Purpose: This study aimed to investigate the influence of fall risk, fear of falling, and social support on older inpatients' fall prevention behaviors. Methods: The participants were 105 older inpatients. Data were analyzed using descriptive statistics, independent t-test, One-way ANOVA, Pearson's correlation coefficients, and multiple regression analysis using the SPSS program. Results: The mean fall risk, fear of falling, social support, and all prevention behaviors were 58.05±28.27, 2.47±0.67, 3.58±0.63, and 2.93±0.48, respectively. There were significant differences in fall prevention behaviors by sex, and fall related to education experience. Fall prevention behaviors were associated with fall risk, and fear of falling. The factors influencing the fall prevention behaviors of the older inpatients were sex, fall related to educational experience, fall risk, and fear of falling. It was found that 21% could explain fall prevention behaviors. Conclusion: The results indicate a need to develop a fall prevention education program, including intervention, to reduce the fear of falling among older inpatients.
The purpose of this study was to describe about elders with dementia, their families, and their living environment by visiting the households where demented elders resided. The findings will be used as a basis to develop future individualized adjustment programs for demented elders and their families living in communities. The study participants were 64 demented elders and their families who were registered to a dementia counseling center at Nam-Gu community health center located in Inchon, Korea. Data were collected for two months, from May to June 2001. The length of data collection for each home visit ranged from 1.5 to 2.5 hours. Conclusion are as follows based on these study findings: Those demented elders had more than one chronic health problem in addition to their pre-existing dementia condition. Two thirds of the demented elders were not receiving any specific treatment for dementia. They showed a moderate level of independence in basic ADL, but were mostly residing at home because of lack of ability to perform more delicate and complicate routine daily activities by themselves. In addition, the primary caregivers were not well adjusted to the care-giving activities for their demented family members due to the lack of knowledge and information about dementia. The caregivers were mostly women including daughters-in-law, woman spouses and daughters, over a half of whom perceived their physical and mental health status as poor. Their image toward the demented elders was considerably negative. while their level of knowledge on dementia was moderate. The burden for the care-giving was high, whereas their coping method was passive. As the difference in image toward elderly before and after the onset of dementia in their family member increases, the caregiver burden also increased. The main resource of social support for the caregivers was their children. The caregivers showed high level of needs for knowledge and information on dementia, and day care service was the most preferred type of service by the caregivers. There was lack of safety in the living environments for the demented elders and their families, and in the surrounding environments to prevent dementia-related symptoms. Considering that home-based family care-giving is the most culturally appropriate model of providing care for the demented elders in Korea, we need to develop and apply an individualized adjustment program for the demented elders and their families.
There has been pointed out that a great portion of hospitalized patients stay in hospitals longer than necessary, often even after the completion of necessary care. This causes that hospital resources are not used efficiently. In order to identify underlying forces in postponing inpatients' discharge, this study aimed to investigate reasons for long-term stay of patients admitted in general hospitals. A total of 135 patients, who were staying at 7 general hospitals in Inchon and Kyonggi-Do for more than 60 days, were surveyed with a self-administered questionnaire between April 3 and April 10, 2000. Medical reasons including incompleteness of necessary care, difficulty in receiving outpatient-based care, and being under physical therapy were the most significant factors associated with long-term stay, followed by the lack of familial resources to take care of patients after discharge. Financial problems such as inability to pay for hospital bills were not significant factors influencing long-term stay. Regression analyses were conducted for medical reasons, familial resources, and financial problems, respectively. It was shown that receiving physical therapy and the number of admission in the past were significant predictors for medical reasons. The lack of familial resources as a reason for long-term stay had a positive relationship with the degree of need for aid in daily living. It may be recommended for the hospitals to cope with administrative problems due to the patients' long-term stay, considering the reasons of it, and their characteristics. And also, institutional efforts like vitalizing the home care service systems by hospitals as the continuing care after discharge should be needed.
Purpose: This study aimed to shed light on the experiences of women caregivers' in nursing homes who provide care for older men; it focuses on sexual behavior and provides baseline data that can help improve quality of care. Methods: Colaizzi's phenomenological method was used for data analysis. Participants included 10 caregivers working at a nursing home. Face-to-face in-depth interviews were conducted for data collection between January and February, 2020. Results: Five theme clusters were identified based on women caregivers' experiences of sexual behavior when caring for older men: "The weight of charge and exhausted stamina", "Unwillingness to face awkwardness", "Unacceptable sexual desire by older men", "An incurable wounded heart", and "Hardship in its entirety becomes my share of responsibility". The analysis further yielded ten themes. The participants stated that was hard to accept the older men's sexual behavior through relationships with the older men. Conclusion: These results should be considered in planning caring interventions for older men with respect to sexual behavior in nursing homes. The findings support the need for educational strategies and programs to improve the ability of nursing home caregivers to adequately care for older men, with consideration for their sexual behavior and for developing relevant policy supports.
In Korea, there is a need for safe and convenient elderly housing so that older people can enjoy a good quality of life and perform various daily activities while they maintain their health and well-being. Thus, this study is to suggest community -based housing settings for the elderly. We analyzed living space characteristics of the Continuing Care Retirement Communty(CCRC) in the U.S. In particular, we focused on CCRCs in the Oregon area and examined two types of settings: 1) a tower setting and 2) a town setting. The CCRC living arrangements include independent living, assisted living, nursing care, and memory care. We visited six CCRCs in Oregon during January through May in 2015. The field observations and floor plan surveys were conducted for data collection. The data analysis revealed that there are two types of arrangements: the suburban type and the urban type. Element analysis of the living unit designs for each CCRC type demonstrated typical space configurations. It was found that home care services were provided from local communities. It is suggested that community-based housing for the elderly should include mixed housing types so that community facilities can be shared. Medical services in the community were offered through healthcare institutions, disease prevention centers, welfare centers, and sports facilities. In order to apply these community-based care systems to elderly Korean housing, it is important to develop a community based on independent homes that share services and welfare facilities.
Purpose: The number of nursing home residents has been increasing in Korea. Nurses need to know the lived experience of relocation of the elderly to long-term care facilities. However, studies on this issue has not yet been conducted in Korea. Therefore, this study was carried out to understand the experience of institutionalization of the elderly using a phenomenological approach. Method: There were 11 participants who consisted of 5 men and 6 women living in 3 different nursing homes. The data was collected through in-depth interviews and participant observation from June 1999 to October 2001, and analyzed by Colaizzi's phenomenological method. Result: A total of 275 meaningful statements related to the experience of institutionalization by the elderly were obtained. Of the 275 statements, 175 were found as the statements with more general forms. 28 themes were grouped into 8 theme clusters. These theme clusters included' resentment at their sons', irresistible admission', 'humiliation on institutionalization', 'being unbearable to shock', 'grief to unfamiliarity', 'being in agony of outwards', 'regret for self-life' and 'comfort for new residence'. Conclusion: Based on these results. we can understand the lived experience of institutionalization of the Korean elderly. We need to develop a nursing program to help the elderly cope with this crisis, and studies about the familys experience on their parents admission to institutionalized facility warrant further exploration.
This study was described as midwife's role and obstacle of midwife's role expansion. Midwife as primary medical personal who practices for a mother and infant health care and promotion of mother infant interaction. As the trend of increasing natural childbirth, midwifery has to provide childbearing care those who want delivery in a midwifery center. This study conducted to survey for 44 midwives who work at the midwifery center. The results of the study as fellows. 1. Most of the midwives role was care of pregnancy, delivery, postpartum women and babies. Another role was conducted educational classes childbirth, breast feeding, contraception and sexual education. 2. Some midwives role perform breech, vaccum delivery, episiotomy and suture, pitocin induction and augmentation, ultrasonogram, giving medication, anesthesia, collecting specimen from Pap smear and vaginal discharge. Midwife perform these roles without medical law support. 3. Most of the obstacles of the midwife role was the medical law limitation. Midwives want revise medical law to perform simple treatment for childbearing women and babies. 4. Half of the midwives refer cases to medical doctor in case of complication of women and newborns. 5. Current frequency of home birth rate is slightly higher than before and me cases like to have delivery under water. Finally, midwife and midwifery have to prepare to meet childbearing woman, baby and family's need. For activation and expansion of midwife's role, every midwife has to be aware of medical law accurately and they must know what practice they can do and what practice they can not do.
The purpose of this study was to explore the maternal caretaking behaviors during the first 3 days postpartum. The sample consisted of 132 healthy, married women and their baby was full-term & normal also. Data were collected from April to June 1995 and analyzed using SPSS. The results of this study are as follows : 1. Primiparous mothers feel difficulty in performance of caretaking behavior for their baby was especially bathing, umblical cord care, identify the crying & baby's condition. But mothers feel doing so well was diaper change, clothing, immunization etc. 2. The results revealed that caretaking behaviors was not related to age, education & family. After the birth of a baby, the baby needs caring of parents especially mother. So, the mother feel difficulty in performance of maternal role. It means higher educational need for mother. Nurses were the primary source for caretaking behavior during early postpartum period at hospital and home also. Therefore the nurse need the strategies of nursing intervention to increase for caretaking activites of primiparous mothers. Further research is needed to determine which strategies affect the mother's competence of their caretaking behavior.
본 연구는 유아의 점심 식사장소에 따라 영양소와 식품섭취량에 차이가 있는지를 조사하였다. 제5기 (2010 ~ 2012년) 국민건강영양조사 원시자료를 이용하여 개인별 24시간 회상에 의한 식품섭취조사 항목 중에서 점심 식사장소가 유아원/유치원일 경우 보육기관 점심식사군으로, 점심 식사장소가 가정과 이웃집/친척집일 경우 가정 점심식사군으로 나누어 복합표본설계 요소를 반영한 통계분석을 실시하였다. 대상자수는 1~2세에서 보육기관 점심식사군 179명, 가정 점심식사군 367명이었고, 3 ~ 5세에서 보육기관 점심식사군 549명, 가정 점심식사군 225명이었다. 연령군별 하루 총에너지섭취량, 점심과 저녁 식사로부터 에너지 섭취량은 두 집단 간 차이가 없었으나, 아침 에너지섭취비율이 보육기관 점심식사군에서 낮은 반면에 간식 에너지섭취비율은 더 높았다. 3 ~ 5세 유아에서는 나트륨, 칼륨 및 비타민 C 섭취량이 보육기관 점심식사군에서 많았으며, 리보플라빈 섭취량은 가정 점심식사군에서 유의하게 많았다. 점심으로부터 섭취한 식품군을 비교한 결과 1~2세 보육기관 점심식사군에서 채소류 섭취량이 유의하게 많은 반면에 과일류, 난류, 우유류 섭취량은 가정 점심식사군에서 많았다. 3 ~ 5세 보육기관 점심식사군의 점심으로부터 채소류, 두류, 어패류 섭취량이 유의하게 많은 반면에 우유류 섭취량은 가정 점심식사군에서 많았다. 3~5세 유아들이 보육기관 점심으로부터 채소류, 두류, 어패류를 더 많이 섭취하고 칼륨과 비타민 C를 많이 섭취하여 보육기관 점심이 영양소 균형과 식품 다양성 측면에서 가정 점심보다 나은 것으로 판단된다. 결론적으로 유아의 성장 발육을 위한 적절한 영양 공급은 물론, 올바른 식습관 형성을 위해서 보육기관 급식은 물론, 가정 식사에서도 채소류, 생선류 등 다양한 식재료를 활용하는 노력이 요구된다.
Primary school is regarded as an important period when many health-related behaviors and life-styles begin to be formed. Acquiring them through school heath education has a strong influence on the health promotion of not only the family but also the community. The objectives of this study were to evaluate current oral health care of elementary schools in Chungnam province and to provide information for further development in elementary school oral health. We performed a questionnaire survey to 280 health teachers and among them, 155 teachers answered. The result of this study were as follows: 1. Sixty five percent of the health teachers had little interest in oral health. Major information sources for teaching oral health were books in 58.1% of the 155 teachers and 83.2% of teachers spent 30 minutes to 1hour per day in oral health care practice for the students. 2. Contents of the oral health education were composed of regular and special curriculums, and an average of education time during a semester was 2.6 hours in 3rd grade, and 1.3 hours in first and second grade. 60.6% of the teachers made the children practice the proper method of tooth brushing during the education time. 3. Major problems in oral health education were insufficient time, lack of equipment and difficulty in teaching method. The educational media were tooth models among 91.0% and OHP among 85.2% of the teachers. The tooth model was usually used in first to fourth grades and OHP in fifth to sixth grades. But 63.9% health teachers need to develop stronger educational methods using multimedia. 4. Meanwhile the most important strategy of oral health in urban schools was health education, that of rural schools was fluoride mouth-rinsing programme. Fluoride mouth-rinsing programmes were performed by 60.0% of the elementary school. Periodic dental examination was performed in all elementary schools. 98.2% of the schools sent the results home through school notification letters, but post-examination management was performed in only 67.1% of them 64.5% of the health teachers do follow-ups on the oral disease of the children after the examination. Only 0.7% of the schools have oral health education plans for the students' parents. Considering these major strategies for elementary school oral health care were health education, practicing proper methods of tooth brushing, periodic dental examinations, and fluoride mouth-rinsing programmes. But health teachers need more time for oral health education, practicing and management, and developing education materials. With regard to the high demand for oral health education and poor follow-up after periodic examination, the oral health education in elementary school should be considered as a formal educational course for more proper management of oral health, including application of major strategies to the children in earlier grades and efforts for increasing recognition and participation of the parents.
