  • Purpose. This study aimed to explore family caregivers' experiences with ventilator-dependent patients at home. Methods. The number of patients using mechanical ventilators at home is expected to increase and family caregivers must be able to care for them. However, few studies focus on the experiences of family caregivers. We conducted in-depth interviews with 11 family caregivers who cared for a ventilator-dependent patient at home in South Korea. Data were analyzed using phenomenological method of enquiry. Results. Statements generated from the interviews on the meaning of the family caregiver's experiences were organized into 27 themes, 5 theme clusters, and 2 categories. The theme clusters included endurance under the burden situation, role strain as a caregiver, separation from others, trying to find coping methods, and oriental ethnical customs. Family caregivers of patients using domestic mechanical ventilators need systematic education and emotional support to cope with the challenges of managing ventilator equipment and learning new ways of communicating with patients on the ventilators. Conclusion. These findings may contribute to family caregivers' knowledge and competence, thereby allowing them to better support their ventilator-dependent family members.

  • Background : In order to improve the quality of life of dying patients, they need to receive not only the physical, psychological, social, and spiritual care, but also systematic and continuous care to die with dignity. However, no adequate medical services are available for these terminal cancer patients. We studied their behavior patterns of health care utilization to understand more of their medical and social needs. Methods : We investigated 108 bereaved families through the telephone interview with structured questionnaires. They were randomly selected through the retrospective chart review of the terminal patients who passed away due to cancer. Results : Most of the terminal cancer patients received their care from proper medical services including admission to hospital (45.4%), outpatient clinic (22.2%), emergency room (16.7%), and oriental medicine (12.0%). But during the terminal phase of their illness, 32.4% of patients never received medical care including oriental medicine, and 28.7% received alterative natural care. 26 bereaved families (24.1%) pointed out the indifference of medical staff as a problem receiving proper hospital care, and 22 (20.4%) emphasized emotional strain of their helplessness with the patients' suffering as a problem of caring at home. Over 90% suggested availability of continuous care, hospice care, home care, and 24 hour telephone service to be improved. Conclusions : Due to various reasons, adequate medical care is not delivered to the terminal cancer patients in our present medical system. These problems can be approached with the establishment of proper education and medical delivery system. The role of comprehensive medical specialty cannot be overly emphasized to accomplish this most effectively.

Methods: Participants included 293 high school students. The data were collected from March 4 to 29, 2019, and were analyzed using t-test, ANOVA, Pearson's correlation coefficient, and multiple linear regression with SPSS ver. 21. Results: Mental health was positively correlated with social pressure related to appearance and body image dissatisfaction. Factors influencing mental health were peer pressure (β=.28, p<.001) and parental pressure (β=.22, p<.001) related to appearance; body image dissatisfaction (β =.11, p=.034); "high" (β=-.18, p=.004) and "moderate" (β=-.22, p<.001) perceived school achievement; and "satisfied" response to school satisfaction (β=-.27, p=.028). These factors explained 32% of the variance in mental health. • This study purposed to examine elderly welfare facilities by type, to analyze their local variations, and ultimately to contribute to the expansion of elderly welfare infrastructure. The results are expected to help inspect elderly welfare infrastructure for providing the aged with social services before the execution of 'the insurance for elderly long term care' and establish welfare facilities by area in the future. For these purposes, we used the national data "The Current State of Elderly Welfare Facilities in 2007" produced by the Ministry of Health and Welfare. We digitized elderly welfare facilities in 163 cities and counties by type and analyzed them by area. We also examined the differences in the local distribution of representative elderly welfare facilities such as elderly welfare centers, home based facilities (home helper centers), asylums for the aged and elderly care facilities in 16 cities and provinces. Furthermore, we analyzed differences and problems in their local distribution urban areas, mixed areas of urban and rural communities, and rural areas. In addition, we studied the current state of institutionalized care and home based care, which are two major directions of current elderly welfare policies, based on the local distribution of facilities and analyzed differences in the trends according to area. According to these results, the urban had more home based care facilities than the rural. However, the rural had more institutionalized care facilities than urban. Also, each local self-governing body showed unique characteristics. Therefore, these results suggest that we need to establish elderly welfare policies based on the distribution of facility types by area.

  • This study was done to identify basic information in classifying nursing diagnoses and nursing interventions needed for the further development of computerized nursing care plans. Data were collected by reviewing charts of 123 home care clients who had active disease, for whom at least one nursing diagnosis was on the chart, and who had been discharged. Data included demographics, medical orders, nursing diagnoses and nursing interventions. The results of the study, which found the most frequent medical diagnoses to be cancer (40.7%) and brain injury (26.8%), showed that 'Impaired Skin Integrity'(18.3%), 'Risk for Infection'(15.0%), 'Altered Nutrition, Less than Body Requirements'(13.8%), and 'Risk for Impaired Skin Integ rity'(9.9%) were the most frequent nursing diagnoses. 'Pressure Ulcer Care'(28.4%) was the most frequent intervention for 'Impaired Skin Integrity', 'Infection Protection'(16.0%) for 'Risk of Infection', 'Nutrition Counseling'(26.8%) for 'Altered Nutrition' and 'Positioning'(22.0%) for 'Risk for Skin Integrity Impairment', Comparison of interventions with the Nursing Intervention Classification(NIC) showed that the most frequent interventions were in the domain 'Basic Physiological' (33.94%), followed by 'Behavioral'(27.8%), and 'Complex Physiological' (22.6%). Interventions related to teaching family to give care at home could not be classified in the NIC scheme. Examination of the frequency of NIC interventions showed that for the domain 'Activity & Exercise Management', 75% of the interventions were used, but for seven domains, none were used. For the domain 'Immobility Management', 93% of the times that an intervention was used, it was 'Positioning', for the domain 'Tissue Perfusion Management', 'IV Therapy' (59.1%) and for the domain 'Elimination Management', 'Tube Care: Urinary'(54.0%). The nursing diagnoses 'Altered Urinary Elimination' and 'Im paired Physical Mobility' were both used with these clients, but neither 'Fluid Volume Deficit' nor 'Risk of Fluid Volume Deficit' were used rather 'IV Therapy' was an intervention for 'Altered Nutrition, Less than Body Requirements', A comparison of clients with cancer and those with brain injury showed that interventions for the nursing diagnosis 'Impaired Skin Integrity' were more frequent for the clients with cancer, interventions for 'Risk of Infection' were similar for the two groups but for clients with cancer there were more interventions for' Altered Nutrition'. Examination of the nursing diagnoses leading to the intervention 'Positioning' showed that for both groups, it was either 'Impaired Skin Integrity' or 'Risk for Skin Integrity Impairment'. This study identified a need for further refinement in the classification of nursing interventions to include those unique to home care and that for the purposes of computerization identification of the nursing activities to be included in each intervention needs to be done.
