Purpose: This study examined the characteristics of in-home injuries of children in low-income families and sought to identify the factors affecting parental in-home injury prevention practices. Methods: A cross-sectional descriptive survey design was applied, using questionnaires on in-home injury characteristics in children, parental in-home injury prevention practices, parental perceptions and knowledge on childhood injuries, and the Parental Stress Index. We queried 169 parents of children less than 5years of age who were enrolled in Nutrition Plus Projects at community health centers. Results: Overall, 92.7% of children had experienced in-home injuries, with sliding crashes and bumping injuries as the most frequent type of injury. The recovery rate with a scar after injury was 26.3%. Parental practices for in-home injury prevention were higher according parental age, educational status, and previous learning experiences regarding in-home safety and injury prevention. The two most significant factors affecting parental in-home injury prevention practices were age and parental perception of childhood injuries as being controllable and preventable. Conclusions: Considering the high risk of in-home childhood injuries in low-income families, safety education and the promotion of injury prevention practices for parents are recommended. The strategy to enhance the parental perception on preventing childhood injuries needs to be addressed.
Since patients who have suffered from strokes have chronic health problems, the development of a protocol for home care nursing is required. Therefore, this study was done to test the effects of such a protocol on stroke patients. This study was conducted by using a single group pre-post design. The subjects for the study were 13 home care nurses and 30 home care patients who had a stroke. Data collection was done from October 1997 to February 1998. The collected data was analyzed using the Wilcoxon signed rank test and paired t-test and the results were as follows : 1. The first hypothesis that 'working satisfaction of home nurses would be higher after using the home care protocol' was supported. 2. The second hypothesis that 'patients' satisfaction would be higher after using the home care protocol' was rejected. 3. The third hypothesis that 'the level of ADL of patients would be higher after using the home care protocol' was supported. In conclusion, by using a home care protocol, the satisfaction of nurses' work and the ADL of patients increased, and the patients' satisfaction also increased without statistical significance. The above results confirm that the application of a home care protocol is very effective. Thus, this protocol can be helpful in the quality care of stroke patients. Therefore, developing and organizing protocols, according to health problem, can be useful for beginner nurses who interact with high risk patients.
Purpose: To identify factors that affect the job stress of visiting nurses at health centers in Seoul, Korea, who perform home care services. Methods: Data were collected in the period from November 20 to December 10, 2008, from 206 visiting nurses working at 25 health centers in Seoul metropolitan city. Self-report questionnaires of the Organization-Based Self-Esteem (OBSE) and the Korean Occupational Stress Scale (KOSS) were used. Data were analyzed by the SPSS win 15.0. Results: Several findings were obtained from the study. First, the mean of job stress was 52.4 points, which was higher than that of Korean women. Second, it was shown that by the sub-domain of job stress, the mean value belonged to the top 50% in the four sub-domains of physical environment, job demand, interpersonal conflict, and organizational system. Additionally, it belonged to the top 25% in the sub-domain of job insecurity, wherein the job stress was the highest. Third, the self-esteem of the nurses had the most significant effect on their job stress. Conclusion: Stress related to job insecurity was the highest among the visiting nurses at health centers in Seoul who perform the custom home care service thus, institutional support is urgently needed to alleviate such stress and secure their employment.
Purpose: This study investigated the degree of, and factors influencing the spiritual care among home healthcare nurses. Methods: The subjects were 168 advanced practice nurses working in 94 hospital-based facilities for home health care located in Seoul, Gyeonggi-do, and Incheon Metropolitan City. Data were collected from January 14, 2019, to March 7, 2019, using structured questionnaires on spiritual care and related variables based on the literature. Data were analyzed by t-test, one-way ANOVA, Pearson correlation, and multiple regression analysis (stepwise) using SPSS Version 23.0 program. Results: The results showed that the degree of spiritual care of home health care nurses was 3.44 points out of a possible 5 points. Factors affecting nurses' spiritual care were spiritual well-being(𝛽=.36, p<.001), recognition of spiritual care as the nature of nursing(𝛽=.17, p=.016) and role of nurses(𝛽=.22 p=.002), regular-based education for spiritual care(𝛽=.23, p<.001), patients' discomfort with spiritual discussion(𝛽=-.18, p=.001), and nurses' experience in home health care area(𝛽=.14, p=.010), which explained 57.2% of the observed variation in spiritual care. Conclusion: This study suggests the need to develop regular-based education programs for facilitating spiritual care and strengthening home health care nurses' spiritual well-being and their positive perceptions toward spiritual care.
In order to confirm the level of professional commitment and organizational commitment of the home health care nurses and the public health nurses, this study was carried out by using the Commitment Inventory developed by Meyer and Allen and modified by Rhee and others. To compare the commitment level between two professionals, data was collected through self-administered questionnaires from the 61 home health care nurses and the 134 public health nurses working in 25 public health centers in Seoul. The results are as follows: 1. Commitment level of' the Home Health Care Nurses ($4.7{\pm}0.7$) was significantly higher than that of the Public Health Nurses($4.4{\pm}0.7$). The level. of the professional commitment of the home health care nurses($5.0{\pm}0.9$) was higher than that of the Public Health Nurses($4.5{\pm}0.8$). The level of the organizational commitment of the of Home Health Care Nurses($4.5{\pm}0.7$) was higher than that of the public health nurses($4.3{\pm}0.6$). 2. The higher of affective professional commitment was shown in the home health care nurses, and the higher level of continuance professional commitment was shown in the public health nurses. The higher levels of normative professional commitment and affective organizational commitment were shown in the Home Health Care Nurses, and the higher level of continuance organizational commitment was shown in the home health care nurses. The higher level of normative organizational commitment was shown in the home health care nurses. 3. The level of professional commitment was statistically different in age and educational level. The level of affective professional commitment of the of home health nurses with higher-educated was higher than that of the lower-educated group. The level of organizational commitment of the Home Health Nurses in higher age was higher than that in lower age.
Purpose: This study was to investigate the effect of home visiting care service and to evaluate the effect from the cost-benefit perspective. Methods: Target participants were enrolled in 2007~2008 for home visiting care and provided with a home visiting nursing service for more than 18 months in J Ward of S City. Of 391 participants, 244 who satisfied the inclusion criteria were used in the final analysis. Cost-benefit analysis was done using the net benefit and benefit/cost ratio. Results: After providing the home care nursing service, the blood pressure control rate increased from 50.8% to 75.4%. Of the subjects, 39.8% maintained their blood pressure level within the target range. As a whole, the net benefit of home visiting care per person ranged from 434,964.86 to 447,112.43 won and the benefit/cost ratio ranged from 2.82 to 2.84. Conclusion: Home visiting care for vulnerable populations with hypertension was effective in both maintaining blood pressure and reducing blood pressure to the target range. Therefore these results are especially useful for establishing the value of home visiting services for policy makers as well as for prioritizing vulnerable populations.
Purpose: The purpose of this study was to examine the relationships between frailty, nutritional status, positive thinking, family function, and health conservation and to identify the factors influencing health conservation of the elderly at home. Methods: The research design was a descriptive survey using a convenience sampling. Data were collected from 142 elders using self-reported questionnaires. Data were analyzed using the SPSS/WIN 20.0 program for descriptive statistics, Pearson's correlation coefficients, and multiple linear regression. Results: The average health conservation score was 98.85. There were significant correlations between frailty, nutritional status, positive thinking, family function and health conservation. As a result of the multiple linear regression analysis, positive thinking, perceived health status, spouse and frailty accounted for 69% of the variance in health conservation of the elderly at home. Conclusion: These influencing factors on health conservation can be taken into account in the development of nursing intervention programs for improving health conservation of the elderly at home.
This study was conducted to assess need for education on the infection control by examining the knowledge on infection control in home care of the home care trainees who had completed an infection control course. Using 64 items questionnaire. home care trainees of eight home care education institutions who took classes related with infection control. The level of knowledge evaluated subjectively was 4.24point (full mark: 5 point). The correct answer rates for handwashing and gloving. cleaning patients' rooms. preventing sharp injury, preventing exposure to potential infectious agents, home care bag technique were 84.5% 86.4% 70.7% 65.3% 76.2% representatively. For the knowledge on the infection control principles to prevent catheter related infection, indwelling catheter related infection, tracheal tube related pneumonia. L-tube related infection, oxygen therapy related infection. and wound infection were 62.8% 27.8% 39.1% 87.8% 76.5% 80.5% representatively. The correct answer rates varied depending upon the educational institutes (p< = .0001), educational levels (p= .001), workplaces (p<.0001), and the experience of infection control education (p= .001). Considering these results. a standardized curriculum must be developed and implemented after analyzing the existing infection control curricula of the home care education institutes. and guide books for infection control in home care should be developed and distributed. In addition. continuous infection control education be provided to the home care nurses through reeducation so they can acquire new knowledge needed for carrying out infection control activities effectively.
Purpose: This study is know the home care nursing specialists' copying method for life-threatening emergency situation occurred while changing the tracheotomy tube in the patient's home through case-based learning. Methods: Two cases were analyzed using structured critical reflection while changing the tracheal tube. In the first case, the patient presented with dyspnea, cyanosis, and low oxygen saturation when connected to home mechanical ventilator after replacement of the tracheotomy tube. In the second case, replacement of the tracheal tube was difficult due to a narrowed airway from proliferated granulation tissue. Results: From the case-based learning. using a critical reflection instrument, home care nursing specialists indicated that it was important to explain the possibility of danger to the client and that nurses must check the operation of the ambu-bag to prepare for an emergency when replacing a tracheotomy tube. Moreover, they stated a need for two nurses during client visits when there is the possibility of an emergency. Conclusion: Case-based learning through critical reflection provides actual practice-focused knowledge that is helpful to home care nurse specialists who face emergency situations in a restricted environment. Therefore, we hope that home care nursing specialists will use this method to strengthen their professional knowledge.
This study was carried out to investigate agreement, content and demand for home nursing care of hospital inpatients in brain, spain and musculoskeletal diseases. The data was collected by interviewing with 242 patients who were hospitalized in university hospital on Taegu, from September 6,60 October 9, 1993. Of 242 patients, 66.1% agreed to home nursing care system and rate of agreement was highest between 30 years to 49 years of age as 40.4% in general characteristics. The rate of agreement according to type of diagnosis was highest inpatients with spinal diseases as 75.6% according to functional status was the highest in patients who had daily living activity freely as 69.4% according to prognosis in patients at terminal stage as 80.0% and the rate of agreement to home nursing care of patients who wanted early discharge was 73.9% The first-ranking reasons of agreement to home nursing care was asking for continuous relationship with doctor as 37.3% and there was statistically significant difference in reasons of agreement to home nursing care according to functional status of patients. The first-ranking reason of early discharge among patients who wanted early discharge(74.8%) was because of long time stay in hospital. Among 23 items of nursing activity that patient wanted, the first-ranking item was recovery promotion, prevention of complication, education and counseling for health as 76.4%, drug management was 2nd-ranking item as 62.1% and the third was regular checking of vital signs as 55.9%, The lowest item of demand for home nursing care was hospice care(3.9%) and airway keep(9.1%).
