patients want to utilized system with charging. 68.3% of patients with charging want to use this system because of long waiting time at pharmacy depart. 50% of patients who do not want to use door-to-door do not use this system because of incorrect delivery. The affecting factors to utilize the system were sex, waiting time, fee. Conclusion : The model for door to door delivery system. 1. door-to-door personnel reside in hospital and the patient want to utilize the system apply for the delivery with charging. 2. The applied drugs dispense at spare time. 3. Delivery company gathers drug at appointed time and delivers. 4. The delivery fee is 2,000-3,000 won. 5. To prevent from loss and changing the drug the name of patient on packet are printed and drug packet are sealed. 6. The company submit the confirm sheet which are written that the patient received drug correctly to hospital. 7. The delivery time of drug is reserved for the convenience of receiving.

노인장기요양 등급인정자와 등급 외자의 지역사회복지서비스 이용 실태분석 (Analysis on the Use of Welfare Services of Elderly Long-term Care Grade Accredited and Unidentified)

  • 노인들은 건강상태가 다소 좋지 않더라도 지역사회에 지속적으로 거주하기 원한다. 그러나 노인의 건강 및 기능상태에 맞는 통합적인 돌봄지원 시스템의 부재로 생활시설을 선택하는 경우가 증가하고 있다. 본 연구는 2017년 노인실태조사를 활용하여 장기요양인정을 신청한 노인들을 대상으로 욕구에 맞는 서비스를 이용하고 있는지를 탐색적으로 논의하고자 한다. 분석결과 첫째, 장기요양인정을 통해 등급을 받은 노인 중에서 경증 노인은 방문요양 등 재가서비스를 주로 이용하고 있었다. 그러나 경증 노인 중에서 일부는 요양시설에 입소하고 있어서 기능 상태에 맞지 않는 서비스를 이용하고 있었다. 둘째, 방문요양서비스가 주야간보호서비스에 비해 월등히 이용이 높아서 노인 상태에 맞는 복합적인 재가서비스가 이루어지지 못하고 있었다. 셋째, 등급 외 노인의 경우 등급인정 노인에 비하여 일상생활 수행을 위한 도움을 충분히 받지 못하고 있었으며, 경로당이나 노인복지관 등 지역사회복지서비스 이용도 낮았다. 따라서 장기요양인정자의 경우 건강 및 기능상태가 경증 임에도 지역사회에 계속 거주하지 못하고 시설에 입소하는 경우가 발생하고 있고, 등급 외 노인의 경우 필요한 지역사회 돌봄 서비스를 적절이 이용하지 못해서 장기요양인정자로 상태가 악화될 가능성이 높은 것으로 판단된다.

  • Hospice Care is the best way to care for terminally ill patients and their family members. However most of them can not receive the appropriate hospice service because the Korean health delivery system is mainly be focussed on acutly ill patients. This study was carried out to clarify the situation of hospice in Korea and to develop a hospice care delivery system model which is appropriate in the Korean context. The theoretical framework of this study that hospice care delivery system is composed of hospice resources with personnel, facilities, etc., government and non-government hospice organization, hospice finances, hospice management and hospice delivery, was taken from the Health Delivery System of WHO(1984). Data was obtained through data analysis of litreature, interview, questionairs, visiting and Delphi Technique, from October 1998 to April 1999 involving 56 hospices, 1 hospice research center, 3 non-government hospice organizations, 20 experts who have had hospice experience for more than 3 years(mean is 9 years and 5 months) and officials or members of 3 non-government hospice organizations. There are 61 hospices in Korea. Even though hospice personnel have tried to study and to provide qualified hospice serices, there is nor any formal hospice linkage or network in Korea. This is the result of this survey made to clarify the situation of Korean hospice. Results of the study by Delphi Technique were as follows: 1.Hospice Resources: Key hospice personnel were found to be hospice coordinator, doctor, nurse, clergy, social worker, volunteers. Necessary qualifications for all personnel was that they conditions were resulted as have good health, receive hospice education and have communication skills. Education for hospice personnel is divided into (i)basic training and (ii)special education, e.g. palliative medicine course for hospice specialist or palliative care course in master degree for hospice nurse specialist. Hospice facilities could be developed by adding a living room, a space for family members, a prayer room, a church, an interview room, a kitchen, a dining room, a bath facility, a hall for music, art or work therapy, volunteers' room, garden, etc. to hospital facilities. 2.Hospice Organization: Whilst there are three non-government hospice organizations active at present, in the near future an hospice officer in the Health&Welfare Ministry plus a government Hospice body are necessary. However a non-government council to further integrate hospice development is also strongly recommended. 3.Hospice Finances: A New insurance standards, I.e. the charge for hospice care services, public information and tax reduction for donations were found suggested as methods to rise the hospice budget. 4.Hospice Management: Two divisions of hospice management/care were considered to be necessary in future. The role of the hospice officer in the Health & Welfare Ministry would be quality control of hospice teams and facilities involved/associated with hospice insurance standards. New non-government integrating councils role supporting the development of hospice care, not insurance covered. 5.Hospice delivery: Linkage&networking between hospice facilities and first, second, third level medical institutions are needed in order to provide varied and continous hospice care. Hospice Acts need to be established within the limits of medical law with regards to standards for professional staff members, educational programs, etc. The results of this study could be utilizes towards the development to two hospice care delivery system models, A and B. Model A is based on the hospital, especially the hospice unit, because in this setting is more easily available the new medical insurance for hospice care. Therefore a hospice team is organized in the hospital and may operate in the hospice unit and in the home hospice care service. After Model A is set up and operating, Model B will be the next stage, in which medical insurance cover will be extended to home hospice care service. This model(B) is also based on the hospital, but the focus of the hospital hospice unit will be moved to home hospice care which is connected by local physicians, national public health centers, community parties as like churches or volunteer groups. Model B will contribute to the care of terminally ill patients and their family members and also assist hospital administrators in cost-effectiveness.

여성의 건강을 위한 간호전달체계 모형개발 - 조산원 중심으로 - (Model Development of Nursing Care System for Women's Health : Based on Nurse-Midwifery Clinic)

  • The purposesof the study are to analyze the community nursing center in U.S.A and to develop the model of nursing care system based on nurse-midwifery clinic in community for women's health in Korea. 1. In America nursing center is defined as nurse-anchored system of primary care delivery or neighborhood health center. Nursing centers are identified the following four types: (1) community outreach centers, which are similar to traditional public health clinics: (2) institutional-based centers following the mission of a large institution, such as a hospital or university: (3) wellness/health promotion centers, which offer screening, education, counseling, triage, and health maintenance services: and (4) independent practice. Nursing centers are a concept of services provided by nurses in practice arrangements in a community. Nursing centers offer a variety of services, ranging from primary care provided by advanced practice nurses with medical acute management and nursing care to the more traditional education, health promotion, screening wellness and coordination services. Some services, such as the care provided by advanced practice nurses are reimbursed under various insurance plan in some instances and states, where as others, such as preventive and educational services, are not. Thus, lack of reimbursement has threatened the survival of some centers. Licensing of nursing centers varies by state and program and accreditation of nursing centers is also limited. 52% of centers are affiliated with another facility and 48% are freestanding centers. The number of registered nurse at the nursing centers ranges from just one to 115, with a mean of eight RNs peragency and a median of three. Nursing centers avail ability varies: 14% are open 24 hours, 27% have variable short hours, 23% are open 6-7 days per week, and 36% are open Monday- Friday. As the result of my visiting three health centers in Seattle and San Francisco, the women's primary care nurse practitioners focus on a systematic and comprehensive assessment of the health status of women and diagnosis and management of common physical and psychosocial health concerns of women in ambulatory settings. Therapeutic nursing strategies are directed toward self-care, risk reoduction, health surveillance, stress reduction, healthy nutrition, social support, healthy coping, psychological well-being, and pharmacological therapy. They function as primary care providers for the well ness and illness care of women from adolescence through the older adult years and pregnant families. 2. In Korea a nurse-midwife practices independently for pregnant women's health including childbearing family at her own clinic in community. Her services are reimbursed under national health insurance but they are not paid on a fee-for-service schedule covering items. Analyzing the nursing centers in America, I suggest that nurse-midwifery clinics offer primary care for women and home care for chronic ill patients. The health law and health insurance policy should be reovised in order to expand nurse-midwife's and home care nurse's roles at nurse-midwifery clinic.

외국인 관광객 대상 위법행위 실태와 관광경찰제도에 관한 연구 (Study on Safety Measures for Foreign Tourists Visiting Korea)

  • 이 연구는 한국을 방문하는 외래 관광객의 관광불편 및 범죄에 대한 실태와 효율적인 제도 정책적 대안을 제시하기 위한 연구이다. 지난 2012년 우리나라를 찾은 외래 관광객은 1100만 명을 넘어섰다. 사상 처음으로 외래 관광객 1000만 명 시대를 맞은 것이다. 최근 3년은 두 자릿수 증가율을 기록해 경제협력개발기구(OECD) 주요국 중 최고 수준이다. 자연스럽게 관광산업 규모도 지난 5년간 괄목할 만한 성장을 이뤘다. 관광수입은 2007년 61억 달러에서 2012년에는 140억 달러로 불과 5년 만에 두 배 이상 늘어났다. 그러나 한국을 방문하는 관광객이 느끼는 불편과 관광지 및 관광객을 대상으로 한 범죄는 날로 증가하고 있으며, 이러한 상황을 적절히 통제하기 위한 경찰의 대응은 현실적인 한계로 인해 부족한 실정이다. 따라서 국내 방문 외국인 관광객의 관광불편 및 범죄를 효과적으로 대응할 수 있는 대안으로서의 관광경찰제도의 보완과 개선에 관한 논의는 이제 구체적인 방향으로 모색해야 한다. 이를 위해 본 연구에서는 관광불편 및 불법행위의 실태 및 문제점을 고찰하고, 안전대책으로서의 관광경찰제도 개선방안에 대해 고찰하였다.

미국과 캐나다 노인밀집도시의 노인주거관련 사회적지원에 관한 연구 : 농촌지역 소도시를 중심으로 (A Study on Social Supports for the Elderly Housing in Senior Concentrated Cities in the United States and Canada : Focused on Small Cities along Rural Counties)

  • The purpose of this study is to explore social supports for elderly housing and their residential lives in small cities along rural counties of the United States and Canada, and suggest future implications for age-concentrated rural villages in Korea. In this study, five small and medium cities in non-metropolitan counties of California and Ontario province were visited and elderly residents and service experts were interviewed about their perceptions of community integrated social support networks for senior residences. The senior housing complexes were built due to influx of both metropolitan and rural residents seeking warm localities, traffic connections, business purposes in active production areas. and leisure attractions. There are five main social support networks for senior housing issues in these areas. First, the areas are claimed for senior zones and accordingly health industries are encouraged by local authorities. Second, the community is homogeneously constructed as a senior friendly environment and include features such as an RV park and mobile cottages. Third, senior-helping seniors are offered active work through golf-cluster active retirement communities. Fourth, traditional theme production camps are mobilized by the elderly workers. Lastly, an information system is maintained for screening volunteers and for senior abuse prevention. On the other hand, residential lives are occasionally negatively influenced by unbalanced concentrations of elderly facilities such as nursing stations and funeral homes. For the future of Korean rural elderly policies, suggestions are made as follows: first, an integrated urban and rural township that contains attractive places for early retiring people who seek a warm atmosphere in later life needs to be constructed. Second, an integrated model retirement village of urban and rural retirement life needs to be initiated as a measure of evaluating the adaptation process of movers in senior concentrated zones. Third, a cooperation system among governmental ministries needs to be formed with the long- term goal of establishing a traditional rural town of independent housing districts and medical facilities in rural areas. Fourth, productive and active lifestyles need to be maintained as the local community and government develop successful retirement rural villages, by limiting the expansion of nursing related facilities. Finally, generation integrated visiting welfare programs and services need to be further developed for the housing areas especially in the winter, when social integration and activity are relatively low.

보건진료소(保健診療所)와 업무실태(業務實態)와 개선방안(改善方案) (Performance State and Improvement Countermeasure of Primary Health Care Posts)

  • 보건진료소의 보건의료환경 및 근무여건, 업무현황과 개선에 대한 보건진료원의 의견을 파악하여 향후 보건진료소의 발전 방안을 제시하는데 도움을 주고자 경상북도 소재 보건진료소(1996년 330개소, 1999년 313개소)의 운영상황보고서에 의한 업무 변화량을 분석하였으며, 보건진료원 280명의 설문자료를 분석하였다. 운영상황보고서에 의한 보건진료소의 관할인구 추이는 1996년에 비해 1999년에 전반적으로 감소하였으나 노인인구는 증가하였다. 운영상황보고서에 의한 보건진료소의 업무 활동상황은 1996년도에 비해 전반적으로 증가하였고, 진찰 및 투약관리, 검사, 성인병 및 만성질환관리, 노인건강, 가정방문이 특히 증가하였으며, 전염병 관리와 결핵관리에서 부분적인 감소를 보였다. 재정운영상태는 50.4%가 운영이 잘된다고 하였고, 1.4%만이 운영하기 곤란하다고 하였는데, 현 근무지 근무연수가 많을수록(p<0.05), 그리고 도시근교일수확, 인구가 많을수록, 진찰 및 투약건수가 많을수록(p<0.01) 운영이 잘 된다고 하였다. 보건진료원의 직업적 긍지는 전반적으로 긍정적이었는데, 그 중 하는 일의 중요함이 94.6%로 가장 긍정적이었고, 역할과 임무에 대한 만족정도는 현 근무지 근무연수에 따라 차이가 있었다(p<0.05). 보건진료원들의 보건기관과 민간의료기관과의 협조정도는 대체로 긍정적이었는데, 보건소와 협조정도는 연령이 많을수록, 근무 경력이 길수록 긍정적인 응답률이 유의하게 높았으며(p<0.01), 현 근무지 근무연수에 따라 차이가 있었다(p<0.05). 보건진료원들은 운영협의회, 마을건강원, 지역사회조직과도 협조적이다는 응답이 모두 70% 이상이었다. 보건진료소 사업계획서는 96.4%가 적성하였으며, 제2기 지역보건의료계획서 작성에는 11.4%만이 참여하였다. 관할지역주민의 혈압과 흡연여부를 70% 이상 파악하고 있는 보건 진료원은 각각 88.2%, 63.9% 였는데, 혈압파악률은 보건진료원의 연령이 많을수록(p<0.01), 교육정도가 전문대학 이하인 경우(p<0.05)에서 높았다. 보건진료원의 지난 3년 동안에 보수교육외 교육 참여율, 연구사업 참여율은 각각 27.5%로 저조했으며, 보건진료소 수입으로 주민환원 사업을 실시한 경우는 65.4%였다. 보건진료원들이 생각하는 보건진료소의 필요 정도는 국가적 측면, 소속시군 측면, 관할지역 측면에서 모두 95% 이상이 매우 필요 또는 필요하다고 하였으며, 53.9%가 보건진료소의 역할이 증대되어야 한다고 하였다. 지금까지 폐쇄 및 폐쇄 예정된 보건진료소에 대하여 보건진료원은 담당 부서의 보건진료소 업무에 대한 인식부족 및 행정편의, 보건진료원의 신분이 법률로 보장되지 않은 별정직이어서, 단체장의 의지 등을 주된 이유로 제시하였다. 보건진료원은 향후 보건진료소에 대한 평가기준에 대해 노인 및 만성질환자 등의 보건의료서비스 대상자수, 주민의 의견, 인구 규모, 일상생활권을 고려한 교통상황 등의 순으로 제시하였으며, 보건진료원에 대한 평가기준으로는 보건사업실적, 주민과의 유대정도, 진료실적, 행정 및 업무처리 능력 등의 순으로 제시하였다. 객관적인 평가 후 일정기준이하의 보건진료소에 대한 향후 대처 방안에 대해서 보건진료원은 현 구조 유지하면서 업무개선, 도시 의료취약지역으로의 보건진료소 위치 조정 등을 많이 제시하였다. 보건진료소의 가장 필요한 개선부문으로는 절반 이상인 52.5%가 보건진료원의 업무조정이라고 하였으며, 향후 보건진료소가 중점적으로 추진하여야 한 사업으로는 당뇨 및 고혈압 관리, 방문보건사업, 노인보건사업 등이 중요하다고 하였다. 향후 보건진료소가 일차보건의료의 가치체계를 잘 반영하는 조직이 되기 위하여는 지역사회 보건의료요구에 부합하는 업무개선이 이루어져야 하겠으며, 만성질환관리사업, 방문보건사업, 노인보건사업 등이 활성화 되어야 하겠다.
