Purpose: This study was a survey to provide basic data about nursing interventions for improving the quality of life among family caregivers for the elderly by examining their care burden, burnout, and quality of life, and by confirming the correlation between each of them. Method: The subjects were 215 people in Seoul and Gyeonggi Province who understood the purpose of this study and participated voluntarily from April 1 to June 4, 2007. Data was analyzed by the SAS program. Result: 1. Concerning primary caregivers of the elderly, it was found that their care burden was slightly high, burnout was high and quality of life was good on the whole. 2. When it comes to the correlation among care burden, burnout, and quality of life among family caregivers, it was found that burnout increases in proportion to care burden, quality of life decreases as care burden increases, and bigger burnout leads to a lower quality of life. Conclusion: The quality of life among primary caregivers should be improved by reducing their care burden and burnout. Thereby, a priority might be considered for admission to nearby, comfortable nursing homes or failing that, home visiting services, rather than family support in the home.

Patients with mental disability who are classified as dental severely disabled, have poor oral health status and many difficulties in taking health services. The purpose of this study was to investigate factors of influencing in visiting oral health care services. After receiving approval from institutional review board in Seoul National University, 39 participants were recruited and the oral health examination and questionnaire were taken. Data were analyzed using one-way ANOVA, independent t-test, hierarchical linear regression for predicting influence of each participants' characteristics on oral examination time and care time. In regression model, the higher the disability grade and the lower the degree of cooperation, the more oral examination time increased. However possibility of communication was not significantly influenced. Also, the higher the disability grade and the lower the degree of cooperation, the more oral care time increased. However possibility of communication was not significantly influenced. These results are expected to provide objective data for introduction and establishment of visiting dental care.

Objective: The purpose of this research was to define the factors influencing turnover intention of customized home health care nurses. Methods: Research participants were 237 nurses who worked in the customized home health care of 33 public health centers in Daegu city and Gyeong-Buk Province. The data were collected by self report questionnaires from July, 1 to August, 30, 2011. The data were analyzed by t-test, ANOVA, correlation coefficient, multiple stepwise regression using the SPSS program. Results: The average score for turnover intention was 3.12 (5 score) and job satisfaction was 2.41 (4 score). The scores for sub-categories of job satisfaction were operational factors, 1.99, personnel factors, 2.03, work factors, 2.64 and relationship factors, 3.04. Marital status, religion, visiting nurses career, employment status, work area were the significant factors related to turnover intention in these subjects. There was a significant negative correlation between job satisfaction and turnover intention (r=-0.354, p<0.001). Significant factors were job satisfaction, marital status, employment status (Cum $R^2$=0.198, F=17.179, p<0.001) that explained 19.8% of turnover intention. Conclusions: Customized home health care nurses management policy which includes job satisfaction and general characteristics of visiting nurses should be established to enhance of quality of customized home health care service.
The urgent needs to establish hospice care systems in Korea arise from the following reasons: 0) a drastic increase in chronically ill patients with the increase of aged population: (2) rapid changes in living environment from the traditional habitation (e. g., Many Koreans living in apartment complexes, which is the most popular form of modern residence in recent years, prefer to die in the hospital.): the overall increase in patients with advanced cancer: (4) recent trends in early discharge of terminally ill patients from the limited hospital facilities to accomodate other medical insurance beneficiaries; (5) easy acceptance of euthanasia owing to the recent social atmosphere that belittles the dignity of human life; (6) medical and nursing care of AIDS patient in terminal stage; (7) and the problem associated with inhumane medical care system, overtreatment, and groundless fears against narcotics. Terminally ill patients were used to be treated in the hospital in the past. In these days, however, they are forced to have home cares with little assistance from the qualified medical personnel because of insufficient hospital facilities, which are even short for the need of emergency patients and provide priority cares to medical insurance beneficiaries with other acute problems. And yet, neither are there any administrative organizations nor systematic medical studies that deal with the level of terminally ill patient's need, their family's problems and resources of hospice care systems in Korea. Thus, most patients are not able to get appropriate medical care at the terminal stage of their lives. The objective of this study is to make comprehensive database for various hospice care organization currently in operation, link them through medical information system, and develop an easily accessible hospice care model that meets the need of most Korean people. Our survey results may be summarized as follows: Nationally there are 40 organizations that provide partial or full hospice care. However, these organizations are not linked to any formal medical service network. Furthermore, the objective of hospice care, care principles, personnel with appropriate training, educational programs, standard for care, costs, consulting service to patients' family members, the extent of medical care from professional staff members, status of hospice facility, and management of those institutions are neither clearly defined nor organized compared to the international hospice care standards. The surveys on patients of terminal stage. grouped in hospice and non-hospice care patients. reveal what they want visiting nursing care to help their pain control. psychological. social and spiritual demands. While the more than 90% of hospice care patients want to reduce their pains. the non-hospice care patients. in addition to their desire for pain control. demanded more psychological. social and spiritual helps as well. The results of this research could be utilized to 0) define the standard of hospice care. (2) provide the guidance for hospice medical care costs. (3) establish the database of hospice care systems. (4) develop softwares. (5) build communication network through Medinet. and (6) provide an organized visiting home nursing care system. These information should be a valuable resource to many medical staffs who are involved in cancer therapy. nursing care. and social welfare programs.
Purpose: The purpose of this study is to identify factors affecting the urinary incontinence (UI) prevalence and the quality of life among elderly adult women who are subject to home visiting healthcare services. Methods: A cross-sectional study was conducted with 400 women aged 60 or over who were registered for home health care services at one health center in Gangwon-do. Data were collected using a structured questionnaire consisting of UI severity, UI type, and the quality of life. The UI severity was assessed using International Consultation of Incontinence Questionnaire-Short Form (ICIQ-SF), the quality of life using Incontinence Quality of Life (I-QOL), and type of UI using Questionnaire for Urinary Incontinence Diagnosis (QUID). The data were analyzed by using x2 test, independent t-test, one-way ANOVA, Pearson's Correlation, binary logistic, and multiple lineal regression. Results: The prevalence of UI is 51.7%. The mean score of ICIQ-SF was 9.70±4.04 for women with UI and 0.04±0.51 for women without UI (t=-33.67, p<.001). As the frequency of day time urination (OR=1.34), the subjective health status (OR=1.45), and the educational status (OR=0.90) were higher, the risk of UI prevalence was the higher. The factors affecting I-QOL were ICIQ-SF score, mixed UI, subjective health status, and nighttime urination (adjusted R2=.61). Conclusion: The results of this study showed that UI severity, mixed UI, subjective health status, and day time urination affected the quality of life. It suggests that the assessment for the severity and type of UI be needed to prevent UI or improve the quality of life of UI vulnerable elderly adult women.
This study was conducted as a survey to find out middle-aged persons's housing preferences for a future elderly housing and to grasp background characteristics of persons influencing the preferences and the effect of elderly's aging situations on the changes of their preferences. A sample of 300 middle-aged people was selected purposely from parents of university students living in Gwang-ju city. Questionnaires were administered to parents by students and analysed with SPSS Windows 10 program. Middle-aged persons's housing preferences were differed according to elderly's aging situations, in the case of healthy situation, they tended to prefer to stay present house without their children in irrespective to couple or single, whereas in the case of unhealthy situation, they tended to prefer to live with their children. and couples showed preferences for planned housing for the elderly and singles showed preferences for elderly housing facilities with care services. The most important characteristics of elderly housing among middle-aged people was a places where they can enjoy leisure and provide home-help services, located in the suburbs. Also middle-aged persons were likely to prefer to a green spaces such as garden, path for walking, and madang for the elderly housing. Needs for community care services (NCCS) were required above the average. Among the items of NCCS, a visiting medical examination was the most demanding, and a visiting nursing services, a emergency calling bell, and home repairs were sequently followed in the level of needs. The characteristics of the elderly housing which preferred to and needs for community care services were influenced by middle-aged persons's age, gender, subjective evaluation of economic abilities, tenure status, the cost of living, and the size of housing. This findings suggests that it must to be considered to develop various types of the elderly housing depending on socioeconomic status.
Purpose: The rapid change to an aging society generates an increase in the incidence of chronic diseases. Many chronic patients have been facing their illnesses without enough preparation. In order to solve these problems, we designed and tested a public healthcare service based on ubiquitous technology. Method: Telemedicine has emerged as new medical care system of chronic disease. However, public potential of its technology is difficult to know under current traditional health care system. In this work, we developed a Personal Digital Access (PDA) phone based healthcare system by trained visiting nurses for elderly patients. A field test was performed by SeongBuk Public Health Center in Seoul, Republic of Korea (ROK). Result: Surveys were generated to assess the effects of this system compared to conventional public health system. Findings of trials demonstrate that healthcare coordination enhanced by PDA phone technology is satisfactory to the patients and nurses compared to previous one because prompt responses arouse their recognition of health. Conclusion: Ubiquitous healthcare system applied to public health service proved to be efficient and time-saving to monitor and control the chronic illnesses of large population.

Objective: The purpose of this study was to identify the predictors and frailty level in the frail elderly receiving home visiting health care services. Methods: The subjects were 177 frail elders aged over 65 registered in the home visiting health care services of three public health centers in Daegu. The data collection was performed from June 9 to June 24, 2015. This study used descriptive statistics, t-test, ANOVA, scheffe test and stepwise multiple regression by SPSS Win 18.0 program. Results: The mean of the frailty score was 10.05 (${\pm}4.52$). Age and life satisfaction were the significant factors related to the frailty score in frail elderly. Health promotion behavior, empowerment, social participation and perceived health status had a negative correlation with the frailty score. Thirty seven point four percent of the variance in the frailty score can be explained by perceived health status (${\beta}=-0.398$, p<0.001), health promotion behavior (${\beta}=-0.251$, p<0.001) and age (${\beta}=0.232$, p<0.001)(Cum $R^2=0.374$, F=25.744, p<0.001). Perceived health status was the most important factor related to the frailty score in our study. Conclusions: An integrative care program which includes these significant variables of subjects is essential to prevent the deterioration of frailty in frail elderly.
Purpose: The purpose of this study was to understand levels of both mental health literacy of depression and intention of help-seeking, and then to identify the relationship of them in Korean older adults. Methods: Participants in this cross-sectional survey were 395 persons over 65 years old receiving customized home visiting health care services at Chungnam province. Data were collected through interviews by visiting nurses in 2011. Results: The proportion of participants with depression was 61.8% ($6.7{\pm}3.6$). 78.2% of subjects appeared to have help-seeking intention for solving depression. The proportions of participants with ability to recognize depression was 69.1%. Although the level about knowledge and belief in self-help interventions were varied according to questionnaires, subjects understood self-help intervention of smoking accurately (86.3%) and physical activity (85.5%). Ability to recognize depression, knowledge and belief about self-help managements, and opinion of medication for treatment among health literacy variables measured in this study were related to help-seeking intention. In addition, women, visual impairment, and lower depression scores were related to help-seeking intention. Conclusion: Results demonstrate that it is necessary to improve depression health literacy to manage effectively depression of vulnerable elderly in communities. These results could be used in developing mental health literacy programs.

Purpose: The purpose of this study was to analyze the role and tasks of nurses who were working for the elderly in the visiting health services at the public health centers. Methods: Literature reviews, two rounds of meetings with 5 experts and a two-round Delphi technique with 15 experts were performed in this study. Results: The nurses' role and job analysis revealed 5 roles, 16 duties and, 71 tasks. The nurses' roles, including discovery and registration of households/groups in visiting health service in the community, case manager, administrative management, program planning, operation and evaluation, and development of job competency. Sixteen duties included client registration and management, need assessment and plan establishment, education, consultation and support, seasonal health care, prevention and monitoring of infectious diseases, basic nursing care, chronic disease management, linkage and utilization of resources, team cooperation and coordination, home environment management, monitoring and support for intervention outcomes, evaluation, administrative management, program planning, operation and evaluation, development of professional competency and, adoption of fourth industrial revolution technology. Conclusions: Based on the results, the government should provide sufficient nursing personnel to provide universal preventive health services for the elderly and a job training program to perform these roles well.
